[Primary lymphoma of the liver: an uncommon disease. Ultrasonic and x-ray computed tomographic aspects. Apropos of a case with review of the literature].

Although the very rare primary non-Hodgkin's lymphoma is resectable and responds to chemotherapy it has a very poor prognosis since the outcome was rapidly fatal in 7 of the 11 cases reported in the literature. Particular features in a patient with a primary non-Hodgkin's lymphoma of cirrhotic liver were the association of portal hypertension and portal thrombosis related to tumor invasion. Findings on ultrasound imaging and histologic diagnosis from guided needle puncture biopsy appear to be previously unreported aspects of this affection.

[Colorectal complications of radiological examinations].

These complications are uncommon, but can be very serious. They are dominated by perforations, in the case of barium enemas. The use of rigid canulas and/or balloons are often implicated in rectal perforations which occur most readily in rectal tissue damaged by a biopsy. Colonic perforations usually occur in damaged tissues (various forms of colitis, cancer, colostomy). The hydrostatic pressure seems to be incriminated as a precipitating factor in the development of spasms in the presence of an underlying obstruction. The other complications are much less common. In children, perforation usually occurs during reduction of an intestinal intussusception. The complications of embolisation of the abdominal arteries consist of necrosis; the left colonic flexure is at greatest risk because of its blood supply. Emergency embolisation can be performed provided certain rules are respected.

[Colonic involvement in systemic mastocytosis (author's transl)].

The most common gastrointestinal lesions encountered in systemic mastocytosis are the gastric, duodenal and small bowell lesions. The authors present a case of diffuse colonic involvement diagnosed on a double contrast examination. The radiological signs correlated with the endoscopical and histological findings are due to a submucosal edema and a polymorphic cellular infiltrate. The involvement of the colon is probably more frequent then previously thought. It could explain the diarrhea encountered in some patients affected by this disease.

Colonic Diseases↗

[Digestive manifestations of dermatomyositis (author's transl)].

The principal digestive anomalies detected by radiological examination in dermatomyositis are the motor and functional consequences of the smooth and striated muscle lesions in these tissues. Vascular changes cause ulcerations, located in any part of the digestive tract, but more frequently in the esophagus, duodenum, and small intestine. These ulcerations sometimes develop into pseudodiverticulae.

[Radiological exploration of the Budd-Chiari syndrome. Study of 13 cases].

Using 13 cases of Budd-Chiari syndrome, the authors describe their method of radiological investigation in order to reach a positive and sometimes aetiological diagnosis. Hepatic venous thrombosis can be confirmed only on the basis of findings at hepatic phlebography. Cavography, which precedes retrograde catheterisation of the hepatic veins, provides only presumptive evidence and sometimes reveals total thrombosis of the inferior vena cava. Coeliac and superior mesenteric, or even hyperselective hepatic arteriography, is of great value if there is inversion of the portal flow. Otherwise it only leads to a suspicion of the diagnosis, and makes it possible to determine the state of the portal trunk and to seek an aetiology.

[Radiological exploration of alveolar echinococcosis. Concerning two cases with hepatic and splenic localization (author's transl)].

Two cases of alveolar echinococcosis were observed, the first in a 30 year old man, suffering from a hepatic localization, of the classical icteric and tumoral form, the second in a 26 year old patient, with an exclusively splenic localization, a very exceptional possibility. In both cases, coeliac arteriography showed vascular lesions which led one to suspect the diagnosis. To our knowledge, this is the first case of alveolar echinococcosis explored by arteriography. Furthermore, in the first patient, retrograde catheterization of the biliary tract was carried out, which showed very marked changes of the biliary tract. Inspite of the amount of information provided by the radiological investigations, the diagnosis was not made until the surgical intervention.

[Nasopharyngeal fibroma. Significance of super-selective angiography and embolization].

The authors describe a case of naso-pharyngeal fibroma which they treated, in order to prevent per-operative haemorrhage, by means of embolisation of the arterial pedicles. The result was very satisfactory, the operation being practically bloodless. They stress the advantages of angiography for obtaining information about these tumours. And, thinking ahead on the subject of embolisation, they weigh up the risk of haemorrhage due to surgery (which remains considerable whatever methods have been used hitherto) against the possible risk of embolism which appears to be negligible if it is carried out by a radiologist who is expert in this method.
