Continuous brachial plexus block for replantation in the upper extremity.

For replantation surgery, continuous brachial plexus block was employed using supraclavicular or axillary approach. Of a total of fifty blocks, forty-seven (94%) gave adequate surgical analgesia throughout the operations. No serious complication appeared in this series. Pneumothorax was detected in one supraclavicular block, but no surgical drainage was required. One patient had paraesthesia in the blocked arm, but it subsided within a month. No toxic reaction was recognised in spite of high dose of anaesthetic agents. Thus continuous brachial plexus block has been ascertained to be safe, reliable, and useful for the replantation and other prolonged operations in the upper extremity.

4-Aminobutyraldehyde dehydrogenase activity in rat brain.

An enzyme with NAD+-dependent 4-aminobutyraldehyde dehydrogenase activity was purified about 360-fold from rat brain extract. AMP-Sepharose chromatography was effective in separating the enzyme from other NAD+-dependent aldehyde dehydrogenases included in the extract. The KmS for the substrates NAD+ and 4-aminobutyraldehyde were 4.8 x 10(-4) and 8.3 x 10(-5) M, respectively. The pH optimum for the enzyme was about 8.0. The ratio of activities toward 4-aminobutyraldehyde, propionaldehyde, succinate semialdehyde, and benzaldehyde was 1.00:0.17:0.24:0.09:0.03 when the activity toward 4-aminobutyraldehyde was set equal to 1.00. The enzyme activity in subcellular fractions of rat brain was localized in cytosol.

Indications, applications and future prospects of diagnostic examinations in clinical cardiology.

Nowadays there are various kinds of diagnostic examinations in the field of clinical cardiology. In this field, information concerning structure, dimension (hypertrophy and dilatation) and cardiac function is essentially important. For the diagnosis of valvular and congenital heart diseases and of cardiomyopathy, ultrasonic examinations are more useful; for ischemic heart disease, nuclear medicine is superior. Angiocardiography provides as much information but it is invasive. A combination of an exercise-test with the examinations mentioned above is useful for the detection of left ventricular and coronary reserves. The determinants of left ventricular reserve, being related to the prognosis of the patients, can be analyzed by exercise echocardiography. Exercise echocardiography can reveal instantaneous changes in myocardial ischemia during exercise. Regional and global left ventricular performances revealed by exercise radionuclide angiocardiography can lead us to a new interpretation of an exercise ECG.

Effect of pyridoxal phosphate deficiency on aromatic L-amino acid decarboxylase activity with L-DOPA and L-5-hydroxytryptophan as substrates in rats.

This paper describes the distribution of aromatic L-amino acid decarboxylase (AADC) activities in fourteen tissues (eight peripheral tissues and six brain regions) of semicarbazide (SC)-treated rats, using both L-DOPA and L-5-hydroxytryptophan (L-5-HTP) as substrates. The distribution of pyridoxal phosphate (PLP) was also measured in control and SC-treated rats. SC-treatment decreased the PLP concentration in all tissues (about 50-60% of control). AADC activities towards L-DOPA and L-5-HTP as substrates were also decreased significantly in almost all tissues of SC-treated rats. After the addition of exogenous PLP in vitro, AADC activities were recovered only partially in most tissues, but the recovery patterns were not parallel between L-DOPA and L-5-HTP as substrates. L-DOPA decarboxylase activity was more sensitive to PLP-deficiency than L-5-HTP decarboxylase activity in the same tissues. Serum AADC activities were decreased drastically using both L-DOPA and L-5-HTP as substrates. No serum AADC activity was detected in SC-treated rats using L-DOPA as substrate, but low activity was detected in the same sample using L-5-HTP as the substrate; both activities recovered completely after in vitro addition of 10 microM PLP in the incubation mixtures.

Primary chylopericardium.

In a 23-year-old man with massive chylous pericardial effusion, oral administration of Sudan III and radio-active 131iodine-labelled triolein revealed an abnormal communication between the lymphatic system and the pericardial sac. Despite partial pericardiectomy and resection of the thoracic duct, the fluid reaccumulated, and a postoperative lymphangiogram showed an obstructed left thoracic duct. Resection of this left thoracic duct resulted in full recovery. This is the first reported case of this disease with a duplicate thoracic duct. Careful attention must be paid to the possibility of this variation before and during surgery.

Influence of ergometer exercise on aortic pressure-diameter relationship in healthy men.

In order to investigate the change of the aortic pressure-diameter relationship during exercise, M-mode echograms and/or two-dimensional echograms of the aortic root and of the abdominal aorta were taken before and during supine ergometer exercise in 15 healthy men. The difference of the angle of the ultrasonic beam to the aorta between before and during exercise was too small to influence the measured values. The exercise, which elevated heart rate to 100 beats/min, caused a shift of the pressure-diameter relationship curve of the aortic root to a lower diameter (on an average, end-systolic diameter: from 3.1 to 2.9 cm, p less than 0.001; end-diastolic: from 2.9 to 2.7 cm, p less than 0.001). This was also seen in the abdominal aorta. Stroke volume showed no change during exercise. Total peripheral vascular resistance decreased. Central aortic volume decreased. In conclusion, exercise induces active constriction of aorta with dilatation of arterioles. This might be beneficial to push the blood into the peripheral tissues.

Neuromuscular blocking in acutely tetanus intoxicated mice.

