[Therapy and compliance in angina pectoris--analysis of a physician and patient survey].

Positive compliance is essential for the successful treatment of coronary heart disease. To study the reasons for non-compliance of patients with angina pectoris 50 patients out of a sample of 2000 persons were recruited. Randomized telephone-calls were used. The 50 angina pectoris patients were interviewed using a semi-structured questionnaire. Another questionnaire was sent to all general practitioners and specialists for internal diseases in Austria. 675 doctors replied. The responding physicians reported a 68.9% positive compliance of the patients to pharmaceutical treatment of angina pectoris. Nevertheless most of the physicians believe that about half of the patients sometimes forget to take their medicine or change their therapeutic scheme or don't take the prescribed drugs if they have no symptoms. Patients suffering from angina pectoris confirm compliance problems, but significantly less often than doctors.

[The Delphi panel "atherosclerosis report"--concept and experiences].

While the overall mortality in Austria is decreasing and life expectancy is increasing the mortality rate from ischaemic heart disease has been stable for more than 15 years despite therapeutic progress. In many countries well-established prevention programs have helped to decrease the cardiovascular mortality by more than the overall mortality. In order to provide the basis of a nationwide program to reduce morbidity from cardiovascular diseases, their epidemiological importance must be documented convincingly, as well as the available strategies for their prevention and therapy. The most convincing form of documentation is a joint statement by leading medical experts. Hence, the "Delphi panel" method was chosen as the most appropriate approach. In Austria, the Institute of Social Medicine, University of Vienna and the Austrian Society of Hygiene, Microbiology and Preventive Medicine published the "Atherosklerosebericht" (atherosclerotic report), result of a written Delphi panel, in June 1988. Concept and practical experiences are reported below.

[Total cholesterol findings in Austria--an overview of epidemiologic studies].

Hyperlipoproteinaemia is the most important public health risk factor for ischaemic heart disease. This publication gives a review of six Austrian epidemiological studies covering more than 87,000 people in all. The results are not representative for Austria, but give a picture of the epidemiology in Austria. Mean values were calculated age-standardized (age-adjusted when ten-year age groups are registered) to the Austrian population of 1985, for the age group 20 to 79 years. This age range covers 99.77% of the adult male population aged 20 or more, and 95.70% of the women. Mean total cholesterol level for the six studies is 224.2 mg/dl in men and 227.3 mg/dl in women. These results are similar to those of a representative study in the province of Vorarlberg in western Austria (220.0 mg/dl, and 227.4 mg/dl respectively). Variation range is 20.4 mg/dl in men, and 22.5 mg/dl in women; the coefficient of variation is 3.9% in both, men and women.

[Endemic struma and its relation to various drinking water factors in 4 communities of the Suhl district].

By means of anamnestic and clinical examinations carried out on 1,055 test persons of all age groups in four adjacent communities of the County of Suhl an average frequency of struma of 53.9% was stated. The strumata of the size stage I with a diffuse condition and a preference of the younger age groups and the female sex prevailed. The prophylactic and medicamentous measures of the combat against struma introduced up to 1984 are to be estimated as ineffective. The distinctly greater formation of struma (64.6%) which is to be proved in one community might be caused by a correlating significantly higher nitrate content of the drinking water with existing considerable deficiency of iodine. No connection could be found between the frequency of struma and the calcium and magnesium content of the drinking water as well as the hardness of water and the consumption of potassium permanganate. Within the framework of a complex combat against goitre an improvement of the iodised common salt prophylaxis, measures for the decrease of the nitrate content in drinking water and soil as well as a more effective registration and treatment of the strumata are necessary.

[The incidence of hyperthyroidism in the Suhl district in 1985 and 1987].

An increase of the number of treated hyperthyroidosis-patients within 2 years until 174% was found out in the years 1985 and 1987 by a comparative prescription analysis in the course of 3 months. Sex- and age incidence of the patients did not change in the control period. A more effective iodine prophylaxis of the population has to be supposed as cause for it. The portion of treated patients in the district increased on an average from 7.6 patients/10,000 population in 1985 to 13.2 patients/10,000 population in 1987.

[The prevalence of hyperlipidemia and obesity in Vienna health examinations 1986].

A random sample of 20% of people taking part in the free health check-ups provided by the city of Vienna in 1986 (707 men and 1022 women), aged 20 to 79 years (mean age men 48.1 +/- 15.8, women 49.6 +/- 15.1 respectively) were retrospectively evaluated to determine coronary heart disease risk according to the recommendations of the Austrian Cholesterol Consensus Conference. Age-standardized mean total-cholesterol is 230.0 mg/dl in men and 235.8 mg/dl in women. Age-standardized percentage of normal lipid levels (i.e. total-cholesterol below 200 mg/dl) is 29.7% in men, 25.7% in women respectively. The high-risk group (i.e. total-cholesterol more than 250 mg/dl) has an age-standardized prevalence of 28.5% in men and 35.8% in women. The age-standardized percentage of obesity according to the guidelines of the National Institutes of Health is 14.2 in men, 17.2 in women respectively. The comparison of age-standardized men-values of total-cholesterol, LDL-cholesterol, triglycerides and Quetelet-index, as well as normal lipid levels according to the Austrian recommendations and percentage of obesity, in the age groups 20 to 39, 40 to 59 and 60 to 79 years shows a statistical significant difference of each parameter between age-group 20 to 39 and age-group 40 to 59 in men, but no significant difference in any parameter between age-group 40 to 59 and 60 to 79 in men. On the contrary in women differences are significant between age-group 20 to 39 and 40 to 59, as well as between age-group 40 to 59 and 60 to 79 in each parameter.(ABSTRACT TRUNCATED AT 250 WORDS)

[CEA studies in endometrial cancer].

