T cell activation and T cell receptor variable region gene usage in measles.

T cell activation and T cell receptor variable (V) regions were studied with monoclonal antibodies in peripheral blood lymphocytes from 22 patients with measles. Increased (greater than 5%) activated T cells (HLA-DR+ CD3+ cells) were noted in 14 of the 22 patients. Elevations of V beta 5+ and V beta 8+ T cells were observed in two and four patients, respectively, and appeared to be associated with T cell activation. The duration of fever was significantly prolonged in those with increased (greater than 10%) activated T cells (p less than 0.01). These results suggest that T cell activation and the preferential expansion of V beta 8+ and V beta 5+ T cells are associated with the pathogenic process of measles.

FR 113680: a novel tripeptide substance P antagonist with NK1 receptor selectivity.

1. We have discovered a novel tripeptide substance P (SP) antagonist, FR 113680 [N alpha-[N alpha-(N alpha-acetyl-L-threonyl)-N'-formyl-D- tryptophyl]-N-methyl-N-phenylmethyl-L-phenylalaninamide]. In binding experiments, FR 113680 inhibited [3H]-SP binding to guinea-pig lung membranes (NK1) in a competitive manner but had not effect on [3H]-SP binding to rat cerebral cortical membranes (NK1), [3H]-neurokinin A ([3H]-NKA) binding to rat duodenum smooth muscle membranes (NK2) and [3H]-eledoisin (Ele) binding to rat cerebral cortical membranes (NK3). 2. In bioassay experiments, FR 113680 dose-dependently inhibited SP-induced guinea-pig ileum contraction (NK1), but did not inhibit either NKA-induced rat vas deferens contraction (NK2) or neurokinin B (NKB)-induced contraction of rat portal vein (NK3). According to Schild plot analysis, the inhibitory effect of FR 113680 on SP-induced guinea-pig ileum contraction is competitive and the pA2 value is 7.53. 3. The inactivity of FR 113680 on NK1 receptors in rat compared to guinea-pig may represent species-specific forms of the NK1 receptor. 4. These findings suggest that FR 113680 interacts selectively with the NK1 neurokinin receptor.

Pharmacological profile of a high affinity dipeptide NK1 receptor antagonist, FK888.

1. In our search for compounds that inhibit the binding of [3H]-substance P (SP) to guinea-pig lung membranes, the dipeptide SP antagonist, FK888, was developed by chemical modification of the parent compound, (D-Pro4, D-Trp7,9,10, Phe11)SP4-11. 2. In a [3H]-SP binding assay using guinea-pig lung membranes and rat brain cortical synaptic membranes, FK888 displaced [3H]-SP binding with a Ki value of 0.69 +/- 0.13 nM and 0.45 +/- 0.17 microM, respectively, in a competitive manner. 3. FK888 inhibited the contraction of guinea-pig isolated ileum induced by SP in the presence of atropine and indomethacin (a NK1 receptor bioassay) with a pA2 value of 9.29 (8.60-9.98). 4. FK888 inhibited contractions of rat vas deferens by NKA (a NK2 receptor bioassay) and of rat portal vein by NKB (a NK3 receptor bioassay) at concentrations at least 10,000 times greater than that required to inhibit contractions of guinea-pig ileum. 5. FK888 also inhibited SP-induced airway oedema in guinea-pig after both intravenous and oral administration. 6. These data demonstrate that FK888 is a potent and selective NK1 antagonist which is active both in vitro and in vivo.

Characteristics of vasoinhibitory action of FK 453 (a pyrazolo-pyridine derivative), a new antihypertensive agent with diuretic action in isolated rabbit aorta.

