[Effect of the number of transferred embryos on multiple pregnancy].

OBJECTIVE: The aim of this retrospective study was to assess the influence of the number of transferred embryos on multiple pregnancy. DESIGN: Retrospective clinical study. SETTING: Department of Obstetric and Gynecology FN UP, Olomouc. METHODS: Over the last seven years 572 embryotransfers (ET) have been performed in our centre. RESULTS: Of the 173 pregnancies (P) achieved there were 56% singletons, 35% twins and 9% triplets. One embryo was transferred in 10% of cases and all 8 pregnancies (15% P/ET) ended with a live birth. Two embryos were transferred in 17% of cases. The pregnancy rate was 20% P/ET, 55% of pregnancies were twins and 10% pregnancies were lost in abortion or ectopic pregnancy. Three embryos were transferred in most cases (62%). The pregnancy rate was the highest in this group (36% P/ET) with 13% twins and 3% triplets. The overall pregnancy loss was 16%, the highest loss, 27%, recorded in triplets. Four embryos were transferred in 11% of cases. The pregnancy rate decreased to 28% P/ET, as well as the number of twins (6%), while the number of triplets increased slightly (5%). Pregnancy loss occurred only in singleton pregnancies. CONCLUSION: We can conclude that in our setting the pregnancy rate increases with a higher number of embryos up to three embryos transferred. Unfortunately, the higher pregnancy rate with three embryos transferred is on the expense of a higher risk and losses due to multiple pregnancy. Therefore we recommend to transfer two embryos unless there are some specific reasons.

[The EUROPOP Project. European Programme of Occupational Risks and Pregnancy Outcomes].

Efforts to obtain an objective view of the working and living conditions of European women and in particular the influence of these conditions on the course of pregnancy were the reason why in 1994 within research activities of the EC a project EUROPOP (European Programme of Occupational Risk and Pregnancy Outcome) was adopted and started. Seventeen countries incl. the Czech Republic were asked to participate. The research proper was conducted in 57 maternity institutions. In the Czech Republic the Olomouc region was selected with the Gynaecological and Obstetric Clinic in Olomouc as the coordinating centre. All 13 gynaecological and obstetric departments of the Olomouc catchment area were included in the trial.

[EUROPOP--European Programme of Occupational Risks and Pregnancy Outcome--part II].

As continuation of their previous paper the authors submit the results of the European study EUROPOP pertaining to the effect of occupation on the course and outcome of pregnancy. The paper contains the first part of results where the authors compare groups of women with premature deliveries (22nd-36th week of pregnancy) with control groups 1/10 deliveries in term (> or = 37 weeks). The groups of abortions in the second trimester which were studied by the authors were relatively small (Czech Republic 49 and in the all-European study 229). To keep the study within a reasonable limits the tables are not presented. Comparison of results describing home work, living conditions of pregnant women, prenatal care and medical problems as well as data on the outcome of pregnancy are presented in part III.

[EUROPOP--European Programme of Occupational Risks and Pregnancy Outcome--part III].

As continuation of the previous paper ther authors submit the second part of the results of the European programme EUROPOP which evaluates the relationship between work, living conditions of pregnant women, prenatal care and medical data on the outcome of pregnancy. The authors compare groups of women, who had premature deliveries (22nd-36 week of gestation) with control groups 1/10 deliveries in term (> or = 37 weeks). Groups of abortions in the second trimester which were investigated were relatively small (Czech Republic 49 and all-European study 229). To keep the paper within reasonable limits the tables are not presented.

[Results of a study of serum hormones and biochemical findings in seminal plasma in subfertile men examined at the Clinic for Gynecology and Obstetrics in Olomouc 1995-1996].

Based on a comparative study in a group of healthy men and men suffering from oligospermia and azoospermia, the authors consider it useful to examine FSH, LH, PRL and testosterone serum levels for possible evidence of testicular hypofunction and for detection of stress situations associated with raised PRL levels. As to seminal plasma, the authors consider it useful and relevant to assess citric acid levels, total acid phosphatases and in particular fructose with alpha-glucosidase.

[Results of IVF and ET 1992-95 at the Gynecological-Obstetrical Clinic in Olomouc--comparison of stimulation protocols].

The authors compared three types of stimulation protocols used in their department in 1992-1995. In the standard protocol they evaluated the use of hMG and FSH with regard to the number of abolished cycles and the number of achieved pregnancies. They compared protocol No. 1 "standard: CC + G (FSH), protocol No. 2 "short": GnRH-a + hMG (FSH) and protocol No. 3 "long" GnRH-a + hMG (FSH). They evaluated the number of abolished cycles, the effectiveness of the protocols as regards the number of obtained oocytes, the number of performed embryo transfers, the number of clinical pregnancies/embryo transfers and the number of cryopreservations. The abortion rate in relation to the used protocol was also evaluated.

[Risk of fluid overload syndrome during hysteroscopy procedures].

At present increasingly liquid media are preferred to gaseous ones in hysteroscopic procedures. In operations the former media are used almost exclusively. The use of liquid media involves, however, some risks, the greatest one being the so-called fluid overload syndrome. It develops as a result of overload and upset of the milieu interior due to excessive escape of the distension medium. Medium escapes also via the oviducts into the peritoneal cavity. It is therefore necessary to adhere to safety measures which include in particular selection of a suitable medium, the time factor, correct surgical procedure and specially continuous control of consumption of the medium.

Fetal fibronectin in patients at increased risk for premature birth.

OBJECTIVES: The objective of this prospective study was to evaluate fetal fibronectin bedside test (Mast Diagnostica) to rule out preterm labor. STUDY DESIGN: 46 patients from between 25 and 36 weeks were included in the study over a six months period. All patients underwent only one cervical sampling at the beginning of the study. The association between the fetal fibronectine test results and various outcomes were analysed. RESULTS: 5 of the 16 women whose test results were positive had early labor and 4 of the 30 patients with negative test results had preterm delivery. The bed side test had a sensitivity of 55.6%, a specificity of 70.3%, a positive predictive value of 31.3%, and a negative predictive value of 86.7%. The addition of tocolytic therapy and intensive care resulted probably in decrease of the predictive capacity of a positive fetal fibronectine test, odds ratio 2.95 (95% confidence interval 0.54-16.85). However interval from diagnosis to delivery was shorter (P < 0.025), cervical findings (Bishop score) worse and betamimetic treatment was more frequent (P < 0.01) in the group with positive test results. CONCLUSIONS: Positive fetal fibronectine test results indicates a significant risk for the subsequent early delivery and negative test result is a good indication that subsequent preterm labor and early delivery are unlikely to occur.
