Menstrual cycle effects on a VDT-based simulation task: cognitive indices and subjective ratings.

It is unclear whether the menstrual cycle affects the basics of performance, mainly task performance and cognitive functions. We examined menstrual cycle effects on VDT-based task performance, cognitive indices based on the signal-detection theory, and subjective ratings in questionnaires. We experimented in three phases: premenstrual, menstrual and postmenstrual phases. Previous studies reported premenstrual tension syndrome (PMST) in the premenstrual phase. Women used physiological and psychological strategies to cope during the menstrual phase. However, task performance, cognitive indices and subjective ratings showed no menstrual cycle effects. Thus, we can conclude that the information processing for the VDT-based task was not affected by menstrual cycle.

Human T-cell leukemia virus type 1 Tax activates transcription of the human fra-1 gene through multiple cis elements responsive to transmembrane signals.

We have shown that Tax1 of human T-cell leukemia virus type 1 stimulates the expression of several cellular immediate-early genes (M. Fujii, T. Niki, T. Mori, T. Matsuda, M. Matsui, N. Nomura, and M. Seiki, Oncogene 6:1023-1029, 1991). In this study, the 5'-flanking region of the human fra-1 gene, which is a Tax1-inducible fos-related gene, was isolated and Tax1 or serum-responsive cis elements were analyzed to obtain further insight into the mechanism of Tax1 action. The 62-bp sequence starting 46 nucleotides upstream from the translation initiation site showed 71% homology with the sequence surrounding the TATA box of the c-fos promoter. Regulatory motifs identified in the c-fos promoter, such as an Ets-binding site, E boxes, a CArG box, c-fos AP-1 sites, and two retinoblastoma control elements, were also found upstream of the c-fos homology region. A 502-bp fragment containing these motifs mediated transcriptional activation by Tax1 or by serum in a transient transfection assay. Three independent Tax1-responsive regions (TRRs) were identified, and mutations in each revealed that one of the retinoblastoma control elements in TRR1 and the c-fos AP-1 sites in TRR2 and TRR3 were essential for the activation. Although TRR2 contains a CArG box-like sequence, it was a weak binding site for p67SRF, if it bound at all, and was not required for activation. All three TRRs could also mediate the signals stimulated by serum. Thus, Tax1 appears to activate fra-1 gene expression by means of a part of the cellular machinery similar to that which mediates growth signals.

Purification and characterization of a 59-kilodalton protein that specifically binds to NF-kappa B-binding motifs of the defense protein genes of Sarcophaga peregrina (the flesh fly).

Various Sarcophaga peregrina (flesh fly) defense protein genes were shown to be activated when NIH-Sape-4 cells were cultured with bacterial lipopolysaccharides or beta-1,3-glucan. The 5' upstream regions of the defense protein genes were found to have common motifs showing similarity to the mammalian NF-kappa B-binding consensus sequence. A protein with affinity to the NF-kappa B-binding motif of the Sarcophaga lectin promoter was identified and purified to near homogeneity. This 59-kDa protein also bound to the NF-kappa B-binding motifs of other defense protein genes, e.g., sarcotoxin I and sarcotoxin II genes. This protein was found in both the cytoplasmic and the nuclear fractions of the cells, and it appeared to migrate from the cytoplasm to the nucleus on treatment of the cells with lipopolysaccharides. This 59-kDa protein is probably a transcriptional regulator of the genes for defense proteins of S. peregrina.

Lethal posttransfusion graft-versus-host disease. Report of four cases.

Four cases of posttransfusion graft-versus-host disease are reported. Three were caused by high-dose transfusion of non-irradiated fresh blood during open-heart surgery and 1 by the contaminated lymphocytes in packed red blood cells transfused for renal injury. In 1 patient, change of HLA type of peripheral lymphocytes was demonstrated.

Alteration of pulmonary blood flow distribution in patients with congenital left to right cardiac shunt--pre- and postoperative study with macroaggregates of 99mTc-labeled human serum albumin.