The vasoinhibitory effect of FK 453 was examined in isolated rabbit aorta. FK 453 inhibited contractile responses to norepinephrine, angiotensin-I and KCl. Pretreatment of the tissue with FK 453 failed to affect the relaxing effect of verapamil on the KCl response and the inhibitory effect of prazosin on the phenylephrine response. FK 453 inhibited both the residual norepinephrine response and the subsequent Ca2+ response in a Ca(2+)-free medium containing EGTA and nifedipine. The inhibitory effect of a combined treatment with either FK 453 plus nitroglycerin or FK 453 plus theophylline, but not with FK 453 plus M & B 22,948 (2-o-propoxyphenyl-8-azapurine-6-one; May & Baker), was much greater than that of any single treatment. Pretreatment with FK 453 also potentiated relaxing effects of nitroglycerin and isoproterenol on the PGF2 alpha response. The effect of a combined treatment with FK 453 plus theophylline, but not with FK 453 plus M & B 22,948, was much greater than that of any single treatment. FK 453 also inhibited the activity of phosphodiesterase from canine aorta to convert cyclic [3H]-GMP and cyclic [3H]-AMP to 5'-GMP and 5'-AMP, respectively. These results suggest that the inhibitory action of FK 453 is not due to inhibition of voltage-operated Ca2+ channels or alpha-adrenoceptors, but due to increase in cyclic GMP level.

Imbalance between thrombin and plasmin activity in disseminated intravascular coagulation. Assessment by the thrombin-antithrombin-III complex/plasmin-alpha-2-antiplasmin complex ratio.

We investigated the imbalance between thrombin and plasmin activity in vivo with various grades of severity of disseminated intravascular coagulation (DIC) in relation to the underlying diseases. Plasma thrombin-antithrombin-III complex (TAT) and plasmin-alpha 2-antiplasmin complex (PAP) levels were measured in 133 blood samples obtained from patients with DIC. The TAT/PAP ratio was higher in patients with sepsis or solid cancer than in those with hematologic malignancies. In acute promyelocytic leukemia (APL), the TAT levels were the highest, but the PAP levels were even higher and the TAT/PAP ratio was the lowest. As for the severity of DIC, in mild DIC, both thrombin and plasmin activities were increased. In moderate DIC, the TAT/PAP ratio increased, and thrombin activity was much more predominant. However, in severe DIC, the ratio decreased, and plasmin activity became excessive. In 3 patients with acute myeloblastic leukemia, APL and pancreatic cancer, respectively, the PAP level remained high during heparin therapy although the TAT level was decreased. When tranexamic acid was given, the PAP level was selectively reduced, and the TAT/PAP ratio was markedly decreased along with clinical improvement. These results indicate that monitoring of the TAT/PAP ratio may contribute to decisions regarding the institution and performance of combination therapy for DIC using anticoagulants and antifibrinolytic agents.

Studies on antiinflammatory agents. I. Synthesis and pharmacological properties of 2'-phenoxymethanesulfonanilide derivatives.

Various 2'-phenoxymethanesulfonanilide derivatives were synthesized and evaluated for antiinflammatory and analgesic activities. Some compounds bearing an electron-attracting substituent at the 4'-position strongly inhibited adjuvant-induced arthritis in rats and acetic acid-induced writhing syndrome in mice without causing gastro-intestinal irritation. Among them, 4'-cyano-(FK867) and 4'-acetyl-(FK3311) 2'-(2,4-difluorophenoxy)methanesulfonanilides were selected as the candidates for further development.

New 2-aryliminoimidazolidines. II. Synthesis and antihypertensive activity of 2-(biphenylimino)-imidazolidines.

For improvement of the duration of action of FR35447 (I), 2-(biphenylimino)imidazolidines (III) were synthesized and their hypotensive activity was tested against conscious normotensive rats. 2-(4'-Fluoro-[1,1'-biphenyl]-2-ylimino)imidazolidine (IIIl) exhibited superior hypotensive potency and was comparable to clonidine (II) in its duration of action. The structure-activity relationships of III are also described.

Identification of effective component from a traditional herbal medicine and the inhibitory effects on experimental glomerular lesion in mice.

Effective constituents for renal disease were partially purified from a herbal prescription. The efficacy on renal diseases was examined using an experimental model system of glomerular lesion in mice induced by Agkistrodon acutus venom (Ac1-P). The extract of boiling water of the herbal prescription (P-3) was first fractionated by ether into an acidic fraction and a mixture of basic and neutral fractions. The acidic fraction was proved to be more effective and then further examined by thin layer chromatography and spectrophotometry. One of the main components was confirmed to be caffeic acid which had inhibitory effect on renal failure in mice by Ac1-P. This effect was considered to be caused by caffeic acid inhibiting the proteolytic enzyme activity of Ac1-P. Caffeic acid should thus have prophylactic or inhibitory effect on glomerular disease in which proteolytic enzymes may have pathogenic roles.

