[Relationship between an involvement of Oddi's sphincter in a choledochopancreatic junction and clinical appearances in 120 patients with anomalous arrangement of the pancreatico-biliary ductal system].

We studied relationship between an involvement of Oddi's sphincter in a choledochopancreatic junction and clinical appearances in 120 patients with anomalous arrangement of the pancreaticobiliary ductal system. The 120 composed of 83 with a sphincteric mechanism at the junction of the common bile and pancreatic duct and 37 without the sphincteric mechanism. We found that influences of sphincteric mechanism at the junction closely related to the length of the common duct between an orifice of the papilla of Vater and a junction of the common bile and pancreatic duct. And in all 83 with a sphincteric mechanism at the junction the length of the common duct showed less than 1.5 cm, and in all 37 without a sphincteric mechanism at the junction the common duct showed more than 1.5 cm in length. There was no correlation between a distribution of the sphincter muscle and clinical appearances, association with congenital choledochal cyst, cholelithiasis and bile duct cancer in the 120 patients.

[Real time microscopic image transmission system by using Hi-vision technique].

Using a high definition television (HDTV) technique, we have designed and developed the real time microscopic image transmission system. The department of laboratory and pathology, 500 m apart, are connected by this system on a light fiber line. It consists of an electrically controlled microscope, a HDTV camera, a HDTV color monitor, and a video transmitter on the laboratory side, while on the pathology a remote microscope controller, a video receiver, and a HDTV color monitor of 22.5 (H)-39.5(W) cm in CRT size. By monitoring real time noiseless picture, pathologists can select the objective lenses, move the stage, and adjust the focus of microscope at their own office. Three-month trial revealed that this prototype system was quite useful for prompt histo-pathological diagnosis during surgical operations. So far as we know, this microscopic image transmission system with Hi-vision TV is the first one in practical use in this field.

Clear cell carcinoma of the human ovary synthesizes and secretes a transferrin with microheterogeneity of lectin affinity.

Human ovarian clear cell carcinoma cell line (transferrin (Tf)-non-producer), HAC 2, cells were adapted to grow in chemically defined synthetic medium when the cells were cultured with medium containing 10 micrograms/ml of insulin at least for 6 months. They synthesized and secreted constantly the 80 kDa protein immunologically similar to human serum Tf (15 +/- 12 ng/ml/10(7) cells/3 days). By sensitive lectin-affinity electrophoresis followed by antibody-affinity blotting technique, a concanavalin A weakly bound or unbound, lentil lectin, a strongly reactive abnormal band, which was rarely found in human serum Tf, was detectable in the Tf synthesized by HAC 2 cells (HACTf). These findings suggest that the HACTf may act as one of the autocrine growth factors and that this heterogeneity of HACTf for lectin affinity is ascribed to differences in the carbohydrate moiety of the Tf.

Purification and characterization of P47gag-crk expressed in insect cells.

The crk oncogene product, P47gag-crk, was expressed and purified using a baculovirus expression system. Approximately 2-10 mg of P47gag-crk was produced in 10(9) insect cells infected with a recombinant baculovirus. Partially purified P47gag-crk was obtained by precipitation in a low salt buffer and gel filtration. A better purification of P47gag-crk was achieved by immunoaffinity chromatography, resulting in a single band by Coomassie Blue staining. The insect cells expressing P47gag-crk showed an increase in protein-phosphotyrosine content, which is a characteristic feature of crk-transformed cells. Moreover, like P47gag-crk produced in chicken or rat cells, P47gag-crk produced in insect cells associated in vitro with a tyrosine kinase and its substrates from Crk-3Y1 cells. Peptide mapping of P47gag-crk expressed in insect, rat, and chicken cells showed that similar sites were phosphorylated in these proteins. These data suggest that P47gag-crk expressed in insect cells is functional and will be useful for the further analysis of this protein.

Purification and characterization of a rat brain aldehyde dehydrogenase able to metabolize gamma-aminobutyraldehyde to gamma-aminobutyric acid.

