[Clinical statistics on inpatients and operations at the Department of Urology, the Center for Adult Diseases, Osaka, 1984-1988].

Statistical observation on inpatients and operations at our department between January 1984 and December 1988 revealed the following results: 1) The total number of inpatients was 1962 (male: 1658, female: 304). The most frequent diseases were bladder cancer (30.0%), benign prostatic hypertrophy (19.2%), prostatic cancer (10.6%) and renal cancer (6.7%). 2) The total number of operations was 1699. The most frequent operations were transurethral resection (TUR) of bladder tumor (22.8%), TUR prostate (20.7%), TUR biopsy (6.5%) and total cystectomy (5.4%).

[A case of primary ureteral squamous cell carcinoma associated with calculus].

A case of primary squamous cell carcinoma of the ureter associated with ureteral calculus is presented. A 66-year-old woman was admitted to our hospital, with the chief complaint of macroscopic hematuria in October, 1985. A kidney-ureter-bladder X-ray and drip intravenous pyelography failed to reveal the calculus shadow clearly. However, computerized tomographic scan revealed the calculus shadow clearly adjacent to the ureteral tumor, and retrograde pyelography revealed the filling defect on a third lower portion of the left ureter. She was diagnosed with tumor of the ureter associated with the calculus. She underwent complete nephroureterectomy with excision of a periureteral cuff of the bladder. The tumor was diagnosed histologically as squamous cell carcinoma. Metastases and recurrence of tumor have never occurred on July, 1987. Forty-four cases of the primary ureteral squamous cell carcinoma could be collected in the Japanese literature. Moreover, we deal with this disease associated with the ureteral calculus.

Melanocytes in odontoameloblastoma. A case report.

A case of pigmented odontoameloblastoma occurring in the maxilla of an 11-year-old Japanese girl is reported. In addition to the characteristic histopathologic features of odontoameloblastoma, dendritic cells containing melanin pigment were widely distributed in the epithelial element of the ameloblastoma. Fine granules and coarse aggregates of melanin pigment were also distributed in the epithelial cells. The possible histogenesis of melanocytes in the odontogenic lesions was discussed, although no conclusion could be drawn from the present observation as to the origin of these cells in odontoameloblastoma.

[An autopsy case of a patient with myasthenia gravis who showed various symptoms of collagen diseases and complicated with malignant thymoma].

A 37-year-old man suffered from photosensitivity and urinary casts with serological findings of positive anti-DNA antibody, LE cells and false positive VD reaction in September of 1979. He developed general fatigue, dyspnea and diplopia with ptosis of bilateral eyelids in November of 1979, which were improved by the anti-cholinesterase drugs. In January of 1980, he had an attack of unconsciousness and his chest X-ray film showed several tumorous shadows in the anterior mediastinum and middle and lower lung fields. Treating him with chemotherapy of VEMP, the pulmonary shadows disappeared. However, he developed severe muscle weakness with an elevated CPK (430 mU/ml) and a myogenic EMG pattern along with an increased anti-acetylcholine receptor antibody (243 n Mol/l), dysphagia and eyelid-ptosis. He died in September of 1985 and his autopsy disclosed a malignant thymoma of mixed type in the anterior mediastinum and an atrophy and fibrosis with infiltration of inflammatory cells in the striated muscles.

Preventive effect of pravastatin sodium, a potent inhibitor of 3-hydroxy-3-methylglutaryl coenzyme A reductase, on coronary atherosclerosis and xanthoma in WHHL rabbits.

In this paper, we examined whether the development of atherosclerosis in the Watanabe heritable hyperlipidemic (WHHL) rabbit, an animal model of familial hypercholesterolemia in man, could be prevented by the reduction of serum cholesterol levels. Pravastatin sodium (the generic name of CS-514), a potent inhibitor of 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase, was used as a cholesterol-lowering drug. The drug was administered orally to 12 WHHL rabbits (2-3 months old) at a dose of 50 mg/kg per day for 24 weeks, and 13 animals were given water as control. In the treated group, serum cholesterol, phospholipid and triacylglycerol levels were significantly reduced by 28%, 32% and 16%, respectively, as compared with those of the control group. Although the prevention of development of the aortic atherosclerosis was not significant, the progression of coronary atherosclerosis was significantly prevented. The incidence of atherosclerosis in four main coronary arteries was reduced from 42% (control group) to 19% (treated group, P less than 0.01), and the development of lesion of coronary arteries evaluated by area of lesion was reduced from 19.7% (control group) to 9.1% (treated group, P less than 0.05). Histopathological findings supported the above observations. In addition, development of xanthoma in digital joints was also reduced from 90.4% (control group) to 58.3% (treated group, P less than 0.005). These results suggest that the development of coronary atherosclerosis and xanthoma in WHHL rabbit was reduced by continuous reduction of serum cholesterol levels treated with pravastatin sodium.

