Ultrasonic measurement of femur length as a predictor of fetal gestational age.

Ultrasonic determination of fetal femur length is useful as a parameter in the determination of fetal gestational age. One hundred thirty measurements were made in pregnancies at 14 to 36 weeks' gestation. The measurements were plotted against gestational age, and we found that a strong correlation exists between fetal femur length and fetal gestational age, with 95% confidence limits of plus or minus seven days. This ultrasound parameter of fetal age assessment should be added to the other methods available for accurate dating of pregnancy.

[Arterial vascular occlusion in penicillin allergy (author's transl)].

A nodular exanthema of the skin was observed in an 8-year-old girl. She had been treated with penicillin a few days ago because of a gastrointestinal infection. Continuation of penicillin treatment led to occlusion of larger arteries with gangrene of the forefoot. This was accompanied by septic temperatures and superinfected skin lesions. Angiologic and angiographic investigations showed a right-sided femoro-popliteal occlusion and occlusions of the arteries of the lower leg. On the left side there was spasm of the superficial femoral artery. After cessation of penicillin and high-dose steroid therapy rapid restitution occurred. Arterial spasms regressed, the occlusions remained persistent, but there was adequate collateral blood supply and ischaemic acral necroses healed. An allergic reaction of the intermediary type involving major arteries can be considered causative underlying pathology.

Induction of DNA single-strand breaks by T2 toxin, a trichothecene metabolite of fusarium: effect on lymphoid organs and liver.

T2 toxin, a trichothecene metabolite produced by Fusarium species, contaminates cereals harvested and stored under damp and cold conditions. These substances are responsible for Alimentary Toxic Aleukia (ATA), a severe human disease, and numerous animal intoxications. The action of T2 toxin on DNA was studied by using Parodi's alkaline elution technique coupled with a microfluorimetric determination of DNA. In vivo the effect of the toxin was studied on liver, spleen and thymus, and in vitro on a primary culture of rat hepatocytes and on splenic and thymic lymphocytes stimulated by PHA. Under our experimental conditions, in vivo and in vitro, no damage was observed for the hepatic DNA. By contrast, the DNA of lymphoid organs was severely damaged by the toxin. In vitro, T2 toxin induced severe damage to the DNA molecule with low concentrations (5 ng/ml culture) and for short exposure (2 h). In vivo, a moderate amount of DNA breaks was observed in splenic and thymic lymphocytes 3 h after administration of T2 toxin to mice (3 mg/kg). Reversibility occurred 24 h later under these conditions in vivo, indicating DNA repair. The results agree with the preferential cytotoxicity of T2 toxin for lymphoid cells. The relation between DNA damage, mutagenicity and carcinogenic properties of T2 toxin is discussed.

Response to long-term deferoxamine therapy in thalassemia.

We evaluated decreasing deferoxamine-induced urinary iron excretion during intensive chelation therapy in four children with thalassemia. Patients received daily intramuscular or subcutaneous therapy as well as intermittent intravenous infusions of high doses of DFO. Iron excretion fell by more than 80% in three patients and decreased by 45% in the fourth. Ferritin concentrations returned to normal or near normal values in all patients. Serum iron concentration and transferrin saturation steadily declined in one of four patients. Supplemental vitamin C was no longer required for normal vitamin C stores or maximum iron excretion in one patient after 26 months of chelation therapy. Interruption of chelation therapy was not followed by increased iron excretion after resumption of treatment with DFO. Decreasing DFO-induced iron excretion occurs during long-term, intensive chelation therapy, and may be the result of substantial reduction of excessive iron stores rather than of tachyphylaxis or transient depletion of an intracellular chelatable iron pool.

Is medicine a social science?

The question, "Is medicine as social science?" can be understood in three different ways. One interpretation suggests that medicine is merely a social science, which is obviously false. Another interpretation is that medicine might be part a social science. The third interpretation of the question is, "Is the social scientific dimension of medicine very important:" Three claims are considered about the social scientific dimension of medicine. Although these claims are shown to be untrue, they nevertheless call attention to neglected aspects of medicine.

