Immunocytochemical localization of estrogen and progesterone receptors (ER & PgR) in breast cancer.

The localization of estrogen receptors (ER) and progesterone receptors (PgR) in human breast cancer was examined light and electron microscopically by using specific monoclonal antibodies. Among the breast cancer patients, ER-positive (ER[+]) cases were shown by enzyme immunoassay (EIA) to account for 58.9% (63/107), and 63.5% of these cases were women in the postmenopausal state. The PgR-positive (PgR[+]) rate was 50.5% (54/107), with 44.4% of these positive cases being postmenopausal women. The rates of ER(+) and PgR(+), ER negative (ER[-]) and PgR negative (PgR[-]), and ER(+) and PgR(-) cases were 43.9% (47/107), 34.6% (37/107) and 15.0% (16/107), respectively. Although ER(-) and PgR(+) cases were in few number, they were found (6.5%, 7/107). These results correlated well with those obtained by the immunocytochemical method. In either case, i.e anti-ER or anti-PgR reaction, positive nuclei and negative nuclei were found intermingled with each other in a given visual field. In electron microscopy, both anti-ER and anti-PgR antibodies bound to sites in the euchromatin area of the nucleus.

Calcium ion-dependent monoclonal antibody against human fibrinogen: preparation, characterization, and application to fibrinogen purification.

We have produced a high-affinity monoclonal antibody classified as IgG1 with kappa-type light chains that recognizes the calcium ion(Ca2+)-dependent conformation of the D-domain of human fibrinogen. Binding of fibrinogen in solution to the insolubilized antibody increased in the presence of increasing concentrations of up to 2 mM Ca2+, the half-maximal binding being reached at 130 microM Ca2+. The dissociation constant was estimated to be 1.6 x 10(-8) M at 2 mM Ca2+. The antibody was found also to be dependent on other divalent metal ions including Zn2+, Mn2+, Co2+ and Cu2+, but not Ba2+, Mg2+ or Sr2+. The synthetic Gly-Pro-Arg-Pro-amide peptide, which has recently been shown to bind to close proximity to the calcium binding site in the D-domain, was unable to elicit the conformation for the antigen to be recognized by this antibody. This antibody was found to be a suitable ligand for the immunoaffinity chromatography of normal and abnormal fibrinogens directly from citrated plasma depleted of the vitamin K-dependent proteins or heparinized plasma by eliminating the precipitation procedure widely adopted in conventional techniques of fibrinogen purification. Indeed, fibrinogen Marburg I with the A alpha chains depleted of the carboxy-terminal A alpha(461-610) residue segment has been purified by this technique, although this dysfibrinogen was difficult to purify by conventional precipitation techniques.

[A case of multiple myeloma complicated by acute transverse myelopathy due to epidural abscess].

A 51-year-old man developed fever and lumbago followed by rapidly progressive bilateral sensory disturbance below the 9th thoracic spinal cord level, flaccid paraplegia, urinary obstruction and constipation. Based on radiological examinations and laboratory findings, a diagnosis of transverse myelopathy due to epidural abscess was made. A series of MRI studies revealed multiple abscess formation in the paravertebral muscles. Hypergammaglobulinemia with M protein was observed continuously, and further examination revealed multiple myeloma in the early stage. Since it has been reported that several different immunosuppressive mechanisms precede the development of bone lesions in multiple myeloma, these mechanisms may have played an important role in the rapid progression of the abscess in this patient. Multiple myeloma is not only important as one of the disorders underlying epidural abscess of unknown etiology, but important in predisposing to severe infection as a result of the immunosuppressive mechanisms present starting in the early stage of the disease.

[Intra-arterial infusion chemotherapy for advanced liver metastases from lung cancer via implantable vascular access: report of six cases].

Six patients with advanced liver metastases judged as a survival limiting factor from lung cancer were treated with intra-arterial infusion chemotherapy employing totally implantable vascular access. The treatment regimen included 5-FU/CDDP/EPIR/MMC/etoposide in various combinations and this therapy was repeated as long as possible. There were no serious side-effects or complications, such as bone marrow suppression or gastrointestinal symptoms. The therapy could be continued for a mean duration of 7.2 months (ranging from 2 to 13 months). Cause of death was due to extra-hepatic lesion in all 5 patients. The direct effect on liver metastases reviewed on CT scan was 83.3% (CR: 1, PR: 4, NC: 1), and the median survival period was 108 days (for small-cell group: 92 days; for non-small-cell group: 407 days). We conclude that this therapy is useful in the treatment of liver metastases from lung cancer because of its limited toxicity and high direct effect.

