A case of tuberculosis and adenocarcinoma co-existing in the ileocecal region in a 61-yr-old woman is described. The Japanese literature is reviewed and several points discussed regarding the epidemiological and morphological features in a colon carcinoma associated with tuberculosis. (1) Females predominate by a ratio of 17:9 in such cases. (2) The tumor has been found in the right side of the colon in 17 out of 26 cases. (3) The most characteristic histological finding is that of the cancer, which shows a well differentiated adenocarcinoma with a tendency to produce a mucin. These unique findings are consistent with those in our case. It is of interest that these unique findings may support the possibility that the cancer originated from a tuberculous lesion.

Fifty patients with renal (33 patients), renal and ureteral (4 patients) and ureteral (13 patients) stones were treated by percutaneous nephrolithotripsy (PNL) between November 15, 1984 and December 14, 1985. Complete stone removal was achieved in 39 cases (78%). Four of the 11 cases of incomplete stone removal were among the first 5 cases we treated. We have not experienced any severe complications in our 50 cases. The cause of the unsuccessful attempts, complications and other factors of PNL are briefly discussed.

4-Aminobutyraldehyde (ABAL) was shown to be converted to gamma-aminobutyric acid (GABA) in the rat retina in vivo as well as in vitro. The results suggest that ABAL can easily cross the blood-retinal barrier and be rapidly oxidized to GABA in the retina. The conversion of ABAL to GABA also occurred in retinas treated with kainic acid, which greatly reduced glutamic acid decarboxylase (GAD) activity. Therefore, it is suggested that the conversion in the retina is independent of the GAD pathway and may occur not only in GABAergic neurons but also in glia or photoreceptor cells.
The clinical value of bronchoscopic findings as a prognostic factor has been studied in 136 cases with an inoperable adenocarcinoma of the lung. The survival time in 107 cases with pathological bronchoscopic findings was significantly shorter than in 29 cases without pathological bronchoscopic findings. In the three subtypes of bronchoscopic findings, classified according to the tumor proliferation pattern, i.e. polypoid type, submucosal type and exposing type, the exposing type proved to have the worst prognosis. In addition, our study suggests that the level of infiltrated bronchus is related to the survival time. The more central the tumor infiltration, the worse the prognosis.

We have reviewed our experiences with needle aspiration cytology in five cases of malignant lymphoma of the thyroid. From the original cytologic reports, one case was correctly diagnosed as malignant lymphoma. Three were misdiagnosed as anaplastic carcinoma of the small-cell type, and two of these three were subsequently revised to malignant lymphoma at the second aspiration. Another patient was misdiagnosed as chronic thyroiditis at the first and second aspirations. Following the third aspiration, the condition was correctly diagnosed as malignant lymphoma. In the smears correctly diagnosed as malignant lymphoma, many malignant cells were distributed as isolated cells and showed monotonous features. Sometimes, cleaved cells or prominent nucleoli were visible.
Clostridium perfringens type A enterotoxin bound rapidly to HeLa and Vero cells in the absence of extracellular Ca2+ at 37 degrees C. The bound toxin rapidly (within 2 min) caused influx of Na+ and efflux of K+ and Mg2+. Changes in membrane permeability occurred in the absence or presence of extracellular Ca2+ and to the similar extents at 37 degrees C and 4 degrees C, in contrast to the subsequent bleb and balloon formation, which required both extracellular Ca2+ and incubation at 37 degrees C. Substances with molecular weights of over ca. 200 protected the cells from the morphological alterations induced by the toxin, whereas substances with molecular weights of less than ca. 200 did not. The mechanism of the primary action of the enterotoxin is discussed.

Abnormal properties of antithrombin III have been found in a 55-year-old male who has been thrombophilic over the last seven years. They are characterized by defective inhibition of thrombin and activated blood coagulation factor X, reduced affinity to heparin and partial immunological identity with the normal molecule. The antithrombin III molecule, however, preserves a single-chain structure and an apparently identical molecular weight with that of the normal molecule. It is, thus, very unlikely that the impaired functions of antithrombin III in the patient's plasma are induced by possible proteolytic modifications of the molecule by thrombin or other related activated blood coagulation factors. Since no other members of his immediate family have been found to be affected, the abnormality may be acquired rather than genetically determined, although further investigation is necessary for the elucidation of the abnormality of the molecule.
The effects of tolbutamide on the activities of fructose-6-phosphate,2-kinase and fructose-2,6-bisphosphatase were examined using rat hepatocytes. Tolbutamide stimulated fructose-6-phosphate,2-kinase activity and inhibited fructose-2,6-bisphosphatase activity, resulting in an increase of fructose-2,6-bisphosphate level. Changes in the activities of the enzyme by tolbutamide were due to variation in the Km value, but not dependent on alteration of Vmax. Glucagon inhibition of fructose-2,6-bisphosphate formation resulting from an inactivation of fructose-6-phosphate,2-kinase and an activation of fructose-2,6-bisphosphatase was released by tolbutamide. Tolbutamide stimulation of fructose-2,6-bisphosphate formation through regulation of fructose-6-phosphate,2-kinase/fructose-2,6-bisphosphatase may produce enhancement of glycolysis and inhibition of gluconeogenesis in the liver.

We examined the morphologic characteristics of the corneal endothelium in three groups of contact lens wearers: those who had used daily-wear soft contact lenses for an average of 6.3 years, long-term (greater than 20 years) users of hard contact lenses, and former users of hard contact lenses who had worn them for an average of 9.6 years but who had discontinued them for an average of 4.3 years. When compared to age-matched controls, all groups had similar endothelial cell densities but demonstrated significant increases in variation of cellular size (polymegethism) and shape (pleomorphism). We found that daily-wear soft contact lenses can cause endothelial polymegethism and pleomorphism similar to those caused by hard contact lenses and that the endothelial changes induced by hard contact lenses are more profound with greater duration of wear and are apparently not completely reversible.
High molecular weight alkaline phosphatase (HMW-ALP) was measured in the sera of 126 patients with lung cancer to determine its diagnostic value in the detection of hepatic metastasis. This isoenzyme was found in 21 of 24 patients with hepatic metastasis and in 27 of 102 patients without hepatic metastasis. When 10 U/L was used as a cut-off value, the sensitivity, specificity, and accuracy of this test were 71%, 89%, and 86%, respectively. From the standpoint of histologic type, this test was most useful in patients with small cell carcinoma. HMW-ALP was not detected in the sera of 15 controls. It is concluded that HMW-ALP is a useful marker for hepatic metastasis in patients with lung cancer.
