Adult T-cell leukemia initially manifesting as facial diplegia.

We report an unusual case of adult T-cell leukemia (ATL) initially manifesting as facial palsy due to ATL meningitis, which was caused by lymphoid cells in the cerebrospinal fluid (CSF) with a different phenotype from those in the blood. A DNA southern blot analysis of the blood cells confirmed the diagnosis of ATL. The aberrant cells in the CSF contained nuclei with simple indentations and responded well to the initial chemotherapy, while leukemic cells in the blood showed lobulated or convoluted nuclei and were resistant to treatment. Flow cytometry performed before treatment demonstrated that the majority of the cells in the CSF were CD3+CD4+CD8-CD25+, consistent with ATL, whereas they expressed the CD45R antigen, which the blood ATL cells did not. These differences in the subphenotype, cell morphology, and responsiveness to treatment among the ATL cells in blood and CSF suggest that a single clone or subclone with heterogeneous nature was not generated, before the final development of ATL.

Interstitial hyperthermia of experimental brain tumor using implant heating system.

New experimental system of induction hyperthermia for brain tumor using ferromagnetic implant with low Curie point has been developed. The metal implant is cylindrical needle and made of Fe-Pt alloy with low Curie point suitable for hyperthermia (50-60 degrees C). Induction coil and generator which produce maximum power of 200W and variable frequency of 100-500kHz, yielding magnetic power of 16.7Oe, have been developed. Interstitial hyperthermia was made on rat brain tumor model (T9 gliosarcoma) by this system. Significant effects of single hyperthermia (45 degrees C for 30 minutes) were observed by the extension of life span and morphological changes of the tumor.

Circulating CD4+CD8+ cells in myasthenia gravis: supplementary immunological parameter for long-term prognosis.

Twenty patients with myasthenia gravis (MG) were studied prospectively for up to 5 years after thymectomy, in order to clarify the relationships between disease severity, anti-acetylcholine receptor antibody (anti-AChR) titres, proportions of circulating CD4+CD8+ cells (CD4+CD8+ cell level) and major lymphocyte subsets. The CD4+CD8+ cell levels were closely related to the clinical change within 1 year after surgery in 8 patients who showed a preoperative elevation in the cell levels. This group of patients consisted of six thymomatous and two non-thymomatous patients; the latter were both negative for anti-AChR. The anti-AChR titres generally changed in parallel with the clinical state in 9 of the 16 patients who were followed up for more than a year after thymectomy, and the CD4+CD8+ cell levels were useful in predicting the clinical course in 6 of the above 9 patients and 3 other patients, including antibody-negative cases. The present study suggests that the CD4+CD8+ cell levels may serve as an indicator for long-term prognosis of MG.

Novel restriction fragment length polymorphism of the growth hormone gene in inbred rats.

A novel restriction fragment length polymorphism in inbred rats was detected by Southern blot analysis with rat growth hormone cDNA as a probe. Four alleles, characterized by PstI fragments of 1.2, 1.1, 0.9, and 0.7 kb, respectively, were detected in 27 strains examined. The same distribution of polymorphisms was observed on digestion of DNAs of these strains with three other enzymes, PvuII, HindIII, and BamHI. Moreover, the same differences in length of allelic restriction fragments were obtained with these restriction enzymes as with PstI. These findings suggested that the polymorphism was caused by insertion or deletion of variable DNA segments in the second intron of the growth hormone gene. Linkage analyses using backcross progeny provided no evidence for close linkage between the restriction fragment length polymorphism locus and 10 other loci examined.

Adrenal myelolipoma discovered incidentally on abdominal CT and MR imaging.

This report describes an adrenal myelolipoma occurring in a 39-year-old man first noted incidentally by computed tomography (CT). The radiographic findings of this neoplasm are briefly discussed and magnetic resonance (MR) imaging of myelolipoma is also described. Myelolipomas can be distinguished from nonfunctioning adenomas by differences in signal intensity on T2-weighted images.

Cloning of ara, a putative Arabidopsis thaliana gene homologous to the ras-related gene family.

