Screening for mt-DNA mutations in optic neuritis of unknown cause.

OBJECTIVE: To investigate mitochondrial DNA (mt-DNA) mutations in optic neuritis of unknown cause (ONUC) and to assess the practical value of mt-DNA mutation detection in etiologically and differentially diagnosing ONUC. METHODS: Thirty patients with ONUC were screened for mt-DNA mutations of nt11778, nt3460 and nt15257 by using SSCP, mutation-specific primer PCR and sequencing. RESULTS: mt-DNA mutations were found in twelve of thirty ONUC patients. All of the mutations were at nt11778 position, but no one at nt3460 and nt15257. CONCLUSIONS: Forty percent (12/30) of ONUC patients were caused by an mt-DNA mutation. Combined with other routine measures, screening for mt-DNA mutations in ONUC patients is of great significance in diagnosing ONUC etiologically and differentially.

Intravesical immunotoxin as adjuvant therapy to prevent the recurrence of bladder cancer.

OBJECTIVE: To assess the intravesical application of immunotoxin as adjuvant therapy to prevent recurrence after tumor resection in bladder cancer patients. METHODS: An anti-human immunotoxin against bladder carcinoma, BDI-1-RT, was prepared and its in vitro targeting cytotoxicity estimated. The immunoreactivity of BDI-1-RT with human bladder cancer tissue of different grades and stages was detected by immunohistochemical analysis. After safety test, intravesical administration of BDI-1-RT was performed in 31 patients while mitomycin C (MMC) was used in 36 patients serving as a control group. The recurrence rates and side effects in both groups were recorded. In addition, the development of human anti-mouse antibodies (HAMA) was determined by ELISA, to assess the potential safety of this immunotoxin. RESULTS: In our study, BDI-1-RT had immunoreactivity with 81.6% of bladder transitional cell carcinomas. The immunoreactivity of BDI-1-RT correlated with tumor grade. High-grade carcinoma had stronger staining than low-grade (P < 0.05). There was no significant difference between the BDI-1-RT group (10%) and MMC group (19.3%) in recurrence rate (P > 0.05). Side effects, including systemic and local, were more frequent in the MMC group (11 of 36 patients versus 2 of 31, P < 0.05). HAMA was not detected in any of 7 patients. CONCLUSION: Immunotoxin may have considerable potential in the prophylaxis of bladder transition cell carcinoma.

[Detecting embryo sex by fluorescence in-situ hybridization in preimplantation genetic diagnosis].

OBJECTIVE: Applying fluorescence in situ hybridization(FISH) in preimplantation gender diagnosis. METHODS: Ovarian hyperstimulation was performed in 2 hemophilia A carriers and 2 patients with Y chromosome abnormality. Embryo sex was identified by single blastomere FISH after embryo biopsy. Female embryos were transferred into uterus. RESULTS: A total of 110 cumulus-oocyte complex were retrieved in 5 treatment cycles. Among them, 68.2% showed normal fertilized pronuclei and 55 embryos were available for embryo biopsy. The success rate of biopsy was 85.5%, with further cleavage rate of 61.7%. In FISH procedure, one cell was lost during fixation, led to 97.9% fixation rate. Totally, 18 female embryos were diagnosed and 16 were transferred into uterus. Two clinical and one biochemical pregnancies were achieved. The diagnosis was confirmed by the following analysis of aminocyte and embryonic buds after embryo reduction respectively. CONCLUSIONS: FISH is an efficient and accurate technique for determination of the sex of human preimplantation embryos. Selective abortion and birth of affected child can be avoided by preimplantation gender diagnosis.

[Treatment of nonunion in childhood by allogeneic decalcified bone graft].

OBJECTIVE: To investigate the effect of allogeneic decalcified bone graft in the treatment of nonunion in children. METHODS: From April 1990 to September 1997, 17 children with nonunion were adopted in this study. Among them, there were 10 boys and 7 girls, the age ranged from 2 to 13 years. The allogeneic decalcified bone graft taken from fresh corpse by aseptic manipulation were used to repair 3 cases of congenital nonunion and 14 cases of acquired nonunion. RESULTS: All cases were followed up 2 to 9 years after operation, 9 cases were cured successfully and 7 cases were cured after twice operations. One case of congenital pseudoarthrosis of the tibia was operated twice and there were callus grown half years after the second operation, but reoccurred after one year. Although there were 1.5 cm to 3 cm shortening deformity of extremity including another 2 cases of congenital pseudoarthrosis of the tibia and 5 cases of nonunion caused by chronic osteomyelitis, the function of joint was normal. CONCLUSION: Treatment of nonunion in children by allogeneic decalcified bone graft is a valuable technique in clinical practice. It is characterised by high capacity of osteoinduction, low antigenicity, rapid bony union, plentiful source of bone graft and convenient utilization.

