The effects of an ergot alkaloid derivative (Hydergine) on aspects of psychomotor performance, arousal, and cognitive processing ability.

Hydergine, 12 mg per day for two weeks, has some direct activity on a variety of tasks of mental and cognitive performance. These results add support to biochemical findings that implicate the ergot alkaloids (particularly co-dergo-crine, Hydergine) in cellular activity likely to increase cortical arousal and awareness. The high dose of Hydergine used in this volunteer study was exceptionally well tolerated and did produce significant results on individual measures of central nervous system activity which might suggest the use of similar doses in patient populations. The findings of a hangover of activity after drug withdrawal and the fact that some CNS activity (serial subtraction of 17s) is not obvious until two weeks of medication would suggest the need for pharmacokinetic measures to be taken in conjunction with psychologic assessments. It would seem that a two-week schedule of repeated doses is the minimum required to produce an effect on CNS activity, but even with such a dose regimen it appears that the drug continues to exert some effect after its withdrawal.

Segmental colonic transit time.

Mean segmental transit time of radiopaque markers through the right colon, left colon and rectosigmoid areas of adults and children has been calculated form their distribution on consecutive plain films of the abdomen. Overall mean transit does not differ significantly in the large bowel between adults and children. However, there are regional differences within the colon in relation to age. Mean transit time in the right colon is 13.8 hours in adults and 7.7 in children (p less than 0.01). Corresponding values in the left colon are 14.1 and 8.7 hours (p less than 0.02) and, in the rectum, 11 and 12.4 hours (p = NS). The percentage of the mean total large bowel transit time spent in the right colon, left colon and rectosigmoid area are respectively for adults and children 33 +/- 4 and 28 +/- 3 per cent (p = NS); 39 +/- 4 and 32 +/- 4 per cent (p = NS); and 28 +/- 4 and 41 +/- 4 per cent (p less than 0.05), indicating a relative stagnation in the rectosigmoid area of children. These physiologic differences may have implications in diseased states.

The N-terminal fraction of desmoglein 3 encompassing its immunodominant domain is present in human serum: implications for pemphigus vulgaris autoimmunity.

Pemphigus vulgaris (PV) is considered as an autoimmune disease against a tissue-restricted antigen, desmoglein 3, a 130 kDa glycoprotein expressed by keratinocytes of skin and mucous membranes. Therefore, a breakdown of peripheral tolerance is generally invoked to explain this horror autotoxicus. The availability of a self-antigen and the strength of antigenic stimulation represent critical points in the regulation of immune system homeostasis. Our study shows for the first time that the immunodominant fraction of the PV self-antigen is present in sera of healthy individuals and patients as a circulating 30 kDa fragment (sDsg3). These findings provide a good explanation for the N-terminal specificity of antibody production and peptide recognition in PV patients by B and T cell, respectively. Moreover, the presence of the sDsg3 in human sera could allow to reconsider pemphigus as a disease against a circulating antigen; once produced, PV-autoantibodies also recognize the 130 kDa epidermal antigen desmoglein 3 on keratinocyte surface (kDsg3), thus triggering the acantholysis and the clinical manifestations of pemphigus.

[Complete rectal prolapse in a patient with Hirschsprung disease: a clinical case].

The disease of Mya-Hirschsprung (HD) it's rare and congenital, usually diagnosed in child age, but that it can also remain unknown until the adult age. It's associated to genetic mutations and it can involve other pathologies and malformations. The variability of the anatomopathological and clinical phenomena is correlated to the length of the aganglionic segment. The Authors describe the clinical case of one young female patient, who presented chronic constipation (less than 3 evacuations to week), tenesm, meteorism, abdominal pain. To the inspection of the anal region after the evacuation the presence of complete rectal prolapse (3) cylinders of the length of 30 cm was appraised, with presence of ulcerations of the mucosa. The patient came subordinate to diagnostic study that they demonstrated the presence of HD. The Authors operated the patient with the technique of Frykman and Goldberg. In the follow-up the patient had the complete remission of the symptomatology and resumption of the rectoanal inhibitory reflex, remarkable diminished in the preoperative manometric examination. The pathogenesis of the association of HD and rectal prolapse goes searched in the presence of ultrashort HD (aganglionic defect of a limited segment of rectum), pathological variety somewhat rare that can determine subocclusive phenomena and that it favors the prolapse of the rectum for the continuous evacuating strains.

[The safe transportation of children in automobiles].

The Author emphasizes the significant number of children who sustain fatal accidents during their transport on motor vehicles and relates the Act recently promulgated by the Italian Parliament on the mandatory use of particular restraining devices as well as safety harness for children.