A putative Arabidopsis thaliana gene, ara, homologous to the ras-related gene family was isolated from a genomic library by low-stringency hybridization with a v-Ha-ras probe. The putative gene has a coding frame for a 218-residue protein (Mr 24,210), which is interrupted by one intron. The amino acid residues conserved in proteins of the ras gene family, which are important for GTP binding, GTP hydrolysis and membrane attachment, are also present in the ara protein. An unusually long homopyrimidine-homopurine tract was found about 100 bp upstream from the methionine start codon. A unique sequence, 5'-AACAAAACCAAAGTGATATGC-3', was found three times in the 5'-noncoding region and once in the intron.

A new mutant strain of Gunn-LA Wistar rats with genetic deficiencies in bilirubin and androsterone uridine diphosphate-glucuronosyltransferases.

Hooded Gunn rats which have low-activity of 4-nitrophenol uridine diphosphate-glucuronosyltransferase (GT) and defect in bilirubin GT were crossed with albino LA Wistar rats with a defect in androsterone GT. From F2 and F3 progeny were selected Gunn-LA Wistar rats which have defective bilirubin and androsterone GTs and low-active 4-nitrophenol GT. These rats had either hooded or albino coat color. In order to establish a uniform genetic background, albino male Gunn-LA Wistar rats were crossed with heterozygous albino female Gunn-LA Wistar rats in terms of bilirubin GT. The offsprings were all albino and were classified to Gunn-LA Wistar rats and heterozygous Gunn-LA Wistar rats by assaying their hepatic GT activities or by the appearance of jaundice. The linkage relationships between GT and coat color genes are discussed.

Separation and purification of uridine diphosphate-glucuronosyltransferases by chromatofocusing on a high-performance liquid chromatograph.

A rapid method for the separation and purification of uridine diphosphate-glucuronosyltransferases (GT) was developed with the use of chromatofocusing on a high-performance liquid chromatograph. GT isoenzymes solubilized from hepatic microsomes of Wistar rats were separated on a Mono P column, a pre-packed column for chromatofocusing. Using 4-nitrophenol, testosterone and androsterone as substrates, four fractions with different GT activities were separated in a pH gradient from 9.5 to 7.0. Two isoenzymes, testosterone GT and androsterone GT were purified to apparent homogeneity. They were eluted at pH 8.9 and 8.0 and had subunit molecular weight values of 50000 and 52000, respectively. Approximately 10 mg of solubilized microsomal proteins was applied and the elution was completed within 2 h. Addition of N-nitrodiethylamine, an in vitro activator of GT activity, enhanced the GT activity toward 4-nitrophenol in the three fractions. This chromatographic analysis confirmed the absence of androsterone GT isoenzyme in LA Wistar rats, a mutant strain in terms of androsterone glucuronidation.

Spatial distribution of exercise-induced ST-segment depression and U-wave inversion in identifying the ischemic site in patients with coronary artery disease.

Eighty-seven unipolar electrocardiograms were simultaneously recorded before and after symptom-limited treadmill exercise in 75 patients with coronary artery narrowing (greater than equal to 70%) and without previous myocardial infarction. Body surface distributions of ST segment depression were divided into 3 types; upper, lower, and diffuse types. Body surface distributions of U-wave inversion were divided into 2 types; upper, and lower types. These distribution patterns were compared with the location of ischemia determined by T1-201 exercise myocardial perfusion imaging. For ST-segment depression, a considerable number of patients had diffuse-type ST depression, whether the site of ischemia was anterior (22/32, 69%), inferior (18/27, 67%) or both (5/5, 100%). However, upper-type ST depression was associated with anterior ischemia, and lower-type ST depression, with inferior ischemia. The sensitivity and specificity of the spatial distribution of ST depression in identifying the myocardial ischemic site were 27% and 95% for anterior ischemia. The sensitivity and specificity of the spatial distribution of St depression in identifying the myocardial ischemic site were 27% and 95% for anterior ischemia respectively, and 28% and 88% for inferior ischemia, respectively. The incidence of U-wave inversion was moderate (29/75, 39%), but the distribution pattern was specific for the site of ischemia; upper-type U inversion associated with anterior ischemia, and lower type with inferior ischemia. The sensitivity and specificity were 59% and 100% for anterior ischemia respectively, and 22% and 100% for inferior ischemia respectively. By a combination of ST-depression and U-inversion, the sensitivity and specificity were 78% and 95% for anterior ischemia, and 44% and 88% for inferior ischemia. Body surface electrocardiographic mapping provided important information in the non-invasive diagnosis of the site of myocardial ischemia.

