Correlation between titer of antibody to hepatitis B core antigen and presence of viral antigens in the liver.

Liver specimens of 31 autopsied cases of liver cirrhosis who had had detectable levels of antibody to hepatitis B core antigen (anti-HBc) inthe serum were stained for hepatitis B core antigen (HBcAg) and hepatitis B surface antigen (HBSAg) by the direct immunofluorescence method. Their premortem serum samples were tested for HBSAg, antibody to HBSAg (anti-HBS) and anti-HBC. Persistent hepatitis B virus (HBV) infection as judged by circulating and/or liver HB antigens was identified in 18 patients, and all of them revealed a high titer of anti-HBC ranging from 2(11) to 2(16) by the immune adherence hemagglutination method. In contrast, anti-HBC titer of the remaining 13 patients without detectable HB antigens was less than 2(9), and the geometric mean titer of anti-HBC of the patients with persistent HBV infection was significantly higher than that of the patients without (13.9+/-1.55 versus 7.23+/-1.30; t test, P less than 0.001). A combination of circulating anti-HBS and hepatic HB antigens was found in one patient, whose serum revealed an anti-HBC titer of 2(12). On the basis of these results, a high titer of anti-HBC in the serum (immune adherence hemagglutination titer of 2(11) or more) seems to be a reliable indicator of persistent HBV infection in the liver.

Sclerema neonatorum associated with systemic fibrosis and endocardial fibroelastosis.

A female newborn infant whose illness was diagnosed as sclerema edematosum on the basis of diffuse edematous skin induration having rapidly occurred until her death on the third day of life, was pathologically investigated. Autopsy revealed notable interstitial fibrosis of the subcuteneous adipose tissue, variable increases of fibrous connective tissue in various visceral organs, particularly prominent in the alimentary tract, as well as endocardial fibroelastosis of the heart. Although the skin lesion is similar to that of sclerema neonatorum presented by HUGHES et al., systemic fibrosis in the visceral organs has not been reported in the literature with respect of this disease. Furthermore, this case presented striking histopathological differences from scleroderma neonatorum and infantile subcutaneous fat necrosis in many respects. It is conceivable that this case is a peculiar disorder at least pathologically different from the previously known cutaneous indurative disorders of newborn infants. Possible pathogenesis of this case was briefly discussed.

Carcinoembryonic character of gamma-glutamyltranspeptidase in primary hepatocellular carcinoma.

As a characteristic isoenzyme of gamma-glutamyl transpeptidase (gamma-GTP) has been detected in sera of patients with primary hepatocellular carcinoma, which migrates to alpha-globulin region on polyacrylamide-gel electrophoresis, biochemical studies on human fetal liver, Morris hepatoma (7316-A, 7794-A), and human hepatoma was carried out to elucidate its carcinoembryonic character. The highest distribution of the enzyme was found in particle fraction of human fetal liver as well as of Morris hepatoma, and an isoenzyme of gamma-GTP with the same electrophoretic mobility as detected in human hepatoma was obtained, which reflected to the pattern of the serum zymograms. Histochemically, the enzyme was distributed in plasma membrane of the fetal hepatocytes and Morris hepatoma cells, while it was distributed diffusely throughout the cytoplasm of human hepatoma cells. These findings may strongly suggest that gamma-GTP in hepatoma has a carcinoembryonic character and the detection of a serum isoenzyme in the alpha-globulin region is a quite diagnostic as well as the detection of alpha-fetoprotein in the field of neoplasma of the liver. The physicochemical and kinetic properties of the enzyme in human hepatoma were also discussed.

Cytological characterization and histogenesis of Ewing's sarcoma.

In addition to a light-microscopical and histochemical investigation of primary and metastic lesions in 27 cases of Ewing's sarcoma, biopsy materials from the primary bone lesions of 7 patients with this neoplasm were examined histochemically, enzyme-cytochemically and electron microscopically to elucidate the histogenesis and nature of the neoplasm. Ultrastructural observation has revealed that besides intracytoplasmic and extracellular deposition of glycogen the tumor cells possess several cytological features characterized by intracytoplasmic microfilaments of varying thickness up to 80 A, occasional appearance of dense patches, fat droplets, desmosomal connections and reminiscent attachment bodies. The tumor cells are mostly round, oval or polygonal in shape, but spindle or elongated cells are intermingled and occasionally contain well-developed rough endoplasmic reticulum, resembling pericytes or fibroblasts. In the intercellular spaces amongst the tumor cells, varying amounts of variable-shaped amorphous materials are found, which are compatible with acid mucopolysaccharides and glycoproteins histochemically verified. These findings may suggest that Ewing's sarcoma is a highly malignant neoplasm originating from a transitional cell developed from pericytes to vascular smooth muscle cells in the bone marrow.

Tumor growth of the reticuloendothelial system.

From the author's hitherto studies on the reticuloendothelial system (RES) it was concluded that the RES is not a single cell system of identical origin, morphology and function but is a group of several types of cells of different origin. From this point of view the heterogeneity of tumors of the RES was studied to reveal the following results. Tumors of the "Reticuloendothel" (Aschoff) reveal pictures of endothelioma, while tumors of histiocytes in connective tissue show findings of fibrohistiocytoma. Histiocytes and reticulum cells of lymph nodes are respectively independent cells, and reticulum cells do not partake in the in the development of histiocytosis or histiocytic sarcoma. Follicular lymphoma is a neoplastic growth of reticulum cells having desmosomes in lymph follicles, and tumor cells of the majority of reticulum cell sarcoma are similar to the cells forming the lymph node anlage in the early fetal stage (lymphoreticular cell). Ewing's sarcoma is considered to be a kind of angiopericytoma.
