[Gynecological urologic disease: a summary of clinical experience during the last 4 years at Wakayama Red Cross Hospital].

From April 1979 to March 1983, sixty seven patients with gynecological urologic disease were experienced at our department. Forty one of them were inpatients, i.e., 15.3% of the total number of female in patients admitted during the same period. Thirty three urologic operations were performed on them which corresponded to 16.1% of the total operations on female patients. Ten of these patients had ureteral obstruction following gynecological operation, 8 had ureteral fistula, 7 had vesicovaginal fistula, 11 had tumor invasion to urinary tract, 14 had radiation cystitis, 16 had neurogenic bladder dysfunction, and 1 had other complication. The treatment of these patients and the results are summarized and discussed.

Rice bran treatment for hypercalciuric patients with urinary calculous disease.

We report on the efficacy of rice bran in hypercalciuric patients with urinary calculous disease. After the administration of defatted rice bran at a dosage of 20 gm. daily for 4 weeks urinary calcium excretion was reduced significantly from 402 plus or minus 102 to 291 plus or minus 123 mg. per 24 hours. In 6 patients who showed a reduction of urinary calcium excretion an increase in urinary calcium excretion was observed 4 weeks after stopping the administration. All 7 patients who previously received defatted rice bran were given processed or rice bran free of phytin at a dosage of 20 gm. daily. The value of urinary calcium excretion increased again during the administration of processed rice bran but was less marked than that of the preceding 6 patients. Although it is interesting to debate whether the influence of rice bran on the urinary calcium excretion depends on the action of phytin or of other constituents defatted rice bran should be effective in reducing urinary excretion of calcium. These results would come up to the expectations of the patients with recurrent urinary calculous disease.

Relation between Friend leukemia virus-induced leukemia and genetic control of the host.

Hematological assays of inbred specific pathogen-free (SPF) mice of ten different strains inoculated with Friend leukemia virus (FLV) were performed chronologically to assess whether the genetic control of the host may play an important role in viral oncogenicity. Mice strains C57BL/6J, B10 (H-2b) and B10D2 (H-2d) were FLV-resistant, BALB/c, DBA/2N (H-2d), RFM (H-2f), AKR and 80% of CBA/JN (H-2k) were FLV-sensitive (polycythemia) and C3H/He, B10Br and 20% of CBA/JN (H-2k) were FLV-sensitive (anemia). Only the AKR strain mice showed a spontaneous regression of splenomegaly. These results indicate that there is not a strong but a weak correlation between the H-2 haplotype and the reaction to FLV. The main phenomenon in the anemic mice was the monotonous proliferation of the naked blastic cell, whereas that in the polycythemic mice was the enormous increase of the mature erythroblast and the decrease of the naked blastic cell in the later phase. These facts suggest that the naked blastic cell in the mice with polycythemia are reactive and that in the mice anemia truly neoplastic.

[Pharmacokinetics of an ampicillin suppository (KS-R1) in mice, rats and rabbits].

KS-R1, a new suppository of ampicillin (ABPC) sodium, was rectally administered to study its absorption, excretion and distribution in mice, rats and rabbits. Concentrations of ABPC in plasma when 12.5 and 25.0 mg/kg of KS-R1 were rectally administered to experimental animals reached the peak levels rapidly. The values were 7.3 and 13.9 micrograms/ml in mice, 7.7 and 15.9 micrograms/ml in rats, and 14.1 and 35.3 micrograms/ml in rabbits, respectively. All of these values were about 3.5 times as high as those attained by oral administration of the same doses. Concentrations of ABPC in tissues when 25.0 mg/kg of KS-R1 was rectally administered to rats reached the peak level rapidly, as in the concentration in plasma. The concentrations distributed into various tissues were found as follows; liver not equal to kidney greater than spleen greater than heart greater than lung. Urinary recoveries of ABPC after rectal administration of 12.5 and 25.0 mg/kg were 23.4 and 28.4% (0 approximately 12 hours) in rats while 79.4 and 75.4% (0 approximately 6 hours) in rabbits, respectively. When 25.0 mg/kg of KS-R1 was administered to rats, 10.7% of the dose was excreted into bile during 6 hours after administration. The relative bioavailabilities of KS-R1, calculated on the basis of AUC and urinary recovery after parenteral administration of ABPC sodium, were 81.0 to 87.5% in mice, 37.6 to 45.9% in rats and 75.6 to 101% in rabbits, which were 1.5 to 2.8 times higher than those of ABPC orally administered.

[Effects of rectal administration of an ampicillin suppository (KS-R1) on cecal microflora and cecal weight in mice].

