[Adult T-cell leukemia associated with pure red cell aplasia-like lesion].

An adult T cell leukemia associated with pure red cell aplasia-like lesion was described in this paper. A 51 year-old woman was admitted because of headache and palpitation in October 1988. On admission, physical examination showed marked pallor but no detectable superficial lymphadenopathies. Hepatosplenomegaly was not observed. The blood examination revealed normocytic anemia with Hb of 6.6 g/dl and marked leukocytosis of 18,800/microliters with 43% ATL cells. The bone marrow aspirate showed moderate infiltration of ATL cells and a few erythroblasts. The bone marrow biopsy disclosed moderate infiltration of ATL cells, only a few erythroblasts with maturation arrest and marked fibrosis. The erythropoietin in serum was elevated (686 IU/microliters). To clarify the mechanism of development of the PRCA-like lesion, the peripheral blood lymphocytes (ATL cells) or serum of the patient was added to in vitro erythroid colony formation. The patient's serum increased BFU-E but either serum or lymphocytes didn't inhibit the growth of CFU-E compared with control.

Study on a new index of fetal respiratory function: a novel optical analysis of the amniotic fluid by the vacuum-dried infrared attenuated total reflection (ATR) method.

For the purpose of detecting fetal pulmonary maturity with real-time convenience but reliable accuracy, infrared spectrophotometry of amniotic fluid lipids was introduced using our newly developed optical device. The peak areas at the wave number of 1740 cm-1 on the infrared spectra (called "PA1740") were measured in 29 specimens sampled from 29 pregnant women, and compared with the Lecithin/Sphingomyelin (L/S) ratio of the same samples. Statistical analysis revealed that the PA1740 was almost as reliable as the L/S ratio, and these indices illustrated pulmonary immaturity in cases with a PA1740 less than 0.9 and an L/S ratio less than 2.0. PA1740 is absolutely independent of the effect on the concentration of lecithin in the amniotic fluid because of a combination of the vacuum-drying process and the absorbance correction, and can be completed in less than 20 minutes from sampling of the amniotic fluid by a very simple manipulation. In conclusion, the new index, PA1740, will be a more practical method for prediction of the fetal pulmonary maturity than methods such as the well known L/S ratio.

[Rheological properties of elastomeric impression materials].

The rheological properties of elastomeric impression materials are important because they are major determinants in the handling characteristics and adaptation to the soft and hard tissues of the mouth. Of particular interest is the rheology of the elastomeric impression materials as supplied by the manufacture since this strongly influences the ease with which the two pastes can be mixed and placed. Additionally, the flow characteristics of the mixed impression materials are also important as these will have an important bearing on the accuracy and reproduction of surface detail which are influenced by the working time. Generally, rheological properties can be defined as the study of the flow characteristics of viscos materials such as elastomeric impression materials. In the context of impression materials used in dentistry the variation of these flow characteristics over short time intervals is extremely important in the making of clinically acceptable impressions. The purpose of these studies were to measure a variety of rheological characteristic for the unmixed elastomeric impression materials and to relate this information to clinical dental procedures. The elastomeric impression materials used in this study were polysulfides, condensation cured silicone, addition cured silicone rubbers in common clinical use and specimens of adding increasing increments of filler (SiO2, TiO2 or PbO2) to a main constituent part (Thiokol LP2, Chloridparaffin, Silicone or Vinylsilicone) of each impression paste. The viscosity of these materials were measured by using the Ishida-Giken cone and plate high shear rheometer. The changes in shear stress with time were recorded for a shear rate of 5 s-1 at 23 degrees C. The relation of shear rate to shear stress were accomplished with a range of shear rates from 0 s-1 to 20 s-1. The viscosity behavior which were the flow characteristic, apparent viscosity, the yield value and the areas of hysterisis loop, were determined from both the shear stress-time and the shear stress-shear rate curves. The results of this study of elastomer impression material pastes have shown that: 1. All impression pastes exhibited non-Newtonian viscosity characteristic, apparent viscosity decreasing with increasing shear rate. 2. Surflex F base pastes (polysulfide rubber), Flexicone injection type base paste (condensation-cured silicone rubber) and Exaflex injection type base paste (addition cured silicone rubber) were pseudoplastic behavior, and other impression pastes were virtually thixotropic behavior. 3. All impression paste had yield value, especially Surflex F accelerator pastes which had a markedly higher yield value.(ABSTRACT TRUNCATED AT 400 WORDS)

[A case of myxedema with diffuse myocardial fibrosis proven by endomyocardial biopsy].

