[Combined administration of futraful and esquinone for the treatment of advanced ovarian cancer].

Thirty-seven patients with advanced ovarian cancer were treated with futraful (FT-207) and esquinone (CQ). These patients had infiltrated tumors to the pelvic and peritoneal cavity and also distant metastasis, so they were not operated perfectly. There were evaluable 12 patients out of 37. FT-207 + CQ is an effective drug combination against advanced ovarian cancer with an overall response rete of 40.9%, a complete response rate of 4.5% (1/22), partial response rate of 36.4% (8/22), no change rate of 27.3% (6/22) and progressive rate of 31.8% (7/22). Patients with adenocarcinoma showed a response rate of 66.7% (8/12). According to the stage, the patients in stage III showed response rate of 50.0% (6/12), in stage IV showed a response rate of 37.5% (3/8). The total dose of FT-207 used in the study was 38.3g in the progressive disease group and 177.6 in the good response group. The group of long term administration of FT-207 and CQ included many patients with good response.

Regulation by human chorionic gonadotropin of protein kinases in rabbit endometrium.

The effect of human chorionic gonadotropin (CG) on protein kinase levels has been studied in rabbit endometrium. A good correlation was observed between alterations of total protein kinase activity and DNA content in the tissue. The level of type I cyclic AMP-dependent protein kinase had slightly increased by 6 h after human CG treatment, and then decreased later. In contrast, the type II enzyme gradually increased after 3 days. The increase of the activity at 7 days was mainly due to that of type II enzyme, and the ratio of type II to type I enzyme increased. The activity pattern at the later stage closely resembled that of treatment with progesterone.

Effect of ACTH infusion on induction of uterine contraction in prolonged human pregnancy in association with endocrine function.

The role of hormonal interactions in the mechanism initiating labor in women was studied. The serum levels in women was studied. The serum levels of cortisol, progesterone, estradiol estriol, human chorionic somatomammotropin, and prolactin were measured simultaneously before, during and after infusion of ACTH at a rate of 50 microgram/h in 100 ml of physiological saline or infusion of physiological saline without ACTH (as control) at a rate of 100 ml/h into 14 pregnant women of 10 days or more beyond term. In five (group A) of the 10 women infused with ACTH. uterine contractions were induced within 5-15 min after beginning of ACTH infusion. Although the serum progesterone levels in group A were slightly, but not significantly, lower than those in the other five women (group B) in whom uterine contractions were not induced, the ratio of progesterone to estriol was 13/1 in group A and 18/1 in group B, respectively (P less than 0.05). The endocrine levels in group A may be favorable for the initiation of uterine contraction. The serum cortisol levels in group A were higher, but not significantly higher, than those in group B after ACTH infusion. And the serum estriol levels 6 h after ACTH infusion were significantly lower than control levels before ACTH infusion (P less than 0.01 in group A, P less than 0.05 in group B). To the contrast, in all the four control women infused with physiological saline only (group C), neither uterine contractions nor changes in the serum levels of ACTH and cortisol were found. This work may show that ACTH infusion induced uterine contractions by increased corticosteroids via placenta in lower ratio of progesterone to estrogen but it does not provide any precise information on the mechanism of initiation of human labor.

Effect of progestogen on glycogen metabolism in the endometrium of infertile patients during the menstrual cycle.

The glycogen content and glycogen synthetase and glycogen phosphorylase levels were studied in endometrial samples obtained from 19 normal and 37 infertile patients during the menstrual cycle before and after administration of progestogen. Each of the above groups received the progestogen Lyndiol (lynestrenol, 5.0 mg, and mestranol, 0.15 mg) daily for 7 days during the follicular phase of the menstrual cycle. In both groups an increase in the endometrial glycogen deposition and an increase in glycogen synthetase enzyme levels were seen. When the administration of progestogen was started on day 7 after ovulation, during the luteal phase, the glycogen content of endometrial tissue from infertile patients increased significantly; no change was found in endometrial samples from normal patients. No difference was found in the serum progesterone levels of normal and infertile patients in the midsecretory phase of menstrual cycle, and Lyndiol reduced the serum level of progesterone to approximately that found during the follicular phase in untreated normal women. These studies suggest that the proliferative endometrium of infertile patients may be less stimulated by ovarian estrogen than is normal endometrium, whereas endometrial tissue obtained from both groups during the luteal phase responded similarly to progesterone in glycogen synthesis and storage.

