[Clinical evaluation of intra-arterial infusion of hyperthermo-chemotherapy in gastric cancer].

We performed intra-arterial infusion hyperthermochemotherapy by retaining an intra-arterial reservoir in 17 lesions of 12 patients with non-resectable, metastatic or recurrent gastric cancers. The 12 patients consisted of one with a primary gastric cancer lesion, 6 with a solitary gastric cancer lesion metastasizing to the liver, 4 with gastric cancer accompanied by hepatic metastasis, lymph node metastasis or local recurrence, and one with a gastric cancer lesion metastasizing to Douglas' pouch. A catheter was retained in the hepatic artery of all 6 patients with a solitary gastric cancer lesion metastasizing to the liver, and a catheter was retained in the aorta of the patient with a primary lesion, 3 of the 4 patients with two or more metastatic lesions, and the patient with a lesion metastasizing to Douglas' pouch. The duration of each hyperthermia session was 50 minutes, and one or two sessions were performed within a week. One course consisting of 5 or 6 sessions was repeated. Antineoplastic drugs such as MMC, 5-FU, ADR, epi-ADR, CDDP and VP-16 were injected in bolus form or administered serially through the reservoir. Nine of the 12 patients had polypharmacy. One to 3 courses or 4 to 20 sessions at maximum (average 9.8 sessions) were given. The rate of efficacy of intra-arterial infusion hyperthermochemotherapy was 44% for hepatic metastasis and 25% for lymph node metastasis. The local recurrent lesions, the lesion metastasizing to Douglas' pouch and the primary lesion did not respond to therapy.

[Deformities due to finger sucking].

Finger sucking can result in deformities of the fingers as well as the deformities well-known in orthodontics. Besides skin changes resulting from chronic irritation, more serious deformities in position or function of the fingers appear to occur. In these cases it is mostly the index finger that is affected. Upon ceasure of the habit the skin changes prove to be reversible; the deformities in position and function of the fingers however do not always display spontaneous improvement. A review of relevant literature and an account of two patients with characteristic case histories is given.

Gastrocnemius muscle transposition for bony and soft-tissue defects.

For patients with chronic osteomyelitis, extensive scarring or soft tissue defects around the knee joint, transposition of the gastrocnemius muscle should be considered, because of its constant anatomy, easy dissection, versatility and moulding features combined with an excellent immunocompetence related to the high oxygen environment. Three patients were treated with transposition of the medial gastrocnemius muscle. Follow-up (12-15 months) showed a good result without recurrence of infection.

[Ray amputation of the hand].

The aim of a ray amputation is to improve the cosmetic appearance and function of the hand after finger amputation. Pain, reasons of appearance and impaired function of amputation stumps are good indications. The patient's occupation, hobbies, handedness and emotional attitude regarding the amputation should be assessed when ray amputation is considered.

Chronic painful sesamoids of the thumb.

In patients with persistent localized pain at the level of a sesamoid bone of the thumb not obviously related to trauma, sesamoidectomy gives excellent results. Often no X-ray or histological abnormalities are found, leaving the origin of pain problems still unsolved.

Effects of zirconia addition on fracture toughness and bending strength of dental porcelains.

Zirconia dispersed composite porcelains with glass and aluminous porcelain as matrix were prepared as models of dental porcelains. The bending strength and fracture toughness of the composite porcelains were examined. The bending strength and fracture toughness of composite porcelains containing 50 wt% zirconia were 20 to 80% greater than in glass alone. However, bending strength and fracture toughness decreased upon the addition of zirconia at more than 50 wt%. Moreover, in the case of aluminous porcelain as matrix, fracture toughness increased to a maximum value of 2.6 M Pa.m1/2by addition of 23 wt% zirconia, twice the toughness of glass alone. On the other hand, no increase of bending strength was observed in this case. Deflection and bowing of cracks as well as microcracking effects were related to these increases of mechanical properties in zirconia dispersed composite porcelains.

Viscosity of dental porcelains in glass transition range.

Changes in the viscosity of dental porcelain as a function of temperature are the controlling factor determining incompatible stresses in porcelain-fused-to-metal restorations. A new method was used to estimate viscosity using a viscoelastic analytical model that considers the relation between the heating rate and deformation temperature (Td) of dental porcelain with stress applied during heating. The activation energy of viscous flow and the viscous constant in the Arrhenius equation were calculated for six kinds of commercial body and opaque porcelains. The effect of the heating rate on the instantaneous coefficient of thermal expansion was also measured. Tg onset was graphically determined using data on thermal deformation and agreed well with the strain point as defined by viscosity. The data gathered in this study was also compared to the experimental results of previous works by others.

Effects of preparation conditions in aqueous solution on properties of hydroxyapatites.

Hydroxyapatite powders (HAP: Ca10(PO4)6(OH)2) were prepared from calcium nitrate and di-ammonium hydrogen phosphate at various temperatures in aqueous solution in the absence and presence of carbon dioxide. In the absence of carbon dioxide, the crystallinities as well as the calcium to phosphate ratio (Ca/P ratio) of HAP powders increased with an increase in the preparation temperature. Calcium ion deficiency caused low Ca/P ratios of HAP powders. Such HAP powders decomposed thermally to beta-tricalcium phosphate (beta-TCP: Ca3(PO4)2) at 800 degrees C in 3 hours whereas HAP powders with sufficient calcium were stable at 1200 degrees C for 3 hours. In the presence of carbon dioxide as well the crystallinities of HAP powders increased with an increase in the preparation temperature, but carbonate-bearing HAPs were obtained instead of pure HAP. In addition, calcium carbonate was obtained along with carbonate-bearing HAP prepared below 60 degrees C.