We report a middle-aged woman with myxedema heart who presented both clinical features resembling dilated cardiomyopathy and the diffuse myocardial fibrosis proven by endomyocardial biopsy. Thirty years previously, when she was 36 years old, partial thyroidectomy had been performed after a diagnosis of hyperthyroidism was made. Four years later, she experienced dry skin and peripheral edema, and hypothyroidism was diagnosed. Several months after, replacement therapy for hypothyroidism improved her symptoms. However, the therapy was discontinued because of her ignorance of the disease. Twenty six years later (64 years old), she felt exertional dyspnea, and was admitted to Fukuoka University Hospital for evaluation of her cardiac state. Thyroid function test revealed primary hypothyroidism with low T3, low T4 and high TSH levels. Cardiothoracic ratio on chest X-ray film was 69%. Electrocardiogram showed low voltage in the limb leads and intraventricular conduction disturbance. Echocardiogram demonstrated marked dilatation and severely reduced wall motion of the left ventricle and pericardial effusion. Left ventriculogram showed diffuse hypokinesis with 27% of the ejection fraction. No significant stenosis was observed on coronary arteriogram. Seventeen-month replacement therapy did not improve these cardiac findings significantly. Transvenous right ventricular endomyocardial biopsy demonstrated diffuse myocardial fibrosis without inflammatory infiltrate, which was interpreted as a sequel of interstitial lesions of the myxedema heart such as edema or mucoid infiltration. This pathological finding suggests that long-standing untreated hypothyroidism can cause irreversible myocardial damage.

[A rare adult case of double-outlet right ventricle without pulmonary stenosis: an autopsy case].

Double outlet right ventricle (DORV) is a rare cardiac malformation especially in adulthood. We report a woman with DORV who survived to the age of 33 years. She had not been seriously limited, before she visited our hospital for cardiac evaluation at the age of 25 years. She was cyanotic, and had clubbing of fingers and toes and facial edema. Holosystolic murmur and diastolic regurgitant murmur were audible along the left sternal border. Chest X-ray showed cardiomegaly and enlarged pulmonary trunks. Electrocardiography showed right axis deviation and biventricular hypertrophy. Laboratory examination revealed polycythemia (Hb: 22.4 g/dl), increased levels of hepatic enzymes due to congestive liver and marked hypoxemia (Pao2: 40 mmHg), Diagnosis of DORV was made with cardioangiography. Surgical repair was not indicated. Thereafter, she experienced recurrent heart failure which progressed gradually. She died probably of ventricular arrhythmia at the age of 33 years. At autopsy, the heart showed dilatation of the right atrium and ventricle, and hypertrophy of the bilateral ventricles. Both the pulmonary artery without stenosis and the aorta originated completely from the right ventricle, which were almost normally positioned with bilateral conus. Large ventricule septal defect was found at the subaortic portion. Patent ductus arteriosus and preductal aortic coarctation co-existed. To our knowledge, few cases with DORV who survived over 30 years have been reported, and they all manifested pulmonary stenosis. The present case is a very rare DORV that survived over 30 years without the complication of pulmonary stenosis.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of progressive systemic sclerosis associated with mutilans-type arthropathy and suspected Felty's syndrome].

A patient who developed mutilans-type arthropathy, splenomegaly, leukopenia, leg ulcer and massive hydroxyapatite accumulation during the course of progressive systemic sclerosis (PSS) was reported. A 56-years-old female had suffered Raynaud's phenomenon since the beginning of her third decade. She developed multiple symmetrical arthritis and morning stiffness at the age of 29, and was treated with NSAIDs and low dose corticosteroids under the diagnosis of rheumatoid arthritis (RA) Because of dysphagia and diarrhea, she was admitted in Niigata-Kenritsu Senami Hospital in September, 1987. Physical and roentgenographic examinations revealed diffuse scleroderma, mutilans-type arthropathy, lung fibrosis, splenomegaly and right leg ulcer. Laboratory examinations showed leukopenia, high titer of anti-DNA antibody, positive anti-Scl-70 antibody and mild hypocomplementemia. These findings suggested that she had PSS and Felty's syndrome. Furthermore, massive subcutaneous and intraarticular hydroxyapatite accumulation were noticed. The leg ulcer and laboratory data gradually improved with the combination therapy of corticosteroids, D-penicillamine and plasmapheresis. Although it has been well recognized that PSS patients reveal frequently the articular lesions similar to these of RA, severe mutilans-type arthropathy seen in this case is extremely rare. The joint contracture might be induced by hydroxyapatite accumulation, of which the early diagnosis seems to be very important in long-standing PSS patients.

Anti-human T-cell lymphotrophic virus type I antibody-positive adult T-cell leukemia/lymphoma with no monoclonal proviral DNA. A clinicopathologic and immunologic study.

