Left arm thermography in early and maturity onset obesity.

Left-arm thermography and adipose-tissue biopsy of triceps skinfold have been performed in seven patients with obesity of early onset (EO), seven with maturity-onset obesity (MO) and seven non-obese control subjects (C). Obese patients were within the same range for age, physical activity, excess body-fat content and skinfold thickness; mean fat-cell diameter was found to be significantly higher in the group MO than either EO (P less than 0.05) or C subjects (P less than 0.01). Colour thermograms, as evaluated by a well-standardized score system, showed that skin temperature was higher in MO than in EO or C cases (P less than 0.05). The combination of fat-cell hyperplasia and lower thermal losses from the skin, indicative of a lower rate of energy expenditure, may explain the higher frequency of therapeutically refractory cases in EO than MO patients.

Protein-sparing modified fast in the treatment of severe obesity: weight loss and nitrogen balance data.

Nitrogen balance has been measured in 25 (16 f, 9m) patients with severe obesity submitted for four weeks to total fasting (TF) or different versions of protein-sparing modified fast (PSMF). The hospitalized patients, divided into four age-matched groups, were treated as follows:Group 1, were submitted to TF; Group 2 were treated by 80 kcal-PSMF (17 g protein=2.6 g N, traces of fat and carbohydrate); Group 3 were submitted to 180 kcal-PSMF (40 g protein = 6.4 g N, 2 g fat, traces carbohydrate) and Group IV, to 80/180 kcal-PSMF (80 kcal during the 1st and 2nd week and 180 kcal during the 3rd and 4th week of treatment). Weight loss was similar (-11 kg) in Gps 1, 2 and 4; lower (-8 kg) in Gp 3. Mean daily Nitrogen loss was significantly lower (P < 0.05) in Gps 3 and 4 than in Gps 1 and 2. Nitrogen loss was significantly reduced from the 3rd week onward in Gps 1 (P < 0.005), 2 (P < 0.05) and 4 (P < 0.002). During 180 kcal-PSMF nitrogen loss was significantly lower (P < 0.005) only when this dietary regimen was preceded by 80 kcal-PSMF for two weeks (Gp 4). These observations suggest a new approach to the treatment of severe obesity by PSMF and show that is is possible to further reduce the early catabolic phase which commonly arises during this type of therapy.

Acyl-coenzyme A--cholesterol acyltransferase activity in human liver.

1. In the presence of CoA and ATP, human liver microsomes catalyse the incorporation of [14C]oleate or [14C]cholesterol into cholesteryl oleate, thus demonstrating the presence of acyl-coenzyme A-cholesterol acyltransferase (cholesterol acyltransferase) in human liver. 2. The enzyme has properties similar to those of rat liver enzyme and with both the concentration of endogenous cholesterol in the microsomal fraction is adequate to support a constant initial rate of esterification. However, unlike the rat liver enzyme, the human cholesterol acyltransferase does not efficiently utilize added cholesterol as substrate. 3. The activity of cholesterol acyltransferase in human liver was 25% of that measured in rat liver under similar conditions of assay.

Blood lipids in Italy. Regional differences.

Serum cholesterol and triglyceride levels have been evaluated in samples from fasting males aged 20--59 in Northern (Brisighella), Central (Rome) and Southern (Pozzuoli) Italy. Regularly performed quality controls between laboratories assured comparability of data. A statisitically significant difference of mean serum cholesterol and triglyceride levels was observed for most age-groups in the 3 different areas, lower values being found in the southern population as compared to the central and northern ones. These results support previous findings and the thesis that large differences in blood lipid levels may still exist even within the same country and that they at least in part may be culturally determined in connection with different dietary habits.