The effects of tetanus toxin on neuromuscular transmission of mice in acute intoxication produced by intravenous injection of a large amount of the toxin were examined by (1) recording the phrenic nerve impulses, the electromyograms (EMGs) of the diaphragm and the electrocardiograms, and (2) the evoked EMGs of the gastrocnemius muscle in response to electrical stimulation of the sciatic nerve. The evoked EMGs of the gastrocnemius muscle were analyzed in terms of kinetic and tonic components by their different latencies. Just before death of animals, the EMGs of the diaphragm appeared with some delay relative to the corresponding phrenic discharges. Finally, the EMG of the diaphragm disappeared even in the presence of phrenic discharge, but cardiac electrical activities continued. The amplitudes of the evoked EMGs of the gastrocnemius muscle invariably became low before death, but the muscle action potential could be recorded by direct muscle stimulation for several minutes after death. The latencies of the evoked EMGs were constant until about the middle of the survival time when the latencies suddenly became prolonged. The longer latency was the same as that of the tonic action potentials. Thus, in acutely tetanus-intoxicated mice, neuromuscular transmission was blocked rapidly and the kinetic component of the muscle was blocked earlier than the tonic component.

[Cytologic studies in relation to the effect of treatment of lung cancer].

Since the diagnosis of lung cancer is made cytologically in most instances, the study of cytologic materials of lung cancer may be useful to plan the treatment schedule, to judge the effect of treatment and to find the recurrence of the lesion in central type lung cancer. Shibata et al. observed the correlation of morphologic changes of irradiated cancer cells and radiation doses, and found the close relation between them. In order to detect disappearance of cancer cell in the cytologic specimens, more than over 1000 r. was necessary for small cell carcinoma, more than 2000 r. For large cell carcinoma, more than 3000 r. For squamous cell carcinoma and more than 5000 r for adenocarcinoma, respectively. It suggests that small cell carcinoma is more radiosensitive. To predict the degree of response to radiotherapy, it is most important to decide the cell typing of obtained cancer cells. We investigated whether certain cytologic characteristics of small cell carcinoma could be correlated with response to chemotherapy. The cancer cells in the good response group more frequently showed finely granular chromatin evenly distributed through the nuclei. The cancer cells in the no/response group predominantly showed deeply stained nuclei with coarsely granular chromatin distributed evenly or pale nuclei with unevenly distributed chromatin. These findings may be an indicator to predict the degree of response to chemotherapy. Cytologic grading of irradiatic changes reported by Shibata et al. may be useful for judging the effect of treatment of lung cancer. We introduce three cases of lung cancer in this paper and reported the correlation of morphologic changes of cancer cells and the effect of treatment of those cases.

[Evaluation of left ventricular asynergy by parasternal and subcostal M-mode echocardiography].

In 47 patients with old myocardial infarction (MI), parasternal and subcostal M-mode echocardiograms (M-mode) guided by the two-dimensional echocardiogram (2D) were recorded to evaluate left ventricular asynergy quantitatively, and were compared with 2D findings. By placing the transducer at the left sternal border, the short-axis views of the left ventricle (LV) by 2D at the level of the chorda tendineae and papillary muscle were recorded. The LV wall was divided into 4 segments; including (1) anterior wall (AW) and anterior septum (AS), (2) lateral wall (LW), (3) posterior wall (PW), and (0) inferior wall (IW) and posterior septum (PS), and asynergy was analyzed on moving images. The AS and PW were recorded by parasternal M-mode, and the PS and LW were recorded by subcostal approach. Asynergy by M-mode was defined when septal amplitude was less than 3 mm, LW or PW amplitude was less than 9 mm, % systolic thickening (% ST) of the septum was less than 17%, and % ST of the LW or PW was less than 25%. Of 25 patients with anterior MI, asynergy of the AW and AS wass s present in 19, LW asynergy in 10, PW asynergy in 2, and IW and PS asynergy in 1 by 2D, meanwhile, M-mode detected asynergy of AS in 21, and LW asynergy in 15. Of 15 patients with inferior MI, asynergy of the PW and PS was present in 4 and 7, respectively by 2D, but by M-mode asynergy was present in 11 and 14, respectively. In 31 patients underwent left ventricular cineangiography, detection rate of asynergy by angiography was compared with that by echocardiography. In 124 segments by cineangiography, wall motion characteristics were correctly identified in 83% by 2D and 91% by M-mode. Of 25 patients with anterior MI, amplitude of the AS was 3 approximately -5 mm in 19, and %ST of the AS wa 0 approximately 6% in 2, but amplitude of the PS was within normal range in 24. Of 15 patients with inferior MI, amplitude of the AS was within normal range in all, and amplitude of PS was 3 approximately -8 mm in 13 and %ST of PS was 10% in 1. This study shows that combined use of parasternal and subcostal M-mode detects asynergy more sensitively than 2D alone even in its quantitative sense, and therefore, not only 2D but M-mode in essential for evaluation of LV asynergy. Asynergy of PS was present in inferior MI, and this segment was not injured in anterior MI, while AS asynergy was present in anterior MI. When analysing asynergy of the interventricular septum, it should be subdivided into two parts including AS and PS. Subcostal M-mode detected PS asynergy that was not visualized by routine cineangiography. In inferior MI, subcostal M-mode is recommended for detection of PS asynergy.