KS-R1 is a new rectal preparation of ampicillin (ABPC) sodium. Changes of cecal microflora, changes of cecal weight and ABPC concentrations in the cecum after rectal administration of KS-R1 to mice were compared with those after oral and subcutaneous administration of ABPC. When KS-R1 was administered to mice by 25.0 mg/kg 3 times a day for 10 days continuously, a little changes were observed in the number of Enterobacteriaceae, Staphylococcus and Lactobacillus in the cecum. However, at the fifth day after completing the rectal administrations, normal level of microflora was gained. This change of microflora closely resemble those in case when ABPC sodium was administered subcutaneously. On the other hand, when ABPC was administered orally, changes of microflora were large, i.e. the number of Enterobacteriaceae and Streptococcus increased remarkably, the number of Fusobacterium increased and the number of Staphylococcus and Bacteroides decreased. Recovery of normal level of microflora was late. The cecal weight did not show changes but the increase observed in the case when ABPC was orally administered. This increase was due to the increase of moisture in the cecum. The concentrations of ABPC in the cecum when administered by 25.0 mg/kg were in the order of oral administration greater than rectal administration (KS-R1) greater than subcutaneous administration. The highest concentrations were respectively 219.6, 54.1 and 5.1 micrograms/g. Changes of microflora in the cecum was great in proportion to the concentrations of ABPC.(ABSTRACT TRUNCATED AT 250 WORDS)

[Experimental and clinical evaluation latamoxef in newborn and premature infants].

Basic and clinical studies of latamoxef (LMOX) were carried out in neonates. In basic study, these neonates consisted of 16 mature babies and 12 premature babies. LMOX was administered at dose of 10 and 20 mg/kg, either as a single intravenous injection or as a 60 minutes intravenous drip infusion. Both the mature babies and the premature babies were divided into 3 subgroups as a function of the number of days after birth (0--3, 4--7 and 8--25 days). A clinical study of LMOX was performed in 12 neonates aged between 0--35 days, consisting 6 males and 6 females. (Purulent meningitis 4 cases, septicemia 1 case, bronchopneumonia 5 cases, pertussis pneumonia 1 case, urinary tract infection 1 case). Serum concentration and urinary excretion 10 mg/kg, one shot intravenous injection In the 3 subgroups of neonates the peak serum concentrations of LMOX were found to range from 14.6 to 28.9 micrograms/ml. Although there was no significant difference, the half-life of the drug became shorter as the age of the neonates increased, these values were 4.46, 3.85 and 3.30 hours, respectively. 10 mg/kg, 60 minutes intravenous drip infusion. As above, the peak LMOX serum concentrations were found to range from 23.7 to 38.9 micrograms/ml, the half-lives of the 3 subgroups were 4.83, 2.48 and 3.01 hours, respectively. And urinary excretions were ranged from 46.0 to 56.5% for 6 hours. 20 mg/kg, one shot intravenous injection The peak serum concentrations were found to range from 31.0--82.5 micrograms/ml, and it was found out 3.29--15.9 micrograms/ml at 8 hours after the injection. There was a tendency for the half-life to be shorter in more mature subjects in 3 subgroups. 20 mg/kg, 60 minutes intravenous drip infusion In the 3 subgroups, the peak concentration was the level existing at the end of the intravenous drip infusion, and that showed a range of 41.8--58.6 micrograms/ml. Half-lives were found out the significant difference to their age, these showed 4.08, 2.31 and 2.52 hours. Cerebrospinal fluid concentrations of LMOX Cerebrospinal fluid concentrations of LMOX were studied in 2 cases at the dose about 50 mg/kg. In 1 case, that's meningitis estimated E. coli organism, the cerebrospinal fluid concentrations of LMOX were found to range from 29.0 to 49.9 micrograms/ml in that acute state. In another case from N. meningitidis, that values were found to range 12.1 to 21.3 micrograms/ml. These cerebrospinal levels were superior value at it's penetration ratio. [Case of carcinoid tumor of the breast].

A 74-year-old female with a left breast mass was admitted to the Cancer Institute Hospital and underwent mastectomy. Upon study of a frozen section, a diagnosis of breast carcinoma was made. Histopathological examination revealed a non-invasive tumor whose slightly eosinophilic rich cytoplasm was positive for PAS and Alcian-green staining and argyrophilic staining. The tumor showed medullary arrangement, partly with glandular and rosette-like structures. Electron microscopically, many dense core granules (170 +/- 30 nm in diameter) were diffusely distributed in almost all the tumor cytoplasm, they were thought to be neurosecretory granules. A diagnosis of mucocarcinoid of the breast was made. The patient experienced no dyshormonal disorder during the course of the disease and is alive and well without recurrence.

Histopathological study of early lesions of pancreatoduodenal cancer.

The early lesions of pancreatoduodenal cancer, which was defined as those in which the site of origin is confined in a single certain anatomical part and the mode of early extension can be pursued, were found in 28 of 52 surgical specimens examined; 12 in the pancreatic portion of the common bile duct (PCBD), 12 in the common channel, 1 each in the terminal portion of the common bile duct and the terminal portion of the pancreatic duct, and 2 in the peripapillary zone of the duodenum. There were roughly two types of tumor growth; one annular flat-raised, histologically scirrhous tubular adenocarcinoma, principally found in PCBD and the other polypoid, histologically papillary or papillotubular adenocarcinoma, found everywhere. The staining properties of mucin was of no value in differential diagnosis for the site of origin. One typical carcinoid was observed in PCBD. The surface spread of carcinoma beyond the originating part was seen in 10 cases (36%), lymphatic invasion in 23 cases (82%), node metastasis in 9 cases (32%), and venous invasion in 6 cases (21%). Only 2 cases were mucosal carcinomas. From these figures it can be concluded that the carcinoma of this region is of high grade malignancy.

