Role of monoamines in experimental spinal cord injury in rats. Relationship between Na+-K+-ATPase and lipid peroxidation.

A spinal cord injury was produced in Wistar rats by extradural compression of the cord with a Sugita aneurysm clip for 5 seconds. During a 2-week observation period following the injury, the tissue norepinephrine (NE), dopamine (DA), and serotonin (5-HT) concentrations decreased uniformly at and below the injured site. The chemical denervation of NE or 5-HT neurons produced by the intraspinal injection of 6-hydroxydopamine (6-OHDA) or 5,7-dihydroxytryptamine (5,7-DHT) 2 weeks before the injury did not cause a marked difference in the extent of hemorrhagic necrosis of the spinal cord after trauma as compared to control animals without pretreatment. In the rats pretreated with 6-OHDA, NE was decreased to less than 30% of control (non-pretreated) values, and, beginning at 5 days after injury, motor performance (assessed quantitatively with the inclined-plane method) was significantly improved compared to results in the non-pretreated control rats. The rats pretreated with 5,7-DHT showed no change from control animals. Spinal cord samples from non-pretreated control animals obtained at the injury site 30 minutes after the compression injury showed a marked decrease in the activity of synaptosomal Na+-K+-ATPase (adenosine triphosphatase) of about 50%, and an increase in both thiobarbituric acid reaction substance (about 170%) and cyclic guanine monophosphate (about 150%). The NE-denervated rats showed no significant changes in these three parameters. The results indicated that NE released after crush injury may impair the neuronal cell membrane around the lesion site by induction of lipid peroxidation. The possible mechanisms by which released NE may alter membrane function are discussed.

[Changes in serum pancreatic enzymes and pancreatic secretory trypsin inhibitor in patients with severe acute pancreatitis].

Serum pancreatic enzymes (amylase, trypsin, pancreatic elastase 1, pancreatic phospholipase A2) and serum pancreatic secretory trypsin inhibitor (PSTI) were measured in 22 patients with moderate or severe acute pancreatitis. Serum levels of all pancreatic enzymes were elevated at the initial determination, but they fell rapidly to normal in both moderate and severe pancreatitis. In contrast, PSTI in severe pancreatitis increased after admission and reached the maximum on the second to the forth day after onset. There was a significant positive correlation between the level of PSTI and that of acute phase reactant (fibrinogen, alpha 1-antitrypsin), and serum PSTI in severe acute pancreatitis changed as if it was one of acute phase reactants. There was also a significant negative correlation between the level of serum PSTI and that of alpha 2-macroglobulin.

[Phase II study of 5'-deoxy-5-fluorouridine (5'-DFUR) in patients with malignant cancer--a multi-institutional cooperative study].

A phase II study of oral 5'-deoxy-5-fluorouridine (5'-DFUR) was conducted on a multi-institutional basis. Six hundred and ninety-two cases were entered into this study and evaluated by the extramural review committee. In 437 evaluable cases the response rate was 16.0%, 13 CR and 57 PR. The response rate of gastric cancer was 14.3%, 1 CR and 19 PR out of 140 evaluable cases, that of colorectal cancer was 9.2%, 1 CR and 6 PR out of 76 evaluable cases, and that of breast cancer was 35.9%, 11 CR and 26 PR out of 103 evaluable cases. The response rate was high with a long-lasting efficacy in breast cancer. Although the number of cases was small, PR was observed in 3 cases of head & neck, and in 1 case each of esophagus, gall-bladder and thyroid cancer. The optimal daily dosage was considered to be 800-1,200 mg/body/day. The tumor began to decrease in size within 8 weeks of treatment in most of the responded cases. Side effects were observed in 44.4% of 513 cases. Diarrhea occurred with the highest rate (26.3%), but this was controllable.

[Preoperative cancer chemotherapy for gastric cancer].

