Esophagocutaneous fistula after anterior cervical spine surgery and successful treatment using a sternocleidomastoid muscle flap. A case report.

An esophagocutaneous fistula following anterior cervical fusion is rare. A 61-year-old man had cervical myelopathy because of ossification of the posterior longitudinal ligament of the cervical spine. Anterior decompression of the cervical spine and anterior fusion with strut bone grafting were performed. A second anterior fusion was done because the graft was dislodged after the patient fell out of bed one month after surgery. An esophagocutaneous fistula occurred three months after the second anterior surgery. One of the causes of this esophagocutaneous fistula was considered to be a pressure necrosis of the esophagus because of to projection of the bone graft. Conservative treatment, which consisted of wound drainage and intravenous administration of antibiotics, was tried but was unsuccessful. A good result was achieved by cancellous bone grafting, closure of the esophageal fistula, and transposition of a sternocleidomastoid muscle flap to the interspace between the esophagus and the cervical spine.

Hypocholesterolaemic factor from gallbladder cancer cells.

A patient is reported with drug-resistant familial hypercholesterolaemia in whom serum cholesterol fell after the onset of gallbladder cancer with intraperitoneal invasion. Cancer cells were obtained from ascitic fluid and a cultured cell line established. Incubation of the culture medium of these cells with skin fibroblasts from the patient and from normal subjects increased low-density lipoprotein (LDL) receptor activity in a dose-dependent manner. These results show that gallbladder cancer cells from this patient secrete a substance that stimulates LDL receptor activity. This substance may have contributed towards reduction of serum cholesterol.

Polymorphism of age-related changes in interleukin (IL) production: differential changes of T helper subpopulations, synthesizing IL 2, IL 3 and IL 4.

Inbred mice were examined for strain differences in age-associated changes in the capacity to synthesize interleukin (IL), i.e. IL 2, IL 3 and IL 4 after stimulation with phorbol myristate acetate (PMA) and calcium ionophore (A23187). Production of IL 2 remains constant in A/J, DBA/1 and DBA/2 mice and decreases with age in one of the strains examined (C57BL/6J). Strain differences in age-associated change of synthesis did not show the relation between youthful synthetic capacity and magnitude of later decrease ("economic correction") which is observed in several other systems. This difference between different types of polymorphisms is attributed to an age-associated defect in intrinsic capacity to synthesize IL 2 which may occur in only one of the tested strains, C57BL/6J. In contrast to IL 2 production, the quantities of IL 3 and IL 4 increase progressively, with advancing age, in mice of the three strains tested. T cells from old mice contain a greater frequency of cells producing IL 3, than do those of young mice. In addition, synthesis of IL 3 is induced at a relatively lower concentration of PMA in cells from old animals and this may be a consequence of different signal requirements of the two subsets of the T helper cells, but also a change in intrinsic properties of these cells. Since IL 3 and IL 4 production, but not IL 2 production, increases with age, it is reasonable to conclude that this reflects an expansion of a T helper cell population which secretes IL 3 and IL 4, but not IL 2, presumably TH2.

Analysis of MRI and SPECT in patients with acute head injury.

Traumatic lesions defined by magnetic resonance (MR) imaging were divided into two groups according to findings on computed tomography (CT). This classification reflected difference in the regional cerebral blood flow (rCBF). In the contusional lesions which CT could demonstrate, rCBF varied from hyperperfusion to hypoperfusion, while it was almost always decreased in the lesions which CT could not detect. These results suggest that the former may include a mixture of brain oedema and hyperemia and the latter may imply brain oedema. MR imaging can reveal the minor oedema which CT fails to show in patients with acute head injury.

Effects of endoscopic variceal sclerotherapy using GT XIII on blood coagulation tests and the renal kallikrein-kinin system.

