[Folate determination in serum for clinical use (author's transl)].

Folate deficiency may be diagnosed in man by measuring serum folate activity using either a microbiological procedure with L. casei or a radioassay. The microbiological procedure tends to be time consuming and tedious, as compared with the radioassay; nevertheless it allows the accurate determination of each folate derivative that is normally present in serum. The comparison of serum folate levels obtained by the two methods shows an adequate correlation. However, it results from individual samples show that the values obtained by the radioassay are lower than the microbiological ones by 10-30%. The lower results in the radioisotope test may be caused by the presence of high levels of unsaturated endogenous protein binders in serum and/or by the difference in affinity of lactoglobulin - the binding protein used in the test - for folic acid (standard) and 5-methyl FH4 or 10-formil FH4 (serum).

A planned prospective study of combined treatment in stage III epithelial ovarian cancer.

In a planned prospective study 39 consecutive patients with stage III ovarian cancer have been admitted in two years. All patients have received initial extensive surgery followed on as out patients basis by 5 courses of A-Cy chemotherapy and by a second operation and further non cross resistant chemotherapy with Platinum, Hexamethyl-Melamine, Polymelphalan; 24 have been found pathologically free of disease at second surgery, and 34/39 are clinically free of disease while reporting. These data, exactly collected and evaluated are consistent with our (previously reported) empyrical observation that adjuvant surgery interspersed with courses of aggressive chemotherapy may render free of disease the majority of patients, prolong survival, and possibly cure a portion of this population.

Contact hysteroscopy in the diagnosis of endometrial carcinoma.

The contact hysteroscopy is undoubtedly useful every time an exploration of the uterine cavity is necessary to confirm or deny a diagnostic suspicion. For the typical aspect of the neoplastic lesion which has been confirmed by traditional endoscopy, contact hysteroscopy permits the diagnosis of an endometrial carcinoma with high reliability, modifying the therapeutic approach and consequently the prognosis.

Fibrinolysis in ovarian cancer.

Coagulation disorders, including thromboembolic phenomena and disseminated intravascular coagulation are recognised as a complication of neoplastic disease. In the present study the Fibrinogen Degradation Products (F.D.P.) were determined in 26 patients with Advanced Ovarian Cancer (stage III and IV FIGO) (mean = 38.1; S.D. = 37.6) and in ten healthy patients considered as controls (mean = 2.7; S.D. = 13). The significative difference (p less than 0.05) between the two groups points in evidence an increasing activation of Ffibrynolytic processes. The F.D.P. determination has revealed a good correlation (p less than 0.001) with the tumor extension as showed by the difference between third stage with large tumor masses (late) (mean = 44.5; S.D. = 24.5) and third stage with minimal residual disease after reductive surgery (early) (mean = 6.7; S.D. = 5.7).

Carcinoma of the vulva. Non-mutilant surgical therapy, a clinical case.

Radical non-mutilant surgical therapy of the vulva carcinoma; a clinical case. The case of a 30 year old patient affected by invasive carcinoma of the vulva is reported. The patient was submitted to a first operation for tumorectomy with inguinal lymphadenectomy according to the non-mutilant method, and after four months to a further operation for tumorectomy. The treatment was completed with radiant therapy. After three years the patient is living without any illness, and her condition of life is better, compared with the period before surgical intervention.

Distribution of pteroylglutamates in rat liver during regeneration after partial hepatectomy.

1. Liver pteroylpolyglutamate distribution was studied during regeneration after partial hepatectomy in rats maintained under controlled feeding conditions. 2. Pteroylhexaglutamate, pteroylpentaglutamate and pteroyltetraglutamate concentrations decrease from 12 to 72 h after operation, then increase and reach normal values at 180 h. Pteroyltriglutamate concentration, already high at 12 h, remains so in the subsequent periods. Pteroyldiglutamate concentration was unchanged. Monoglutamate concentrations at first decrease, and at 180 h exceed normal values. 3. The decrease in polyglutamate derivatives with a high number of glutamate residues, at present considered to be the coenzyme forms of folate, could be related not to a decreased synthesis, but to a greater requirement for these compounds during the early periods of regeneration, when biosynthetic processes are markedly increased. It is indeed probable that the increased availability of the preferred substrate of pteroylpolyglutamate synthetase, i.e. tetrahydrofolate, enhances conversion of folate into coenzyme forms.

Synthesis of pteroylpolyglutamates in rat liver during regeneration after partial hepatectomy.

The liver content of higher polyglutamate forms of folic acid, which in partially hepatectomized rats is markedly lower as compared with sham operated animals, is normalized by the vitamin administration. The greater availability of folic acid and consequently of the tetrahydrofolates, promotes the synthesis of actual coenzymic forms; so their content, in regenerating liver, is maintained at normal values in spite of a higher requirement caused by the exaltation of biosynthetic processes.