Scanning electron and light microscopic observations on the healing process after sintered bone implantation in rats.

The healing process after implantation of sintered bone in the rat parietal bone was compared with that of synthetic hydroxyapatite using both scanning electron and light microscopy. The results showed that the differences between the sintered natural bone and the synthetic hydroxyapatite implantations were in the states of bone union and the bioresorbability of the implanted materials, even though both materials consist of the same hydroxyapatite. In the sintered bone implantation, the newly formed bone invaded into the material at 1 to 2 weeks after implantation. The sintered bone surface on the dura mater side was completely covered by the new bone at 5 weeks. It is noteworthy that bone resorbing areas characterized by Howship's lacunae were observed on the sintered bone surface at 2 weeks and the material was replaced by new bone. Light microscopy, which revealed the invasion and the development of the new bone into the sintered bone, supported the scanning electron microscopic observations. In the synthetic hydroxyapatite, the new bone adhered closely to the material just like the sintered bone implantation. The new bone did not invade into the synthetic hydroxyapatite. There was no evidence of the resorption of the hydroxyapatite. This shows that the natural and the biological structures of the sintered bone offer an advantageous environment to fluid circulation and ingrowth after implantation.

Detection of genomic variability among isolates of Campylobacter jejuni from chickens by crossed-field gel electrophoresis.

Digestion with Sal I facilitated the subclassification of 41 strains of Campylobacter jejuni into seven types, and digestion with Sma I enabled subclassification into twelve types. Sma I was potentially more useful for the detection of variability among the 41 strains, but both restriction enzymes seemed to be potentially useful for detecting variability among crossed-field gel electrophoresis profiles of the strains. The results clearly demonstrated that C. jejuni strains from different sources and with different routes of transmission had invaded the three farms investigated. At farms Sa and Ai, approximately 70% (23 strains) of isolates of C. jejuni (33 strains) were subclassified into the two major genotypes (I and II) on the basis of cleavage profiles with both Sal I and Sma I. These two major genotypes appeared to have invaded, expanded in and occupied the two chicken farms. All nine strains from farm Ai belonged genotype I.

[Changes of tympanic temperature by stellate ganglion block].

The effects of stellate ganglion block (SGB) on the temperature of tympanic membrane were determined clinically. Thirty patients received SGB with 8 ml of 1 % mepivacaine. The tympanic temperature was measured using radiation non-contact tympanic membrane thermometer before and after administration of SGB for 30 min on the side where SGB was given. Before SGB the tympanic temperature was 37.29 +/- 0.08 degrees C, and there was no difference in the readings between the two sides. The tympanic temperature dropped significantly 5 min after SGB and reached its lowest value of 36.90 +/- 0.08 degrees C 15 min later. This drop persisted for more than 30 min after SGB. The fact that therapeutic effect of SGB is partly due to vasodilation and improvement in blood flow to the affected region was demonstrated, but these effects in internal carotid arterial system have not yet been studied in detail. Tympanic temperature has been proposed as a valid index of brain temperature in man. The mechanism of brain cooling has been suggested that countercurrent heat exchange takes place between carotid and jugular blood flow. Therefore the result of the study suggests that SGB could enhance an increase in the brain blood flow.

Molecular subtyping of clinical isolates of Escherichia coli from patients with lower urinary tract infections in Japan.

Crossed-field gel electrophoresis was used to analyse the chromosomal genomic DNA from 41 clinical isolates of Escherichia coli serotypes O-2, O-4, O-6 and O-75 from patients with lower urinary tract infections after digestion of the DNA with Not I and Xba I, respectively. Not I generated 28 distinctly different genomic profiles after crossed-field gel electrophoresis (CFGE) from 34 strains and Xba I generated 35 distinctly different genomic profiles from 41 strains. Not I failed to cleave the genomic DNA from one strain of E. coli serotype O-6 and from all of six strains of serotype O-75. Both enzymes were shown to have equal potential utility for detection of variability in CFGE profiles among 34 strains because both classified 34 of the 41 strains into 28 profiles in a similar manner. No examples of essentially identical restriction profiles among strains of different serotypes were obtained.

[Congenital dermoid cyst over the anterior fontanelle: case report].

