Control of VX2 carcinoma cell growth in culture by calcium, calmodulin, and prostaglandins.

Based on our in vivo observation that growth of VX2 carcinoma transplanted in rabbits paralleled development of hypercalcemia, we studied the regulation of VX2 tumor growth using a clonal cell line isolated from VX2 tumor (VX2-L). VX2-L cell growth was dependent on prostaglandins released by the cultured cells into the medium, since indomethacin suppressed VX2-L growth, and prostaglandins A2, E1, E2, F1 alpha, and F2 alpha stimulated VX2-L proliferation. In contrast, prostaglandins D2 and I2 inhibited VX2-L proliferation. In contrast to previous reports, increases in extracellular calcium concentration promoted VX2-L growth not only directly but indirectly through augmentation of prostaglandin E synthesis. Antagonists of the intracellular calcium binding protein calmodulin inhibited cell replication. Increases in extracellular calcium also stimulated production of a nonprostaglandin macromolecular bone-resorbing factor. This factor may account for the hypercalcemia which we were unable to block by indomethacin. These results suggest a close relationship between VX2-L growth, prostaglandin production, and hypercalcemia. It is proposed that calcium blockers and anticalmodulin drugs might be powerful anticancer and/or antihypercalcemic agents for malignant cells such as VX2-L.

[Preliminary report of adjuvant chemo-endocrine therapy for breast cancer].

Preliminary analysis of adjuvant chemo-endocrine therapy for 193 breast cancer patients was performed. The patients consisted of 38 cases of Stage I, 124 cases of Stage II and 31 cases of Stage III. Therapeutic regimen was randomly divided into three groups; (1) Tamoxifen (TAM), (2) TAM + ftorafur (FT-E) and (3) TAM + FT-E + Adriamycin. Side effects among the three therapeutic groups were comparatively studied. Anorexia and nausea were observed in 1.4%, 17.1% and 60% of the patients, respectively. Leucopenia of less than 3,000 and alopecia were remarkably seen in the patients treated with regimen 3. Liver dysfunction was observed in 10.9% of group 1, 29.5% of group 2 and 13.3% of group 3, respectively. Gastrointestinal symptoms and liver dysfunction were important side effects.

[Intra-arterial infusion therapy with angiotensin II in gastric carcinoma].

This report describes the clinical use of MMC localized intra-arterial infusion therapy in combination with Angiotensin II for advanced gastric carcinoma which is not surgically treatable, along with some basic considerations. Since blood vessels regenerated within a tumor no longer have automatic adjustment of blood flow, there is no vascular contraction mechanism operation even when Angiotensin II is administered. The blood flow within the tumor shows a relative increase so the amount of the carcinostatic substance reaching the tumor is considered to increase. Using MMC intra-arterial infusion together with Angiotensin II, the authors were able to increase the antitumorigenic effect of the carcinostatic agent in patients with advanced gastric carcinoma. In an experiment using VX2 tumor transplanted into domesticated rabbits, the group treated with angiotensin II showed a marked reduction in the tumor. At the clinical level, differentiated gastric carcinoma patients with liver metastases very often evidenced favorable effects when treated with Angiotensin II in combination with the MMC intra-arterial infusion method. A randomized clinical study of 30 patients using either Angiotensin II together or without intraarterial infusion revealed that in 4 advanced gastric carcinoma patients could PR be obtained by the combination therapy, the non-combination treated group showed no favorable effects. In Borrmann type IV gastric carcinoma, the present combination therapy displayed no effect, while a switch to MTX-5FU sequential therapy yielded very often good results. Since the ability of the agent to reach the tumor is markedly enhanced in the Angiotensin II-intraarterial infusion approach to chemotherapy, progress in the latter science is anticipated.

[Adjusted relative survival rate and causes of death after resection of gastric cancer].

To estimate how many of 1,302 patients with curative resection of gastric cancer died of recurrence or other diseases, the cumulative survival rate (act), relative survival rate (rl) and adjusted relative survival rate (adj) were analyzed. Adj was computed excluding gastric cancer death, because the death rate of this disease can not be ignored in the Japanese population. The death rate from gastric cancer can be estimated by [1.0-(adj)] and that from other causes by [(adj)-(act)]. It is more appropriate to calculate adj for the patients with mucosal or submucosal cancer, and rl can be substituted for adj for those with deeper invasion.

Biochemical studies on the purine metabolism of four cases with hereditary xanthinuria.