Fixation of C3 to platelets in vitro by antiplatelet antibody from patients with immune thrombocytopenic purpura.

Chronic immune thrombocytopenic purpura (ITP) is due to platelet destruction by an IgG antiplatelet antibody. Recent studies by some investigators suggest in vivo fixation of C3 by the antiplatelet antibody in ITP patients; other groups have not confirmed these findings. In the present studies, purified IgG produced in vitro by splenic cells from two patients with ITP resulted in C3 fixation to target platelets. Since cell-bound C3 increases the efficiency of cell destruction in many experimental antibody systems, the ability of the IgG antibody in patients with ITP to fix C3 may influence the severity of their disease.

Processing of information about self by neurotics.

This article examines the possibility that neurotic people exhibit consistent idiosyncracies of cognitive information processing. Specifically, it was hypothesized that if a subject is presented with both negative and positive information which is said to refer to the subject's personality then neurotic people should exhibit a greater than normal tendency to selectively process the self-depreciatory rather than the self-appreciatory information. An experimental investigation with both clinically neurotic and normal samples employed a variety of experimental measures in conjunction with three personality measures (Eysenck Personality Questionnaire, Repression-Sensitization scale, and Social Self-esteem), and provided evidence for the existence of such an association.

Separation by counter-current distribution of rat and chicken erythrocytes of different age, and its application to the assay of enzyme activities.

The separation of cells with different ages from erythrocyte populations of adult rats and young or adult chickens have been achieved by counter-current distribution (CCD). A thin-layer CCD apparatus has been employed. Erythrocytes from blood samples taken at different times after 59Fe i.p. injection were separated by CCD. By compilation in a "composite curve" of the hemoglobin and radioactivity CCD profiles obtained for each erythrocyte population, the distribution of cells according to age can be inferred. Young erythrocytes of rats are located at the right part of the CCD curves, while older cells are distributed towards the left. An opposite distribution has been found for erythrocytes from adult or young chickens. As a first attempt for the application of the CCD procedure to the assay of enzyme activities, it was found a decrease in phytase activity as the age of chicken erythrocytes increases and an increase in phosphoglycerate kinase and phosphofructokinase as the age of rat erythrocytes increases.

[Aseptic arthritis with positive serology for Yersinia pseudotuberculosis. Apropos of 6 new cases].

Six personal observations of aseptic acute arthritis with positive serology for yersinia pseudotuberculosis (YPT) have been related, the specific serodiagnosis was clearly positive (greater than 1/500) for all the patients and the rate variations of repeated examinations were constantly observed. These cases have been compared with 14 sporadic observations of the existing studies where such reactive arthritis were referred to a recent YPT infection, this confrontation leads us to specify the characteristics of this inflammatory rheumatism : starting by a prearthritic phase with variable combination of fever, diarrhea, abdominal pains, erythema nodosum, usually polyarticular affection (90%) mostly of the knees (89%), ankle-joints, wrists and elbows, marked inflammatory clinical and biological signs but without X. Rays abnormalities. The course which seems hastened by anti-inflammatory medication ends up several weeks or months later without recurrences or secondary diseases by a recovery. The presence of HLA B27 antigen in 50% out of the 10 tested cases enables us to discover the intervention of a predisposing genetic factor. A semiologic and nosologic comparison is established with polyarthritis of YEC infection. The discussion concerning the 18 observations where only serology based diagnosis exists, turns mainly on the reality of sporadic YPT rheumatism. If it is not yet possible to assert with certitude the authenticity of such YPT reactive arthritis there is several arguments in support of that, particularly the existence of post-septicemia aseptic polyarthritis and the existence of similar joint-affection during the far-east scarlatiniform fever which is a special form of YPT infection.
