Renal function in 153 manic-depressive patients treated with lithium for more than five years.

Renal function was examined in 153 manic-depressive patients treated with lithium for more than 5 years, mean 10 years. No significant change was detectable in plasma creatinine. Glomerular filtration rate (GFR) decreased slightly, but significantly, and not until after 17 years of treatment did the regression line reach the lower confidence limit in the reference material. GFR was generally only moderately decreased. Renal concentrating capacity was significantly reduced during the whole investigation period and did not change with time. GFR was independent of the dosage pattern. The diuresis did not differ markedly in patients given one or three daily doses. In a two-dose group predominantly treated with slow-release tablets, the diuresis was somewhat higher in 75% of the patients but much higher for the rest of the group. Since the prophylactic effect of lithium was the same in the one-dose group (mean dosage 21 mmol/day) as in the two-dose and three-dose groups (mean dosage 27-28 mmol/day), our data indicate that generally employed lithium doses may be reduced somewhat without loss of prophylactic efficacy.

Prognosis in glomerulonephritis. A follow-up study of 395 consecutive, biopsy-verified cases. I. Classification, renal histology and outcome. Report from a Copenhagen study group of renal diseases.

Between 1967 and 1977, 395 consecutive cases of glomerulonephritis (GN) were collected by a Copenhagen study group. The diagnosis was established by histological and biochemical criteria. Light microscopy investigations of thin silver-stained sections were applied. In a follow-up in 1980 all cases were categorized by one of the following end points: death without uremia, uremia, recovery, or censored cases. The course is presented in figures showing the cumulated distribution of outcomes in relation to observation time. Each histological subgroup of GN had its own characteristic course with respect to initial rates of changes in the renal state, as well as to frequency of recovery, uremia and death. The prognosis was good in minimal changes GN and proliferative GN, bad in unclassified GN and worst in extracapillary GN. When part of a connective tissue disease, GN carried a poor prognosis. We conclude that histological classification of GN based on light microscopy offers a reliable means of predicting the long-term prognosis.

Acute lymphoblastic leukemia. Possible origin from a mediastinal germ cell tumor.

The case of acute lymphoblastic leukemia developing shortly after the diagnosis and treatment of a mediastinal germ cell tumor, is reported. The close temporal relationship of these two rare diseases and the presence of a population of mononuclear cells with features of lymphoblasts in the resected germ cell tumor, suggest that the leukemic process originated from the mediastinal tumor.

Increased sleep following intracerebroventricular injection of the delta sleep-inducing peptide in rats.

Synthetic delta sleep-inducing peptide (DSIP) was injected intracerebroventricularly into rats in a dose of 7 nmol/kg. A significant, 20% increase in total slow wave sleep (and total sleep) was found in the DSIP-injected rats, compared to a baseline recording following a sham injection procedure. Rats injected with the Ringer solution vehicle did not change their sleep compared to their baseline recording.

Effect of high pressure on EEG burst suppression dose of thiopental in rats.

The anesthetic induction dose of thiopental in rats was studied at 1 ATA air, 1 ATA helium-oxygen (He-O2), 4 ATA air, and at 4 ATA air plus 67 ATA helium (71 ATA). The compression rate was 0.3 ATA/min, and 1 h was spent at pressure before the experiment started. Thiopental was infused at a rate of 7.5 mg X kg-1 X min-1. The depth of anesthesia was assessed by electroencephalographic (EEG) recording using burst suppression of 1 s as the biological end point. The mean induction dose at 1 ATA air was 53.1 mg/kg and at 71 ATA was 74.7 mg/kg, an increase of 41%. The induction doses at 1 ATA He-O2 and 4 ATA air were 46.4 mg/kg and 45.3 mg/kg, respectively.

The prognosis of insulin dependent diabetes mellitus and the importance of supervision.

1451 patients with IDDM, onset before 1953 and before the age of 30 were followed until death or until 1976. Survival with diabetes, relative survival and the influence of supervision on survival were examined. It is shown that only 50% of the patients survived more than 30 diabetes years. The patients had an overmortality of more than 600% in relation to age and sex matched non-diabetics. Frequent supervisions in the out-patient clinic reduced as well the overmortality as the prevalence of severe complications significantly.