Restriction fragment length polymorphism of cardiac myosin heavy chain gene in rats and its strain distribution.

The distribution of an RFLP in EcoRI fragments of the cardiac myosin heavy chain gene among 29 strains of laboratory rats was examined. Southern blot hybridization of rat genomic DNAs with rat cardiac myosin heavy chain cDNA as a probe demonstrated an interstrain variation in one of eight EcoRI fragments. Of the 28 inbred strains examined, 10 had a fragment of 10 kbp, whereas 18 had a fragment of 7.5 kbp. The 15 samples of the remaining strain (Iar: WI outbred stock) had fragments of either 7.5 kbp or 10 and 7.5 kbp, indicating that this strain has maintained heterogeneity of these fragments.

[Interstitial radiotherapy of carcinoma of the tongue-comparison between Jacobson-Yamamoto grading and prognosis].

One hundred and one patients of less than seventy years old with T1, T2 and N0 carcinoma of the tongue were graded according to the Jacobson-Yamamoto's grading (1-4 D). All patients were also classified into two groups such as less than fifty years old group and fifty to seventy years old group. Patients with grade 3, 4 C and 4 D carcinoma had more metastases to the cervical lymph nodes than grade 1, 2. Patients with T2N0 carcinoma showed approximately same five year cervical lymph node metastases free survival rate. However, the five year survival rate of less than fifty years old group was significantly lower than that of fifty to seventy years old group. In T2N0, grade 3-4 D carcinoma cases, more dead cases were found in less than fifty years old group than in fifty to seventy years old group.

[Study on follow-up of patients with biopsy-proven breast disease].

Eight hundred forty-one patients with biopsy-proven benign breast disease were followed annually in prospective manner for 1-10 years to analyse breast cancer development at aspect of histologic features, age at entry and mammographic patterns by Wolfe. No association was found between patients who developed cancer and those who did not with respect to the initial histologic features. The rate of breast cancer development in patients in thirties was the highest. The breast cancer development was found most often in less than three years follow-up period. The breast parenchymal patterns at entry related to cancer development. In 10 cancer patients, 8 patients showed DY pattern and 2 showed P2 patterns. The present results demonstrate that the patients with biopsy-proven benign breast disease in thirties, in less than 3 years follow up period and have parenchymal patterns of P2 or DY should be followed carefully.

[Effects of an anti-prostaglandin agent added to the irrigation solution on damage to the anterior segment in monkey eyes induced by pars plana vitrectomy].

Flurbiprofen is a water-soluble, non-steroid anti-inflammatory agent and is capable of marked inhibition of prostaglandin synthesis. In a previous study by the authors, it was determined that the optimum concentration of flurbiprofen for intraocular irrigation solution in monkey retinas is 4 to 40 micrograms/ml. The present study was conducted to determine the optimum concentration of flurbiprofen for the irrigation solution, as well as to evaluate its toxic effects on the anterior segment tissues. In a pilot experiment, flurbiprofen was injected directly into the anterior chamber of pigmented Japanese rabbits to investigate its overall toxicity and to quantify the concentration at which it could become toxic to the cornea. This experiment showed that aqueous concentrations of flurbiprofen greater than 1,000 micrograms/ml were toxic. In a subsequent study, the twelve eyes of six Japanese monkeys (Macaca fuscata) underwent vitrectomy using balanced salt solution (BSS) containing concentrations of flurbiprofen from 4 to 800 micrograms/ml and BSS alone. This experiment showed that BSS containing 4 to 100 micrograms/ml of flurbiprofen prevented post-operative occurrence of edema in non-pigmented epithelial cells in the ciliary body. Furthermore, it was observed that infiltration of inflammatory cells into the trabecular meshwork and swollen mitochondria in the corneal endothelial cells were likewise prevented. This is thought to be due to the pharmacological action of flurbiprofen; which has an inhibitory effect on leucocyte migration and prostaglandin synthesis as well as stabilization of the cell membrane. However, these effects were not observed in the eyes vitrectomized with flurbiprofen concentrations between 400 and 800 micrograms/ml.(ABSTRACT TRUNCATED AT 250 WORDS)

[Macular diseases in the elderly person].

