Functional differentiation of various types of cardiomyopathies by echocardiography.

Echocardiography was used to compare the left ventricular function in 35 cases of congestive cardiomyopathy (COCM), 16 cases of obstructive and 28 cases of non-obstructive asymmetric septal hypertrophy, 43 cases of scleroderma heart disease, 21 cases of ischaemic cardiomyopathy (ICM), 2 cases of restrictive cardiomyopathy, and one case of cardiac amyloidosis. COCM was characterized by left ventricular dilatation, decreased contractility and signs of elevated end-diastolic pressure. In asymmetric septal hypertrophy there was a decreased distensibility of the obstructed left ventricle, both in early and end-diastole, but the pump function remained normal. In scleroderma and amyloid heart disease both the contractility and distensibility of the small, stiff heart were reduced. In restrictive cardiomyopathy the only abnormality was the impaired end-diastolic distensibility. Reduced contractility confined primarily to the interventricular septum and impaired early diastolic distensibility are the characteristic features distinguishing ischaemic cardiomyopathy from COCM. However, echocardiography is not suitable for individual differentiation of ICM and COCM. The COCM can be differentiated from left ventricular aneurysm by M-mode sector scanning technique.

Absorption, distribution and elimination of drotaverine.

14C-drotaverine [1(3',4'-diethoxybenzale)-6, 7-diethoxy-1, 2,3, 4-tetrahydroisoquinoline. HCl; No-SpaR] is well absorbed after subcutaneous and oral administration in mice. Its distribution is not specific. After intravenous administration the drug penetrates rapidly into every organ as indicated by whole body autoradiography. In the first hours the concentration of drotaverine was higher in the intestinal wall than in the other tissues. The concentrations of drotaverine in the organs decrease soon after administration and the drug is excreted mainly with the bile as beta-glucuronide; 60% of the dose was in the bile collected during 5 hours. During 96 hours of observation, 67% of the radioactivity administered was found in the stools while only 20% of it was eliminated with urine.

Echocardiographic study of left ventricular posterior wall motion in angina pectoris.

Echocardiographic study of left ventricular posterior wall motion was performed on 22 healthy subjects, on 7 cardiac patients without angina, and on 12 patients with angina pectoris, at rest and during supine exercise on a bicycle ergometer. Resting values did not differ significantly between patients with angina and control subjects. During submaximal ergometric exercise, the systolic excursion of the left ventricular posterior wall as well as the maximal systolic and diastolic velocities increased considerably in all groups. The degree of the changes was essentially similar in patients with angina and in healthy subjects. During chest pain, however, all three parameters of posterior wall motion decreased markedly, especially the systolic excursion, which fell well below the resting level. This phenomenon is attributed to the reduction of the work of the heart during chest pain.
