[A case of apnea during spinal anesthesia after intravenous injection of sedatives].

During spinal anesthesia for appendectomy (sensory level: -Th7), apnea occurred for about 20 minutes by intravenous administration of diazepam. Since the patient showed hyperventilation sign before the incident, this apnea might be caused by low PaCO2 and stopping respiratory stimulation from psychological unrest when diazepam, pentazocine and droperidol were administered. In the postoperative period, this patient showed normal neurologic and psychosomatic functions. In such a case as this, long duration of apnea might damage brain by severe hypoxia. Apnea after hyperventilation during spinal anesthesia as observed in this case could lead to a serious accident.

[A clinical study of the radiosensitization of aclarubicin].

Sixty six patients with an advanced or a relapsed cancer have been treated with a combination of radiation and aclarubicin (ACR). The average irradiation dose was 45 +/- 15 Gy, and the combined total dose of ACR ranged from 130 to 260 mg, depending upon the following differing schedules: 10 mg x 3 or 5/w, 20 mg x 2 or 3/w and 20 mg x 2/d x 2/w. The tumor response attained was 70%, including 30% who achieved a complete response. A high response rate also was observed in rather radioresistant cancerous tumor, such as those of the lung, stomach and esophagus. The adverse effects, such as anorexia, nausea, and so on, were found increase with an increase in the dose of ACR. It thus was concluded that for clinical usefulness, 10 mg of ACR should administered every other day in combination with a definite program of radiotherapy.

[Chemotherapy of stage III-IV Hodgkin's disease. A retrospective analysis of the 53 cases collected from 9 institutions in Japan].

The clinical data of 53 patients with Stage III-IV Hodgkin's disease collected from 9 institutions in Japan were analyzed for the efficacy of chemotherapy. CR rate (78%) and five-year relapse-free survival (RFS) rate (61%) were higher in the patients treated with VEPA/CHOP regimen than those in patients with VEMP/BONP or MOPP/C-MOPP regimen, although the difference was not statistically significant because of the small number of the patients. As to 14 patients treated with CHOP regimen, CR rate was 87% and RFS curve trend toward plateau at 67% after 2 years and 3 months from the initiation of chemotherapy. Salvage therapy with adriamycin-based combination chemotherapy achieved CRs in 9 of 14 (64%) patients who had been treated with VEMP, BONP, modified MOPP or C-MOPP regimen. The CHOP regimen was effective in the treatment of III-IV Hodgkin's disease but an alternative multidrug chemotherapy with ADM, CPM, VCR, BLM, etoposide and procarbazine is recommended for achieving a higher CR rate and a better RFS. Prospective study is needed to establish the standard chemotherapy for Hodgkin's disease in Japan.

[Role of free radicals in the treatment of cancer].

Free radicals including active oxygen species attack various cells by destroying lipids, proteins and enzymes, and often result in cell death. Recently, free radicals have been found to participate in the mechanism of anticancer therapy such as irradiation, chemotherapy, laser and immunotherapy. Since tumor cells contain fewer scavengers of radicals, it would be reasonable to look for a way to produce a large amount of free radicals in tumor tissues for future anticancer therapy. It should be noted that a variety of side effects induced by anticancer therapy are often related to the production of free radicals in normal organs.

[Adhesive efficiency of new bonding agents to dentin on bonding agent using urethane acrylate oligomer].

The tensile bond strength of new bonding agents on human dentin treated with 37% phosphoric acid solution for 60 sec to a photocurable composite resin was measured and the fractured surface was then observed by SEM. The new bonding agents consisted of various 20 wt% high molecular monomers (MW. 1500, 5000, urethane acrylate oligomer) as a base monomer. The best bonding agent was a mixture of 20 wt% 1500 molecular monomer as a base monomer and the value of the tensile bond strength was 91 +/- 17 kgf/cm2. The tensile bond test and SEM image of the fracture surface suggested that a high molecular monomer as a base monomer promoted the adhesive efficiency to human dentin.

[The trial use of alpha-IFN in treating a case of chronic myelomonocytic leukemia with splenic infarction].

A 75-year-old man, previously diagnosed as having chronic myelomonocytic leukemia, suffered an attack of severe left hypochondralgia in July 1986. A splenic infarction was diagnosed by both ultrasound tomography and computerized tomography. The patient was treated with alpha-Interferon (600 M.U./day i.m.) for cytoreduction in order to prevent a recurrence of the splenic infarction. Twenty-one days later, the peripheral white blood cell count decreased from 44,110 microliters to 9800/microliters and the monocytoid immature cells disappeared. However, severe dementia appeared and so alpha-Interferon therapy was abandoned. In this report the beneficial effects and side effects of alpha-interferon in the treatment of chronic myelomonocytic leukemia are discussed.

[The successful surgery of lung abscess formed from pulmonary aspergilloma in a patient with recurrence of acute promyelocytic leukemia during remission induction therapy].