Specific regulation by steroid hormones of protein kinases in the endometrium. 1. Alteration by estrogen and progesterone in levels of protein kinases in rabbit endometrium.

The alteration in activities of multiple protein kinases has been studied in the endometrium of a rabbit treated with estrogen and progesterone. The administration of estrogen or progesterone to the castrated rabbit resulted in a remarkable increase of total activity in the cytosol fraction of the endometrium. The administration of estrogen caused an increase of type I adenosine-3',5'-monophosphate-dependent (cAMP-dependent) protein kinase and a slight decrease of type II cAMP-dependent protein kinase. In contrast, the treatment with progesterone after priming administration of estrogen brought about an increase of type II cAMP-dependent protein kinase and a decrease of type I cAMP-dependent protein kinase. Therefore, the activity ratio of type II to type I decreased by estrogen and increased by progesterone. The simultaneous administration of cycloheximide abolished the stimulatory effect of respective hormones on the level of each protein kinase. The activity profile of protein kinases on DEAE-cellulose column after ovulation caused by the administration of human chorionic gonadotropin to a non-castrated rabbit was similar to that of the rabbit treated with progesterone. The results presented demonstrate the specific regulation by the steroid hormones of de novo synthesis of protein kinases in the target organ.

Specific regulation by steroid hormones of protein kinases in the endometrium. 2. Alteration in levels of protein kinases in human endometrium during the menstrual cycle.

The alteration in the levels of multiple protein kinases has been studied in human endometrium during the menstrual cycle. Protein kinases were examined in the cytosol fraction and separated by DEAE-cellulose column chromatography. The level of type I adenosine 3',5'-monophosphate-dependent (cAMP-dependent) protein kinase was higher than that of type II cAMP-dependent protein kinase at the proliferative phase. At the late secretory phase, type I cAMP-dependent protein kinase markedly decreased, whereas type II cAMP-dependent protein kinase remained at the same level. Therefore, type II cAMP-dependent protein kinase was exclusively predominant at this phase. cAMP-dependent and cAMP-independent protein kinases with protamine increased more at the late secretory phase than at the proliferative phase. The results presented demonstrate a specific alteration in protein kinase activities during the menstrual cycle and are compatible with those obtained in rabbit endometrium (the preceding paper).

Cell-mediated immunity in human pregnancy: Changes in lymphocyte reactivity during pregnancy and postpartum.

The function of thymus-dependent lymphocytes (T lymphocytes) was studied in women during pregnancy and labor and postpartum by evaluating the blastogenesis of peripheral lymphocytes, which were stimulated with phytohemagglutin-P (PHA-P) in both whole-blood semimicroculture and purifed lymphocyte culture. Data from 353 random samples (203 women) and 50 serial specimens from 10 women revealed that PHA-P induced-lymphocyte blastogenesis was significantly (p less than 0.005) reduced during pregnancy and labor but rapidly returned to normal several days after artificial termination in the early stage of pregnancy as well as after full-term delivery. These results indicate that the T-lymphocyte function in maternal peripheral blood is depressed by causes related to pregnancy. It seems very likely that depressed T-lymphocyte function during pregnancy is caused by inhibitory factors in the blood plasma derived from the feto-placental unit. Questions relating to the inhibitory factors in maternal plasma are discussed.

Rapid assays of urinary estriol in pregnant women.

Two devices designed for rapid assay of urinary estriol, the E3 KIT and E3 HAIR KIT, were evaluated using urine samples taken from pregnant women. Additionally, the clinical application of these devices in pregnancy management was examined. Correlations were high between an established method and the E3 KIT method (r = 0.819, p less than 0.001) and between the E3 HAIR KIT method and the E3 KIT method ( r = 0.936, p less than 0.01). E3 HAIR KIT assays showing E3 values of less than 20 microgram/ml (positive at 100-fold dilution of the urine) after the 36th week of pregnancy suggested fetoplacental dysfunction and indicated that more detailed tests were required. The E3 KIT, which provided rapid quantitative assay of urinary estriol, was capable of measuring 10 samples in approximately 3 hours. The E3 HAIR KIT provided semiquantitative assays of 10 samples in 2 hours. The speed, simplicity, and accuracy of the E3 HAIR KIT indicates its high clinical value as a prospective large-scale screening method for fetoplacental function.