[Strength and transparency of dental porcelain consisting of high refractive germanate-glass and alumina crystal].

A translucent aluminous porcelain was developed for dentistry. The effects of refractive indexes and sintering behaviors on transparency and strength of the aluminous porcelains consisting of high refractive germanate-glass (Na2O-TiO2-GeO2) and alumina crystal powders were examined. The various germanate-glass specimens with a high refractive index were made by fusion at about 1,300 degrees C. The refractive indexes of fused Na2O-TiO2-GeO2 glass specimens were 1.64-1.76, heightened with an increasing TiO2 content. The sintered aluminous porcelains were made from the mixed compacts consisting of 80 wt% germanate-glass and 20 wt% alumina at the densification temperature of 580-820 degrees C. Sintered aluminous porcelains prepared with high refractive germante-glass had a high transparency compared with the other aluminous porcelains, with almost the same transparency as a commercial feldspathic porcelain (body). Aluminous porcelains had lower transparency with different refractive index due to generation of crystals following the crystallization of glass matrix than that without crystallizing property. Bending strength value was 120 MPa, which is similar to that for the glass-alumina ceramics with the same content of alumina volume as germanate-glass aluminous porcelains. Non-crystallized aluminous porcelain had a higher strength compared with the crystallized one.

Dermodress: a temporary skin substitute for coverage of exposed vital structures in the hand.

A chemically treated bovine skin, Dermodress, is presented as a new type of biological dressing for exposed tendons, bones, and nerves following extensive thermal or severe soft-tissue injuries to the hand. Its use enables the surgeon to postpone closure of defects with grafts or flaps until the presence of healthy, viable tissue is secured. The vital structures underneath the covering are kept moist, thus preventing necrosis and desiccation. Because of its adherence properties, Dermodress use also allows early mobilization and bathing, thereby accelerating rapid return of function.

Use of Van der Meulen principles in repair of hypospadias cripples without chordee.

The principle of Van der Meulen procedure was used for correction of 14 hypospadias cripples. The advantages and drawbacks are discussed. Follow-up ranged from one to five years. None of our patients needed a second intervention. Only one complication (urine leakage) occurred, which could be treated by application of Histoacryl glue at regular intervals and slight compression during voiding.

Rotational deformity of the index finger caused by reversed finger sucking.

Five patients with deformities of the index finger are presented. We believe all five are due to finger sucking. The deformity usually consists of a combination of rotation, radial deviation in the metacarpal phalangeal joint, and hyperextension in the proximal interphalangeal joint; one of these is more pronounced. In our series the rotational deformity and deviation were the most prominent features. Cause, incidence, and other factors influencing this deformity as well as spontaneous regression and therapy are discussed.

The free parascapular flap.

The parascapular flap is a new, reliable and thin flap with a constant anatomy of the vascular pedicle. The flap can be taken with the lateral border of the scapula or used in combination with the latissimus dorsi muscle. Primary closure of the donor site can be achieved with a flap size up to 30 X 15 cm. The various applications of the parascapular flap are discussed and the results of four patients are presented.

Increased production of tumor necrosis factor and prostaglandin E2 by monocytes in cancer patients and its unique modulation by their plasma.

We investigated the role of monocytes in the production of tumor necrosis factor (TNF) and prostaglandin E2 (PGE2) in 77 cancer patients with malignancies of the digestive tract, using 30 normal individuals and 18 noncancer patients as controls. Monocytes were incubated with lipopolysaccharide for 20 h, and TNF production and PGE2 production were analyzed by bioassays. Elevated levels of TNF (greater than 512 U/ml) and PGE2 (greater than 8 ng/ml) production were demonstrated in many cancer patients when these factors were induced in the medium with 10% fetal bovine serum. The elevated level of TNF was seen to be restricted for the most part to patients with malignancies. Thus, 51 out of 59 cancer patients (86%), consisting of 44 primary cancer patients and 15 recurrent cancer patients, showed an increased level of TNF. In contrast, almost all of 18 postoperative cancer patients showed TNF levels comparable to those of normal individuals. Furthermore, 16 primary cancer patients were also demonstrated to have reduced levels of TNF production by monocytes after curative operation. When 10% cancer-patient plasma was added to the induction culture, TNF production by monocytes was drastically suppressed in the cancer patients. Interestingly, the same addition of plasma induced a prominent enhancement of PGE2 production in the cancer patients. The plasma of noncancer patients did not modulate production of these factors. No TNF activity was found in the plasma of cancer patients, but such plasma did contain an increased level of PGE2 (100-300 pg/ml). Although PGE2 (greater than 2 ng/ml) was able to suppress TNF production by monocytes, the addition of 10% plasma PGE2 was not enough to induce suppression. An unknown factor(s) in the plasma of cancer patients may uniquely modulate the elevated TNF and PGE2 production in these patients.

Bilateral hip resection for closure of trochanteric pressure sores: case report.

Bilateral resection of the proximal femur, an orthopaedic procedure advocated in selected cases of infected hip joints, can be useful in the treatment of extensive pressure sores in paraplegics. It is especially indicated in those cases where a total thigh flap is considered with the advantage of saving the limb, and by shortening the leg providing more skin and soft tissue for closure of the pressure sores.