An enzyme which catalyses dehydrogenation of gamma-aminobutyraldehyde (ABAL) to gamma-aminobutyric acid (GABA) was purified to homogeneity from rat brain tissues by using DEAE-cellulose and affinity chromatography on 5'-AMP-Sepharose, phosphocellulose and Blue Agarose, followed by gel filtration. Such an enzyme was first purified from mammalian brain tissues, and was identified as an isoenzyme of aldehyde dehydrogenase. It has an Mr of 210,000 determined by polyacrylamide-gradient-gel electrophoresis, and appeared to be composed of subunits of Mr 50,000. The close similarity of substrate specificity toward acetaldehyde, propionaldehyde and glycolaldehyde between the enzyme and other aldehyde dehydrogenases previously reported was observed. But substrate specificity of the enzyme toward ABAL was higher than those of aldehyde dehydrogenases from human liver (E1 and E2), and was lower than those of ABAL dehydrogenases from human liver (E3), Escherichia coli and Pseudomonas species. The Mr and relative amino acid composition of the enzyme are also similar to those of E1 and E2. The existence of this enzyme in mammalian brain seems to be related to a glutamate decarboxylase-independent pathway (alternative pathway) for GABA synthesis from putrescine.

Binding of transforming protein, P47gag-crk, to a broad range of phosphotyrosine-containing proteins.

Although the oncogene product of CT10 virus, P47gag-crk, does not itself phosphorylate proteins at tyrosine residues, it elevates phosphotyrosine in transformed cells. The P47gag-crk oncoprotein contains SH2 and SH3 domains, which are conserved in several proteins involved in signal transduction, including nonreceptor tyrosine kinases. P47gag-crk bound in vitro to phosphotyrosine-containing proteins from crk-transformed cells and from cells transformed by oncogenic tyrosine kinases. The association between P47gag-crk and p60v-src, a phosphotyrosine-containing protein, was abolished by dephosphorylation of p60v-src. This suggests that the SH2 and SH3 regions function to regulate protein interactions in a phosphotyrosine-dependent manner.

Anti-human factor IX monoclonal antibodies specific for calcium ion-induced conformations.

Four monoclonal antibodies have been produced, which are specific for the Ca2+ or Sr2(+)-induced conformation of human factor IX. Certain, but not all, gamma-carboxy-glutamic acid residues in factor IX are involved in the epitope expression together with the conformation stabilized by the adjacent region of Gla-domain and a disulfide bridge. All the antibodies interfered with the binding of factor IX to phospholipids and inhibited the procoagulant activity of factors IX and IXa beta.

[Adenoid cystic carcinoma of the lacrimal gland: report of a case].

Adenoid cystic carcinoma of the lacrimal gland is rarely reported in Japan compared with other countries in Europe and the U.S.A. Here is a report of this rare case. A 26-year-old female had complained of left visual disturbance and left exophthalmos with orbital pain. CT scan and MRI revealed a lacrimal gland tumor in that eye. The tumor was removed via a left frontal craniotomy, and diagnosed as adenoid cystic carcinoma. After operation, she was locally irradiated with a total dose of 5,000 rads using 15 MeV Lineac source. Chemotherapy was started with 5-FU after radiation therapy. An abnormal uptake was observed in the sternum in the bone scintigraphy performed after operation, suggesting metastasis. Visual disturbance improved and exophthalmos and orbital pain disappeared. At the last follow-up at 8 months after operation, she had no signs and symptoms of recurrence or metastasis. Adenoid cystic carcinoma is reported to be the most frequent among the malignant tumors of the lacrimal gland in the Western countries. Prognosis is generally unfavorable with the mortality of 45% to 70%. The 5-year and 10-year survival rate are 45% and 25%, respectively. The mean post-operative survival time is 4.5 years. Since the tumor grows infiltratively into the surrounding tissues, radical excision with total exenteration of the orbit is the treatment of choice. However, it is a difficult decision whether or not to remove the eye with the intact visual function. As for operation, an approach from above through the frontal craniotomy provides a wider operative field and an easier access to the tumor than the lateral approach employed by ophthalmologists.

[Meningiomas in the elderly: analysis of 34 patients].