Macular diseases in the elderly, such as age-related macular degeneration, idiopathic senile macular hole and epiretinal membrane of the macular area were studied. In 75 normal subjects aged from 20 to 78 years, retinal sensitivity in the central 10 degree visual field were examined using automated static quantitative perimetry. For background luminance of 31.5 asb, a significant reciprocal correlation was demonstrated between individual mean sensitivity and age. The influence of age on the decrease in sensitivity was proved to differ according to different test locations. To enhance contrast, an image processing procedure was applied for fluorescein angiographs of age-related pathologies which resulted in better recognition of age-related RPE pathologies were recognized. The senile disciform macular degeneration (SDMD) study group sponsored by the Ministry of Health and Welfare performed an epidemiological survey to estimate the number of patients with SDMD. The epidemiological estimation was 6,000 to 13,000 patients in the entire Japanese population. 133 eyes of uni- or bi-lateral senile macular degeneration without choroidal neovascularization and 156 opposite eyes of patients with unilateral SDMD were followed-up for choroidal neovascularization development. Choroidal neovascularization development was confirmed in 15 eyes, 5.2%. In 13 of the 15 eyes, choroidal neovascularization was proved to develop through serous RPE detachment. Also, serous drusen were shown to be to predisposed to choroidal neovascularization through serous RPE detachment. Therefore, it was concluded that senile macular degeneration should be classified into the atrophic form, predisciform or intermediate form and disciform form. In the author's previous paper, it was reported that the navel-like lesion would be a macular lesion predisposing to a senile macular hole. 49 opposite eye of patients with one eye affected by a macular hole were follow-up for macular hole development. At the initial examination, the navel-like lesion was observed in 5 of the 49 eyes. During the course of observation, navel-like lesions developed in one of the other 27 eyes with other abnormalities and in 4 of the 17 eyes without any abnormality. Finally, macular holes developed in 11 of the 49 eyes; in 10 eyes with a navel-like lesion and one eye with another abnormality. We found a 17 year old female Japanese monkey with pre-macular holes in both eyes and clinicopathological correlative study was carried out. In her right eye, photoreceptor cell loss at the foveola, circumferential retinal detachment around the area of cell loss, cystoid spaces in the detached retina, and very thin residual tissue covering the foveolar lesion were observed.(ABSTRACT TRUNCATED AT 400 WORDS)

[Complications of IABP in the postoperative management of open-heart surgery--a review of 108 cases in the last 10 years].

During the past decade from May 1978 to July 1988, intraaortic balloon pumping (IABP) was used in a total of 108 patients following open-heart surgery at our department. Eleven complications depend on IABP have occurred in nine of theses patients, i.e. five of circulatory disorder on account of ischemia of the lower extremity ipsilateral to balloon catheter insertion, three of ischemia of the abdominal viscera, one of aorto-esophageal fistula, one of gas embolism resulting of balloon rupture and one of damage of the abdominal aorta caused by a Fogarty balloon catheter. Five of these patients died of these complications depend on IABP. Especially, four of five patients died of vascular accidents. These complications were due to operation of the balloon catheter or the driving unit, bat that was no complication related to insert of the balloon catheter. Then, we have to paid more careful attention to driving the intra-aortic balloon.

[A case report of ultrasonic decalcification for calcified bicuspid aortic valve stenosis].

Calcified stenotic aortic valve are usually replaced by the artificial valve. Recently, we are trying to decalcify the calcification of the aortic valve by ultrasonic energy. This case report reviewed a small physique, 47 year old female, who had the calcified stenotic bicuspid aortic valve and the narrow aortic valve ring (18 mm diameter). The ultrasonic decalcification was performed by the ultrasonic surgical instrument, SUS-201D (ALOKA), ultrasonic energy output range from 25% to 45%. After the ultrasonic decalcification, the movability of the aortic cusps were improved and the aortic opening was enlarged. The systolic pressure gradient across the aortic valve was improved from 100 mmHg to 20 mmHg. This method "ultrasonic decalcification" is very useful technique for the repair of the calcified aortic valve stenosis.