[Anterior urethral diverticular stones and retrocaval ureter in male: a case report and review of literature in Japan].

A case of diverticular stones in the male anterior urethra with retrocaval ureter is reported. A 26-year-old man visited our hospital for examination, who had experienced spontaneous stone discharge a few days earlier. Computed tomographic (CT) scan with ureteral catheterization and urethrography revealed a retrocaval ureter and urethral diverticular stones. Resection of urethral diverticulum with 7 stones and right ureteroplasty were performed. The urethrography and drip infusion pyelography (DIP) 9 months after operation showed no abnormal findings. The largest stone was 28 x 22 x 20 mm in size and 20 g in weight. The main components were ammonium dihydrogen-urate (70%), carbonate apatite and struvite. Histological feature of the epithelium of the urethral diverticulum indicated normal skin with hairs. Pathological diagnosis was para-urethral dermoid cyst. Our case is the 67th case of the male urethral diverticular stones and the first case of those with retrocaval ureter in Japan.

[Clinical studies on imipenem/cilastatin sodium in the field of obstetrics and gynecology].

The efficacy and safety of imipenem/cilastatin sodium (IPM/CS) were studied in patients with obstetric and gynecologic infections and in those given the drug as prophylaxis against postoperative infections. The following results were obtained: 1. Efficacy rates were 96.0% (48/50) in patients with obstetric and gynecologic infections and 100% (28/28) in those with urinary tract or other infections. The overall efficacy rate was 97.4% (76/78). Bacteriologically, 30 organisms were isolated from 28 patients. The eradication rate was 95.2% (20/21) and the efficacy rate was 96.4% (27/28). 2. Changes in blood elastase before and after treatment were compared with those in CRP, WBC, and ESR in the patients with obstetric and gynecologic infections. The changes in elastase were similar to those in CRP. 3. The efficacy rate was 98.0% (48/49) in the patients given prophylaxis against postoperative obstetric and gynecologic infections. 4. An adverse reaction was observed in only one patient (diarrhea), and abnormal laboratory findings were noted in 2 patients (elevation of GOT and GPT). These results indicate that IPM/CS is very useful for the treatment of obstetric and gynecologic infections.

[Usefulness of magnetic resonance imaging (MRI) in evaluation of left ventricular apical aneurysm].

Twenty three consecutive cases of left ventricular aneurysm due to antero-septal myocardial infarction in normal sinus rhythm were studied to decide whether or not magnetic resonance imaging (MRI) can evaluate aneurysm of the left ventricular apex. The apex, as well as the base, of the left ventricle was clearly imaged in 21 out of 23 cases. Poor images were obtained in two cases who showed frequent premature ventricular beats during this procedure. Two-dimensional echocardiography has weak points when visualizing left ventricular apex because, for one thing, the sector angle is maximally 90 degrees, and for another, the cardiac apex is too near the probe for observation of the characteristic of the left ventricular apical wall. However, MRI can make up for the above weak points and can provide supportive information by visualizing the entire left ventricular apex.

[Fast MR imaging of liver metastasis using FLASH and FISP--optimal sequences for T1- and T2*-weighted images].

This paper deals with a study to obtain the optimal sequence of gradient echo (GE) for T1- and T2*-weighted images similar to T1- and T2-weighted images of spin echo (SE). Two GE sequences, fast low angle shot (FLASH) and fast imaging with steady-state precession (FISP), were performed in 15 cases of liver metastasis in various combination of flip angle (FA), repetition time (TR), and echo time (TE). The optimal combinations were summarized as follows: 1) T1-weighted FLASH image with FA of 40 degrees, TR of 22 msec and TE of 10 msec, 2) T1-weighted FISP image with FA of 70 degrees, TR of 100 msec, TE of 10 msec, 3) both T2*-weighted FLASH and FISP images with FA of 10 degrees, TR of 100 msec and TE of 30 msec. Not only to provide the adequate T1- and T2*-weighted images but also to enable breath-holding MR imaging, GE sequences can optionally take place SE in cases of deteriorated images caused by moving artifacts. Other applications support the re-examination and further detailing when required, conveniently rather in short time.