We report a case of a 35-month-old Japanese boy harboring a dermoid cyst in the region of the anterior fontanelle, which is rarely reported among Japanese. A small soft subcutaneous mass was noticed in the mid-frontal region at the age of 6 months. On admission, a soft, nontender round mass (2.5cm in diameter) was located over the anterior fontanelle. Transillumination was negative. Neurological examination yielded normal findings. MRI showed a well-circumscribed mass, which was slightly hypointense on T1W1 and hyperintense on T2W1. Midsagittal MRI showed no intracranial extension. At operation, a well-encapsulated mass was found beneath the pericranium, which was easily dissected from the underlying dura mater and removed. The tumor contained caseous material and several hairs. Microscopically, the cyst wall consisted of connective tissues lined by stratified squamous epithelium. Sebaceous glands and hair follicles were found in the subepithelial layer. It is important to confirm preoperatively whether or not the extracranial mass extends into the intracranial cavity. For this purpose, midsagittal or coronal MRI seems to be most useful.

[Effects of centrally administered NPY on stress induced gastric ulcers in the rat].

We investigated the central effects of neuropeptide Y (NPY) on stress-induced gastric ulcers in water immersion rats. Intracisternally administered NPY dose-dependently aggravated, but intravenously administered NPY did not affect stress ulcers in rats. Intraperitoneal pre-treatment of atropine completely blocked the central effect of NPY, but neither pyrilamine nor cimetidine affected. Intracisternal administration of NPY aggravated stress ulcers not through histamine mediated mechanism but by parasympathetic pathway. It is suggested that centrally administered NPY acted by the inhibition on gastric motility and by the suppression on gastric mucosal blood flow rather than by the stimulation on gastric secretion in rats.

Plasma endothelin-1 levels after exercise in patients with congenital heart disease.

We have previously reported that plasma endothelin-1 (ET-1) levels were significantly increased after exercise in healthy athletes. In the present study, venous plasma ET-1 levels were measured to investigate whether the role of ET-1 during exercise differs between healthy athletes and patients with congenital heart disease. Seven patients (mean age 13 years) performed symptom-limited treadmill exercise (Bruce protocol). All patients were in class I of the NYHA functional classification. The mean end-exercise O2 utilization was 32 ml/min/kg (166% of ventilatory threshold). Blood sampling was carried out in the seated position. The plasma ET-1 level was 1.21 pg/ml before exercise and did not alter immediately after or 30 min after exercise. The plasma level of norepinephrine was markedly elevated immediately after exercise and returned to the basal level 30 min after exercise. The present study demonstrated that exercise failed to alter plasma ET-1 levels in patients with congenital heart disease.

[Effect of endogenous and inhaled nitric oxide on pulmonary microcirculation].

The sites of action of endogenous and inhaled nitric oxide (NO) were reassured during hypoxic pulmonary vasoconstriction. Lungs of 21 adult cats were perfused in situ with autologous blood in zone-3 conditions. Capillary pressures were measured by the double-occlusion techniques and pressures in arterioles and venules 70-100 microns in diameter were measured by the servo-null micropuncture technique, both during normoxia (FiO2 = 0.3) and during hypoxia (FiO2 = 0.02). The effects of NG-nitro-L-arginine methyl ester (L-NAME, 10 mg/kg), an inhibitor of NO synthase, and of inhaled NO (5-100 ppm) were also measured. The PO2 of the prefusate decreased from 187.6 +/- 5.3 mmHg during normoxia to 25.7 +/- 1.3 mmHg during hypoxia, and further decreased to 20.8 +/- 2.2 mmHg during hypoxia with 50 ppm NO (p < 0.05, compared with hypoxia only). Increases in pulmonary vascular pressure drop in response to hypoxia were 4.8 +/- 1.0 cmH2O and 9.1 +/- 1.4 cmH2O in non-treated and L-NAME-treated lungs, respectively (p < 0.05). L-NAME significantly increased hypoxic construction in the venous segment. The concentration of exhaled NO increased from 13 +/- 4 ppb during normoxia to 18 +/- 4 ppb during hypoxia (p < 0.1). Inhaled NO lowered not only pulmonary artery pressure but also capillary pressure in a dose-dependent manner, which reduced hypoxic pulmonary vasoconstriction. Pulmonary veins were more sensitive to inhaled NO than were arteries. Inhaled NO (50 ppm) dilated vessels smaller than 70 to 100 microns in diameter, by 39% (p < 0.05), and dilated venules greater than 100 microns in diameter by 26% (p < 0.05), but did not significantly dilate arterioles greater than 100 microns in diameter (11%). Inhaled NO did not significantly change the ratio of wet weight to dry weight. We conclude that both endogenous and inhaled NO attenuate hypoxic pulmonary vasoconstriction, with significant pulmonary venous dilation. The main site of action of inhaled NO is vessels smaller than 100 microns in diameter and venules greater than 100 microns in diameter. Inhaled NO (5-100 ppm) does not cause interstitial edema.