The purine metabolism of four cases with marked hypouricemia (serum uric acid concentration of less than 0.018 mmol/l) from three Japanese families was investigated. Erythrocyte adenosine deaminase (EC 3.5.4.4) and purine-nucleoside phosphorylase (EC 2.4.2.1) activities of the patients were within the normal ranges. Urinary hypoxanthine and xanthine concentrations were 0.096-0.397 mmol/l and 0.743-1.717 mmol/l, respectively. Xanthine oxidase (EC 1.2.3.2) activities in the jejunal mucosa of the two normal controls were 0.257 and 0.283 units/g protein, while those of three of the patients were extremely low and could not be determined. The findings of these biochemical features may indicate that the four patients have hereditary xanthinuria. In order to study the purine metabolism in the hypouricemic condition of this disorder, a single oral dose of allopurinol (4-hydroxypyrazolo[3,4-d]pyrimidine) was administered in one case. The excretion pattern of allopurinol and oxypurinol (4,6-dihydroxypyrazolo[3,4-d]pyrimidine) in the urine of the patient was similar to that of a normal control male. These data suggest that some residual enzyme activity may be functioning in vivo, although the presence of xanthine oxidase could not be detected.

Postoperative cancer therapy for patients with carcinoma of the thoracic esophagus: an attempt of aggressive chemotherapy combined with proper nutritional support for patients with distant node metastasis.

The prognosis of patients with advanced thoracic esophageal carcinoma is still poor. In general, patients having distant lymph-node metastasis do not survive for more than 3 years. Therefore, a new therapeutic regimen employing aggressive chemotherapy for these patients has been attempted. In this regimen, three kinds of anticancer drugs Pepleomycin, and twice as much as Adriamycin and Mitomycin of the conventional level for a short period of time are used. Eleven cases were treated with this chemotherapy under active enteral and parenteral nutritional support of 45 to 50 kcal/kg daily. Two patients over 70 years of age died of pneumonitis. Four patients with recurrence of cancer in the lungs, liver and cervical nodes died after 10, 12, 13 and 14 months following surgery, respectively. Five patients have been alive for more than 2 years. These results indicate that this therapy is more effective than other conventional therapies. It has been shown that aggressive chemotherapy combined with proper nutritional support is effective for patients with node metastasis.

[Anti-tumor effects of adriamycin, 4'-Epi-adriamycin, THP-adriamycin and mitoxantrone on rat mammary cancer induced by 7,12-dimethyl benz (A) anthracene].

Comparative studies of the anti-tumor effects of Adriamycin, 4'-Epi-Adriamycin, THP-Adriamycin and Mitoxantrone on rat mammary cancer induced by 7, 12-DMBA were performed. All 4 drugs were effective on the tumors, three administrations of Adriamycin, 4'-Epi-Adriamycin or Mitoxantrone at 1-week intervals showed remarkable tumor regression in each group. Mitoxantrone was not so effective in comparison with the other 3 drugs. Decrease in the body weight of the rats was most remarkably observed in the Mitoxantrone-treated group.

Benzethonium chloride resistance in Pseudomonas aeruginosa isolated from clinical lesions.

The benzethonium chloride resistance of 341 strains of Pseudomonas aeruginosa isolated from Jikei University Hospital was determined. The distribution pattern of the susceptibility to benzethonium chloride clearly revealed two peaks, and the resistance was differentiated by 1000 micrograms/ml (0.1%) of benzethonium chloride. The frequency of resistance to benzethonium chloride was 51.6%. Furthermore, the frequencies of resistance to SM, TC, CP, KM, GM, PIP, Hg, Cd, As and chlorhexidine were 42.5, 15.8, 41.3, 29.6, 14.0, 8.2, 88.3, 97.9, 97.1, and 74.5%, respectively.

[Adjuvant chemotherapy after surgery in advanced gastric cancer].

Two hundred and fifty-four patients with advanced gastric cancer underwent radical surgery and ftorafur (FT) or 5-fluorouracil (5-FU) was orally administered as the adjuvant chemotherapy (ACT). Recurrence after ACT were analyzed by the quantification method II. As a result, it was found that: 1) ACT over 2 years could lower the recurrence rate, and careful follow-up is still necessary up to 4 years, and 2) after the remission induction therapy with mitomycin C and/or 5-FU and cytarabine, 12 mg/kg/day of FT for the first year and 8 mg for the second year are advisable.

Survey of resistance to metals and volatilization activity of Hg-resistant R plasmids in Citrobacter isolated from clinical lesions in Japan.

Clinical isolates of Citrobacter (277 strains) were studied for the resistance to four metals and four drugs. The distribution patterns of their susceptibility to Hg, Cd and As clearly revealed two peaks, but only a single peak of resistance to Pb. The frequencies of resistance to Hg, Cd, As, SM, TC, CP and KM were 31.8, 94.2, 57.8, 51.3, 44.8, 35.7 and 28.9%, respectively. And we selected 88 mercury resistant strains, and they were tested for their ability to transfer the resistance to the mercury and drug sensitive recipients of E. coli K12-ML1410-Nx and E. coli JE17-Rif. Among 88 strains of Hg resistant Citrobacter, 80 R plasmids with Hg resistance could be demonstrated. It should be noted that transferable R plasmids with Hg resistance were demonstrated in 91% of the Hg-resistant isolates. Furthermore, we tested the volatilization of mercury by strains containing these mercury resistance plasmids, by using radioactive 203Hg2+. All of these isolates of Citrobacter have volatilization activity of Hg2+. Also, all of these volatilization activity is inducible.