The pulmonary blood flow in 84 patients with left to right cardiac shunts was preoperatively and postoperatively examined by scintigram with macroaggregates of 99mTc-labeled human serum albumin. Lung biopsy was also performed in 28 patients. The regional pulmonary blood flow distribution curves obtained from perpendicular scintillation to the long axis of the right lung in the supine position were classified into 4 patterns (normal and types 1 to 3). The normal pattern displayed uniform distribution, however type 1 demonstrated a lack of upper corner in the curve. Type 2 showed 2 peaks (a lower peak in the upper and a higher peak in the lower part of the lung) with a dip between them. Finally only 1 peak in the lower part of the lung was manifest in type 3. The right Upper/Lower lung count ratio, R(U/L), expressed as the mean +/- standard deviation in each curve type was 0.82 +/- 0.12 (n = 23), 0.71 +/- 0.11 (n = 28), 0.6 +/- 0.08 (n = 17) and 0.58 +/- 0.18 (n = 16), respectively. The count ratios from the 3 abnormal types were statistically significant compared to the normal curve type (p < 0.01). In addition, the type 1 and 2 curves were significant compared to type 3 (p < 0.05-0.01). In patients under 3 years of age, lower R (U/L) count ratio expressed more severe pulmonary hypertension. Negative correlations were observed with the peak pulmonary/peak systemic pressure ratio, Pp/Ps, (r = -0.66, p < 0.01) and with the pulmonary vascular resistance index, PVRI, (r = -0.45, p < 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

Dome osteotomy of the pelvis for osteoarthritis secondary to hip dysplasia. An over five-year follow-up study.

We performed Kawamura's dome osteotomy of the pelvis, with simultaneous distal transfer of the greater trochanter on 101 hips in 91 patients with osteoarthritis secondary to hip dysplasia. The mean age at operation was 30 years (15 to 55), and follow-up was for a mean of 8.3 years (5 to 14). Clinical evaluation using the Merle d'Aubigné score showed 92% excellent or good results. Radiologically, 91 hips had good acetabular remodelling and showed no signs of progression of osteoarthritis. In ten hips the osteoarthritic process progressed despite the osteotomy and six of these eventually underwent total hip replacement. Factors which were significantly associated with a poor outcome included an advanced stage of osteoarthritis, valgus deformity of the proximal femur, old age at the time of operation and postoperative persistence of abductor insufficiency.

New retinoids inhibiting the growth of human acute promyelocytic leukemia cell line HL-60 without inducing differentiation.

A series of acid homologs with longer side chain length was synthesized and their biological activities to inhibit growth and to induce differentiation of human acute promyelocytic leukemia cell line HL-60 were analyzed. It was found that ethyl alpha-retinylidene propionate (E) shows interesting activity. This compound inhibited growth of HL-60 cells with almost the same potency of retinoic acid. However, unlike retinoic acid, the potency of this compound to induce differentiation of HL-60 cells into neutrophiles was almost negligible. Ethyl alpha-retinylidene propionate (E) is an unprecedented retinoid which inhibits growth of HL-60 cells without inducing cell differentiation. The Z isomer [ethyl alpha-retinylidene propionate (Z)] also showed similar unique effects on HL-60 cells. Interestingly, both of the E and Z isomers of alpha-retinylidene propionic acid showed only very weak activities to inhibit growth and to induce differentiation of HL-60 cells. It was suggested that ethyl alpha-retinylidene propionate acts on HL-60 cells in the intact form without suffering hydrolysis, and also that the ester group plays important role for the unique activity. Biological activities of retinylidene acetic acid (E and Z) which lacks methyl group at alpha-position of alpha-retinylidene propionic acid, on HL-60 cells were weaker than those of the isomers of alpha-retinylidene propionic acid. Esterification of the isomers of retinylidene acetic acid resulted in further decrease of biological activities. Biological activities of ethyl retinoate on HL-60 cells were also very weak. It was indicated that methyl group present at alpha-position of ethyl alpha-retinylidene propionate plays an important role for the unique biological activity. It was further indicated that the effect of esterification of carboxyl group of retinoids on the biological activity is not constant, but varies depending on the structure. Pretreatment of HL-60 cells with ethyl alpha-retinylidene propionate (E) apparently did not influence the process of induction of differentiation of HL-60 cells into neutrophiles by retinoic acid. It was further shown that ethyl alpha-retinylidene propionate (E) prolongs the time necessary for induction of differentiation of HL-60 cells by retinoic acid, but does not influence the level of final rate of differentiation.