This is a case report of a successful case of surgical treatment of a lung abscess formed from pulmonary aspergilloma in a patient with a recurrence of acute promyelocytic leukemia during remission induction therapy. A 39 year-old woman was admitted to our hospital for remission induction therapy of a recurrence of acute promyelocytic leukemia. We were successful in bringing her to a state of complete remission by a therapy of a intermediate-dose of Ara-C. Pulmonary aspergilloma was formed in the right lung during therapy. We performed transbronchial infusions to Amphotericin B which dissipated the fungus ball. However, a lung abscess was formed. We needed radical therapy, and there were no complications in the following surgical treatment. Recently, along with the tendency for the use of stronger chemotherapy treatment regimes for acute leukemia, there has been an increase of the number of mycosis cases such as the pulmonary aspergilloma case reported here. Accordingly, surgical treatment is becoming more and more a matter of necessity in the clinical course of leukemia patients.

[A case of aortitis syndrome which presented coarctation of aorta due to intraluminal flap formation in the middle of the thoracic aorta].

We present a rare case of aortitis syndrome associated with dilatation of aorta and coarctation-like effect due to the intraluminal flap formation originated from dissected wall of the aorta. A 31-year-old woman was admitted to our hospital complaining of shortness of breath, palpitation and cough. On admission, her physical status showed congestive heart failure and hypertension of upper extremities and hypotension of lower extremities. Bruits were audible over the neck, the anterior chest and the back. Serological studies showed active inflammation. Chest X-ray film showed upper mediastinal widening, cardiomegaly and pulmonary edema. Aortitis syndrome was strongly suggested by these clinical findings, so that prednisolone therapy was started on 3rd hospital day. Special examinations were performed several days later when inflammatory changes showed a tendency to improve. Chest CT scan, RI angiography and MRI studies showed an aneurysmal dilatation from the ascending aorta to the mid-thoracic aorta. Aortography demonstrated a flap at the terminal portion of this aneurysmal dilatation and grade II (Sellars) aortic regurgitation. There was a pressure difference of 80 mmHg between the parts abutting cranial and caudal sides of the flap. A surgical operation was, then, performed to correct the pressure difference. The dissected wall was extruded toward the aortic lumen creating a flap (2 cm in length). This flap was resected and an artificial graft was inserted. Histologically, the flap consisted of adventitia, media and intima.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of pseudolymphoma of the lung with cough and fever].

This case report presents a 48-year-old woman who had been suffering from cough and fever for 5 years. A large mass in the right lung was pointed out. Even after various examinations, we couldn't resolve the difficult diagnostic problems involved, but it was clear that there were no signs of a malignant tumor. We suspected a large benign tumor or pulmonary sequestration. Right lower lobectomy was performed and histological examinations of the resected tumor revealed lymphoproliferative disorder. Microscopic examinations of the specimen disclosed monotonous lymphoid cells proliferation (small to medium size) without definite lymph follicles or germinal centers. Lymph follicles were clearly demonstrated with the immunoperoxidase method. These findings were interpreted to be consistent with what has been recently called pseudolymphoma of the lung and prompted discussions on the reasonableness of criteria for the diagnosis of various pseudolymphoma and effective immunological examinations for the clarification of the etiology.

[Changes in autonomic nerve function during the normal menstrual cycle measured by the coefficient of variation of R-R intervals].

The purpose of this study was to determine if there is a change in autonomic nerve function during the menstrual cycle. The subjects were 20 females (average age 26.1 years +/- 4.6) with a normal menstrual cycle. The coefficient of variation of R-R intervals (CV R.R) was measured to investigate autonomic function in the menstrual, follicular, ovulatory, luteal, and premenstrual phases. Average CV R-R for all phases was 5.2 +/- 1.9%. And the CV R-R tended to be lower in those in their 30s than in those in their 20s. And no noticeable difference was seen in the CV R-R among the 5 phases of the menstrual cycle. On the other hand, the CV R-R of 11 females with premenstrual syndrome was low in the ovulatory, luteal and premenstrual phases. These results, which provide basic data for clinical use, suggest the following. (1) The age of subjects should be taken into consideration. (2) Changes in the CV R-R during the menstrual cycle are negligible. (3) However, in those showing symptoms associated with the menstrual cycle such as premenstrual syndrome, changes during the menstrual cycle should be taken into account. At the same time psychological changes in the subjects were evaluated by the following tests: Cornell Medical Index, Taylor's manifest anxiety scale, and Zung's self-rating depression scale. The results of these tests did not vary significantly during the menstrual cycle.

[Role of lipid peroxidation and polymorphonuclear leukocytes-derived oxygen radicals in acute gastric lesions induced by hyperthermic treatment].

Acute gastric lesions were experimentally induced by local hyperthermic treatment (42 degrees C, 40 min). Thiobarbituric acid (TBA) reactants in the gastric mucosa were significantly increased 40 min after the hyperthermia. There were no changes in gastric mucosal blood flow and gastric acid secretion. The total area of gastric erosions and TBA reactant in gastric mucosa of rats deficient in vitamin E were significantly increased compared with those of rats supplemented with vitamin E. The increase in the total area of gastric erosions and TBA reactants in gastric mucosa were significantly inhibited by the treatment of bovine Cu, Zn-SOD and bovine catalase, and by the depletion of polymorphonuclear leukocytes (PMN). Allopurinol did not show any influence on these pathological changes induced by hyperthermia. These results suggest that lipid peroxidation and PMN-derived oxygen radicals may play an important role in the pathogenesis of gastric lesions induced by hyperthermic treatment.

