Modified Blalock-Taussig shunt following long-term administration of prostaglandin E1 for ductus-dependent neonates with cyanotic congenital heart disease.

In the treatment of cyanotic heart diseases in neonates who are ductus dependent, the small size of the pulmonary artery and/or subclavian artery often precludes construction of a sufficient Blalock-Taussig shunt. To avoid an emergency operation and to obtain pulmonary and subclavian arteries of adequate size (3 mm), a low dose of prostaglandin E1 was given to 16 neonates for a mean of 47 days (range 26 to 65 days), and a modified Blalock-Taussig shunt operation was performed in neonates whose mean age was 50 days. The pulmonary artery grew significantly (mean 4.5 mm) and the mean diameter of the subclavian artery was 3 mm, which was large enough for a good shunt at the time of operation. There were two late deaths and one operative death. All surviving patients shunts at the mean follow-up period of 22 months. The shunt was constructed on the same side as the ductus arteriosus so that the ductus could be ligated at the same operation in selected cases; the ductus may not close after discontinuation of prostaglandin E1 infusion and may cause uncontrollable congestive heart failure because of excessive pulmonary blood flow, particularly in patients with a univentricular heart. The ductus, dissected in nine patients, was enlarged and elongated in all and was ligated in four patients without complication.

[In vivo sensitivity test of anti-cancer drugs against malignant lung tumor xeno-graft].

Using the malignant lung tumor xeno-grafts in Nu-mouse, the sensitivity test of anti-cancer drugs was studied. Furthermore, the correlation between its clinical application and chemotherapeutic effects was also looked up. The results obtained were as follows; Take-rate of human lung cancer in Nu-mouse was 53.7% in primary lung cancer and 92.8% in metastatic ones. Those xeno-grafts were able to use as target cell of this test. In these tests, there were some cases which effective drugs were selected, but a few cases which all drugs were non-response. Response rate of primary lung cancer to treat with effective drugs is 25%, but that of metastatic lung tumors with the same treatment is 57%. Therefore, if the aggressive chemotherapy will be trying for malignant lung tumor, this in vivo sensitivity test to select effective drugs shall be one of the useful test.

Malignant fibrous histiocytoma at site of bone infarction in association with DCS.

A malignant fibrous histiocytoma developed at the site of bone infarction in the right femur of a 51-year-old male who had worked as a helmet diver and had experienced two episodes of bends in his young days. The outcome was rapidly fatal despite amputation of the right thigh, and widespread metastasis was noted at the time of autopsy. Histopathological examination of the amputated femur disclosed an intimate relation between the old focus of bone infarction and the tumor tissue, which was composed of a mixture of fibroblast-like cells and histiocyte-like cells. In an analysis of the published cases of malignant fibrous histiocytoma associated with bone infarction, it was found that the patients with bone infarction who had worked under hyperbaric atmosphere are at the risk of developing such a malignant tumor. The prolonged reparative process following bone infarction seems to be an important factor in the pathogenesis of malignant fibrous histiocytoma.