Screening of gene deletions by polymerase chain reaction in Japanese patients with Duchenne muscular dystrophy.

Gene deletions were screened in 49 Japanese Duchenne muscular dystrophy patients from 43 families, using the polymerase chain reaction. Enzymatic amplification was carried out on six regions prone to deletion. Fifteen of 43 families (33%) had gene deletions in at least one of the six regions. This frequency was almost the same as that previously reported in Caucasians. The mid-part of the dystrophin gene was the location most frequently deleted. The frequency of deletion of the region encompassing exon 45 was higher in Japanese families (18.4%) than in Caucasians.

Protective effect of vitamin E on linoleic acid hydroperoxide-induced injury to human endothelial cells.

The protective effects of vitamin E (alpha-tocopherol) and its model compounds, which act as antioxidants, on linoleic acid hydroperoxide-induced injury to human umbilical vein endothelial cells were examined. When incubated at 50 microM with endothelial cells at 37 degrees C for 24 hr, alpha-tocopherol protected the cells from injury, and 2,2,5,7,8-pentamethylchroman-6-ol showed a similar protective effect. Trolox C, a water-soluble vitamin E model compound, had no protective effect. Tocol, a poor antioxidant, proved toxic. During preincubation, alpha-tocopherol was incorporated into the cells at 16.6 nmol/mg protein, while the pentamethylchromanol was incorporated at 0.5 nmol/mg protein; Trolox C was not incorporated at all. The results suggest that agents having both high antioxidant activity and lipophilicity can protect endothelial cells from linoleic acid hydroperoxide-induced injury.

Vitamin E and the susceptibility of erythrocytes and reconstituted liposomes to oxidative stress in aged diabetics.

A remarkable increase in the permeability of erythrocyte ghosts and liposomal membranes composed of erythrocyte lipids from aged diabetics was revealed by measuring [14C]glucose leakage. There were no significant differences in the contents of free cholesterol or phospholipids, or in the cholesterol/phospholipid ratio between diabetic and normal erythrocyte membranes, but significantly higher amounts of unsaturated fatty acids, arachidonic acid and docosahexaenoic acid were observed in the erythrocyte membranes of diabetics. Reconstituted liposomes prepared from aged diabetic erythrocyte lipids were highly susceptible to superoxide-induced oxidative stress. Vitamin E was highly effective in suppressing the peroxidative lysis of liposomes composed of diabetic erythrocyte lipids. The effect of superoxide dismutase (SOD) on the inhibition of peroxidation of unsaturated lipids within liposomal membranes was less than that of vitamin E.

Mathematical modeling for the aging process: normal, abnormal and self-terminating phenomena in spatio-temporal organization.

An elucidation of the aging process is attempted using a simple one-dimensional multicellular system, a prototype of living organisms. This model analysis has the advantage of making us investigate the two types of modes of their dynamical behavior: (i) the local modes of behavior of individual cells and (ii) the global modes of behavior of the total system. At first, each cell is assumed to have biochemically excitable kinetics for local modes, and then what kind of the global modes results with change in intercellular interaction is examined. With a simple interaction as possibly occurs in the early stage of differentiation, the model displays well-coordinated spatio-temporal patterns. This may be interpreted as a normal state. With a more complex interaction as possibly occurs in the late stage of growth, however, the model produces much more erratic patterns. This may refer to an abnormal state. Interestingly, these abnormal patterns can be transformed into normal patterns, when the activity of some parts of this model is turned 'off'; the system can survive at the sacrifice of its parts. This makes us imagine that programmed cell death plays an important role in development during morphogenesis. When individual cells become less sensitive to intercellular signals and still possess intrinsic excitability, then the normal patterns are developed for a short while before being replaced by rather irregular patterns. As time proceeds, however, all activity of them disappears. We call this a 'self-terminating' phenomenon, which may refer to aging. This strongly suggests that the loss of the total system function, leading to death, results from a global mode of system failure but not from a local mode of subsystem failure.