Forty-six sclerotherapy sessions were performed on liver cirrhotics with high-risk esophageal varices using GT XIII, a sclerosant composed of gelatin, thrombin and coagulation factor XIII. GT XIII was effective for the prevention of temporary symptoms and transient hypotension observed in 55 sclerotherapy sessions using thrombin. In 42 (91%) sessions, patients underwent sclerotherapy with no symptoms, and in the other four (9%) sessions, only slight symptoms of general fatigue and headache were observed. Changes in the mean arterial pressure were significantly smaller in sessions using GT XIII than in those using thrombin (-12.3 +/- 13.6 vs. -26.8 +/- 20.7 mmHg, P less than 0.01). Changes in coagulation tests, similar to those of disseminated intravascular coagulation (DIC), were also reduced in sessions using GT XIII. Urinary kallikrein and kinin excretion significantly increased after the procedure (P less than 0.01), indicating activation of the renal kallikrein-kinin system. Increases in urinary kallikrein and kinin excretion showed a significant relationship with the consumed plasma fibrinogen levels (r = -0.51, P less than 0.01 and r = -0.58, P less than 0.01, respectively), and it was suggested that activation of the glandular kallikrein-kinin system caused by abrupt DIC-like changes in the hemostatic system might play a role in manifestations of temporary complications occurring with the use of hemostatic agents containing thrombin.

Anti-inflammatory activity of ginsenoside ro.

Ginsenoside Ro, an oleanane-type saponin has been screened for activity in experimental models of inflammation. Ginsenoside Ro (10,50, and 200 mg/kg, P. O.) inhibited an increase in vascular permeability in mice induced by acetic acid and reduced an acute paw edema in rats induced by compound 48/80 or carrageenin. Ginsenoside Ro did not suppress a developing adjuvant-induced edema in arthritic rats. However, ginsenoside Ro was found to be effective in hypercoagulable state, increase of connective tissue in the artery and calcium effluence from the bone in adjuvant-induced arthritic rats.

[Two cases of Yersinia pseudotuberculosis infection in children].

Yersinia pseudotuberculosis (Y. pseudotuberculosis) infection is an intestinal infectious disease comparable in importance as those with Campylobacter or Salmonella. Clinical symptoms of Y. pseudotuberculosis infection vary. In this report, we will describe the clinical symptoms and immunological conditions of the patients with Y. pseudotuberculosis infection, including 2 or our own cases. Case 1 was a 4 years old male infant admitted to the hospital with major complaints of fever, diarrhea, and vomiting. Kawasaki disease was the most suspected diagnosis from the clinical viewpoint. These symptoms improved by symptomatic treatments. Serum examination during hospitalisation revealed the infection of Y. pseudotuberculosis 4a. Case 2 was a 7 months old male baby with psychomotor developmental delay. The patient was admitted to hospital with major complaints of fever and eruptions. The patient was diagnosed to have a severe infectious disorder based on the clinical symptoms and findings of laboratory tests. Treatments with antibiotics improved the conditions. Serum examination during hospitalisation also revealed the Y. pseudotuberculosis 5a infection. Both of these cases showed decreased cellular immunity during the acute phase of the infection which was normalized with the improvement in clinical conditions. It was thus suggested that Y. pseudotuberculosis had a possibility to influence the cellular immunity of hosts transiently but significantly.

[Therapeutic evaluation of combination therapy using C-425, human native immunoglobulin liquid preparation for i.v. administration, and antibiotics in severe and/or refractory infections in pediatrics].

A newly developed human immunoglobulin liquid preparation for intravenous injection was studied for efficacy, safety, and usefulness in treating severe and/or refractory infections in children receiving antibiotic treatment. It is suggested that C-425 is a useful intravenous preparation of human immunoglobulin for the treatment of severe and/or refractory infections in pediatrics. C-425 was administered to 87 inpatients with severe and/or refractory infections at 23 institutions nationwide. The Committee selected 61 cases for the present analysis. Physicians in charge judged clinical efficacy of C-425 to be "excellent" in 23 cases (40.4%), "good" in 24 (42.1%), "fair" in 7 (12.3%), "poor" in 3 (5.3%), and "unknown" in 4. The efficacy rate was calculated at 82.5% when the "excellent" and "good" cases were combined, and 94.7% when the "fair" cases were also included. According to the Committee's judgement, the efficacy of C-425 was "excellent" in 27 cases (44.3%), "good" in 18 (29.5%), "fair" in 7 (11.5%), and "poor" in 9 (14.8%). The efficacy rate was 73.8% when the "excellent" and "good" cases were combined. The rate increased to 85.2% when the "fair" cases were added. Organisms were identified in 31 cases, and the time course was followed in 19 instances. Organisms were eliminated in 12 cases (63.2%), decreased in number in 2 (10.5%), and persisted in 5 (26.3%). Eradication rate was 63.2%. One of the 87 patients died of fulminant hepatitis 2 days after the end of the treatment. The remaining 86 cases were analyzed for the safety of C-425. A skin rash was observed in one case. Laboratory examination revealed increase in transaminase levels in a total of 8 cases; both in GOT and GPT in 5, in GOT alone in 2, and in GPT alone in 1. These findings were not clinically important.