[Effect of lost bite force on the expression of bFGF in rat molar periodontal ligament].

OBJECTIVE: To observe the dynamic change of the expression of basic fibroblast growth factor (bFGF) in rat molar periodontal ligament(PDL), combined with other experiments to probe into the relation of the expression between bFGF and type I collagen mRNA. METHODS: Animal model was established by extracting the left maxillary molars. Immunohistochemistry was applied. RESULTS: The expression of bFGF obviously increased from the first day to the second week (P < 0.05), Came back to that of normal bite force group at the third week. There were strong negative relative relationship between the expression of bFGF and that of type I collagen mRNA. CONCLUSION: The expression of bFGF increased under lost bite force in rat molar PDL. The effect of bite force on the expression of PDL type I collagen mRNA is closely related to the expression of bFGF.

[Comparative study of technique of laser welding ceramo-alloy by different gap distance].

OBJECTIVE: To investigate the quality of laser welded non-precious base alloy by different gap distance and its clinic probability. METHODS: Laser welded ceramo-alloy CW-PA specimens by different gap distance of 0.0 mm, 0.25 mm, 0.5 mm, Tested joined sample's UTS, microhardness; took SEM examination on its tensile fracture surface and metallographic examination on longitudinal cross section. RESULTS: The study showed that UTS of Laser welded CW-PA by different gap distance of 0.0 mm, 0.25 mm, 0.5 mm is (446.05 +/- 82.50) MPa, (485.97 +/- 62.35) Mpa, (503.32 +/- 65.79) MPa respectively, approach to original rod (421.53 +/- 21.78) MPa (P > 0.05). Microimage at tested samples of gap distance at 0.0 mm, 0.25 mm are good. CONCLUSION: It revealed that 1. Laser welding specimen should be contacted as close as possible 2. The gap distance should be less than 0.5 mm, 0.25 mm is better, and easy to handle.

[Effect of yangxue antai granule in preventing and treating habitual abortion and its effect on short life T-suppressor cell].

OBJECTIVE: To study the influence of T-suppressor cell (Ts cell) on habitual abortion (HA) and to observe the regulatory effect of Yangxue Antai Granule (YXATG) on Ts cell and its effect on fetus preservation. METHODS: Ts cell activity of 56 pregnant women with HA history was tested with method of shot life Ts cell activity (MTT method) during early pregnancy, and were followed up in middle and late pregnancy in 20 cases of them. All the 56 patients received YXATG treatment. Groups of normal early pregnant, normal non-pregnant and non-pregnant women but with history of habitual abortion were involved in this study as control. RESULTS: Ts cell activity in early pregnancy was lower than that of middle and late pregnancy (P < 0.01, P < 0.001), and also lower than that of the control groups (P < 0.01, P < 0.001). The successful rate of fetus preservation of YXATG was 98.2%, and 98.0% of the cases got a normal delivery. CONCLUSION: Low Ts cell activity might be the immunologic etiology of HA. The fetus preservative effect of YXATG might be due to its regulatory effect on Ts cell.

[The clinical significance of true insulin and proinsulin levels in subjects with different glucose tolerance].

OBJECTIVE: To examine the levels of serum true insulin (TI) and proinsulin (PI) in individuals with different glucose tolerance and to assess its clinical significance. METHODS: The levels of true insulin and proinsulin during oral glucose tolerance test (OGTT) were determined with specific BA-ELISA in 135 normal glucose tolerance (NGT) and 86 impaired glucose tolerance (IGT) subjects as well as 101 type II diabetes mellitus (DM) patients. RESULTS: The fasting TI levels showed no significant difference (P > 0.05) among the groups of NGT, IGT and type II DM, but the immunoreactive insulin (IRI) concentration increased significantly in type II DM patients as compared with that in NGT (P < 0.01). Both of the fasting PI levels and the ratio of PI/(PI + TI) were significantly higher in type II DM patients as compared with those in NGT or IGT. The area under the curve of TI (AUC(TI)) was significantly higher (P < 0.05) in IGT than NGT and type II DM. Obese subjects had higher TI levels (P < 0.01) and lower insulin sensitivity (P < 0.05) than those in non-obese, while the ratio of PI/(PI + TI) and AUC(TI) was not significantly different statistically (P > 0.05) between the obese and non-obese subjects. CONCLUSION: Type II DM patients present hyperproinsulinemia rather than hyperinsulinemia and have higher ratio of PI/(PI + TI) as compared with NGT and IGT subjects. Higher TI level and significant insulin resistance were observed in obese subjects, but there is no difference of the PI/(PI + TI) ratio. The fasting PI level and PI/(PI + TI) ratio tend to be the indexes of beta-cell dysfunction.