[The prognosis of intermittent claudication].

The purpose of this study is to obtain the informations about the fate of the limbs and lives of claudicants due to arteriosclerosis obliterans. Two hundred and seven lower limbs of 165 patients suffering from intermittent claudication were observed for an average period of about six years. Eighty-seven limbs of 69 patients were managed nonsurgically (Group A). Ninety-eight percent of limbs in Group A remained unchanged or improved, and no limb was amputated. One hundred and twenty limbs of 96 patients had arterial reconstructions (Group B). Although 75 percent of limbs in Group B had benefits by successful operations, 26 percent of them experienced graft failures, and 2.7 percent of them were amputated. Five year graft patency rates in the limbs with supra- and infra-inguinal reconstructions were 82.1 percent and 65.7 percent, respectively. Operative mortality rate was 2.1 percent, and mortality rate due to late complications was 3.3 percent. The prognosis of intermittent claudication with regular follow-up was relatively good, therefore, it is important to recognize that arterial reconstruction is not the only way to treat limbs with mild intermittent claudication. About 30 percent of claudicants died within 5 years, and 60 percent of them died from cardiac or cerebrovascular disease. The results suggested that much attention should be paid to the lives as well as the limbs of claudicants.

[Risk factors for peptic ulcers treated surgically].

To elucidate the risk factors of peptic ulcer which require surgical treatments, we examined the influences of following factors such as sex, age, smoking, and alcohol in 308 peptic ulcer patients. In the group with gastric ulcers, high age group was likely to be operated. On the contrary, young patients with duodenal ulcers were likely to be treated surgically. Sex, smoking and alcohol were not regarded as risk factors. The correlations between the degree of neurosis and the amounts of smoking and alcohol were analyzed in the cases with duodenal ulcers. In the cases treated surgically, the degree of neurosis showed no or little correlations. In the cases treated conservatively, however, the degree of neurosis showed positive correlation with the amounts of smoking and alcohol.

[Binding of K-18 on the surface of tumor cells--flowcytometrical analysis in vitro].

After K-18 was contacted with various kinds of tumor cell lines in vitro, the binding pattern was examined. K-18 bound on the surface of K-562 cells showed the maximum levels 1 hr later and maintained the binding activity for 24 hrs in the analysis by flowcytometry. K-18 also bound to HeLa S3, KB, RPMI8226, P815, YAC-1 and L1210 cells. Fluorescent intensity on K-562 cells was stronger with whole molecules of K-18 than with F(ab')2 conjugated with melphalan. Normal human IgG could bind more on the surface of K-562 cells than F(ab')2 of IgG. These results suggest that K-18 might bind on tumor cells via IgG Fc receptors on their surface.

[A case of chemotherapy by the transarterial catheter with reservoir against malignant lymphoma of the colon].

We had a case of malignant lymphoma of the colon with two lesions, one in the transverse colon and the other in the ascending colon. The tumor of the transverse colon was resected, but that of the ascending colon was unresectable. Chemotherapy was performed through the catheter with reservoir via the iliac artery into the right colic artery. Three months later, the tumor disappeared completely, and the ascending colon was resected. On histopathological examination, there was no lymphoma cell in the ascending colon. Chemotherapy by a transarterial catheter with reservoir is a new method of treatment against malignant lymphoma.

[Cell population and elastase activity in bronchoalveolar lavage fluid from rats exposed to high concentration of nitrogen dioxide].

The purpose of this study was to clarify what elastase acts primarily to injure the lung tissue when animals are exposed to nitrogen dioxide (NO2). Rats were exposed to 50 ppm NO2 for 5 hours a day on 7 consecutive days. Approximately six animals were sacrificed after the exposure of the 1st, 3rd, 5th and 7th days. On the 1st day, diffuse alveolar edema was seen accompanying deposits of fibrin nets, suggesting the increase of permeability. Inflammatory cell infiltration was seen in terminal bronchioles and alveolar ducts on the 3rd day, however these changes diminished gradually thereafter. The increase of total cell count was seen in bronchoalveolar lavage fluid on day 3 and thereafter. The number of neutrophils was raised markedly on the 3rd day, whereas that of macrophages showed a gradual increase thereafter. The protein concentration of bronchoalveolar lavage fluid increased markedly on the 1st day and was reduced rapidly thereafter. Elastase activity was measured in bronchoalveolar lavage fluid using succinyl-trialanyl-p-nitroanilide as substrate. Total elastase activity was significantly increased on the 1st and 3rd day. Inhibition studies using EDTA and DFP suggested that most of the elastase activity was due to a metaleoprotease. These data suggest that elastase from macrophage may be mainly related to nitrogen dioxide-induced lung injury.
