Evidence of thyrotropin-releasing hormone (TRH) and TRH-binding sites in human nonsecreting pituitary adenomas.

Immunoreactive TRH (IR-TRH) and TRH-binding sites were sought in nonsecreting pituitary adenomas. [3H]TRH bound specifically to cellular membranes from 11 of 12 such adenomas studied, with a dissociation constant (Kd) of 50 +/- 5 (+/- SEM) nmol/L and a maximum number of binding sites of 76 +/- 16 fmol/mg membrane protein (range, 32-229 fmol/mg protein). IR-TRH was detected in all 8 of the tumors in which it was sought. The identity of the IR-TRH was verified by high pressure liquid chromatography. The tumor IR-TRH concentration varied from 45-248 fmol/mg cell protein (mean, 109 +/- 28 fmol/mg cell protein), about half that in normal human pituitary (229 +/- 55 fmol/mg protein). There was no correlation between the number of binding sites and the IR-TRH content. The role of TRH in nonsecreting pituitary adenomas is unknown at this time.

[Involvement of arterial vessels in the blood supply to adenomas of the human pituitary, functional implications].

Among 190 human pituitary adenomas investigated, 140 contained arterial blood vessels (nearly 75 percent). In 160 adenomas (PRL-, GH-secreting or nonfunctioning tumors) arterial vessels were present in 80 to 90 percent of the cases, whereas in 30 corticotropic adenomas (Cushing's disease), 5 only contained arterial vessels. According to data of the literature and to our own findings, such vessels should originate from the anterior pituitary itself or from the surrounding structures (trabecular, interlobar, capsular arteries). This arterial blood supply could appear even at the microadenoma stage and increase as the tumor growths. Consequently, the blood from portal origin will be diluted or excluded, so that tumor cells would escape from hypothalamic control.

[Topographic distribution of mitoses in the adrenal cortex of the rat].

The situation of mitoses in the adrenal glands of the rat has been studied by measuring the distance between each of them (3,061 in the whole) at the surface. The curve of their distribution, logarithmic-normal type, shows that they are the most numerous in germinative zone classically described, but that they appear down to the depth of inner fascicula.

Operability of intracranial meningiomas. Personal series of 353 cases.

The senior author's experience has been reviewed in 353 operated meningiomas; so called "malignant meningiomas" have been discarded. The whole series has been split into two groups: the first (A) includes 273 cases operated before the CT era, the second (B) consists of 80 cases the operability of which was based mainly on the CT. In addition, during the time elapsed to constitute the second series surgery has been refused to 15 patients which were considered for CT and clinical reasons as surgically incurable when one wants to reduce the risk of mortality and morbidity. Therefore, we have reviewed in series B indisputable, questionable and incurable cases with respect to operability in the present condition of our technical possibilities including arterial hypotension, the operative microscope and the CUSA. Considering localization, we have made technical observations with respect to the principles on which operations have been based since 1958 when general anaesthesia became available in La Pitié. Among the patients of the whole series, 14 were admitted for a recurrence. The time interval between surgery and readmission has been given as well as the primary location of the meningiomas. The mortality rate has been 26 out of 353 (7%), and the main causes have been reviewed especially in series B. With regard to series B, we have been able to get in touch with 53 patients out of 80; 90% of them returned to work or family without sequelae. Intracranial meningiomas remain the most curable tumours, nevertheless some of them must be considered incurable like in meningiomatosis or when the dural attachment is extensive. For those cases surgery is not the final answer and we must accept the fact that in the future other kinds of treatment will develop.

[Mucigenous cysts of the pituitary gland. 3 cases].

Three cases of mucous cysts, or Rathke's cyst, of the pituitary gland are reported. The clinical and biological features were comparable to those of a non-secreting adenoma. Although extremely rare, these cysts must be recognized since their particular and, in theory, specific characteristics (wall made up of a ciliated epithelium with mucus cells and cavity filled with a mucoid substance) distinguish them from other intra or suprasellar pathological formations, especially craniopharyngiomas. The 62 cases reported in the literature suggest that they have a favourable prognosis.

[Nonfunctional or nonsecreting hypophyseal adenomas. Review of clinical, biological, neuroradiological, developmental and immunohistochemical characteristics. Apropos of 40 cases].

The authors report 40 cases of functionless pituitary adenomas, investigated by the same protocol and treated in the same way, between 1976 and 1982. The clinical, biological, neuroradiological features, clinical course and anatomopathological findings of this very uniform series are described. A complete immuno-histochemical study was undertaken. These findings suggest that the diagnosis of a true functionless pituitary adenoma should be based on the following criteria: --clinical and biological: absence of hormonal hypersecretion; --histological: absence or very few signs of secretory activity (little ergastoplasma, few mitochondria and Golgi apparatus); --immuno-histochemical: absence of reactivity to immunofluorescence and immuno-peroxydase methods.

[Tumoral medullary compressions in malignant hemopathy (a 28 cases review (author's transl)].

The authors reviewed 28 cases of tumoral medullary compressions in malignant hemopathy. At once they point out 2 groups according to the fact that this compression is or not the revealing symptom of the disease. With regard to different data, that is to say, preoperative clinical examination with complementary investigations, and then results obtained from decompressive laminectomy followed or not with complementary therapy; the elements of the functional prognosis are analysed. They insist on frequency of medullary compression in a neuro surgical environment, as the revealing symptom of the hematologic disease and its immediate decompressive therapy is emphasized.

[The histologic extemporaneous examination in neurosurgery. About 1000 cases (author's transl)].

One thousand extemporaneous examinations performed in the neurosurgical department over a nine years period have been reviewed. The purpose of these examinations was to deal with the following points: - presence or absence of a tumor, - primary or secondary nature of this tumor, - appreciation of the malignancy degree. It seems possible to draw the following conclusions: The frequency of false diagnoses was not significantly different from the one observed in out precedent series. On the other hand, with experience, the number of "approximate diagnoses" was strongly reduced. Some special diagnosis problems were discussed. The importance of the clinical data in establishing a correct diagnosis was emphasized.

GFA and S 100 protein levels as an index for malignancy in human gliomas and neurinomas.

Human glia-specific proteins S 100 and GFA were quantitated by use of a rocket immunoelectrophoresis technique with monospecific antisera. No relation was found between the S 100 protein content of an astrocytoma and its degree of neoplasia. However, the lower the GFA protein content of the astrocytoma, the more malignant it was. Similarly, the more malignant a neurinoma was, the lower was its S 100 protein content. Therefore, the levels of these proteins might be used as indexes of neoplastic dedifferentiation.

[2 cases of alveolar sarcoma. Study by electron microscopy and cytoenzymology].

Study by electron microscopy and light cytoenzymology of two cases of alveolar sarcoma of the thigh occurring in young men indicated it to be an active tumour with an apparently functional organoid arrangement, characterised by an abundance of capillary vessels. The secretory nature of the cell elements was suggested by the marked development of smooth and granular reticular systems. The morphological result of this secretory activity was constituted by vesicular bodies with dense contents, surrounded by a membrane and sometimes containing periodic paracrystalline structures. This intravacuolar crystalline organisation goes hand in hand with an increase in acid phosphatase activity suggesting possible crinophagia. Amongst the different hypotheses advanced in the past, the morphological appearances seen here would most likely correspond with the endocrine nature of this tumour.