[The relationship among proinsulin and true insulin to coronary artery disease].

OBJECTIVE: Whether proinsulin (PI) and true insulin (TI) are independently related to coronary artery disease (CAD). METHODS: 54 patients with CAD diagnosed by coronary angiography and/or intravascular ultrasound, 37 patients without CAD by coronary angiography and/or intravascular ultrasound and 23 healthy volunteers. All subjects had oral glucose tolerance test after an overnight 14 h fast. PI and TI concentrations were detected at different phases by BA-ELISA in which PI cross -reactivity is less than 0.1%. RESULTS: Fasting and post-glucose loading PI and TI levels in both CAD and non-CAD groups significantly increased when compared with the control group (P < 0.05), after adjustment for age, body mass index and waist hip ratio, all statistically significant associations weakened or disappeared among three groups; but there was no significant difference of all the above variables between CAD group and non-CAD group (P > 0.05). CONCLUSION: PI and TI, whether fasting or post-glucose loading, may not be independently related to CAD.

[Using testicular sperm to provide fertility for azoospermia men].

OBJECTIVE: To investigate the effect of intracytoplasmic sperm injection (ICSI) on providing fertility for obstructive and non-obstructive azoospermia men after testicular spermatozoa extraction (TESE). METHODS: TESE and ICSI were performed at the day of ovum pick up. RESULTS: TESE and ICSI were performed successfully for 11 cycles in obstructive azoospermia men; The fertilization rate (FR) and clinical pregnancy rate (PR) were 67.0% (69/103) and 18.2% (2/11), respectively. In non-obstructive azoospermia group (21 cycles) including 3 cases of severe oligozoospermia, only in 13 cycles could sperm be found to perform ICSI, with the FR of 59.0% (72/112); 11 cycles had embryo transfer, and the average embryo number for transfer was 2.8. Three cases got clinical pregnancy. In 8 cycles, TESE were failed. The first case delivered a normal boy in June 1998. CONCLUSIONS: Using testicular sperm for ICSI is relatively difficult, but it is a practical method to provide fertility for obstructive or non-obstructive azoospermia men. Its efficacy, however, is needed to be improved.

[Investigation of prognostic factor and treatment mode for undifferentiated cervical carcinoma].

OBJECTIVE: To analyse the prognostic factor and investigate the treatment mode for undifferentiated cervical carcinoma. METHODS: The recurrent rate, the metastatic rate and the survival rate were analysed retrospectively for 146 patients with stage I-II cervical carcinoma of poor differentiation treated with radical operation, operation/chemotherapy, operation/radiotherapy, operation/chemotherapy/radiotherapy from Jan. 1990 to Oct. 1998. RESULTS: The recurrent rate of patients treated with radical operation/chemotherapy (18.18%) or radical operation/radiotherapy (24.32%) was lower than that of patients only treated with radical operation (36.36%) or operation/chemotherapy/radiotherapy (42.11%) for the cases of stage II or deep muscle invasion (P < 0.05). The recurrent rate of patients treated with radical operation/radiotherapy (22.22%) was lower than that of patients only treated with radical operation (2/2) for the cases with local lymphaden positive (P < 0.05). The investigation also showed that the distant metastatic rate of patients treated with radical operation/chemotherapy was 0.00%, which was lower than that of patients treated with radical operation 5.56% (P < 0.05). The 3-year survival rate was 100.00% and 88.53% respectively for the cases only treated with radical operation/chemotherapy or operation/radiotherapy; and it was lower for the cases treated with radical operation 76.74% (P < 0.05). CONCLUSIONS: The prognosis of undifferentiated cervical carcinoma is related to the lymph node metastasis, the International Federation of Gynecology and Obstetrics stage and the treatment methods. The local recurrent rate and distant metastasis might be reduced by 2 courses of chemotherapy or radiotherapy post-operatively. The recurrent rate could not be reduced effectively by the combination therapy of operation/chemotherapy/radiotherapy.

