Relation between myoglobin and cardiac dysfunction in myocarditis--immunohistochemical study of endomyocardial biopsy specimens.

To investigate the mechanism of cardiac dysfunction in myocarditis, myoglobin, an intracellular oxygen-transport, was immunohistochemically examined in biopsy specimens obtained from the right side of the ventricular septum and left ventricular free wall in 58 patients with myocarditis and 19 controls. Sections 4 microns thick were stained by the indirect immunoperoxidase method using a polyclonal antibody to human myoglobin as the primary antibody. Under light microscopy, the intensity of myoglobin immunoreactivity in the tissue section was semiquantitatively classified from grade 0 to grade 3. Then, the grade of myoglobin staining was compared with clinical, hemodynamic and histopathologic parameters. In right and left ventricular specimens, the grade of myoglobin staining was positively correlated with ejection fraction, but inversely with left ventricular end-diastolic and end-systolic volume indices. The percentage of myocytes with grade 0 was correlated with the number of mononuclear cells in the specimens. In addition, the grade of myoglobin staining in right ventricular specimens was positively correlated with the duration of illness but inversely correlated with the number of mononuclear cells. In 4 patients who had serial biopsies, the ejection fraction was improved and the grade of myoglobin staining was increased in the convalescent stage. These results indicate that myoglobin staining reflects the intensity of myocarditis and a decrease of myoglobin may be important as one of the pathogenetic factors of cardiac dysfunction in myocarditis.

Autosomal recessive lethal mutations in two mutator stocks of Drosophila ananassae.

The frequency of recessive lethals in the 2nd chromosome was examined in two mutator stocks of Drosophila ananassae, ca and ca; px. They are characterized respectively by possessing an extrachromosomal clastogenic mutator in males, and by the retrotransposon "tom", which induces Om mutability only in females. The frequencies of recessive lethal mutations in the 2nd chromosome among progenies from males and females of the ca; px stock are 0.35 and 0.34 percent, respectively. Similarity of these frequencies indicates that tom does not induce recessive lethals in females. In contrast to the ca; px stock, the frequency of recessive lethals in males of the ca mutator stock was estimated to be 1.54 percent for the 2nd chromosome. No visible mutants except Minutes were recovered. Some recessive lethals derived from ca stock males were associated with chromosomal rearrangements. Being consistent with its high rate of Minute mutation it was demonstrated that the ca clastogenic mutator also induced recessive lethals.

Absence of atrial contraction and exercise in patients with isolated atrial fibrillation.

The aim of the study was to assess the effect of absence of atrial contraction during exercise. During the incremental ergometer exercise tests, heart rate, oxygen uptake, and oxygen pulse in patients with isolated atrial fibrillation were compared with those in control subjects at rest, at the exercise level of gas exchange anaerobic threshold, and at peak exercise. The study population consisted of 51 subjects aged 40 years or more: 12 patients with isolated atrial fibrillation and 39 control subjects with normal sinus rhythm. Heart rate in control subjects was lower than that in patients with isolated atrial fibrillation, at rest, anaerobic threshold, and peak exercise (74 +/- 12 vs 85 +/- 8 beats/min at rest, 108 +/- 16 vs 134 +/- 18 beats/min at anaerobic threshold, and 151 +/- 16 vs 173 +/- 22 beats/min at peak exercise, all p less than 0.01). During exercise, oxygen uptake in patients with isolated atrial fibrillation was not significantly different from that in control subjects. Oxygen pulse in patients with isolated atrial fibrillation was lower than that in control subjects during exercise (6.45 +/- 2.04 vs 7.84 +/- 1.63 ml/beat at anaerobic threshold, 7.79 +/- 2.28 vs 9.16 +/- 1.79 ml/beat at peak exercise, both p less than 0.05). In patients with isolated atrial fibrillation, the oxygen pulse might be reduced due to the lack of atrial contraction during exercise. However, the oxygen uptake that represents the exercise capacity would be preserved with the increase in heart rate.

Anti-pituitary antibodies in patients with hypopituitarism and their families: longitudinal observation.