Proviral DNA of adult T-cell leukemia virus (HTLV-I) was examined by the standard Southern blotting method in lymph nodes of 45 patients with anti-HTLV-I antibody (ATLA)-positive adult T-cell leukemia/lymphoma (ATLL). Six of these patients revealed no monoclonal proviral HTLV-I DNA in tumor cells. These six patients showed typical flower cells in peripheral blood; they comprised five cases of the smoldering type and one of lymphoma type. They showed a longer clinical course than ATLL patients with integrated proviral HTLV-I DNA. Five of the six patients were alive from 8 to 36 months after onset; the other patient died 9 months after onset. Histologically, they exhibited features of T-cell malignancy but with absence of the typical cerebriform giant cells that are usually present in ATLL. The tumor cells represented T-cell markers, usually CD4, but CD25 was negative. Rearrangement of the T-cell receptor gene C beta was found in four of the six cases. On the basis of these results, cases of ATLL with no monoclonal proviral HTLV-I DNA should be clinicopathologically differentiated from those with integrated proviral DNA.

A human gallbladder adenocarcinoma cell line.

A cell strain (FU-GBC-1) was established from cancerous ascites of a 68-year-old male patient with well-differentiated adenocarcinoma of the gallbladder. By light and electron microscopy, the cultured cells showed the morphologic features of adenocarcinoma characterized by gland-like structures, intracellular microcystic spaces, and mucous production. Immunoperoxidase stains showed that FU-GBC-1 cells expressed several epithelial tumor antigens including CA 19-9, carcinoembryonic antigen (CEA), and epithelial membrane antigen (EMA). The cell strain has been in continuous culture up to passage 44 for 1 1/2 years, with the population doubling time of 120 hours. The cytogenetic analysis by a G-band technique showed a constant loss of chromosome Y in FU-GBC-1 cells. The modal chromosome number at passage 12 was 82 with a range of 77 to 85. Flow cytometry with an ethidium bromide technique additionally confirmed aneuploid DNA content (4C) in the cultured cells at passage 12 and 35. Inoculation of FU-GBC-1 cells into the dermis of BALB/c nude mice produced transplantable adenocarcinoma identical to the original tumor. Because no continuous cell lines of the well-differentiated type of gallbladder adenocarcinoma have been reported in the literature currently, the newly established cell strain we report may yield a useful system for studying the morphologic and biologic characteristics of gallbladder adenocarcinoma.

Assay of methylmalonyl CoA mutase with high-performance liquid chromatography.

An assay for methylmalonyl CoA mutase activity is described. Succinyl CoA produced in this method is separated from the substrate, methylmalonyl CoA, by reverse-phase high-performance liquid chromatography and is quantified. This method is useful to differentiate mutase apoenzyme deficiency (mut0, mut-) and the defect in deoxyadenosylcobalamin synthesis using fibroblasts cultured in high concentration of supplementary hydroxocobalamin. In methylmalonic acidemia, measurement of lymphocytes mutase activity offers therapeutical and prognostic informations.

Synthesis, processing and degradation in yeast of precursor human lysozyme with newly designed signal sequences.

Recently, we reported that the synthesis of human lysozyme in yeast is inhibited when artificially designed non-native signal sequences are used [Yamamoto et al. (1987) Biochem. Biophys. Res. Commun. 149, 431-436; (1989) Biochemistry 28, 2728-2732]. Pulse/chase experiments described in the present paper have now revealed that precursor lysozymes are actually synthesized with these signal sequences, but that they are spontaneously degraded, suggesting that the enzyme is destabilized by the signal sequences. The data further show a good correlation between the secretory capability of the artificial signal sequences and the efficiency of the processing of precursor lysozyme.

Rearrangement of T-cell receptor delta chain gene as a marker of lineage and clonality in T-cell lymphoproliferative disorders.

