[Clinical studies on imipenem/cilastatin sodium in perinatal use].

The clinical efficacy and the safety of imipenem/cilastatin sodium (IPM/CS) in perinatal infections were studied. The results are summarized below. 1. Clinical efficacy was evaluated in 12 patients with intrauterine infections (endometritis), 3 patients with amniotic fluid infections, 2 patients with puerperal fever and 3 patients with other infections. The drug was administered by intravenous drip infusion at a 0.5g to 1.0g dose as IPM and the total doses during an entire course of treatment were 1.5g to 15g. 2. Clinical efficacies were excellent in 4 (20%) and good in 16 patients (80%) and the efficacy rate was 100%. Ten patients had not improved upon treatments with other previous antibiotics. 3. Infective bacteria were eradicated in 4, decreased in 1, and replaced in 6 patients. 4. No side effects or abnormal clinical laboratory test values were observed in any patient.

[Efficacy of ceftriaxone against infections in the field of obstetrics and gynecology].

Ceftriaxone (CTRX), a newly developed cephalosporin antibiotic, was administered to patients with various bacterial infections in the field of obstetrics and gynecology, and the following results were obtained. 1. Two grams of CTRX was administered once daily by drip infusion for a total dosage of 6 g to 20 g to each of 3 cases of intrauterine infection, 1 case of adnexitis, 1 case of intrapelvic infection and 2 cases of infection of the external genitalia. 2. Clinical efficacy of CTRX was good in all cases. No laboratory abnormalities were observed nor subjective or objective adverse reactions occurred during the treatment. The administration of CTRX at a dose of 2 g once daily is a clinically convenient treatment regimen, and its results in satisfactory clinical efficacy.

Lack of recovery from oxygen-induced damage to colony formation and DNA synthesis in senescent human diploid fibroblasts.

The colony formation and DNA synthesis of cultured human fibroblasts were inhibited under hyperbaric oxygen. When maintained in a growth-arrested state for a few days after oxygen exposure, early passage diploid and SV-40-transformed cells repaired the oxygen-induced damage, while late passage diploid cells did not. In early passage cells to which cycloheximide, a protein synthesis inhibitor, was administered, no recovery from the damage to colony formation was found, indicating that de novo protein synthesis is necessary to the repair process. The repair function of the late passage cells against oxidative damage seems to be impaired. Oxidative stress may be involved in cellular ageing.

Membrane stabilization of vitamin E; interactions of alpha-tocopherol with phospholipids in bilayer liposomes.

13C Spin-lattice relaxation times (T1) of 13C-labeled alpha-tocopherol in three kinds of liposomes varying in their contents of arachidoyl residues have been measured by 13C-NMR spectroscopy. On the basis of T1 values, it is proved that the segmental motion of isoprenoid side chain of alpha-tocopherol tends to increase with an increase in the distance from the chromanol moiety, and that three methyl groups attached on the aromatic ring, have some affinity to unsaturated fatty acid residues rather than those of the isoprenoid side chain. These results are incompatible with the hypothesis of Diplock et al. (1) which 4'a- and 8'a-methyl groups of isoprenoid side chain are fitted in the Z-pockets of arachidoyl chain of polyunsaturated lipids in membrane.

On the pathogenesis of chronic alcoholic pancreatitis from the viewpoint of experimental results in rats.

To elucidate the pathogenesis of chronic alcoholic pancreatitis, light-microscopic and electron microscopic observations of pancreases from Wistar strain rats given 20% ethanol ad lib, instead of water, were performed. Results were as follows: (1) In 3 (11%) rats, focal lobular unit acinar cell damage was observed under light-microscopy. In these cases, the intercalated ducts and centroacinar cells showed proliferation independently of protein plugs in ducts. (2) After ethanol intake, several degenerative changes in the acinar cells and ductular cells were observed under electron microscopy. The earliest and most consistent change was the irregular formation of microvilli and interdigitation between the parenchymal cells, accompanied by widening of the intercellular space, especially between the acinar and centroacinar cells. After three months of ethanol intake, an increase in the lysosomes and mitochondrial alteration occurred in acinar cells, and after six months of ethanol ingestion, there was tortuous ductular proliferation accompanied by the degeneration of the ductular cells. In addition to these parenchymal changes, the capillary endothelium and nervous tissue of the pancreas showed occasional degenerative changes after three to six months of ethanol intake. These results suggest that repeated degeneration and regeneration of the ductular cells results in deformation of ductular structures, with subsequent stenosis, and represents the primary event in the development of chronic alcoholic pancreatitis in rats.

Lethal effects of a linoleic acid hydroperoxide and its autoxidation products, unsaturated aliphatic aldehydes, on human diploid fibroblasts.

The lethal effects of linoleic acid and its hydroperoxide on human diploid fibroblasts were quite similar, and that of the reaction mixture from the autoxidation of the hydroperoxide was considerable. Some unsaturated aliphatic aldehydes, the secondary products of the hydroperoxide autoxidation, were identified and their toxicity toward the cells was examined. Among them, (E,E)-2,4-nonadienal, (E,E)-2,4-decadienal, and (E)-4-hydroxy-2-nonenal were the most toxic; e.g., in the presence of 25 microM nonadienal, decadienal, or hydroxynonenal, 75, 90, or almost 100% of the cells, respectively, underwent lysis within one day. Generally, alkenals were toxic and alkanals non-toxic. The toxicity was enhanced as the number of double bonds in each molecule was increased and also as the carbon chain was lengthened.

[Clinical statistics at the Department of Urology, Kitano Hospital (1968-1986)].

A clinical statistic survey was carried out on the patients, diseases and operations experienced at our department between 1968 and 1986. In 1968, the urological department became independent from the dermato-urological department, and in 1981 the urological ward was established. The 19 years are divided in three periods; first period: 1968-1974 (7 years), second period: 1975-1981 (7 years), and third period: 1982-1986 (5 years). The total number of new outpatients during the 19 years was 50,443. They have gradually but steadily increased and have reached recently about 4,000 per year. The total number of inpatients was 3,422 (male: 2,561, female: 861). The proportion of the elderly patients, especially male, has remarkably increased and that of patients more than 60 years old was 44.9% of all inpatients in the third period. Among the major diseases of the inpatients, tumors has got most remarkable increase of its number and proportion. And among tumors, increases of benign prostatic hyperplasia, bladder cancer, prostatic cancer were prominent and at the third period the proportion of those three has reached 81.3% of all tumors. Percutaneous nephrolithotripsy (PNL) and transurethral ureterolithotripsy (TUUL) were introduced in 1985. Recently endoscopical operations such as transurethral resection (TUR), PNL, TUUL and so forth have become a large part of the urological operations.
