Histologic, immunofluorescent, and antinuclear antibody findings in PUVA-treated patients.

Twenty-two patients with psoriasis were treated with PUVA therapy for over two years. Skin biopsies from sun-exposed skin were evaluated before, during, and after PUVA therapy. No statistically significant epidermal changes were found when looking for hyperkeratosis, atrophy, melanin, or melanocyte numbers. No dyskeratosis was found. Dermal changes, including alterations in elastic tissue and collagen, were not statistically significant. Amyloid deposits were absent. The amount of mucin deposition was found to correlate with the length of PUVA therapy. In four patients, the mucin disappeared after withdrawal of PUVA, suggesting that this change is reversible.

The leukocyte adherence-inhibition assay as a diagnostic test for multiple sclerosis: study of a substance found in MS blood.

We have modified and adapted two well-accepted cancer immunology research techniques to study blood leukocyte phenomena of MS patients. The LAI test of Halliday and Miller and the 3 molar potassium chloride (3M KCl) tumor antigen extraction technique of Dean and McCoy were adapted to extract from pooled MS whole blood an MSRM. The LAI test results of 53 of 58 MS patients (91%) were considered positive, but only three of 75 control subjects (4%) were positive when tested against the MSRM. This indicates that patients with MS had a greater specific reactivity to the MSRM than did the control subjects (healthy individuals and patients with disease other than MS). The specificity and reproducibility of this reaction were tested with materials prepared from various malignant diseases. Sensitized leukocytes showed consistently higher reactivity to antigen extracts prepared from corresponding types of tumors than to extracts prepared from tumors of other histological types. Our results indicate that (1) the LAI test is able to corroborate the neurological diagnosis of MS and (2) there is a blood constituent found only in the MS patient.

Vaginal transposition of the ovary in primates (Papio cynocephalus and Macaca arctoides).

Experimental data indicate that the time and mechanism of ovulation play a significant role in some cases of infertility and in a broad range of reproductive abnormalities. In order to establish a new experimental model for the study of these phenomena in primates, transposition of the ovary from the pelvis into the vaginal fornix was attempted in 2 species of monkeys: (a) Macaca Arctoides and (b) Papio Cynocephalus. Particularly in the latter species, ovarian relocation into the anterior and the posterior vaginal fornices proved feasible. Most ovaries continued functioning in their new location during the 2-5 months of the follow-up, despite macroscopic and microscopic evidence of infection. Post mortem examination of one of the experimental animals several years after the surgical procedure revealed the presence of an intact functioning ovary still in close contact with the vaginal fornix.

Cardiac conduction abnormalities produced by chronic alcoholism.

While conduction disturbances and arrhythmias are seen frequently in alcoholic cardiomyopathy, the specific relationship of these changes to ethyl alcohol has been unclear. To investigate the long-term effects of ethanol upon cardiac conduction, alcoholism was induced in 11 male mongrel dogs for 7 to 33 (mean 14.4) months by feeding up to 36 per cent of total daily calories as ethanol while adequate nutrition was maintained. His and left bundle branch electrograms in the intact anesthetized animals were recorded along with high-speed, high-frequency ECG's. While resting left ventricular pressures, volumes, and stroke outputs were normal, H-Q time was prolonged in the alcoholic animals drinking for longer than one year (35 +/- 3 msec., normals 26 +/- 1 msec.-P less than 0.001). QRS widening (to 80 +/- 4 msec.) was also evident after one year as compared with normals (62 +/- 2 msec.-P less than 0.001), and both H-Q and QRS alterations correlated with duration of intake. These changes were less after shorter ingestion periods, could not be reproduced in normals by acute ethanol infusion, and were not associated with ventricular hypertrophy, inflammation, or necrosis. No abnormalities of atrial conduction were noted. Morphologic correlates of the conduction abnormalities included accumulation of Alcian Blue-positive interstitial material as well as dilatation and localized swelling of the nonspecialized region of the intercalated discs in ventricular muscle and Purkinje fibers. Thus, prolonged ethanol intake in the absence of evident malnutrition resulted in demonstrable intraventricular conduction abnormalities and morphologic alterations which were related to duration of ingestion, consistent with a cumulative toxic effect of ethanol.

Factor analysis of the DSM-III personality disorder clusters: a replication.

The authors describe the replication of Kass et al.'s study where a factor analysis of scaled ratings of DSM-III personality disorders yielded groupings similar to those described in DSM-III. In this replication the authors used scaled ratings from a nationwide sample of psychiatrists on 358 patients. The authors conclude that this replication lends additional support for the DSM-III (now DSM-III-R) approach towards grouping personality disorders into three (or four) clusters.

A comparison of clinical and self-report diagnoses of DSM-III personality disorders in 552 patients.

This report examines the relationship between clinicians' diagnoses of personality disorder and self-report diagnoses of personality disorder obtained from the Personality Diagnostic Questionnaire (PDQ). The results from 552 patients showed general lack of agreement between clinical and self-report diagnoses of DSM-III personality diagnoses. The best agreement obtained was for Borderline Personality Disorder: k = 0.46, and r = .51 for scaled ratings. Possible sources of disagreement including failings of the self-report questionnaire, difficulties in relying upon patients' self-reports, lack of reliability of clinical diagnoses of personality, and possible inherent lack of reliability of several of the DSM-III personality disorders are discussed.

Energy deficits associated with nasogastric feeding in patients with burns.

This study was designed to identify whether significant energy deficits occur when feedings are (1) withheld perioperatively and (2) withheld because of gastric ileus. We studied patients with more than 20% total body surface area burns, who had survived at least one surgery, and who were intubated within 3 days of admission. Energy needs and energy received from enteral feedings were summed across the period during which the patients underwent surgical procedures. The difference between energy needed and energy received was divided into that caused by feedings being withheld perioperatively and that caused by feedings being withheld because of a gastric ileus. Twenty-five patients (9 women and 16 men) were included in the study. The total energy deficit was a mean of 15,969 kcal (31.6% of 50,546 kcal needed); 58% of the energy deficit (9264 kcal of the 15,969 kcal deficit) was caused by feedings being withheld perioperatively and 42% of the energy deficit (6705 kcal of the 15,969 kcal deficit) was caused by feedings being withheld because of gastric ileus. The withholding of feedings perioperatively and because of gastric ileus were both significant contributors to energy deficits. Future studies are necessary to determine whether energy needs being more closely met would result in improved outcomes for these patients.

Critical care nurses are increasingly involved in situations of ethical conflict; yet, they are often left out of consequential decision making. This article discusses ways critical care nurses can become more deeply involved in actual decision making in ethical situations.