Substructure and higher structure of chicken smooth muscle alpha-actinin molecule.

Substructure of chicken gizzard smooth muscle alpha-actinin molecule was deduced by domainal mapping of the proteolytic fragments with alpha-chymotrypsin. There were three chymotryptic cleavage sites (Sites I, II, and III, from the amino terminus). Cleavage at Site I generated two fragments, i.e. an NH2-terminal 36-kDa fragment and a COOH-terminal 70-kDa fragment. The 70-kDa fragment generated either a 55-kDa fragment by cleavage at Site II or a 65-kDa fragment by cleavage at Site III. Purified NH2-terminal 36-kDa fragment bound to F-actin, whereas the 55-kDa fragment formed a dimeric molecule. Circular dichroism and electron microscopic experiments demonstrated that the alpha-helical content of the 55-kDa fragment was 14% higher than that of native gizzard alpha-actinin, coinciding with the apparently rod-shaped configuration of this fragment. A 110-kDa product was generated from two 55-kDa fragments in a cross-linking study with the zero-length cross-linker 1-ethyl-3-(3-dimethylaminopropyl)carbodiimide. Two cross-linkable sites in the 55-kDa, A- and B-site, were shown to be involved in this reaction. Further, it was demonstrated by using N-(7-dimethylamino-4-methyl-3-coumarinyl)maleimide labeling and immunoblotting analyses that the A-site on one 55-kDa fragment was cross-linked to the B-site on the other. These results suggest that smooth muscle alpha-actinin formed an antiparallel dimeric molecule in which the 55-kDa fragments connected the two actin-binding domains composed of the 36-kDa fragments.

Computed tomographic diagnosis of enterovesical fistulae: barium evacuation method.

We reviewed and compared the computed tomographic (CT) findings for 5 patients suspected of having enterovesical fistulae with convential radiographic studies and the results of cystoscopy. Prior to scanning, patients were rectally administered 200 ml of 0.8% wt/vol barium solution, and then evacuated before they were administered 200 ml of olive oil via the anus and 100 ml of olive oil via the urethra. The CT following the barium evacuation successfully demonstrated the enterovesical fistulae in 4 patients. It was also diagnostic in a clinically suspected case in verifying the absence of an enterovesical fistula.

Prevalence of HTLV-1 antibodies in hemodialysis patients in Japan.

The southwestern region of Japan is known as a very high endemic area of human T-cell lymphotropic virus type 1 (HTLV-1), the etiologic agent for adult T-cell leukemia (ATL) and probable causative agent for tropical spastic paraparesis and its Japanese version, HTLV-1-associated myelopathy (HAM). Hemodialysis (HD) patients seem to be at high risk for HTLV-1 infection even in other regions of Japan because they sometimes receive multiple blood transfusions. We examined antibody against ATL-associated antigen (ATLA-Ab) in 1,132 HD patients, including 1,066 patients in nonendemic areas (Chubu and Tokyo) and 66 in a highly endemic area (Okinawa). The HD patients in Okinawa showed the highest prevalence, 21.2% (14/66), while those in the Chubu area showed the lowest, 1.1% (10/846), and those in the Tokyo area an intermediate value, 2.7% (6/220). The prevalence of HD patients in each area was significantly higher than that of local blood donors, reflecting an increased prevalence roughly corresponding to the respective endemic rate. The average prevalence of ATLA-Ab among the HD patients was 2.7% (30/1,132), which was similar to that of HBs antigen (3.2%). In the nonendemic areas, 15 of 16 patients with ATLA-Ab had a history of blood transfusions, showing a significant correlation to the presence of ATLA-Ab (P less than 0.01), although four had family histories related to the endemic area. The relative risk of the presence of ATLA-Ab for HD patients with a history of blood transfusions was calculated as 10.3. In the endemic area of Okinawa, the relationship to blood transfusion was not so close, probably masked by the high background prevalence.

Vesical dome tumors: significance of extravesical extension on CT.

Thirteen cases of vesical dome tumors were diagnosed on the basis of computed tomographic (CT) findings. Six were urachal carcinomas, and seven were of vesical origin. All six urachal carcinomas were characterized by predominantly extravesical growth along the urachus; the seven vesical carcinomas did not demonstrate this finding. Calcification in the soft-tissue-density mass was detected in three of the six urachal carcinomas and in none of the vesical carcinomas. The CT finding of vesical dome tumor was an important factor in the choice of management.