Preoperative cancer chemotherapy for gastric cancer was reviewed with special emphasis on histologic findings and survival. Preoperative chemotherapy with intravenous, split administration of MMC 40 mg caused considerable damage to "micro-solitary metastatic foci" in metastatic lymph nodes. In view of the lipid-adsorbing ability of the lymphatic stream, emulsified 5-FU was used orally in 182 patients with gastric cancer; histologic findings revealed that the emulsified 5-FU enhanced the antitumor efficacy for metastatic lymph nodes as well as the primary lesion. However, the 5-year survival rate for gastric cancer patients undergoing preoperative emulsified 5-FU therapy did not differ from the control, with only the exception of patients with Stage III gastric cancer. On the other hand, combined therapy involving preoperative intra-arterial infusion and surgery was carried out in 62 patients with gastric cancer. These preoperative treatments using MMC, 5-FU, VLB, MTX and/or cytosine arabinoside entailed continuous infusion for 15 to 20 hours; the histologic changes observed revealed marked antitumor effects on the primary focus as well as metastatic lymph nodes. The five-year survival rate for the 62 patients was compared with that for 99 patients with gastric cancer in the corresponding period. The survival rate was analyzed based on the degree of serosal invasion. The overall survivals in the 62 patients were higher than those in the controls for the first 3 years. At 4 to 5 years, the survival rates for both the treated and control groups were approximately equal. In patients without serosal invasion, the survival rates were higher in treated cases than in the controls for the first 2 years. Thirty-nine patients with serosal invasion had significantly higher survival rates than the controls for the first 3 years. The survival rates for the treated patients with cancerous infiltration of ther organs were about the same as those for the corresponding control patients.

The age distribution of human adult cancer and an initiation-manifestation model for carcinogenesis.

The many studies of cancer made in recent years have revealed that the preclinical period of cancer--the period from carcinogenic transformation of a cell to growth of an overt tumor when the diagnosis of "cancer" is made--is longer and has a larger variance than considered formerly. Based on these results, the existing multistage theory of carcinogenesis has been reviewed and modified in this paper. In the newly obtained model, which is designated "initiation-manifestation model," it is assumed that carcinogenesis is a combination of two different processes, i.e., carcinogenic transformation of one cell and the subsequent manifestation of cancer. Further, we assumed that the lengths of these two periods are stochastic variables having gamma distributions, and that they are mutually independent. Applying this model, the actual morbidity of various types of human adult cancer was analyzed.

[Basal encephalomeningocele in an adult--a case report and clinico-anatomical classification].

A 53-year-old male was admitted with complaints, of the recurrent cerebrospinal fluid rhinorrhea and meningitis. Basal encephalomeningocele was revealed by radionuclide cisternography, skull x-ray studies, and CT scan. Then, it was confirmed by operation. In reviewing the literature, we proposed a new classification of fronto-basal encephalomeningocele from clinical standpoints. 1) Anterior type; detectable facial anomalies, hypertelorism, building frontal mass or masses. Reparative operation is easy. 2) Intermediate type; no facial anomalies, cerebrospinal fluid rhinorrhea, meningitis, nasal polyp. Good results can be expected through intracranial approach. 3) Posterior type; associated many congenital anomalies such as coloboma, agenesis of the corpus callosum, and cleft palate and lip. In this type, intracranial operation has potential hazards. In our case, elevated CSF pressure due to hemodialysis for chronic renal failure may result in the late onset of CSF rhinorrhea. Etiology of 10 reported cases of the basal encephalomeningocele in Japan was also discussed.

[A case of infected subdural hematoma (subdural empyema) secondary to septicemia caused by agranulocytosis].

A 76-year-old female was admitted with a headache. She had no febrile course before admission. Computed tomography (CT) demonstrated bilateral frontal hypodense areas without enhanced rim. She was made a diagnosis of a chronic subdural hematoma. Because her general condition was poor and she had no neurological deficit, she was planned to be treated conservatively. On a few days after the admission, she was suffered from agranulocytosis. However, leukopenia disappeared within a few days by the effective treatment and any neurological deficit was not observed in these period. On thirty days after the admission, she rapidly became semicomatous state and showed left side hemiparesis. A subdural empyema was demonstrated by the subsequent operation. Both culture of subdural fluid and urine yielded Escherichia coli. Her neurological deficits cleared after the operation and subsequent antibiotic therapy. We speculated that infection of urinary tract produced a E. coli bacteremia and subsequently infected subdural hematoma occurred by this microorganism. We stressed that when the neurological deterioration was observed during the conservative treatment of chronic subdural hematoma, infected hematoma would be one which should be one differentiated from an enlargement of hematoma. The mechanisms of the rim enhancement observed at CT are also discussed.

[Clinical studies on preoperative abdominal arterial infusion of anticancer agents].

