Three-dimensional atypical structure in intraductal carcinoma differentiating from papilloma and papillomatosis of the breast.
The basic architectural pattern of intraductal proliferative lesions of the breast was established by reconstructing serial sections of luminal spaces and interluminal areas of glandular structures. The materials were surgical specimens from twenty patients with intraductal carcinoma, papilloma, papillomatosis, or so-called borderline lesion. In papilloma and papillomatosis, the luminal spaces were tubular and interconnected forming a three-dimensional (3-D) network, whereas carcinoma was a porous structure with dispersed lumina. The latter represented 3-D atypical structure in intraductal carcinoma. In borderline lesion the architecture was an intermediate type, with separate lumina partially transformed into tubular shapes. It was also confirmed that the porous structure of carcinoma observed in 3-D reconstructions corresponded to the cribriform pattern seen in 2-D figures, while the network of papilloma produced a complex glandular pattern. A geometric parameter was devised to measure the different 3-D patterns of lumina and their 2-D expressions. It was concluded that the 3-D architectural pattern of intraductal proliferations was sufficiently characteristic to be of diagnostic value in differentiating these diseases.
Experimental model for the otitis media with effusion induced by Arthus' reaction.
The Arthus' reaction in the middle ear cavity was studied using histochemical technique. The deposition of immune complex on the basement membrane and the immune complex in the middle ear effusion were the initial features of these animal models. But, these immune complexes were clearly removed from the tympanic cavity during the early phase of reaction. Immune complexes were supposed to be just a initiation of otitis media with effusion. The tubal dysfunction and local immunity of middle ear mucosa caused by the Arthus' reaction seemed to be responsible for the chronicity and relapsing of otitis media with effusion respectively.

Lectin-binding patterns of breast carcinoma: significance on structural atypism.
We have investigated lectin-binding profiles of the human breast epithelia and the structural atypism in intraductal carcinoma. The lectino-histochemical probes using biotin-labeled peanut agglutinin (PNA), soybean agglutinin (SBA) and wheat germ agglutinin (WGA) with avidin-biotin peroxidase complex were performed on formalin-fixed and paraffin-embedded specimens from 24 patients. While normal and benign hyperplastic epithelia were stained almost in luminal borders and apical cytoplasmic surfaces, intraductal carcinoma additionally showed conspicuous staining of intracytoplasmic lumina and supranuclear cytoplasmic margins. The lectin-binding pattern found in papillary hyperplasia was completely different from that in intraductal carcinoma; the former was an irregular network structure along luminal borders, whereas the latter was not only a cribriform but also a rosette structure along intracytoplasmic lumina and supranuclear cytoplasms. Such lectin-binding patterns seemed to reflect the change of cell polarity in malignant transformation. The significance of these observations in elucidation of structural atypism was discussed in relation to 3-D structure of intraductal carcinoma.

[Susceptibility of intraductal papillomas to carcinogenesis based on 3-dimensional reconstruction study].
Surgical specimens from 16 patients with intraductal papilloma were subjected to 3-dimensional reconstruction studies of mammary ducts in order to detect early cancerous foci developing in papilloma, to analyze growth behavior of papilloma and to establish morphological features of papilloma particularly prone to cancerous change. Papilloma of the multiple type (10 cases) originated in the terminal duct lobular units (TDLU), while papilloma of the solitary type (6 cases) originated in the large ducts except for one. In 4 out of the 16 patients small foci of intraductal carcinoma were found during specimen reconstruction. Two of the four patients harbored minimal cancer as small as 3 mm or less. The carcinomas with multifocal origins in the TDLUs were anatomically connected with peripheral papilloma so far as to take the form of "cancer in papilloma". Papilloma preceding carcinoma was of the multiple type in 3 cases and of the solitary type in one. But according to the sites from which papillomas originated, 4 out of the 11 cases originating in the TDLU had cancerous foci, whereas none of the 5 cases originating in the large ducts did. This suggests that the peripheral papilloma is highly susceptible to cancerous change. From a clinical point of view, we proposed a nomenclature of peripheral vs. central papillomas instead of the conventional multiple vs. solitary.
[Genesis and extension of intraductal papillomas of the breast--a three-dimensional morphological study].
Graphic reconstruction of semi-serially sectioned breast specimens allowed a three-dimensional observation of intraductal proliferative lesions and their growth characteristics. Ten surgical specimens with intraductal papillomas obtained by microdochectomy were subjected to this study, and the primary site and extending behavior of papillomas were analysed. The results indicated: (1) In seven out of the ten cases papillomas appeared to arise from terminal duct lobular unit (TDLU) as multiple foci three-dimensionally independent of each other. (2) Secondary ductal involvement by multiple papillomas attained to as far as subsegmental or segmental level. (3) On the other hand solitary papilloma arose from segmental duct without involving TDLU. (4) Both solitary and multiple papillomas transformed and distorted the duct system while sustaining its basically arborescent pattern. (5) Duct papillomatosis, situated within TDLU, was often proved to be connected with multiple papillomas in more proximal ducts, and therefore was regarded as a prepapillomatous lesion. (6) Retention cysts associated with multiple papillomas were accounted for by intraductal proliferations obstructing outflow tracts.

K cell activity of murine-cultured lymphocytic leukemic cell line independent of other Fc receptor functions.
A murine-cultured lymphocytic leukemic cell line, SC-1, showed remarkable cytotoxicity against 51Cr-labeled rabbit erythrocytes as target cells in the presence of antitarget antibodies in vitro. This cytotoxicity was complement independent, inhibited by the addition of aggregated human gamma-globulin, and reduced by pepsin digestion of antitarget IgG. Moreover, SC-1 cells also induced cytotoxicity to trinitrophenyl-modified rabbit erythrocytes in the presence of antitrinitrophenyl antibodies but not unmodified rabbit erythrocytes used as bystander cells. SC-1 cells which showed such K cell activity, however escaped from Fc receptor detection by erythrocyte antibody rosettes and FITC-conjugated aggregated human gamma-globulin binding. In morphological studies using the scanning electron microscope, SC-1 cells to induce cytotoxicity through a light attachment to target cells via 'point contact' of the microvilli. From these findings the characteristics of Fc receptors on effector cells in the mechanism of antibody-dependent cellular cytotoxicity was discussed.