In an attempt to investigate the clinical significance of anti-pituitary antibodies in patients with hypopituitarism, anti-pituitary antibody in plasma was examined in 10 such patients (7 cases of isolated ACTH deficiency, 1 of partial hypopituitarism, and 2 of Sheehan's syndrome), on two or three occasions with an interval of more than 6 months (longitudinal study). In a total of 16 relatives of these 4 patients (2 cases of Sheehan's syndrome, one in each of partial hypopituitarism and isolated ACTH deficiency) and one patient not involved in the longitudinal study, anti-pituitary antibodies were also examined (family study). Anti-pituitary antibodies reacting with rat pituitary cytoplasmic antigens (pituitary cell antibodies: PCA) and pituitary cell surface antibodies (PCSA) reacting with GH3 cells and/or AtT-20 cells were measured with indirect immunofluorescence. The longitudinal study revealed the disappearance of antibodies in 3 patients, 2 PCA positive and one both PCA and PCSA positive. In 3 patients, altered antibody titers or a newly appearing antibody were found during the follow-up period. In 4 patients, the pituitary antibodies were negative during the follow-up periods. Of 16 family members studied, positive PCA was found in 3 members (2 in the families of patients with PCA positive Sheehan's syndrome, and 1 in the family of the patients with PCA positive partial hypopituitarism). Positive PCSA was found in 4 members (one in each of families of patients with partial hypopituitarism and isolated ACTH deficiency and of two cases of Sheehan's syndrome), and weakly positive PCSA was found in one family member of a patients with PCA positive Sheehan's syndrome.(ABSTRACT TRUNCATED AT 250 WORDS)

Intraoperative monitoring of somatosensory evoked potentials in patients with cerebral aneurysm--correlation between central conduction time and postoperative neurological status.

Somatosensory evoked potentials (SEPs) were monitored during 55 aneurysm procedures in 49 patients to investigate the relationship between central conduction time (CCT) and neurological status in the immediate postoperative period. Significant CCT prolongation was observed in 15 cases. The postoperative neurological status deteriorated in nine of the 15 cases, compared with three of 40 cases without appreciable CCT changes. This difference was significant at p less than 0.05. The CCT prolongation was ascribable to temporary vascular occlusion in half of the cases. Intraoperative SEP monitoring is useful in predicting neurological morbidities in the immediate postoperative period in patients with cerebral aneurysm.

Absorption and metabolism of pyridoxamine in mice. I. Pyridoxal as the only form of transport in blood.

[3H]Pyridoxamine was orally administered to mice in physiological amounts, and the distribution of isotope between the six recognized forms of vitamin B6 and pyridoxic acid was determined at different times in the intestine, liver, blood, and brain. After 7 min about 50% of the radioactivity in pyridoxamine had been absorbed by the intestine and transported to the blood and other organs. Labeled pyridoxal phosphate was found in the intestine and liver. Labeled pyridoxamine could not be detected in the peripheral blood, but substantial amounts of labeled pyridoxal and pyridoxal phosphate were found in the blood. However, when a large amount (40-140 nmol) was given, a significant amount of labeled pyridoxamine was found in the blood, together with labeled pyridoxal and pyridoxal phosphate. These results suggest that the intestine and/or liver play a major role in completely converting physiological amounts of pyridoxamine to circulating pyridoxal, which is then taken up and phosphorylated by other organs.

[Usefulness and limitation of immunotherapy of metastatic renal cell carcinoma with autologous lymphokine-activated killer cells and interleukin 2].

Fourteen patients with metastatic renal cell carcinoma (RCC) were treated by systemic administration of autologous lymphokine-activated killer (LAK) cells and interleukin-2 (IL-2). Pulmonary metastases alone were found in 9 cases, pulmonary and mediastinal nodal metastases in 3, and pulmonary and bone metastases in 2. LAK cells, generated by incubation in 2 units/ml of IL 2 for 3-4 days, were intravenously administered once or twice a week. In addition, beginning on the day of the first LAK cell infusion, 1000 units of IL 2 diluted in normal saline were intravenously infused once or twice a day with occasional supplementation of 1000 units of IL-2 on each day of LAK cell infusion. The total number of LAK cells and total amount of IL-2 administered per patient in this study ranged from 0.8 x 10(10) to 6.9 x 10(10) cells and from 3.3 x 10(4) to 21.4 x 10(4) units, respectively. As toxic effects caused by the infusion of LAK cells, headache, shaking chills, fever and leukocytosis were found in all 14 cases. Side effects possibly induced by IL-2 infusion were tolerable fever, fluid retention (body weight gain of 2-3 kg) and eosinophilia. No objective regression of mediastinal nodal or bone metastases was observed. In regard to lung metastases, however, partial and minor responses were observed in 3 and 2 cases, respectively. One of the 3 patients with a partial response was clinically free of disease after undergoing a thoracotomy for resection of residual lesions, but a brain metastasis was detected 10 months after the thoracotomy. The remaining 2 patients are being closely followed up at present. In 3 of 11 patients who showed a minor response, no change or progressive disease, brain metastases were observed during or after the immunotherapy. Furthermore, we examined the possibility of selection of suitable candidates for this therapy on the basis of the degree of in vitro LAK activity against autologous cultured tumor cells in 6 patients, but there was no significant correlation between in vitro autologous tumor cell lysis by LAK cells and the clinical response to immunotherapy. In conclusion, although a complete response could not be obtained, it can be said that this immunotherapy may be effective against RCC, in particular lung metastases, since a partial response was achieved in 3 of 14 patients. However, it should be taken into consideration that this immunotherapeutic approach may have a risk of increasing the frequency of brain metastases.

