Clinical reports on plasma exchange in the Kidney Center, Tokai University School of Medicine.

There were 72 patients (19 with hepatic failure, 10 with fulminant hepatitis, eight with paraquat poisoning, eight with rheumatoid arthritis, five with myasthenia gravis, four with hyperlipidemia, four with systemic arteriosclerosis including brain infarction, three with pemphigus vulgaris, two with multiple myeloma, two with systemic lupus erythematosus, two cases non-specific Ig-G antibody, two cases medication with an anticancer drug, one with multiple sclerosis, one with Crohn's disease with amyloid kidney and one with chronic myeloblastic leukemia) treated by plasma exchange in the Kidney Center, Tokai University School of Medicine from Jan. 1983 to Dec. 1986. We performed plasma exchange using fresh frozen plasma in 40 cases and Lactate-Ringer's solution containing albumin (4.0-5.0%) in 20 cases as the replacement fluid. In 17 cases, we performed double filtration plasma exchange with the recycle system and no replacement fluid. Although PE therapy did not constitute a basic therapy for hyperlipidemia, pemphigus vulgaris, rheumatoid arthritis, myasthenia gravis, and systemic lupus erythematosus, it was effective in relieving severe clinical symptoms. At the present time, conventional plasma exchange does not improve the survival rate of patients with hepatic failure and fulminant hepatitis. Developments of a new artificial liver support apparatus and identity of many toxic substances in hepatic failure are necessary. No hypotension, hypovolemic shock or other significant complications were experienced.

Tumor antigen TA-4: an aid in detecting post-operative recurrences of esophageal carcinoma.

Radioimmunoassay for serum TA-4 was performed in 35 patients with squamous cell carcinoma of the esophagus. The results were positive in 13 of 35 patients (37.1 per cent) in whom squamous cell carcinoma of the esophagus was present at the time of assay. Preoperatively, undetectable TA-4 levels in patients with esophageal carcinoma suggested localized tumor and a good prognosis, whereas, strongly positive TA-4 levels correlated with extensive tumors and a poor prognosis. Postoperatively, a positive result for TA-4 levels indicated the presence of a residual tumor. Negative TA-4 levels, however, did not exclude a residual tumor. Therefore, assessment of periodic TA-4 levels in patients who have undergone resection of esophageal carcinoma may lead to detection of a tumor recurrence.

Deproteinizing methods evaluated for determination of uric acid in serum by reversed-phase liquid chromatography with ultraviolet detection.

We evaluated six deproteinizing methods for determination of uric acid in serum by "high-performance" liquid chromatography with ultraviolet detection: those involving zinc hydroxide, sodium tungstate, trichloroacetic acid, perchloric acid, acetonitrile, and centrifugal ultrafiltration (with Amicon MPS-1 devices). We used a Toyosoda ODS-120A reversed-phase column. The mobile phase was sodium phosphate buffer (40 mmol/L, pH 2.2) containing 20 mL of methanol per liter. Absorbance of the eluate was monitored at 284 nm. The precipitation method with perchloric acid gave high recoveries of uric acid and good precision, and results agreed with those by the uricase-catalase method of Kageyama (Clin Chim Acta 1971;31:421-6).

Liquid chromatography with multichannel ultraviolet detection used for studying disorders of purine metabolism.

We used a reversed-phase "high-performance" liquid-chromatographic system equipped with a multichannel ultraviolet spectrometric detector and a micro-computer for analyzing urine samples from patients with disorders of purine metabolism. This system recorded a series of absorption-spectrum data from a single chromatographic run and stored them for subsequent analysis. Because the retention times and ultraviolet absorption spectra of the eluates were recorded simultaneously, identification of peaks was easy and quite accurate for simultaneous quantification of orotidine, adenine, hypoxanthine, uric acid, xanthine, allopurinol (4-hydroxypyrazolo[3,4-d]pyrimidine), oxypurinol (4,6-dihydroxypyrazolo[3,4-d]pyrimidine), inosine, and 2,8-dihydroxyadenine--compounds extremely difficult or even impossible to quantify simultaneously with a conventional single-wavelength spectrometer. We used this method to investigate purine metabolites in urines from a patient with hereditary xanthinuria, three patients with 2,8-dihydroxyadenine urolithiasis, and a gouty subject taking allopurinol.

Production of pyrogen by polymorphonuclear leukocytes during the course of casein-induced peritonitis in rabbits.

Rabbits with casein-induced peritonitis were febrile only during the early stage (1.5 to 12 hours) of inflammation. At that stage, peritoneal exudate cells (PEC) had preformed endogenous pyrogen (EP), while blood leukocytes and leukocytes obtained at the later stage of the inflammatory process did not. Early PEC consisted of 99% polymorphonuclear leukocytes (PMN). As early PEC and highly purified PMN (almost 100% pure) released almost identical amounts of EP, we concluded that PMN were the EP-producing cells in the early PEC. EP from PMN of rabbits was composed of three factors with similar molecular weight (8,000 to 18,000) but different pI values (pI 7.2, 5.4, and 4.5). EP in the three fractions could not be separated from thymocyte comitogenic factor (TMF) in terms of m.w. and pI values. About 60% of EP activity and 92% of TMF activity in the culture supernatant were present in the pI 7.2 fraction. The pI 7.2 factor was weaker in EP activity and stronger in TMF activity than the other acidic fractions (pI 5.4 and 4.5). After high purification, 42 ng of the pI 7.2 factor had one unit of EP activity: one unit of TMF activity was present in 126 pg of highly purified pI 7.2 factor.

Predictors of root caries in the elderly.

In an attempt to determine the association between overall health status, medication history, and oral hygiene status and root caries, 24 older persons residing independently in the community and 23 residents of a nursing home were interviewed and received oral examinations, using the Oral Hygiene Index and the Root Caries Index. The mean age of this population was 80 yr, with the nursing home subjects somewhat older than community-dwelling subjects. Only six persons had no root surface caries; three subjects had caries on all tooth surfaces with gingival recession. Mandibular teeth, particularly molars and premolars, showed the greatest attack rate. The best predictors of root caries were number of teeth remaining, calculus, plaque, and the use of medications with xerostomic effects. Medications were most predictive of maxillary root caries. The results point to the need to provide interceptive dental therapeutics for high risk geriatric populations such as those using multiple medications and with a poor history of oral care.

[Experimental study on the treatment of metastatic lymph nodes with local hyperthermia and temperature-sensitive liposomes containing bleomycin].

The therapeutic effects of combined treatment with hyperthermia (HT) and temperature-sensitive liposomes (TSL) containing Bleomycin (BLM) were evaluated in metastatic lymph nodes of AH-66 ascites tumor in rats. As a model for metastatic lymph nodes, 4 X 10(6) cells of AH-66 were inoculated into the right popliteal lymph nodes of rats. Seven days after inoculation, when the tumor in popliteal lymph nodes was approximately 8 mm in diameter, TSL-BLM was injected subcutaneously into the right foot pad, and local hyperthermia at 43 degrees C was applied for 20 minutes. The most obvious suppression of tumor growth and excellent survival rate were obtained in rats which received both TSL-BLM and HT, compared with groups receiving HT or TSL-BLM alone. These findings suggest that the combination of TSL-BLM and HT provides an effective cancer chemotherapy for patients with metastatic lymph nodes.