Human monoclonal antibodies from a patient with primary biliary cirrhosis that recognize two distinct autoepitopes in the E2 component of the pyruvate dehydrogenase complex.

Peripheral B lymphocytes from a patient with primary biliary cirrhosis were infected with Epstein-Barr virus, and Epstein-Barr virus-transformed B lymphocytes producing large amounts of IgG antibodies to pyruvate dehydrogenase complex were selected, expanded and fused with the human-mouse heteromyeloma cell line F3B6. The resulting Epstein-Barr virus-transformed B-cell hybrids were repeatedly cloned by limiting dilution, and three stable hybridoma clones producing human monoclonal antibodies to pyruvate dehydrogenase complex were generated. These monoclonal antibodies, designated M18GP8, M37GP11 and M82GP8, specifically bound to pyruvate dehydrogenase complex, and their dissociation constant with pyruvate dehydrogenase complex was calculated to be 2.4 x 10(-11), 2.3 x 10(-10) and 2.6 x 10(-11) mol/L, respectively. These three monoclonal antibodies stained the mouse stomach/kidney cryostat sections in a typical immunofluorescence pattern of antimitochondrial antibody. Furthermore, the enzymatic activity of pyruvate dehydrogenase complex was almost completely inhibited by the three monoclonal antibodies. Western blotting analysis revealed that M18GP8 and M82GP8 reacted with only pyruvate dehydrogenase complex-E2 in contrast to M37GP11, which reacted with both pyruvate dehydrogenase complex-E2 and protein X. The binding of monoclonal antibody M37GP11 to solid-phase pyruvate dehydrogenase complex was partially inhibited by two different synthetic peptides corresponding to both the inner and outer lipoyl-binding domains of pyruvate dehydrogenase complex-E2. These monoclonal antibodies, which are the first human monoclonal antibodies to pyruvate dehydrogenase complex generated from a patient with primary biliary cirrhosis, will be a valuable tool for studying the B-cell autoepitopes in PDC and the mechanism of autoantibody production in primary biliary cirrhosis.

Indocyanine green video-angiographic findings in Harada's disease.

Indocyanine green (ICG) video-angiography was performed in 8 cases of Harada's disease and the findings were compared with those of fluorescein angiography. ICG video-angiography was confirmed to be far superior to fluorescein angiography in evaluating the choroidal vasculature seen in Harada's disease. Before retinal detachment appeared, the early phase of ICG video-angiography revealed decreased numbers of large fluorescent choroidal vessels as well as patchy areas of hypofluorescence. When retinal detachment became prominent, the large fluorescent choroidal vessels seen on ICG video-angiography were significantly fewer in number and patchy areas of hypofluorescence could be seen in the early phase. In the late phase, pooling of dye leakage became prominent. When retinal detachment subsided, the number of fluorescent choroidal vessels had increased. These results suggest that the decreased number of large fluorescent choroidal vessels seen in the early phase of ICG video-angiography indicate circulatory disturbance of the choroidal vessels, the degree of which may correspond to the exudative retinal detachment characteristic of Harada's disease. Patchy areas of hypofluorescence indicated blockage due to multiple choroidal granulomas and Dalen-Fuchs nodules.

Subclassification of preproliferative diabetic retinopathy.