We have operated on 82 patients with intracranial meningiomas in the past 10 years. They ranged in age from 6 to 80 (mean 56.3), 59 patients were female and 23 were male. Thirty-four patients were 60 years of age or older. Of 82 patients, follow-up examination was possible in 71, with a mean follow-up period of 3 10/12 years. Pattern of clinical onset, location of tumors, and histology were compared among groups of approximately 59 years, 60-69 years, and 70 years approximately in age. Of 71 followed-up patients, 11 (15.5%) had expired. Operative mortality in patients aged 60 or older in this series, however, was low at 2.9%.

[Pituitary adenoma associated with neurofibromatosis: case report].

A case of pituitary adenoma associated with multiple neurinomas in the central nervous system was presented. A 52-year-old man was referred to us for surgical treatment of an intradural extramedullary cervical cord tumor. He had been operated on for a cauda equina tumor when he was 43 years old, and again for a glossopharyngeal neurinoma at 49 years of age. His brother expired in childhood, and had multiple subcutaneous nodules. The patient had been complaining of left leg pain, left shoulder pain, and hypesthesia of the right leg and foot. Examination showed 6th cervical nerve root sign. MRI revealed a well-circumscribed extramedullary tumor at the C5 level, and, in addition, incidentally showed an intra- and supra-sellar tumor which was isointensity on T1 weighted and high intensity on T2 weighted images. Myelography showed multiple extramedullary tumors in the lumbar region. Endocrinological study revealed an increased serum prolactin level (818.0ng/ml). The patient had neither café au lait spots nor subcutaneous nodules. A neurinoma of C6 root was totally removed and chromophobe pituitary adenoma was partially removed through a transsphenoidal approach. Neurofibromatosis is known to be associated with many kinds of tumors in the central nervous system. They are usually neurinomas, meningiomas or gliomas, and association of pituitary adenomas has been reported in only three cases, one of which being a prolactin secreting adenoma. Coexistence of multiple primary brain tumors has been also reported apart from phakomatosis. The most common combination is association of glioma and meningioma, and it is probably incidental coexistence due to their high frequency.(ABSTRACT TRUNCATED AT 250 WORDS)

Hydrophobic residues 382-386 of antithrombin III, Ala-Ala-Ala-Ser-Thr, serve as the epitope for an antibody which facilitates hydrolysis of the inhibitor by thrombin.

In the presence of a monoclonal antibody raised against the human thrombin-antithrombin III complex, the reaction between thrombin and antithrombin III proceeded to form preferentially a two-chain form of the inhibitor rather than to follow the major pathway of stable acyl complex formation. We thus propose the term "switching antibody" for an antibody that switches the enzyme-inhibitor reaction (Asakura, S., Matsuda, M., Yoshida, N., Terukina, S., and Kihara, H. (1989) J. Biol. Chem. 264, 13736-13739). By analyzing a CNBr fragment of the thrombin-antithrombin III complex that reacts with the antibody we localized the epitope for the antibody to a strongly hydrophobic residue 382-386 peptide segment, Ala-Ala-Ala-Ser-Thr, of the inhibitor, which is also contiguous with a hydrophobic amino acid Ala at its carboxyl terminus. This particular region should be cryptic in nascent antithrombin III, but could have been exposed to provide the reactive site for the antibody at an early stage of the reaction. Thereby a conformational change may have been induced at or near the reactive site of the complex, facilitating hydrolysis of the inhibitor by the enzyme. Interestingly, this hydrophobic region is highly conserved among members of the serpin family.

[Radiological features of primary central nervous system lymphoma].

Radiological features of 12 cases of primary malignant lymphoma of the central nervous system with histological confirmation were reviewed. Ten patients had primary intracranial lymphoma, and two spinal. Angiography yielded no specific findings. CT appearances varied widely. Any positive relationships were not found between histologic types of tumors and patterns of contrast enhancement in the present cases as well as in those reported previously. Gd-DTPA was used in two patients. All lymphomas showed low to isointensity signals on T1-WI and high signal intensities on rho-WI and T2-WI. Although MRI gave no additional informations to those on CT with regard to the differential diagnosis, it proved to be a very accurate, non-invasive method of providing better delineation of tumor on multiplanar basis. Use of Gd-DTPA improved quality of T1-WI by markedly enhancing the tumor. It is generally difficult to distinguish primary from secondary spinal lymphoma, but two spinal epidural lymphomas in the present series were considered primary, as we could detect no other lesion elsewhere in the body. We expect that MR imaging would lead to an improvement in the detection of lesion in the paravertebral area.