[Expression of ras oncogene p21 in relation to prognostic factors of human breast cancer].

For the purpose of demonstrating the relationship between the expression of ras oncogene p21 protein and clinico-pathological characteristics which reflected the prognosis, 253 women with breast cancer who underwent mastectomy were analyzed. Ras p21 was detected in 133 (52.6%). In histological types, scirrhous carcinomas were more often ras p21-positive, and papillo-tubular carcinoma were usually negative. And histological grade was significantly correlated with ras p21. The degrees of invasion to fat tissues and infiltration into lymphatic vessels were also significantly correlated with ras p21. Tumors with lymph node metastases expressed higher levels of ras p21 than nonmetastasizing tumors in smaller tumors, especially in papillo-tubular carcinomas. And patients with elevated ras expression tended to have a poor prognosis. These results suggested that an elevated ras expression may play an important role in the development of aggressive tumors.

[Significance of reduction surgery for stage IV hepatocellular carcinoma (HCC) and postoperative immunochemotherapy for extension of survival period].

A retrospective analysis of 35 stage IV HCC (26 IV-A case and 9 IV-B cases) which underwent reduction surgery from 1983 suggested a possibility to extend their survival period by decrease in their tumor-mass and subsequent immunochemotherapy for improvement of their depressed immunity. Their operability depended on the clinical stage of accompanying liver cirrhosis and extent of distant organ metastasis. It is of first importance for reduction surgery to select intrahepatic multiple tumors, slow-growing and not rapidly to induce distant organ metastases, among them. Intrahepatic tumors arising from multicentric origins were found in 42% in IV-A cases but 0% in IV-B. DNA ploidy analysis of the multicentric tumors in 8 cases did not show any clear indication of resectable tumors according to DNA index. The present immunochemotherapy is composed of a continuous infusion of IL2 and intermittent one-shot injections of 10mg ADR to the remnant liver by using subcutaneously implanted pump. In patients who could enhance peripheral NK and LAK activities by the immunotherapy, decreases in intra- and extra- hepatic tumors were observed. The 2 year-survival rate was 49% in IV-A, but only one case who is receiving the immunotherapy is surviving over 2 years in IV-B.

Changes in serum immunoglobulin values in kittens after ingestion of colostrum.

Immunoglobulin values were determined in fetal and kitten sera. In the fetal and precolostral kitten sera, only IgG was detected, except in 1 case in which IgM was detected. The IgG, IgA, and IgM were transferred to the kittens through colostrum ingestion with some selectivity. Concentration of the transferred IgG, IgA, and IgM decreased significantly with half-lives of 4.15 +/- 1.29 days, 2.03 +/- 0.33 days, and 2.2 +/- 1.2 days, respectively. As a result of this decrease and increase of de novo immunoglobulin synthesis, IgG, IgA, and IgM were at their lowest values when kittens were 20 to 25 days, 14 to 20 days, and 8 to 10 days old, respectively. After their nadir was reached, IgG values increased gradually, IgA slowly, and IgM rapidly, as a result of de novo immunoglobulin synthesis. When the kittens were 90 days old, their immunoglobulin values were 80% (IgG), 7% (IgA), and 100% (IgM), compared with those of adult cats. These findings suggest that kittens that receive inadequate colostrum from their mothers will be particularly susceptible to infection after they are 5 weeks old.

[Subsets of peripheral blood lymphocytes and ageing in patients with SMON].