One hundred and fifty-five eyes (106 cases) affected by preproliferative diabetic retinopathy were divided into three subgroups according to the severity of retinopathy and were followed for one year. The criteria for and numbers of eyes in each group were as follows: 1) mild type with soft exudates and without apparent nonperfused areas on fluorescein angiography (39 eyes), 2) moderate type with soft exudates and demonstrable nonperfused areas (103 eyes), 3) severe type with soft exudates, nonperfused areas and venous beading (13 eyes). The proportion undergoing photocoagulation was 23% in the mild type, 81% in the moderate type and 100% in the severe type. None of the mild type eyes underwent photocoagulation unless they advanced to a more severe type. Nine of the mild type eyes which progressed to moderate type during the follow-up period underwent focal photocoagulation. Panretinal photocoagulation was performed in 29% of moderate type eyes and 77% of severe type eyes. The courses of these three subgroups were analyzed after one year of follow-up. The proportion developing proliferative retinopathy was 0% in the mild type, 18% in the moderate type and 46% in the severe type. Based on the above results, it was concluded that the subclassification we proposed in this paper can be applied to the management of patients with preproliferative diabetic retinopathy.

Clinical study of venous abnormalities in diabetic retinopathy.

The authors studied 304 eyes (248 cases) affected by diabetic retinopathy in relation to venous abnormalities. The posterior pole and surrounding mid-peripheral area were examined by ophthalmoscopy, slit-lamp examination with contact lens and fluorescein angiography. There were 225 eyes with preproliferative and 79 eyes with proliferative retinopathy. Fifty-six eyes (18%) of the total eyes had venous abnormalities: 17 (8%) of the 225 eyes with preproliferative retinopathy and 39 (49%) of the 79 eyes with proliferative retinopathy. In the 56 eyes with venous abnormalities, 54 (96%) showed beading, 14 (25%) showed looping and 2 (4%) showed duplication. All eyes which showed looping or duplication had proliferative retinopathy. The locations of the venous abnormalities were analyzed: 42% were in secondary branches, 27% in tertiary branches, 18% in primary branches, 10% in fourth branches and 3% in fifth branches. The portion of the fundus affected was upper temporal, lower temporal, upper nasal and lower nasal in 28%, 22%, 29% and 21%, respectively. Based on our results, eyes with looping or duplication on ophthalmoscopic or slit-lamp examination have an extremely high probability of being in some stage of proliferative retinopathy. Retinopathy should be confirmed by fluorescein angiography and photocoagulation should be performed as quickly as possible.

A multicenter study of typical retinitis pigmentosa in Japan.

A nationwide, multicenter study of typical retinitis pigmentosa was carried out in collaboration with 13 university hospitals throughout Japan. A total of 253 patients, 122 males and 131 females, with a wide range of ages (mean 48 years), were registered during a two-month period in 1989. Determination of inheritance pattern revealed 30.2% autosomal recessive cases, 15.4% autosomal dominant, 0.5% X-linked, and 48.9% simplex, indicating a relative decrease in autosomal recessive cases and a relative increase in simplex cases in recent decades. The age at onset, initial symptom, and visual functions including visual acuity, visual field and electroretinogram showed a marked interindividual variability, but statistical analysis demonstrated that visual defects progressed with increasing age and disease duration. A correlation between the phenotypic variation and the genetic type was observed. This survey of retinitis pigmentosa in Japan provides information for counseling and rehabilitation of patients and encourages basic and clinical research of this genetic disease.

[The relationship between P300 latencies, and WISC-R and Wechsler memory scale results in epileptic children].

