Action of tamoxifen on folliculogenesis in the menstrual cycle of infertile patients.

Daily estimations of follicle-stimulating hormone, luteinizing hormone, prolactin, estradiol, and progesterone were made in the serum of eight infertile patients from day 1 through the follicular phase during menstrual cycles before and after tamoxifen therapy. Tamoxifen therapy was found to shorten the follicular phase from 15.4 +/- 0.8 days (mean +/- standard error of the mean) to 14.0 +/- 0.6 days (difference not significant) and to lengthen the luteal phase from 12.8 +/- 0.4 days to 14.1 +/- 0.8 days (P less than 0.05). The mean estradiol concentration in the eight patients during tamoxifen treatment cycles rose on day 8 (3 days after starting tamoxifen treatment) and increased significantly (P less than 0.05) from day 10 to midcycle. The integrated follicular phase estradiol concentration in the tamoxifen treatment cycle increased to 2450.1 +/- 208.1 pg/ml/cycle, and was significantly higher (P less than 0.025) than that in the nontreatment cycle. In contrast, the concentrations of follicle-stimulating hormone, luteinizing hormone, and prolactin during the follicular phase and at the midcycle peak of tamoxifen treatment cycles were not significantly different from those of the nontreatment cycle. These results suggest that the mechanism of tamoxifen in improving folliculogenesis may involve a direct action on the ovary without intervention of the hypothalamic-pituitary system.

Calmodulin and Ca2+- and calmodulin-dependent protein kinase in rat anterior pituitary gland.

Calmodulin and Ca2+- and calmodulin-dependent protein kinase were identified in the rat anterior pituitary gland. The concentration of calmodulin was 1.18 +/- 0.11 microgram/mg protein (n = 7) in the cytosol fraction. The calmodulin of the anterior pituitary gland co-migrated with brain calmodulin on sodium dodecyl sulfate polyacrylamide gel electrophoresis. The Ka value of the partially purified enzyme for Ca2+ was 3.3 microM in the presence of 0.30 microM calmodulin. Trifluoperazine and chlorpromazine, calmodulin-interacting agents, inhibited enzyme activity, with Ki values of 1.3 and 2.6 X 10(-5) M, respectively. The enzyme was resolved into two peaks of activity, with sedimentation coefficients of 5.5 S and 16.5 S, by sucrose density gradient centrifugation. At least nine proteins were phosphorylated by the enzyme in a Ca2+- and calmodulin-dependent manner. In light of these results, the possibility that calmodulin and the calmodulin-activatable protein kinase system are involved in the mediation of the Ca2+ effect on hormone release from the anterior pituitary gland must be given consideration.

Regulation by estrogen and progesterone of protein kinases in rabbit myometrium.

We report on hormonal regulation of protein kinases in the rabbit myometrium. The injection of estrogen or progesterone produced 5.2 +/- 0.7- or 4.7 +/- 0.4-fold increases, respectively, of the total activity when histone was used as the substrate in the cytosol fraction of the tissue. The analysis of the activity by DEAE-cellulose column chromatography revealed that estrogen had no effect on increases in the levels in both types I and II cAMP-dependent protein kinases, on the basis of mg protein, while progesterone produced a 50% decrease in type I cAMP-dependent protein kinase and a 50% increase in type II cAMP-dependent protein kinase. Thus, the activity ratio of types II to type I did not change by treatment with estrogen and increased from 2.8 to 7.9 with progesterone administration. As the activity patterns by treatment with human chorionic gonadotropin closely resembled those seen with progesterone, our findings in case of the latter hormone are probably valid. The results indicate that the levels of protein kinases in the myometrium are altered in response to estrogen and progesterone and may possibly be involved in the physiological functions of the tissue.

Unconjugated estradiol, estriol and total estriol in maternal peripheral vein, cord vein, and cord artery serum at delivery in pregnancies with intrauterine growth retardation.

The levels of unconjugated estradiol (E2), estriol (E3) and total (conjugated plus unconjugated) E3 in maternal vein serum during labor, cord vein serum, and cord artery serum were measured in normal singleton and twin pregnancies with appropriate for dates babies (AFD) and with light for dates babies (LFD). The mean level of total E3 in the maternal vein serum in singleton pregnancy was significantly lower in the LFD group than in the AFD group, but no differences were seen in the mean levels of unconjugated E2 or E3 between the groups. The concentration of unconjugated E2 in the maternal vein serum was significantly higher in the twin group with a large placenta than in the singleton group with a smaller placenta, while the concentration of total E3 in the case of twin pregnancy with LFD was lower than that in singleton pregnancy with AFD but not significantly. No difference in the concentration of total E3 was observed between the cord vein serum and cord artery serum. The present data suggest that the total E3 level in maternal vein serum may be used in evaluating fetal states such as intrauterine growth retardation.