In the years 1983 to 1986 concentrations of CEA in serum samples of 63 female patients with endometrial cancer have been estimated with help of a radioimmunoassay. In all histopathological stages 25 per cent elevated CEA-concentrations could be found pretherapeutically. It is given the semilogarithmic representation of the CEA-values of the individual basic lines in the posttherapeutical follow up. Main field for estimations of CEA-concentrations in serum is only the performance of a posttherapeutical monitoring of the operated and irradiated endometrial cancer.

[Placental protein 12 (PP 12) in cyst and peritoneal fluids and serum samples of patients with ovarian cysts and cystic ovarian tumors].

Placental Protein 12 (PP 12) concentrations were measured by a radioimmunoassay in cyst and peritoneal fluids and in serum samples respectively of patients with tumor like conditions and benign and malignant cystic ovarian tumours. PP 12 can not recommended as a serum tumour marker in ovarian cancer. It must be proved the value of PP 12 in follow up of ovarian cancer with help of more extensive sample material. There are many cysts in tumour like conditions with measurable PP 12-concentrations.

[Circulating T cells in patients with breast or genital cancers in the pre-therapeutic phase].

There was the evaluation of circulating T-cells with the help of the E-rosette-assay in patients with breast cancer (n = 52), carcinoma of the cervix uteri (n = 29), carcinoma of the endometrium (n = 32) and carcinoma of the ovary (n = 7). Twenty seven healthy female blood donors were used as control group. There was no statistically significant difference between the total and active E-rosette values of the patients with tumours and the control group. Patients with breast cancer pT1N0 had significant lower total E-rosette values (Gpt/l) than patients with higher tumour stadium (p less than 0.05). The white blood cell count showed a statistically significant leucocytosis in cancer patient groups versus control persons (p less than 0.05).

[Determining binding proteins of progesterones in patients with breast diseases].

We estimated serum levels of progesterone and the most important binding proteins in follicular and luteal phase at women without breast disease with biphasic or anovulatory cycle and female patients with benign or malignant breast diseases. There was an increase of the median value of acidic alpha-1-glycoprotein in the luteal phase at female patients with premenopausal breast cancer. The importance of serum levels of the bioavailable sexual steroids for the recognition of women with breast diseases is discussed.

Insertional activation of a promoterless thymidine kinase gene.

A plasmid carrying a promoterless herpes simplex virus thymidine kinase gene was transfected via calcium phosphate precipitation into LM (tk-) mouse fibroblast cells. The transfected gene was efficiently expressed, as the transfected cells grew perfectly well in selective hypoxanthine-aminopterin-thymidine medium, suggesting that the thymidine kinase-coding region became linked to a promoterlike element on integration into the recipient genome. To investigate the structure of the surrogate promoter, we first isolated the integrated gene from a genomic library. The nucleotide sequence of the DNA adjacent to the thymidine kinase-coding sequence was then determined. We found, first, that the integration of the transfected DNA apparently occurred by a blunt end ligation mechanism involving no obvious sequence similarities between integrated and recipient DNA and, second, that the 5'-flanking region included a TATA box, two CCAAT boxes, and a GC box element. However, the TATA box motif and the most proximal CCAAT box appeared to be sufficient for full promoter activity, as determined by the transfection efficiencies of appropriate plasmid constructs. Except for these canonical promoter elements, the surrogate promoter had no obvious similarities to known thymidine kinase gene promoters.

[CEA determination in aspired fluid of cystic ovarian tumors and ovarian cysts and in ascites and serum samples of patients with ovarian tumors].

Carcinoembryonic antigen (CEA) was assayed by radioimmunoassay in serum of 47 patients with cystic ovarian tumors, in 51 cases of cyst fluids of ovarian tumors and ovarian cysts, and in 16 cases of peritoneal effusions (ascites). In order to differentiate between cystic tumors and retentional cysts there was found, in consideration of a changed prevalence (0.65), a sensitivity of 55% and a specificity of 94%. The predictive value in regard to a positive assay result was 94% and in regard to a negative one was 53%. Elevated CEA-levels in ovarian tumor cyst fluids are mainly diagnostic for mucinous ovarian tumors. In regard to elevated CEA-levels in cyst fluids of ovarian tumors it is claimed the laparotomy and histological investigation.

[Current resistance status of pathogens isolated from bile and its development since 1976 in the Suhl district].

Bile was collected intraoperatively and through duodenal tubes from patients and was microbiologically investigated. E. coli and coliform bacteria were found to be the most common germs in bile, accounting for 51.6 per cent of all positive findings. Streptococcus faecalis and Proteus spec. increased remarkably to 17 per cent each. Calculated antibiotic therapy was found to depend for good success on knowledge of the latest resistance rates of the most common germs. These were 67 per cent E. coli to ampicillin and 40 per cent for E. coli to tetracycline and 53 or 91 per cent for Str. faecalis or Proteus spec. to tetracycline. Hence, these preparations will have to be abandoned for calculated therapy, say, for cholangitis.
