The effectiveness of plasma exchange in amyotrophic lateral sclerosis (ALS) was tested in four patients. Electromyography (EMG) and clinical examinations were carried out before, during, and after the treatment. The EMG data showed a progressive unaffected motor neuronal decay. Clinically the course of the disease was not dissimilar to that usually encountered, although a momentary improvement was observed in two patients after the first procedure. Plasma exchange appears to be of no value in treating ALS.

We have tested the effect of five aminoglycoside antibiotics (gentamicin, sisomicin, tobramycin, ribostamycin, and amikacin) on the candidacidal activity of human neutrophils in vitro; all of them are inhibitory and can be grouped into three significantly different levels of toxicity. Gentamicin in the most toxic and sisomicin is the least toxic.
Cytocentrifuged preparations of glutaraldehyde-fixed E rosettes were stained for acid alpha-naphthyl-acetate esterase activity (ANAE). On average, 84% of the rosette-forming cells (E+) also stained positively for the enzyme activity (A+). ANAE staining of blood smears of thirty-three healthy controls was compared to E rosetting by a conventional method. A+ and E+ fractional values are highly significantly correlated; the regression of E+ on A+ is E+ = 0.805.A+ + 16.8 (P < 10(-6)). Since the correlation between E+ and A+ cells is both statistically and cytologically significant, we propose blood smear ANAE staining as a simple and convenient method to enumerate circulating T lymphocytes in clinical practice.

Two siblings with recurrent infections were found to have impaired neutrophil motility. The same association of infections (otitis media, bronchitis, chronic diarrhoea) has caused seven fatalities in the paternal side of the family, suggesting genetic implications.

A simple, rapid and sensitive hemolytic micromethod for quantitative and kinetic assessment of the alternative pathway of complement activity is described. The test was performed in microtitre plates using a multiscan photometer for ELISA and permits the simultaneous evaluation of as many as 20 different serum samples.
Data on initial psychiatric evaluations performed in 1995 were compared to assess whether psychiatric consultation for human immunodeficiency virus (HIV)-positive/acquired immunodeficiency syndrome (AIDS) patients provided on-site in an infectious disease (ID) clinic improved compliance and were preferred by staff to evaluations performed in a specialized AIDS psychiatric program. Compliance with initial appointments remained below 50% in both settings, but more patients seen in the ID clinic had received prior psychiatric treatment and medication and they were more likely to receive a psychotropic prescription at this initial visit. The ID clinic staff preferred on-site consultations. Stationing psychiatric consultants in the ID clinic may reach a more impaired population but did not improve compliance with the initial visit.
Twenty-nine children with acute icteric hepatitis were classified as follows after serological tests for the agents of viral hepatitis: hepatitis B 17 patients, hepatitis A 3 patients, hepatitis A + B 1 patient, possible non-A, non-B hepatitis 8 patients. In four of these cases, hepatitis A or non-A, non-B occurred in chronic HBsAg carriers. Hepatitis B virus (HBV) DNA was present in the initial serum of 15 of the 18 patients with acute hepatitis B. About 1 year after onset, HBsAg and HBV DNA were absent in 17 of them (16 of whom developed anti-HBs) and the remaining patient became a chronic HBsAg carrier without HBV DNA in his serum. At the same time, liver biopsy samples from all 29 patients were available for HBV DNA investigation. HBV DNA sequences in the liver were never found in any patient, neither under free form nor integrated into the cellular genome. The absence of HBV DNA integration in any of the liver samples taken about 1 year after the acute phase of hepatitis B suggests that such integration is either unlikely or is transitory during the symptomatic period.
Exposure to passive smoke is a very common event associated with increased susceptibility to respiratory tract infections. Many related adverse effects result from the ability of cigarette smoke extracts to interfere with the immune system, but the mechanism is not yet completely understood. The aim of the present study is to evaluate the intracellular cytokine profile in adenoids and peripheral blood cells of children exposed to passive smoke. Children undergoing adenoidectomy exposed or not exposed to passive smoke were studied. The intracellular cytokine profile of lymphocyte subsets in adenoids and in peripheral blood were evaluated by flow cytometry analysis. Children exposed to tobacco smoke showed a significantly lower percentage of INF-gamma producing CD4+ and CD8+ cells in adenoids. Moreover a significant correlation was observed between the quantity of exposure and reduction in Th1 (CD4+INFgamma+ and CD8+INFgamma+) cells in adenoids. This reduction may be a contributing factor in the increasing susceptibility to respiratory tract infection in children exposed to tobacco smoke.