We analyzed the rearrangement of T-cell receptor (TcR) delta chain gene in 88 cases of lymphoproliferative disorders; 31 acute lymphoblastic leukemias/lymphoblastic lymphomas (ALL/LBL); 27 adult T-cell leukemias/lymphomas, 9 angioimmunoblastic lymphoadenopathies (AILD); 10 T-cell lymphomas (non-Hodgkin's lymphoma); and 11 Hodgkin's disease. All of 9 T-ALL/LBL cases, of which 4 cases have neither beta nor gamma gene rearrangement, had a new rearranged band of TcR delta locus. Ten of 16 B-lineage ALL/LBL had rearranged band(s) or deletion of TcR delta locus. The rearranged bands were recognized in 2 cases of AILD and 1 case of T-cell lymphoma. All cases of adult T-cell leukemias/lymphomas, 4 of AILD, 4 of T-cell lymphoma, and 8 of Hodgkin's disease had deleted TcR delta locus. Heterogeneous findings of TcR delta locus analysis were observed in AILD, T-cell lymphoma, and Hodgkin's disease. In 16 cases with TcR delta rearrangement, the J delta 1 region was frequently used and the J delta 2 region was rearranged in one AILD. It is suspected that J delta 3 was used in one T-ALL/LBL. There was no correlation between the phenotypic pattern of CD3, CD4, CD8 in T-cell disorders and the rearrangement of the TcR delta gene. These findings suggest that the newly identified TcR delta chain gene rearranges at a very early stage of T-cell ontogeny; prior to the other TcR genes and perhaps at almost the same stage with CD7 expression. The TcR delta gene is useful in assessing clonality for the most immature T-cell neoplasms not showing rearrangement of the other TcR genes. This gene is not lineage specific; however, when used in conjunction with immunoglobulin heavy chain gene, it may be a useful tool to distinguish lymphoid lineage of ALL/LBL.

Molecular genetic analysis of a hybrid gene encoding Sta glycophorin of the human erythrocyte membrane.

Sta is an antigen of the human MNSs blood group system carried by a variant glycophorin residing in the erythrocyte membrane. We examined the structure, organization, and inheritance of Sta gene identified in genomic DNA from an Oriental family. Southern blotting detected a useful genetic marker tightly linked to the Sta gene. Differential hybridization and secondary restriction analyses showed that Sta gene is a fusion hybrid of delta and alpha glycophorin genes. Genomic mapping by extensive use of synthetic oligonucleotides, with overlapping sequence specificity, allowed us to define the delta-alpha junction site and disclose the organization of the variant gene. The junction point of Sta hybrid gene is encompassed by an unexpressed exonlike sequence of the delta gene at the 5' site, and an expressed sequence of the alpha gene spanning codons 59 through 71, at the 3' site. Dosage quantification demonstrated the occurrence of Sta gene as a single copy in the genome. Blood group inheritance, evaluated by DNA typing, established the tight linkage of Sta to the alpha M and delta S genes. The data support a single unequal crossing-over event between misaligned delta and alpha genes on the homologous chromosomes as the mechanism for the origin of Sta gene. The Sta gene is similar in overall structure to another delta-alpha hybrid gene, Dantu, but differs from it in junction structure, copy number, gene linkage, and antigen specificity.

Purification and characterization of carboxypeptidase from terminally differentiated rat epidermal cells.

A tissue carboxypeptidase-A-like enzyme was purified to apparent homogeneity from terminally differentiated epidermal cells of 2-day-old rats by potato inhibitor affinity chromatography followed by FPLC Mono Q column chromatography. The enzyme has an Mr of 35,000 as determined by SDS-polyacrylamide gel electrophoresis and HPLC gel filtration. It has a pH optimum of 8.5 for hydrolysis of benzyloxycarbonyl-Phe-Leu (Km = 0.22 mM, kcat = 57.9 s-1). The enzyme does not hydrolyze substrates with Arg, Lys and Pro at the C-terminal and Pro at the penultimate position. Angiotensin I was effectively hydrolyzed (Km = 0.06 mM, kcat = 6.48 s-1) and produced both des-Leu10-angiotensin I and angiotensin II. The enzyme activity, relatively stable at 4 degrees C and pH 8.0-10.5, was inactivated at pH values higher than 12.0 and lower than 5.0 or at 65 degrees C for 10 min. Inhibitor profiles of the epidermal enzyme also differed slightly from those of tissue carboxypeptidase A of pancreatic or mast cell origin.

Important role of the proline residue in the signal sequence that directs the secretion of human lysozyme in Saccharomyces cerevisiae.

To elucidate the role of the proline residue in the engineered signal sequence that directs the secretion of human lysozyme in Saccharomyces cerevisiae, we have remodeled an idealized signal sequence L8 = Met-Arg-(Leu)8-Pro-Leu-Ala-Ala-Leu-Gly [Yamamoto, Y., Taniyama, Y., Kikuchi, M., & Ikehara, M. (1987) Biochem. Biophys. Res. Commun. 149, 431-436] in the vicinity of the proline residue. By analyzing the secretory capability of 10 engineered signal sequences, we have shown the following. (1) The proline residue is important for the secretion of human lysozyme and is allowed at position -4, -5, or -6. (2) The secretory capability of the engineered signal sequences is correlated with their predicted conformations. (3) The functional signal sequences that we have investigated can be generalized as follows: Met-Arg-(Leu)n-Pro-(Xaa)-Ala-Leu-Gly where n equals 6-12 and Xaa is Leu, Ala, or Leu-Ala or can be omitted.