[Adenosine in treatment of rats with spinal cord injury].

OBJECTIVE: To observe the changes of endogenous extracellular adenosine after spinal cord injury (SCI) to rats and the effect of exogenous adenosine on extracellular calcium after SCI and post-injury neurological function. METHODS: A ventral compression injury model of T13 spinal cord was used, and the extracellular fluids were collected consecutively every 20 minutes after injury by using microdialysis. Adenosine in the samples was analyzed using high-pressure liquid chromatography (HPLC) with u.v. detection. The rats received different doses of 2-chloroadenosine (2-CADO), a nonspecific agonist of adenosine receptors, by intrathecal injection 15 minutes before injury. The extracellular fluid was collected every 10 minutes immediately after injury and the calcium was measured using atomic absorption spectrophotometer. Neurological function score, inclined plane angle, and histology were observed 24 hours after injury. RESULTS: A significant increase of adenosine was found immediately after spinal cord injury. The concentration of adenosine peaked at one hour after injury and dropped down to the basal level. There was a positive relation between the increase of adenosine and the severity of SCI. High dose of 2-CADO can significantly significantly inhibit the decrease of extracellular calcium and improve the neurological function of injured rats. CONCLUSIONS: Adenosine could involve the pathological process of secondary spinal cord injury and might play a protective role in SCI.

Lung autotransplantation technique for treating central lung cancer of upper lobe.

OBJECTIVE: To assess the feasibility of applying lung autotransplantation technique in the treatment of central lung cancer of the upper lobe. METHODS: Two patients underwent double-sleeve right upper and middle bilobectomy. Because the length of resected bronchus or pulmonary artery involved by tumor was too long to perform tension-free anastomosis, we had to transplant the lower lobar vein into the proximal stump of the upper lobar vein. Another two patients underwent pneumonectomy firstly, because we could not perform sleeve lobectomy in suit as the tumor extended through the oblique fissure to the margin of the lower lobe. Subsequently, we resected the tumor parts at a separate table, and replanted the preservable part of the lung into the chest. RESULTS: Being followed up to December 1999, the patient 1, 3 and 4 had been alive with tumor free for 31, 18 and 13 months. The patient 1 and 3 had a good living quality but the patient 4 had a very poor activity. The patient 4 received resection of the replanted lung for pulmonary artery bronchus pleural fistula on the 42nd day after operation. The second patient died of tension pneumothorax associated with bronchopleural fistula on the 19th day after operation. CONCLUSION: Lung autotransplantation is an alternative technique for pulmonary preservation in patients with stage III central lung cancer of the upper lobe whose cardiopulmonary function is too poor to undergo pneumonectomy.

[An occlusive evaluation proposal to intra-saccular embolization of intracranial aneurysm].

OBJECTIVE: To explore the imaging criteria for evaluating intracranial aneurysm embolization. METHODS: Angiographic occlusive criteria for intracranial aneurysm embolization (aneurysm not opcified angiographically after embolization, 100% occlusion; a little part of aneurysm neck residual, 95%; neck residual, 90%; neck and a little part of aneurysm cavity residual, 80%; some cavity residual, < 80%) were used by 6 specialists to evaluate 121 aneurysms embolized with MDS (mechanical detachable spiral) and/or GDC (guglielmi detachable coil) from March 1995 to July 1999. The rationality, feasibility and limitation of the criteria were discussed. RESULTS: Among the 121 aneurysms, 100% occlusion was reached in 53 aneurysms (43.8%), 95% in 27 (22.3%), 90% in 16 (13.2%), 80% in 15 (12.4%), and less than 80% in 10 (8.3%) respectively. Good accordance was obtained among different doctors in the occlusive evaluation of aneurysm embolization. CONCLUSIONS: The suggesting criteria are simple and feasible in clinical practice, although the effect of imaging follow-up of embolized aneurysm is unknown and the numerical value of occlusive percentage is arbitrary.

[A study on correlation between the structural parameter of optic disc and visual field in primary open-angle glaucoma].