Recent clinical problems in patients with subacute myelo-optico-neuropathy (SMON) arise mainly from complications associated with the disease. Some of them show the association of autoimmune diseases. Furthermore, it has been reported that some kinds of autoantibody and hyperglobulinemia are frequently seen in patients with SMON. In order to examine whether ageing and clioquinol intoxication in the past may be implicated in the immunological system of the patients, we investigated subsets of peripheral blood lymphocytes including OKT3, OKT4, OKT8, OKIa1 and Leu7, in 31 patients with SMON, who lived in Nagano Prefecture. The levels of OKT8 (% positive cells), which are known to decrease with age actually increased with age in the peripheral blood of patients with SMON. The levels of OKT4 decreased with ageing, as did the OKT4/OKT8 ratios. The percentage of OKIa1 also significantly increased with age in the peripheral blood of patients with SMON compared to that of the controls (p less than 0.01 or p less than 0.001). It is considered unlikely that the effect of ageing on subsets of lymphocytes can be implicated in these immunological abnormalities of SMON patients, but it seems likely that the immune systems (B cell or T cell) of SMON patients are activated by factors other than ageing in subsets of lymphocytes.

[Two cases of posterior keratoconus].

We studied 3 eyes from 2 patients with posterior keratoconus. One patient (a 17-year-old female) had posterior keratoconus bilaterally and the other (a 8-year-old male) had posterior keratoconus in one eye and Peters' anomaly in the contralateral eye. Slit-lamp biomicroscopy disclosed localized thinning with stromal haze underlying the endothelium in the central cornea. Photokeratoscopy revealed no abnormalities suggestive of anterior keratoconus. Wide-field specular microscopy showed the endothelial cells with normal cell density (approximately 3500 cells/mm2) and morphology in both the peripheral and central corneas (adjacent to the hazy areas).

[Role of carcinoembryonic antigen, carbohydrate antigen 19-9 and cytology of bile in diagnosis of biliary and pancreatic cancer].

To evaluate the clinical value in diagnosis of biliary and pancreatic cancer carcinoembryonic antigen (CEA), carbohydrate antigen (CA19-9) and cytology in the bile were examined in 343 patients with benign biliary disease and 71 patients with biliary and pancreatic cancer. An abnormal CEA level was defined as a level of more than 500 ng/ml in the gallbladder bile and 40 ng/ml in the bile duct bile. Increased bile CEA levels were observed in 50.0% in 22 patients with bile duct cancer, 60.0% in 20 with gallbladder cancer and 23.8% in 21 with pancreas cancer. False positive rate in 343 patients with benign diseases was only 1.2%. Bile CA19-9 levels were measured in 195 patients with benign disease and 30 with malignancy. However, the overlap between the values of the 2 groups was too great for differentiation of malignancy from benign disease. The positive rates of bile cytology were 52.2% in 23 patients with bile duct cancer, 40.0% in 15 with gallbladder cancer and 27.3% in 22 with pancreatic cancer. The diagnostic accuracies of combined assessment of bile CEA and cytology were 68.0% in 25 patients with bile duct cancer, 77.3% in 22 with gallbladder cancer and 37.5% in 24 with pancreatic cancer. The sensitivity of this combined test was 60.6% (43/71), and the specificity was 98.8% (339/343). This combined test is considered to be reliable screening test especially for biliary cancer.

Transoral esophagectomy.

A technique for transoral esophagectomy is presented herein. The major advantages of this operative technique are: less tissue dissected, minimal chance of contamination of the surgical field and preservation of the vagus nerve when the occasion requires it. The operation can be done with small incisions, insignificant blood loss and, with more simplicity and safety than in other similar procedures.

[Establishment and characterization of new cell lines from human renal cell carcinoma].

We have characterized seven human renal cell carcinoma cell lines established from primary sites of five patients between 1987 and 1989. Two lines, OUR-20P and OUR-20S, were derived from the OUR-20 cells by cloning with a dilution method 3 months after the primary culture. These three cell lines were tumorigenic in athymic nude mice when inoculated subcutaneously. Examined by a dye uptake method, OUR-20 was highly sensitive to interferon-alpha (IFN-alpha); OUR-20P, OUR-20S and OUR-30 showed slight sensitivities, while the other three cell lines were insensitive. All seven cell lines have been maintained for more than 2 years and over 50 passages in vitro. Cytogenetic analyses performed 1.5 to 3 years after the starts of primary cultures indicated that all seven cell lines, which exhibited different morphologies in phase-contrast micrographs, were aneuploid with modal chromosome numbers 41 to 89.
