[Possibilities for tattoo removal. Report of experiences].

Different motivations are seen in patients with tattoos, which can be seen in a tattooed picture. Two kinds of tattoos can be distinguished: Professional and self-made tattoos. In general, patients with self-made tattoos call for their removal, because the private situation has changed. These tattoos are more difficult in treatment than professional ones, because the colour is located in the whole dermis. The method of treatment depends on size, form and shape of the tattoo. For treatment simple excision of small tattoos, dermabrasion in one or two sessions are recommended. Own experiences and results in 121 patients with tattoos are reported and discussed.

[Treatment of circumscribed lymphangiomas with the argon laser].

Six patients suffering from lymphangioma circumscriptum neviforme and two from lymphangioma circumscriptum localisatum were treated with an argon laser. Lymphangiomas regressed in all patients, with some scar formation. In four of the six neviform lymphangiomas there was recurrence of tiny clear pseudovesicles within 3-4 months following the first treatment. Two patients with localized type and two with neviforme type were without recurrence within 1 year following the first irradiation. A cure of extensive lymphangioma circumscriptum is not possible in our experience. Most of the patients were content with the obtained regression and accepted the slight scar formation and repeated treatments. Laser therapy for cosmetical reasons is not indicated.

[Dye and nickel allergy in the hairdressing salon].

In 73 hairdressers, out of a total number of 3,300 patients, patchtests were performed using substances of a standard series. Patients of various professions excluding hairdressers and patients belonging to an age group comparable with that of hairdressers, served as two control-groups. In the group of hairdressers the most frequently seen positive reactions were against p-phenylendiamin, nickelsulfat, pellidol, cobaltchlorid, p-aminodiphenylamin, and perubalsam. The incidence of positive reactions against p-phenylendiamin and pellidol in this group was significantly higher than in the control group, whereas the higher incidence of nickelsulfat allergy in hairdressers proved not to be significant.

[Malignant melanoma with unknown primary tumor. Case reports of 12 patients with overview].

Report of 12 patients with metastasizing malignant melanoma and unknown primary tumor. Age of patients, therapy and prognosis are alike in patients with known primary tumors. In five patients the medical history or clinical data pointed to regression of the primary malignant melanoma of skin. Metastasis of internal malignant melanomas or the "de novo" origin of malignant melanomas in lymphatic vessels or lymph nodes are discussed.

[Physical and biological principles of the use of lasers in dermatology (author's transl)].

A description of the principle of laser is followed by indicating the specific characteristics of laser light. The biological and physical influences of light are mentioned slightly. Using the laser in medicine primarily means to utilize the thermcal effect of radiation. This effect results both from the parameters of irradiation and thermcal and optical characteristics (absorption and diffraction) of the tissue. The possibilities of employing different systems of laser in dermatology - surgical operations, treatment of skin lesions and light-induced wound healing - are discussed.

[Ichthyosis vulgaris, growth retardation, hair dysplasia, tooth abnormalities, immunologic deficiencies, psychomotor retardation and resorption disorders. Case report of 2 siblings].

Two siblings (4 and 10 years old) with ichthyosis vulgaris, growth and mental retardation, hair dysplasia, teeth abnormalities, recurrent infections and malabsorption are presented. The elder sister of the children as well as the rest of the family are unremarkable. The complex of symptoms does not fit into one of the known Ichthyosis syndromes. Microscopically the dystrophic hairs show changes in diameters, nodules, lack of the medulla, rough cuticle and splits vertical to the axis, sometimes resembling trichorrhexis nodosa. In the scanning electron-microscope there is a clear difference to Netherton's syndrome. The plasma concentrations of the vitamines A, B12 as well as of beta-carotines are low in spite of normal nutrition. During parenteral therapy with vitamin A and B12 symptoms ameliorate slightly.

[Soft radiotherapy of lip cancer].

Reported are 107 patients with 109 carcinomas of the lip. Histologically 83 per cent of the patients had well or moderately well differentiated tumors. With total tumor doses between 6,000 and 9,500 R, in some patients with more than 10,000 R, 92 per cent of patients were cleared, in 88 per cent with a good cosmetical, and in 77 per cent with a good functional result. The used radiation methods are discussed under consideration of time-, dose- and fractionation-factors of Orton and Ellis. Patients should be controlled for a longer period than five years, because recurrences or second tumors are possible even after this time.

[Experiences with the antiestrogen tamoxifen in the therapy of oligozoospermia].

Thirty-three subfertile men with idiopathic normogonadotropic oligozoospermia were treated with daily 2 x 10 mg tamoxifen, a nonsteroidal antiestrogen, for a period of five months. The statistical evaluation of the semen parameters yielded a significant increase of the sperm count and the total sperm output after three months of treatment with a further increase after five months. The percentage of motile spermatozoa increased already after one month and remained significantly increased during the whole treatment period. Progressive motility, sperm morphology, ejaculate volume and seminal plasma fructose were not affected. Side effects were not observed. The conception rate was 38 per cent within a period of one year.