[Prevention and control of nosocomial infection caused by methicillin resistant Staphylococcus aureus in premature infant ward--prevention effect of "povidone iodine solution" wipe of neonatal skin].

In January 1983, a number of premature infants under management in the premature infants' unit of our hospital were found to have bacteremia due to Staphylococcus aureus. By the end of February of the same year, 4 of these infants, who had been treated in the same unit, developed impetigo. The S. aureus responsible for this condition was classified as type IV by a coagulase typing. In a subsequent antimicrobial susceptibility test using the disk diffusion method, this microorganism was found to be resistant to methicillin, erythromycin and lincomycin, and to be susceptible to tetracycline, chloramphenicol and cefmetazole, indicating that it was a methicillin resistant S. aureus (MRSA). Because the result from the coagulase typing agreed with the antimicrobial susceptibility pattern in all cases, we concluded that these cases represented nosocomial infection with MRSA. The source and route of the infection were investigated, and measures taken to prevent bacterial spread from carriers and to keep instruments and environments clean. Although the source of infection was not identified. Then, we tried wiping the body surface of the premature infants with an Isodine solution (10% PVP-I, 1:100 dilution) in order to prevent colonization of the microorganism on the body surface. With this application+, MRSA was no longer detected from the body surface of the premature infants, and no additional MRSA infection occurred in the premature infants' unit. Data collected for premature infants' managed at our hospital in the subsequent 6 years allows us to conclude that MRSA infection can be almost completely controlled by frequent surveys of carriers and appropriate body surface wiping with Isodine solution.(ABSTRACT TRUNCATED AT 250 WORDS)

Age-related degeneracy of T cell repertoire: influence of the aged environment on T cell allorecognition.

The present study was undertaken to understand the mechanism of alteration of the alloreactivity of T cells in aged mice. The experiment utilized bone marrow chimeras revealed that the age-associated changes in alloreactivity are not primarily determined by the bone marrow stem cells but determined by the environment where stem cells differentiate into mature T cells. The limiting dilution analysis and microfluorometry analysis indicated that the changes lie in both frequency of alloreactive T cells and the expression of T cell antigen receptor. Such quantitative and qualitative changes seems to be the causes for declined alloreactivity of T cells in aged mice.

Effects of protopine on blood platelet aggregation. II. Effect on metabolic system of adenosine 3',5'-cyclic monophosphate in platelets.

The mode of action of protopine on rabbit platelet aggregation was investigated in the metabolic system of adenosine 3',5'-cyclic monophosphate (cyclic AMP) in vitro experimental models. The inhibitory activity of protopine on adenosine 5'-diphosphate induced platelet aggregation was increased in the presence of prostaglandin I2 or papaverine in platelets. Protopine elevated content of the basal cyclic AMP accumulation in platelets and enhanced activity of crude adenylate cyclase prepared from platelets, but was ineffective on cyclic AMP phosphodiesterase. It is concluded that protopine has an inhibitory activity on platelet aggregation, activates adenylate cyclase and increases cyclic AMP content in platelets, in addition to other inhibitory actions in the metabolic system of cyclic AMP.

[Pharmacological study on Aconiti Tuber. I. Effect of water extract from Aconiti Tuber on adjuvant-induced arthritis].

Anti-inflammatory activity of a hot water extract (A-ext) of Aconiti Tuber (Aconitum calmicaeli DEBX.) has been studied by using various experimental models. The successive administration of A-ext from the day of injection of adjuvant agent or 5 d after the injection it significantly inhibited the adjuvant-induced arthritis developed in the primary and secondary lesions in rats. However, when A-ext was administered from 11 d after the injection of adjuvant, the arthritic paw edema was not reduced. A-ext did not inhibit an acetic acid-induced increase in vascular permeability in mice and carrageenin-induced paw edema in rats. A-ext inhibited cotton pellet-induced granuloma in rats, but did not suppress the weight of thymus and adrenal gland. In vitro experiment, A-ext contracted the isolated ileum of guinea pig, but the contractive activity was reduced by pretreatment of anti-histamine agent, diphenhydramine, but not by pretreatment of papaverine or atropine. A-ext did not release histamine from peritoneal mast cell. These results suggest that A-ext prevents the adjuvant-induced arthritis and has a histamine-like effect.