OBJECTIVE: To study the consistency between the optic disc neural rim depth (NRD) and visual field defect in patients with primary open angle glaucoma (POAG). METHODS: A system of computerized image analysis was used to measure the relative NRDs of temporal-superior and temporal-inferior area of the optic disc in patients with primary open angle glaucoma. The consistency between the area with relatively larger NRD and the location of severer visual field defect was investigated. RESULTS: In cases with early and middle stage POAG, the value of NRD corresponding to the severer location of the visual field defect was significantly larger than that corresponding to the location without or with mild such defect. Among 54 eyes, there were 46 eyes whose location with larger NRD value was consistent with the position of severer visual field defect, the rate of consistency being 85.2%. CONCLUSION: In cases with early and middle stage POAG, the increase of NRD value at temporal-superior and temporal-inferior area is asymmetrical, and the consistency between the area with larger NRD value and the corresponding position of severer visual field defect is relatively good. NRD is helpful to the early diagnosis of POAG.

[A comparison between latanoprost and timolol in treatment of patients with primary open-angle glaucoma and ocular hypertension].

OBJECTIVE: To evaluate the therapeutic value of latanoprost on glaucoma. METHODS: In an open-label fashion, multicenter, randomized control clinical trial, the efficacy and adverse drug reactions of topical application of 0.005% latanoprost once daily were compared with that of 0.5% timolol twice daily for 12 weeks in patients with open-angle glaucoma or ocular hypertension. RESULTS: The study included 128 patients (63 patients in latanoprost group and 65 patients in timolol group) and 117 patients remained at the end of the study (60 cases in latanoprost group and 57 cases in timolol group). Comparing 12 weeks with baseline diurnal intraocular pressure (IOP), the IOP reduction (mean +/- standard deviation) in latanoprost group was (7.5 +/- 0.3) mm Hg (1 mm Hg = 0.133 kPa) (32%, t = 22.73, P < 0.0001) greater than the reduction in timolol group (6.1 +/- 0.3) mm Hg (26%, t = 17.94, P < 0.0001), the difference between the two groups being significant (F = 9.54, P = 0.0026). Two patients treated with timolol and none treated with latanoprost were withdrawn from the study because of inadequate IOP control; 3 patients with latanoprost had foreign body sensation. In latanoprost group, there was one patient whose eyelashes became darker and longer at the last visit (the 12th week). No ocular and systemic adverse events related to the two drugs were found. CONCLUSION: It is demonstrated that 0.005% latanoprost topically applied once daily is well tolerated and more effective in reducing IOP than 0.5% timolol topically applied twice daily. Thus, latanoprost has the potential to be a new first-line antiglaucoma drug.

[An observation of the immuno-persistence after inoculating with the domestic BRD II strain rubella vaccine among infants and young children].

OBJECTIVE: To study the immuno-persistence after inoculating with the domestic BRD II strain rubella vaccine among infants and young children. METHODS: Hemagglutination inhibition (HI) antibody detection method was used to test on children at age 6 to 18 months without rubella infection or rubella vaccine immunization in Yantai city of Shandong province and were selected for the observation of the immuno-persistence against the domestic BRD II strain rubella vaccine from 1995 to 1998. RESULTS: Positive rates of HI antibody among children of 6, 7, 8, 9, 12 and 18 month-old of inoculation were 94.44%, 97.22%, 96.67%, 100.00%, 100.00%, 100.00% and 93.10%, 93.55%, 96.77%, 96.97%, 100.00%, 100.00% (P > 0.05), in one or two years respectively. The geometric mean reverse titers (GMRTs) were about 50% lower than that after 1 month after 1 year. Similar results were found that after 2 years GMRTs was about 50% lower than that after 1 year of inoculation. There was highly significant difference on GMRTs of HI antibody between infants with 6 to 7 month-olds (29.89) and young children with 8 to 18 month-olds (53.00) after 2 years of inoculation (t = 3.58, P < 0.001). CONCLUSION: The immunization schedule at the first dose for BRD II strain rubella vaccine should be started when the child is 8 month old. For the second dose, the immunization schedule used in other developed countries should be referred adopted in China.

[Direct effects of fluoride on activities of bone morphogenetic protein].

OBJECTIVE: To demonstrate the direct effects of fluoride on activities of bone morphogenetic protein (BMP). METHOD: A complex sample containing BMP and sodium fluoride was implanted under the abdominal skin of young male Wistar rats. The histological characteristics, and activities of alkaline phosphatase and acid phosphatase of the implanted were examined to study the effects of fluoride on biological activities of BMP. RESULTS: More fibroblasts cell were transformed to osteoblasts cell in animals treated by BMP with fluoride than those treated by BMP only. During bone formation induced by BMP, fluoride increased the activities of osteoblasts and accelerate mineralization, while inhibited the activities of osteoclast. CONCLUSION: Fluoride can affect biological activities of BMP directly.