Diagnosis of squamous-cell carcinoma of the lung by sputum cytology: with special reference to correlation of diagnostic accuracy with size and proximal extent of resected tumor.

Sputum cytology was performed in 179 cases of squamous-cell carcinoma of the lung; 134 cases were diagnosed as positive. There were no significant differences in diagnostic accuracy of sputum cytology between tumors sizes. In cases with tumors extending proximally into the main, lobar, or segmental bronchi, the diagnostic accuracy of sputum cytology was significantly higher than in cases where the proximal invasion of tumor was limited to the peripheral bronchi. In cases with tumors 3 cm or less in diameter, when tumors extended proximally into main, lobar, or segmental bronchi, the diagnostic accuracy of sputum cytology was significantly higher than in cases with tumors extending proximally into subsegmental or subsubsegmental bronchi. In peripherally located squamous-cell carcinoma, in cases in which the tumor arose in subsegmental or subsubsegmental bronchi, carcinoma could be detected by sputum cytology even when it was roentgenographically occult.

Ultrastructure of Paneth cells in the intestine of various mammals.

Paneth cells in the following species were observed under an electron microscope: human, rhesus monkey, hare, guinea pig, rat, nude rat, mouse, golden hamster, and insect feeder bat. Secretory granules containing homogeneous electron-dense materials were observed in the Paneth cells of humans, monkeys, hares, guinea pigs, and bats; mouse Paneth-cell granules were bipartite (central core and peripheral halo), and the Paneth cells in rats and golden hamsters had secretory granules showing various electron densities. In humans, monkeys, and bats, immature granules near the Golgi apparatus sometimes showed bipartite substructure. The number and size of secretory granules were also diverse among various animal species. Some lysosome-like bodies were commonly observed in peri- or supranuclear regions, though the size and shape of the bodies differed from cell to cell. In apical cytoplasm, small clear vesicles (100-200 nm diameter) were more-or-less observed in all species examined, and it was especially note that rat Paneth cells contained many clear vesicles. Small dense-cored vesicles (150-200 nm diameter) were rare. It is unlikely that the various ultrastructural features of Paneth cells correlate with the phylogenetical classification.

Comparison of the incidence of 5-azacytidine-induced exencephaly between MT/HokIdr and Slc:ICR mice.

The incidence of 5-azacytidine-induced exencephaly was compared between MT/HokIdr strain (MT) and Slc:ICR strain (ICR) mice. MT mice have a genetic predisposition for exencephaly, but ICR mice do not. Pregnant mice were given 5-azacytidine (1 mg/kg to 100 micrograms/kg) injected intraperitoneally on Day 7.5 of gestation (vaginal plug day = Day 0.5), and fetuses were observed for external malformations on Day 18.5 of gestation. One hundred micrograms/kg 5-azacytidine induced exencephaly in MT mice but not in ICR mice, and 1 mg/kg 5-azacytidine resulted in resorptions in MT mice but caused exencephaly in ICR mice. These results indicated that MT mice had 10-fold more sensitivity to 5-azacytidine than ICR mice. It seems likely that less than effective doses of teratogens for animals without genetic predispositions are still effective in inducing malformations in animals with a genetic predisposition for malformations. When 4-somite-stage embryos of both MT and ICR mice were cultured in rat serum supplemented with 5-azacytidine, 0.02 micrograms/ml 5-azacytidine induced the failure of closure of cephalic neural tube in MT embryos but not in ICR embryos, and 0.2 micrograms/ml 5-azacytidine induced severe growth retardation in MT embryos but in ICR embryos it only induced embryos with smaller heads and fewer somites than in control. These results indicated that MT mouse embryos in culture also had a 10-fold-increased sensitivity to 5-azacytidine compared with ICR mouse embryos, suggesting maternal effects play no significant role in their increased sensitivity to 5-azacytidine.