Operable patients with gastrointestinal cancer were injected once with MMC 0.4 mg/kg, ADM 0.8 mg/kg or KW 2083 1 mg/kg through the abdominal artery 7 to 15 days before operation. Biopsy was made on liver tissues during operation, and changes of serum biochemical test values were observed before and after operation. In 7 cases of MMC 20 mg arterial infusion, 7 cases of ADM 40 mg and 6 cases of KW 2083 50 mg liver biopsy showed fibrosis around the central vein. Eosin bodies, neutrophils or necrotic lesions were observed in some other cases. Serum biochemical test values were transiently affected with anesthesia, but these changes will have no unfavorable influence on the results of operation and following treatment.

[Preclinical studies of intra-arterial chemotherapy using mitomycin C microspheres].

Heated albumin microspheres 45 +/- 8 microns dia. containing 5% mitomycin C were infused into rabbit femoral artery to assess the depot effects. MMC levels were measured in the muscle and VX -2 tumor tissues fed by the femoral artery as well as in the drainage vein blood. Furthermore, the histologic changes in the VX -2 tumor and the MMC microspheres entrapped in the arterioles were surveyed microscopically. Drug concentration in the case of MMC microspheres was maintained at high levels in both tissue and venous blood over 4 hours, but in the rabbits infused conventional MMC, drug levels decreased below the assay limitation 2 hours after injection. The microscopic findings 2 weeks later revealed necrotic VX -2 tumor tissue as well as the MMC microspheres remaining in the arterioles .

[Chemotherapy of unresectable Borrmann type 4 gastric cancer].

We have reported the results of our questionnaires collected from 108 hospitals all over Japan. The mainly used therapies were 5-FU, tegafur, and MMC in singular medications and 5-FU + MMC, MFC and tegafur + MMC in combined medications. The total cases judged as 'effective' in each hospitals were 71. The breakdown is as follows: 1) 'effective for the primary lesion'--47, 33--expansion of affected site proved by radiological and endoscopic views, 6--improvement only by endoscopic findings, 8--shrinkage of palpable tumor size. 2) 'ineffective for the primary lesion'--24, 11--disappearance or decrease in ascites, 13--improvement of sign and symptom. 50% survival period was 2.9 months in ineffective cases and 8.5 months in all effective cases and 10.5 months in effective cases by radiological and endoscopic findings. Draft of the Criteria of Cancer Chemotherapy for Gastric Cancers proposed by Japanese Research Society for Gastric Cancer, which including the evaluation of Borrmann type 4 cancer, was introduced.

[Experimental studies on hepatic cancer chemo-embolization].

Mitomycin microspheres (MMC MS), which contain about 5% of MMC and have an average diameter of 45 +/- 8 microns, were administered into the rat hepatic artery in a preclinical study on antitumor treatment for human hepatic cancer. Plasma GOT and GPT activities increased markedly 24 hours after MMC MS injection; they decreased to within the normal range within 3 days of the injection. Hepatic necrobiosis was observed in the area adjacent to the hepatic arterioles containing MMC MS, but was not observed in rats injected with placebo microspheres. The hepatic vein blood of MMC MS treated rats contained a markedly high level of MMC, compared to rats treated in the conventional fashion with MMC. Furthermore, MMC MS remained within hepatic arterioles for over 3 weeks.

[Experimental hyperthermo-chemotherapy for human gastric carcinoma transplanted in the nude mice].

Human gastric carcinomas serially xenotransplanted into BALB/c nu/nu mice were treated by hyperthermo-chemotherapy with mitomycin C (MMC). The antitumor efficacy was assessed by both single and double treatments of 25-minute hyperthermia (43.5 degrees C) and/or 2.0 mg/kg of MMC (ip). Tumor weight was estimated using Battelle's Columbus Laboratories protocol. To assess DNA biosynthesis in the tumor cells, the xenotransplanted tumors were excised at prescribed times after these treatments 60 minutes after 3H-thymidine injection (ip), and were examined microscopically. In the group treated twice by hyperthermo-chemotherapy marked growth inhibition and microscopic damage of the tumors were observed, while such features were not recorded in groups treated twice by hyperthermia only and chemotherapy only, nor in groups given single hyperthermo-chemotherapy, or hyperhermia and chemotherapy. In the single-treated groups, DNA biosynthesis was remarkably inhibited by hyperthermo-chemotherapy over 24 hours. The present results suggest that repeated treatments of hyperthermo-chemotherapy with MMC may be effective in the treatment of human gastrointestinal cancer.