Immunological studies on an unusual case of primary pulmonary amyloidosis with hilar adenopathy.

A rare case of primary pulmonary amyloidosis with hilar lymphadenopathy is described. Immunological studies showed decreases in serum IgG and IgM levels. However, both pokeweed mitogen (PWM)-induced IgG production and proliferation of peripheral blood B cells were not disordered. The response of peripheral blood lymphocytes (PBL) to stimulation with PWM, phytohemagglutinin, or concanavalin A was found to be normal. In addition, the proportion of E and EAC rosette-forming cells in PBL from this patient did not differ from that in PBL from normal donors. The positive response of skin test to recall antigens also gave an evidence that the patient had not a depression in systemic cell-mediated immunity. These findings contrast with those previously described for systemic amyloidosis in man and in animals.

Beta-adrenergic blockers: ocular penetration and binding to the uveal pigment.

The ocular penetration of two beta-adrenergic blockers, befunolol and timolol, was studied using albino and pigmented rabbits. A 1% ophthalmic solution of 14C-befunolol hydrochloride or 14C-timolol maleate was instilled in one eye of nonsedated rabbits, and the concentrations of radioactive material in the ocular tissues were determined at various intervals. In the albino rabbit, the time courses of the concentration changes in the cornea and aqueous humor could be analyzed on the basis of a compartment theory, and the following coefficients were calculated using a computer; i.e., permeability of the corneal epithelium (kep), the distribution volume in the anterior chamber (Va), the cornea-aqueous transfer coefficient in reference to the corneal volume (kc.ca), the aqueous-cornea transfer coefficient in reference to Va (ka.ac), the loss rate from the anterior chamber (ko) and the steady state distribution ratio between the aqueous and cornea (rac). The following values were obtained for befunolol: kep--2.6 x 10(-3) cm hr-1, kc.ca--0.51 hr-1, ka.ac--0.18 hr-1, ko--2.7 hr-1, Va--250 microliters and rac--0.9. Those of timolol were as follows: kep--2.3 x 10(-3) cm hr-1, kc . ca--0.82 hr-1 and ko--2.5 hr-1. The estimates for Va and rac could not be calculated from the data obtained in the timolol experiment. Assuming that Va and rac for befunolol were also applicable for timolol, ka.ac for timolol was calculated to be 0.25 to 0.29 hr-1. The beta-adrenergic blockers tested here penetrated the cornea rather easily and they were lost from the anterior chamber mainly by diffusion, probably into the blood circulation through the anterior uvea. The data obtained in the pigmented rabbit indicated that these beta-adrenergic blockers are bound to the melanin-containing ocular tissues and are released from there very slowly. It is suggested that in heavily pigmented subjects, beta-adrenergic blockers might be less effective after short-term use and, furthermore, intraocular binding may occur after long-term use.

[An autopsy case of primary intracranial melanoma associated with dermoid cyst (author's transl)].

Primary malignant melanoma of the cerebral nervous system is an uncommon entity. A case of primary intracranial melanoma associated with dermoid cyst is reported. The patient was a 40-year-old man who was admitted to our hospital on May 2, 1979 complaining of headache, double vision, left hemiparesis and Jacksonian seizure. On admission he had a slight degree of papilledema. Left abdominal reflex was absent. Lumbar puncture showed high pressure of the cerebrospinal fluid which was otherwise normal. Plain skull films showed a calcification in the pineal region. CT scan demonstrated multiple high density areas with slight enhancement after contrast medium infusion in the right fronto-parietal region and a well defined high density area associated with an irregular low density area without contrast enhancement in the right pulvinar and the atrium and temporal horn of the right lateral ventricle. However, no tumor stain was seen on angiograms. On May 8, 1979 partial removal of the tumor was performed. Areas of black discoloration were seen in the leptomeninges of the right fronto-parietal lobes. Histopathological examination of this tumor revealed primary intracranial melanoma. Postoperatively DTIC, ACNU and Vincristine were administered intravenously but the tumor showed only little sensitivity. He died 5 months after operation. Autopsy was performed on October 8, 1979. Malignant melanoma in the right fronto-parietal lobes was invading the cerebellar hemisphere and spinal cord, but any tumor was not found in any other parts of the body except for this central nervous system. The tumor in the right pulvinar extending to the atrium and temporal horn of the right lateral ventricle was a dermoid cyst with calcification histopathologically. A case of primary intracranial melanoma associated with dermoid cyst has not been reported in any other associated with dermoid cyst has not been reported in any other literature yet.