[Pharmacological studies on leaf of Arctostaphylos uva-ursi (L.) Spreng. I. Combined effect of 50% methanolic extract from Arctostaphylos uva-ursi (L.) Spreng. (bearberry leaf) and prednisolone on immuno-inflammation].

The effect of 50% methanolic extract (U-ext) from Bearberry leaf on immuno-inflammation was studied by contact dermatitis caused by picryl chloride (PC-CD) in mice. The combined effect of U-ext and prednisolone was also investigated by using similar experimental model. When given orally twice immediately before and 16 h after the application of PC-CD, U-ext did not show an inhibitory effect on the swelling induced by PC-CD while it exhibited a significant therapeutic effect at a dose of 100 mg/kg or more once 24 h after the application. When U-ext (p.o.) and prednisolone (s.c.) in a given portion were simultaneously administered, the inhibitory effect was more potent than that of prednisolone alone in both administration immediately before and 16 h after or once 24 h after the application. Arbutin isolated from U-ext also increased the inhibitory effect of prednisolone similarly to that of U-ext. These results suggest that Bearberry leaf possesses a therapeutic effect against immuno-inflammation induced by PC-CD and also increases the inhibitory effect of prednisolone, and its active principle may be arbutin.

[Pharmacological study on Arctostaphylos uva-ursi (L.) Spreng. II. Combined effects of arbutin and prednisolone or dexamethazone on immuno-inflammation].

A study was carried out to examine the combined effect of arbutin isolated from the leaves of Arctostaphylos uva-ursi (L.) Spreng. (Bearberry leaf) and prednisolone or dexamethazone on Type IV allergic reaction-induced immuno-inflammation. Contact dermatitis caused by picryl chloride (PC-CD) and sheep red cell delayed type hypersensitivity (SRBC-DTH) in mice was not inhibited by the oral application of arbutin at 2 divided doses immediately before and 16 h after the application, but arbutin at doses of 10, 50 mg/kg 24 h after the application speedily decreased the swelling of PC-CD and SRBC-DTH. Arbutin plus prednisolone or dexamethazone showed the inhibitory effect on the swelling of PC-CD and SRBC-DTH stronger than that of prednisolone or dexamethazone alone. Prednisolone and dexamethazone decreased the weight of thymus and spleen in intact, PC-CD and SRBC-DTH mice, but arbutin did not show these effects. These results suggest that arbutin may increase the inhibitory action of prednisolone and dexamethazone on PC-CD and SRBC-DTH, but further investigations are required to understand the mechanism involved.

[Phagocytic activity of leaves of Epimedium species on mouse reticuloendotherial system].

Effects of a 70% methanolic extract (E-S) from the dried leaves of Epimedium sagittatum on a phagocytic activity of reticuloendothelial system were studied by the carbon clearance method in mice. The clearance-rate of carbon significantly increased 1 h after the oral administration of E-S (200 or 500 mg/kg, one time/d for 5d). E-S activated the phagocytosis of carbon by Kupffer cells in the liver. Icariin and epimedin C isolated from E-S also activated the phagocytosis. These results suggest that E-S promotes the phagocytic activity of the reticuloendothelial system in mice and has a stimulatory effect on macrophage.

[Pharmacological study on Agkistrodon blomhoffii blomhoffii Boie. II. Effect of 50% ethanolic extract on phagocytic activity of mouse reticuloendothelial system].

Effect of a 50% ethanolic extract ([M]) from dried whole body of Agkistrodon blomhoffii blomhoffii Boie on a phagocytic activity of reticuloendothelial system was studied by the carbon clearance method in mice. The clearance-rate of carbon was significantly shortened 1 h after the oral administration of [M] (250 or 500 mg/kg, one time/d for 3d). [M] activated the phagocytosis of carbon by macrophage (M phi) in the spleen and the phagocytosis of latex by peritoneal M phi. These results suggested that [M] may promote the phagocytic activity on the reticuloendothelial system in mice and has a stimulatory effect on M phi.
