Non mutilant radical vulvectomy vs radical vulvectomy.

20 women, matched for age, site of primary tumor, histotype and stage, have been selected among patients affected with vulvar carcinoma for a study comparing classic radical vulvectomy vs "non mutilant" radical surgery. Detailed results actually available are discussed, showing that "non mutilant" technique gives results comparable to classic radical vulvectomy in terms of survival and recurrence rate; with minor functional and aesthetic damage, minor side effects and shorter post operative course.

Angular displacements in the upper body of AK amputees during level walking.

The angular displacements of the longitudinal axis of the trunk, and of the latero-lateral axes of pelvis and shoulder girdle were measured in five normal subjects and four AK amputees during level walking at different speeds. Amputees used single axis prostheses with the SACH foot. Spatial measurements were carried out in three dimensions by means of a photogrammetric technique. The time functions of the target angles underwent harmonic analysis. Based on the Fourier coefficients, comparison was made between normal subjects' and amputees' angular displacements. Relevant findings permitted the identification of compensatory mechanisms adopted by amputees at trunk level as well as the assessment of the relationship between these latter mechanisms and those put into action at lower limb level.

[Folic acid metabolism in rapidly growing tissues: synthesis of pteroylpolyglutamates].

Pteroylpolyglutamates are regarded as the functional coenzyme form of folate: hence their synthesis may represent important regulatory step in the folate-dependent processes. To verify this hypothesis the pteroylpolyglutamate content was studied in rapidly growing tissues, which require a large availability of folate coenzymes, essential as carbon atom donors in the synthesis of purine and pyrimidine nucleotides and in the metabolism of some aminoacids. In the developing chick embryo, the higher polyglutamates are the only folate forms present and their content increases progressively during the development of the embryo. In rat liver regenerating after partial hepatectomy the higher polyglutamate forms decrease below the normal values from 12 to 72 h after the operation; the normal values are restored after 180 h. The initial drop is not related to a lower synthesis, but to a greater utilization of these coenzymes during the early periods of regeneration, when biosynthetic processes are markedly increased. In fact the content of polyglutamates in regenerating liver can be maintained at normal levels by increasing the availability of precursors via folic acid administration to hepatectomized rats.

Introduction to chemotherapy and antineoplastic pharmacology in breast cancer.

Tumor cell biology and growth-kinetics of human breast cancer are reviewed with reference to the different action mechanisms of main antitumor drugs. Particular emphasis is given to pharmacokinetics, dosage, mode of administration, effectiveness and side-effects of drugs most commonly used in breast cancer. The importance of tumor Estrogen and Progesterone Receptor assay for the therapy planning is considered and a short review of estrogens, androgens, progestins, antiestrogens, corticoids and aminogluthetimide is presented. The rationale for combined chemotherapy and/or hormonotherapy in breast cancer is finally discussed.

Clinical re-staging in cervicocarcinoma after clinico-instrumental staging.

A review of 193 cases of invasive carcinoma of the cervix was undertaken to determine the efficacy of the procedures used in the staging of this disease. All patients underwent pretreatment chest, pelvis, and spine radiographies, intravenous pyelogram, cystoscopy and proctoscopy. Bone scanning was performed too. These patients were retrospectively staged according to findings of the physical examination only. Each additional examination was then evaluated comparing its results with the initial staging. The column Xray did never change the stage; the other radiologic examinations changed the staging as follows: chest Xray 0.5%, pelvis 1%, intravenous pyelogram 2%. Cystoscopy and proctoscopy changed the clinical stage in 5.1% and 2.5% of the cases, respectively. Bone scanning in 1.5%. A review of 54 selected cases was undertaken to determine the presence of positive nodes in patients with carcinomas at stages I and II. In 16 cases of cervicocarcinoma the hystologic examination showed positive pelvic lymph-nodes. The AA, recommend all the reviewed procedures in the staging of carcinoma of the cervix in initial stages too.

Diagnosis and treatment of preinvasive and microinvasive cervicocarcinoma.

The mortality rate of cervicocarcinoma has significantly decreased over the last few years, thanks to early diagnosis. Prophylactic and therapeutical interventions, at different stages, are therefore necessary to eliminate or, at least, decrease the incidence of this pathology. Between 1975 and 1980, during a cytologic depistage carried out at the Institute of Gynecologic and Obstretic Clinic of Padua University, we examined 72 cases of patients who had undergone the following diagnostic iter: colposcopy, cytology, direct biopsy and conization. Various parameters were analyzed, notably with regard to cases of serious dysplasia, carcinoma in situ, neoplasia with beginning invasion of the glandular necks and microinvasive forms.

A polymeric film responding in diffusion properties to environmental pH stimuli: a model for a self-regulating drug delivery system.

A new polymeric film, ethylene-vinyl-NN-diethylglycinate random copolymer, has been studied to evaluate possible applications of such a material to produce controlled release dosage forms. Protonation of the substituted amino groups took place to different extents according to environmental pH conditions. The polymeric film proved to be permeable only to uncharged species, the rate of diffusion of which increased with the increasing charge of the membrane. This effect might be related to strong polymer-proton interactions affecting structure and transport properties of the film and enhancing the solvation process. The charged form of a number of barbituric acid derivatives did not diffuse through the membrane. The uncharged forms permeated the polymeric sheeting at rates consistent with their partition coefficients and which increased with the increasing charge of the polymer.

Biomechanics of sit-up exercises.

Sit-up exercises performed by four male subjects revealed the presence of initial lumbar "hollowing" (lumbar curvature), or trunk hyperextension, or both prior to trunk flexion. Maximum muscular moment developed at the hip joint, which varied with sit-up exercise, was of moderate order of magnitude. Intraesophageal pressure, cardiopulmonary response, and metabolic response to a commonly used sit-up exercise (curl-up) reveals it to be a light exercise which stresses the muscles of the lower limbs as well as those of the trunk, especially the abdominal muscle group.

[Intermolecular interactions between fluorescent probes bound to linear DNA].

Intermolecular interactions of fluorescent probes bound to linear DNA. Several fluorescent probes can be widely used in determining the base content, the geometry and the possible conformational transitions of linear and circular DNAs in solution. Data are presented showing that Tb3+ and DAPI bind independently to DNA and can therefore be used to monitor the G+C and A+T content of the nucleic acid respectively.
