The erythrocyte sedimentation rate (ESR) is the most frequently employed laboratory indicator of inflammatory activity. The plasma fibrinogen concentration is usually increased in inflammatory disorders and is thought to contribute to the ESR elevation, 6-sulfanilamidoindazole induced arthritis in rats produces very high plasma fibrinogen concentrations and increased blood viscosity but no change in ESR.
Freiberg's infraction is an osteochondrosis of the second metatarsal head, although it may be seen less often in the other metatarsals. It usually occurs in the second decade of life and is more commonly seen in females. The etiology is generally of a traumatic nature and symptoms include painful, limited range of motion of the joint. If untreated, destructive joint changes may lead to a painful degenerative arthritis requiring surgical intervention. The authors discuss joint replacement with a Swanson Silastic flexible hinged toe implant.
Children with hyper-immunoglobulin M (hyper-IgM) syndrome are at increased risk for Pneumocystis carinii pneumonia (PCP), an opportunistic infection often found in immunodeficient hosts. PCP can present with increasing hypoxia, fever, cough, and respiratory distress. We describe a child with hyper-IgM syndrome in whom bronchoalveolar washings were negative for PCP. However, there was an atypical lung response in which caseating granulomas predominated. The histopathology, resembling that found in tuberculosis, stresses the importance of a high index of clinical suspicion and histologic confirmation for early intervention and treatment. Immunocompromised children with rapidly progressive pulmonary disease may require lung biopsy and stains such as GMS to identify PCP.