Hormone treatments and pregnancy alter myosin light chain kinase and calmodulin levels in rabbit myometrium.

Myosin light chain kinase activity and the amount of calmodulin were measured in rabbit myometrium after the rabbits had been treated with oestrogen, progesterone and human chorionic gonadotrophin, and also during pregnancy. Injections of oestrogen and progesterone produced significant increases of of 2.8- and 2.1-fold in the enzyme activity/mg protein and of 1.4- and 1.2-fold in the calmodulin content/mg protein respectively. The increases seen with oestrogen treatment were inhibited by cycloheximide administration, suggesting that protein synthesis is involved in the process. Calmodulin increased about twofold in the particulate fraction under conditions of oestrogen and progesterone treatments and the raised level of calmodulin detected in the cytosol fraction was not due to translocation from the particulate fraction. During pregnancy the specific enzyme activity significantly increased in comparison with the values seen in samples from the non-treated rabbits. These findings suggest that myosin light chain kinase and calmodulin in the myometrium are under hormonal regulation and may play a role in expulsion of the fetus at the latter phase of pregnancy.

[Immunochemotherapy of recurrent and advanced uterine and ovarian cancer using cisplatin].

A combination immunochemotherapy regimen containing cisplatin (20 mg, days 1-5), peplomycin (20 mg, days 2, 9, 16), +/- vinblastine (5 mg, days 1, 2), and picibanil (3-5 KE/week) was performed in twelve patients with advanced or recurrent uterine and ovarian cancers under intravenous hyperalimentation (IVH), except one patient receiving peplomycin by a continuous infusion method using IVH bag (10 mg/day for 5 days). This regimen was repeated every three weeks. Five (71.4%) of seven evaluable patients showed partial response. No patients yielded the complete disappearance of disease. No severe and lethal pulmonary or renal dysfunction occurred and all was well tolerated. In a regimen without vinblastine, myelosuppression, especially thrombocytopenia, occurred later compared to the regimen including vinblastine.

[Mixed mesodermal tumor of the uterine body].

During the 6-year period of 1976 to 1981, we studied 6 cases of mixed mesodermal tumor of the uterus. The incidence of occurrence of the tumor was 0.4% of all the gynecological inpatients, 1.4% of all the uterine malignancies, and 9.5% of the uterine body malignancies, and was higher than those reported by others. The patients were aged 49-75 years (mean 64 years), and 5 out of 6 patients were postmenopausal. Four patients had heterogeneous tumors (3 rhabdomyosarcomas and 1 chondrosarcoma). Two of the 6 patients are now living 34 and 14 months after treatment without clinical evidence of recurrence, respectively. Histologically, one of them showed only benign glandular structures as a component of epithelial elements, and the other one had no epithelial element. These findings suggest that the malignancy of the epithelial components may be one of the most important prognostic factors in the mixed mesodermal tumor of the uterus.

[Fundamental and clinical studies on T-1982 (cefbuperazone) in the field of obstetrics and gynecology].

Fundamental and clinical studies on T-1982 (cefbuperazone), a new cephamycin antibiotic, were carried out in the field of obstetrics and gynecology, and the following results were obtained. In a fundamental study, T-1982 was given to 7 patients with postpanhysterectomy at a dose of 1 g by intravenous drip infusion, and T-1982 concentrations in uterine artery and elbow vein serum and each uterine tissue (fallopian tube, ovary, endometrium, corpus uteri, cervix uteri and portio) were serially determined. The concentration in the endometrium (though the range of values was wide) was found higher than that in the other tissues. In a clinical study, T-1982 was given to 5 patients; 2 patients with parametritis and each one patient with right salpingitis, pyometra and metroendometritis. The efficacy rate was 80%. No side effects nor remarkable abnormalities in hematology, urinalysis and renal and hepatic function were observed during the therapy.

Primary malignant melanoma of the vulva associated with pregnancy: clinical, light and electron microscopic observations.

A case of primary malignant melanoma of the vulva associated with pregnancy in a 31-year-old patient is reported. The primary tumor was first found at week 14 of pregnancy and biopsied on delivery at week 41 of pregnancy. Microscopically, the depth of invasion was Level V. Radical vulvectomy and bilateral inguino-femoral lymphadenectomy were performed and the right inguinal lymph node was positive for metastatic tumor. Chemoimmunotherapy with DTIC, ACNU, VCR and OK432 was undertaken as treatment for this malignant melanoma. Moreover, a comparison of scanning and transmission electron microscopic studies of the original tumor was carried out. This patient is now living 23 months after diagnosis without clinical evidence of recurrence.

Malignant degeneration of benign cystic teratoma of the bilateral ovaries: adenosquamous carcinoma in the right tumor and squamous carcinoma in the left tumor.