[Therapeutic use of lasers in dermatology].

Lasers are used increasingly in dermatology. The use of different laser types for various indications is discussed. Nevi and benign vessel tumors, tattoos, condylomata acuminata, hypertrophic scars, and keloids are indications for laser therapy. The application of lasers in the treatment of malignant tumors, such as malignant melanoma or malignant vessel tumors, is discussed. Finally, the stimulating effects of laser light with low energy density on wound healing is mentioned.

[Malignant melanoma, leukoderma and vitiligo].

We report about two patients with malignant melanoma, who developed vitiligo at a site distant from the primary melanoma after surgery and during consecutive immunotherapy. In two patients a leucoderma was observed within the area, where metastatic lymphnodes had been excised, as also reported in the literature. These depigmentations may be due to an immunologic defense mechanism, but there is no evidence, if they represent a favourable sign in the course of malignant melanoma.

Autoradiographic and planimetric analyses of the inflammatory infiltrate in psoriatic lesions under PUVA-therapy.

The effects of oral 8-methoxypsoralen-photochemotherapy (PUVA-therapy) on the inflammatory infiltrate in 14 psoriatic skin lesions and on normal skin of 7 controls were investigated by means of autoradiography and planimetry. Under PUVA-therapy, one finds a significant decrease in dermal cells per millimeter lesion in psoriasis. Percentage of 3H-TdR-labeled dermal cells in the lesions and total number of dermal cells in the control group do not change significantly. The relative decrease of dermal cells per millimeter lesion in psoriasis is far less significant than the reduction of 3H-TdR-labeled epidermal cells under PUVA-therapy.

Inhibitory effects of 13-cis-retinoic acid on human sebaceous glands.

The effects of 13-cis-retinoic acid, which is clinically very effective in the treatment of severe acne, on sebaceous glands in humans was studied by means of histology, planimetry, and autoradiography. Histologically and planimetrically, one finds a marked decrease of size of sebaceous glands of up to 90% of the pretreatment values after 12 weeks of treatment. Additionally, the ratio of the so-called "differentiating cell pool (DCP)" vs. the so-called "undifferentiating cell pool (UCP)" changed from 2:1 to 1:7, probably indicating a disturbance of differentiation (lipid-production) of sebocytes. The labeling index of sebocytes also regressed significantly under therapy with 13-cis-retinoic acid.

In vivo autoradiography and planimetry of epidermis in psoriatics under PUVA-therapy.

The effects of oral 8-methoxypsoralen-photochemotherapy (PUVA)-therapy) on involved epidermis of nine psoriatics and on normal skin of four control subjects was investigated by means of in vivo autoradiography and planimetry. Under PUVA-therapy there is a significant decrease of 3H-TdR labeled cells per epidermal cross-sectional surface, per length of basal membrane and per length of epidermal surface. There is also a significant decrease of epidermal thickness. In the initial phase of PUVA-therapy the decrease of 3H-TdR labeled cells is the most pronounced. The reduction of epidermal thickness is less than the decrease of 3H-TdR labeled cells.

[Cryoproteinemias].

Biochemical cryoproteins are divided in cryoglobulins (type I-III), plasma-cryoproteins (heparin precipitable factor, cryofibrinogen and cryofibrin), and fibronectin. While cryoglobulinemias and plasma-cryoproteinemias are mainly caused by internal diseases, fibronectin may be a physiological substance. Because of skin lesions as purpura, necrosis or cold urticaria the patients rather often consult dermatologists at first. Furthermore arthralgias, kidney diseases, neurologic symptoms and lung involvement can be observed. For diagnosis a accelerated blood sedimentation rate at 37 degrees C and a decelerated at 4 degrees C is helpful. By further investigations underlying internal diseases have to be disclosed. In the therapeutical efforts the treatment of the underlying diseases is most important. Additional plasmapheresis and immunsuppressive treatment may be successful.

[Familial-hereditary malignant melanoma (author's transl)].

The existence of familial-hereditary malignant melanoma can be accepted for sure. Its frequency is estimated with 1 to 7% of all malignant melanomas. An autosomal-dominant mode of inheritance with reduced penetrance or a polygenetic mode of inheritance are discussed. Patients with numerous inherited, clinical and histological atypical naevi, the so called BK-moles, are reported to have a high incidence of malignant melanoma. Hereditary malignant melanomas begin early in life, have a high frequency of multiple primary lesions and show the coincidence of other malignant tumors in this families. Men are affected as often as women. Five own families are described and a review of the literature is given.