Auditory event-related potentials (ERPs) were recorded in 35 epileptic children of 9 to 13 years of age, and compared with conventional test batteries of cognitive functions. ERPs were elicited with the auditory oddball paradigm. Corrected P300 latencies showed an inverse correlation with the total score (IQ) of WISC-R and that of the Wechsler memory scale (WMS). A strong inverse correlation was also observed with specific items of these test batteries, as follows; "comprehension", "picture completion" and "coding" in WISC-R, and "associated learning", "digit span", "visual reproduction", "mental control", "orientation" and "information" in WMS. These findings suggested that the prolongation of P300 latencies in epileptic children are related with the impairment of cognitive functions. The P300 latency is correlated with a characteristic kind of cognitive function; "simple recognition", "short term memory" and "easy judgement".

[The utility of endoscopic ultrasonography (EUS) in endoscopic mucosal resection of early gastric cancer].

The utility of EUS was evaluated in 27 patients with early gastric cancer undergoing endoscopic mucosal resection over the past 2 years. Accuracy of the assessment of depth of cancerous invasion was studied in 16 patients undergoing EUS before endoscopic mucosal resection. Patients showing no changes in the submucosal (sm) layer or below on EUS included 15 with mucosal (m) cancer and one with sm cancer showing very slight infiltration. Seven patients with m cancer, a negative stump, and no ulcer in the cancer focus at endoscopic mucosal resection, were followed up for more than 1 year after endoscopic mucosal resection. On EUS, four patients showed Ul-IIs changes resembling benign ulcers, two showed Ul-IIIs changes and two showed no changes in the sm layer or below. All patients were negative for cancer in follow up biopsies. No lymphadenopathy was observed. EUS was effective in diagnosing the depth of cancerous invasion in patients undergoing endoscopic mucosal resection and also in clarifying changes in the sm and deeper layers during follow up.

[A case of interstitial pneumonia of polymyositis-dermatomyositis with various pathological findings in open lung biopsy].

A 60-year-old woman was admitted to our hospital with a two month history of dry cough and dyspnea on exertion. A chest roentgenogram revealed diffuse interstitial shadows with a reduction of lower lung volume. Laboratory examinations revealed an increase in CPK and aldolase. There was decreased proximal muscle power, and the findings of a biopsy of the right deltoid were compatible with polymyositis. Myositic symptoms were stable, but the respiratory symptoms worsened, and an open lung biopsy was performed for diagnosis and to determine the best treatment. The histological findings of biopsy materials demonstrated active interstitial pneumonia complicated by cellular interstitial pneumonia, bronchiolitis obliterans organizing pneumonia, usual interstitial pneumonia and lymphoid hyperplasia. The patient responded well to adrenocorticosteroid and immunosuppressive therapy, and is now attending as an out patient. It is well known that PM-DM can be associated with interstitial pneumonia, and this complication is an important prognostic factor clinically. The pathological patterns of interstitial pneumonia in PM-DM may be divided into usual interstitial pneumonia and bronchiolitis obliterans organizing pneumonia. Furthermore, it is well documented that these patterns are concurrent with the response to adrenocorticosteroid and prognostic factors. However, our case of PM-DM, in which various patterns such as rheumatoid arthritis (RA) were pathologically revealed, cannot be considered as having uniform pathological pattern. We consider that pulmonary pathological patterns of PM-DM are very varied, as with RA. It is a very important to evaluate the nature of these patterns and the subsequent clinical course in PM-DM with interstitial pneumonia.

[Examination of cholangiogram of obstructive jaundice using MRI--compared with PTC and ERCP].

PTC and ERCP are most often used in diagnosis of obstructive jaundice. We studied the possibility of clinical diagnosis using MRI in 33 cases of obstructive jaundice. A clinical diagnosis of malignant tumors could be given in 17 cases out of 20 (85%) using MRI if respiratory standstill was possible. An MRI cholangiogram was particularly effective in describing tearful parting bile ducts and was clearer than PTC in describing negative gallbladders. Choledochal stones could be diagnosed in 58% of cases, which was less than the rate for malignant tumors. MRI is not an invasive examination, can be used in diagnosis of obstructive jaundice, and helps in selecting treatment methods such as PTCD and ERBD.