An extremely rare case of malignant degeneration arising separately in benign cystic teratoma of the bilateral ovaries is reported in a 51-year-old woman. The solid part of the right tumour consisted basically of squamous cell carcinoma, but had a small area of adenocarcinoma, whereas the solid part of the left tumour consisted of squamous cell carcinoma. It was found that in both the cystic teratomas, squamous cell carcinoma had developed separately from the columnar epithelium by a process of squamous metaplasia and malignant change in situ. It is suggested that these malignant degenerations of benign cystic teratoma of the bilateral ovaries had occurred independently due to local carcinogenic agents in the constituents of each cyst content.

Tamoxifen in the treatment of infertility associated with luteal phase deficiency.

A group of 17 patients with suspected luteal phase deficiency was treated with tamoxifen. Tamoxifen therapy was found to lengthen the luteal phase in all patients and resulted in pregnancy in 6 of 17 patients. The integrated luteal phase progesterone (P) concentration in the nontreatment cycle of seven patients was significantly lower (P less than 0.01) than that of five normal women. Therapy with tamoxifen increased the P concentration to 186.0 +/- 24.4 ng/ml/cycle (mean +/- standard error of the mean), i.e., twice that of the control cycle. The mean estradiol (E2) concentration at the midcycle peak was about twice that observed during the nontreatment cycle. The glycogen content of the endometrial tissue at the midluteal phase in the tamoxifen cycle was significantly higher (P less than 0.025) than that of endometrial tissue in the nontreatment cycle, indicating improvement of the endometrial function.

Dissociation of alterations in levels of myosin light chain kinase and cyclic AMP-dependent protein kinase in rabbit myometrium during pregnancy.

The alteration in levels of myosin light chain kinase and cyclic AMP-dependent protein kinase was studied in rabbit myometrium during pregnancy. The specific activity of myosin light chain kinase was 160.9 +/- 2.7, 128.1 +/- 3.5, and 126.5 +/- 4.4% at the 10th, 20th and 28th days after pregnancy, respectively, in comparison with the value seen in samples from non-pregnant rabbits. On the other hand, the specific activity of cyclic AMP-dependent protein kinase gradually decreased to 75.9 +/- 1.7, 58.7 +/- 3.5 and 36.9 +/- 1.9% of that of the control at each time with the increase in the time of gestation. The protein kinase inhibitor was not involved in the decrease in cyclic AMP-dependent protein kinase. These results suggest that the dissociation of the dynamic states of the two enzymes may make it possible for the myometrium to efficiently respond to contraction in the last stage of pregnancy.

Pregnancy following combined therapy with thyroid hormone and bromocriptine in a patient with amenorrhea-galactorrhea due to primary hypothyroidism.

Combined therapy with thyroid hormone (desiccated thyroid) and bromocriptine (CB-154) was applied in a 29-year-old patient with amenorrhea-galactorrhea due to primary hypothyroidism. Two months after commencing administration of desiccated thyroid, the elevated serum level of TSH returned to the normal range while that of PRL remained within the supranormal range (from 134 ng/ml to 86.7 ng/ml). However, it fell to the normal range 2 weeks after administration of CB-154 with thyroid hormone. Subsequently, the galactorrhea completely ceased and ovulatory menstruation resumed with 3 months, and conception was achieved directly after that menstrual period. Only desiccated thyroid was administered during pregnancy and the patient delivered a male infant weighing 2,800 g without difficulty.

Endocrine profile of an ovarian androblastoma.

The endocrine profile of a 16-year-old girl with an androblastoma of the left ovary is presented. Calculated ratios of steroid hormones between the intraoperative peripheral vein blood, the left ovarian vein blood, and the left ovarian tumor fluid were progesterone, 1:10.2:39.5; 17-hydroxyprogesterone, 1:18.7:64.7; dehydroepiandrosterone (DHEA), 1:10.4:35.6; androstenedione (A), 1:24.4:92.3; testosterone (T), 1:18.6:69.2; and estradiol (E2), 1:11.0:26.3. The peripheral levels of hormones before left salpingo-oophorectomy were T, 7.5 ng/ml; DHEA, 19.9 ng/ml; A, 12.3 ng/ml; and cortisol, 11.4 micrograms/dl. Corresponding levels at 14 days after surgery were (0.75 ng/ml; 5.84 ng/ml; 1.94 ng/ml; and 15.6 micrograms/dl, respectively. Preoperatively, an elevated basal level of luteinizing hormone (LH) and a normal basal level of follice-stimulating hormone (FSH) (high LH:FSH ratio) were found. These data suggest that 1) androgens from the androblastoma are responsible for virilization despite aromatizing enzyme activities within normal limits, and 2) both the delta 5 and delta 4 pathways are involved in the biosynthesis of androgens, with that of delta 5 being predominant.