[The correlation of the thalamic lesions on MRI with cerebral cortical blood flow in patients with lacunar infarction].

We performed MRI and measured cerebral blood flow (CBF) using 123I-IMP SPECT microsphere model in twenty three right-handed patients with lacunar infarction. Twelve of 23 patients showed chronic deterioration of dysarthria and gait disturbance. The mental function of the patients was evaluated by the Mini-Mental State (MMS) examination. The area of high intensity on T2-weighted images was quantitatively analyzed in the cerebral white matter (WM), lenticular nucleus (LN) and thalamus (THA). The score of MMS was positively correlated with the local CBF in the bilateral frontal, parietal, temporal and occipital cortices (p < 0.05). Also, the area of high intensity in the left THA showed a significant negative correlation with local CBF of the bilateral frontal, parietal, temporal and occipital cortices (p < 0.001). The high intensity areas of the bilateral LN, right WM and right THA had a significant but weaker negative correlation with local CBF of some cortices. These findings suggest that thalamic lesions on the dominant side play an important role in the reduction of cortical blood flow and the deterioration of mental functions in patients with lacunar infarction.

[Unilateral auditory hallucinations due to left temporal lobe ischemia: a case report].

Unilateral auditory hallucinations are a rare lateralization phenomenon experienced in one ear or from one direction. We recently encountered a 63-year-old right-handed man who developed transient unilateral auditory hallucinations associated with pure word deafness. The patient had a past history of myocardial infarction, lung cancer and aortic aneurysm, but no previous psychiatric or convulsive disorders. About six months before admission, he developed right hemiparesis and motor aphasia caused by a hemorrhagic left parietal infarct. These symptoms gradually improved over three weeks. Two days before admission, he suddenly lost the ability to understand spoken words. He concurrently experienced auditory hallucinations arising from the right anterior direction. On admission, he was very embarrassed to simple verbal commands. He was unable to comprehend spoken words and repeat speech, although he could fairly follow written commands. Confrontation naming, reading aloud, comprehension, spontaneous writing remained relatively unaffected, although he occasionally made paraphasic errors. He could distinguish environmental sounds such as a telephone ringing or running water. After gradual improvement of his auditory incomprehension, he began to describe auditory hallucinations of verbal, musical and elementary types. He was fully aware of the hallucinatory nature of his experience, and took some notes. Two days before admission, from the right anterior side of his head he heard a familiar radio announcer reporting news about the earthquake in Osaka Prefecture and the recommended places of refuge. After similar experiences over several days, he repetitively heard a familiar Japanese traditional song from the right side, which was followed by elementary auditory hallucinations such as a car engine and a siren. These symptoms spontaneously disappeared after nine days. Besides his auditory hallucinations, visual hallucinations and illusional emotion were temporarily present. The Wechsler adult intelligence scale revealed a verbal IQ of 91 and a performance IQ of 100. Pure tone audiometry revealed bilateral, mild peripheral sensorineural hearing loss. Brainstem auditory evoked potentials were unrevealing. The EEG showed slow activities in the left temporoparietal region. Magnetic resonance imaging of the brain failed to reveal any relevant abnormalities except for an old hemorrhagic parietal infarct. The SPECT with Tc99m-HMPAO, however, showed reduced blood flow in the left temporal lobe including the first temporal convolution as well as in the left parietal lobe. Based on the SPECT findings, unilateral auditory hallucinations in our patient are considered to have resulted from the left temporal lobe ischemia. Our case indicates that unilateral auditory hallucinations may have a clinicoanatomical correlation with contralateral temporal lobe lesions.

CCK activates excitatory ventral tegmental pathways to the posterior nucleus accumbens.

In urethane-anaesthetized male rats, electrical stimulation of the ventral tegmental area (VTA) produced either excitation (n = 26) or inhibition (n = 5) in the activity of neurones in the posterior nucleus accumbens (PNAc). Local administration of sulfatedcholecystokinin octapeptide (CCK8, 0.03 microgram) into the stimulation site enhanced the neuronal excitability in 10 out of 21 PNAc neurones that demonstrated the excitation to VTA stimulation, but did not affect the remaining 11 neurones. Similar administration, on the other hand, was without effect in any of PNAc neurones (n = 5) that displayed the inhibition to VTA stimulation. These results suggest that CCK8 may act at the VTA to elicit an activation of the excitatory pathways from the VTA to the PNAc.
