[Biventricular thrombosis in nephrotic syndrome with hypercoagulability and hypereosinophilia].

A case of double right and left intraventricular thrombosis diagnosed by 2D echocardiography is reported in a 20 year old man with nephrotic syndrome with eosinophilia and hypercoagulability, admitted as an emergency for a staphylococcal septicaemia in shock and anuria. Anticoagulation with heparin did not prevent two episodes of pulmonary embolism. Complete dissolution of the thrombi was obtained by peripheral administration of fibrinolytic therapy (urokinase and plasminogen). The authors discuss the differential diagnosis of echocardiographic appearances of biventricular masses and possible causes of these thrombi are suggested.

[Treatment of lead poisoning by drinking water using hemofiltration and chelationin a hemodialysed anuric patient].

Lead intoxication was diagnosed in April 1984 in an anuric woman of 65 years of age, whose hemodialysis had begun 9 years previously. Lead poisoning was completely cured by repeated hemofiltration with calcium EDTA. 28.2 mg of lead were extracted during 26 hemofiltration sessions. After the twentieth session, the rate of extracted lead was 500 micrograms per session and the rate of delta-aminolevulinic acid dehydrase was normalized. This exceptional observation allowed a nearly experimental study of the kinetics and metabolism of Ca EDTA chelated lead.

Neurophysiological changes in the in vitro rat hippocampus following chronic lithium administration.

The effect of chronic lithium exposure on the electrophysiological responses of the Schaffer collateral-commissural (SCC) input to the CA1 pyramidal neurons in the in vitro hippocampus was investigated. Experimental animals were intubated intragastrically with lithium carbonate (150 mg/kg) for 3-4 weeks. This treatment produced lithium levels in serum and hippocampus of 0.3-1.3 mM. During the recording period, the hippocampal slice retained a stable lithium concentration of 53% of the initial value. Chronic lithium exposure had a depressive effect on input/output relationships, paired-pulse facilitation and strength of orthodromic inhibition. The antidromic inhibition was virtually intact. No obvious differences were found between control and lithium slices in amplitude, latency or waveforms, of synaptic and antidromic extracellular potentials. These findings are compatible with a major action of lithium on the excitability of SCC axons and synaptic terminals.

[Fluoride osteosis of hydro-telluric origin in a female patient with renal insufficiency. Apropos of a case with histomorphometric analysis].

We report a case of fluorosis due to high water and soil contents in a 23-year-old Algerian woman. The condition, which is infrequent at this age, was promoted by chronic renal failure due to a congenital renal malformation. Roentgenograms showing osteosis with increased bone density were suggestive of fluorosis rather than renal osteodystrophy. Bone tissue fluoride assay confirmed fluorosis. Histomorphometric analysis of bone biopsy specimens showed mixed lesions: major osteocondensation due to bone fluorosis and morphologic and dynamic osteomalacia related to the chronic renal failure.

Recovery from severe closed head injuries: repeat testings with the Halstead-Reitan Neuropsychological Battery.

Evaluated 15 adult patients who were suffering severe, coma-producing closed head injuries for neuropsychological and intellectual impairment with the Halstead-Reitan Neuropsychological Battery and the Wechsler Adult Intelligence Scale. All Ss were tested shortly after the time of their accidents and were reevaluated at approximately 1 year posttrauma to document residual deficits and rate of recovery. A group of 15 neurologically normal control Ss, matched for age, education, and sex, received the same battery of psychological tests. Although the brain-injured group demonstrated improvement on essentially all of the dependent measures from first testing to second, comparisons made with the control group revealed numerous areas of residual impairment, which suggests only partial recovery during the first year posttrauma.

Measurements of local PO2 in the resting skeletal muscles of rats with portocaval anastomosis (PCA) under normoxic conditions.

Direct measurements of local oxygen pressure by means of platinum multiwire electrode were performed in rats to investigate the effect of an end-to-side portocaval shunt procedure on muscle tissue oxygenation. Compared with intact rats and/or sham operated animals, rats with portocaval anastomosis showed a significant increase of muscle tissue oxygenation with nearly bellshaped pO2 histograms. This considerable increase in tissue oxygenation might appear to be due to the well known PCA induced hyperdynamic cardiovascular state.

SV40 deletion mutant (d1861) with agnoprotein shortened by four amino acids.

d1861 is an SV40 deletion mutant which was thought to lack the agnoprotein coding region and was used to verify the role of agnoprotein in the life cycle of SV40. In the present study the region flanking the deletion was sequenced and, in contrast to the available information, it was found that d1861 lacks 12 nt in phase, downstream from the AUG start codon of agnoprotein (residues 347-358). Using the runoff protocol with viral transcriptional complexes (VTC), that in vitro elongate the in vivo preinitiated nascent RNA, it was found that in vivo the major initiation site for late transcription is at residue 325, the same as in wild type (WT). In comparison with WT, d1861 encodes information for agnoprotein shortened by four amino acids and it has been identified in d1861 infected cells. However, pulse-chase experiments indicated that the rate of synthesis of d1861 agnoprotein is slower than that of WT agnoprotein and that it has a turnover rate of 1 hr as compared to 3 hr of WT agnoprotein. The reduced rate of synthesis of d1861 agnoprotein can be explained by nuclease S1 analyses in which the major leader of d1861 16 S RNA, that encodes the agnoprotein, appeared in significantly lower amounts as compared to the major leader of WT 16 S RNA. Furthermore, analysis of the potential secondary structures at the 5' end of the leader of d1861 16 S RNA has revealed stable structures in which the start codon of agnoprotein is sequestered in a stem. The involvement of RNA secondary structures in regulating the synthesis of agnoprotein is discussed.

IgA nephropathy: dimeric IgA-secreting cells are present in episcleral infiltrate.

Recent publications have reported a frequent association between IgA nephropathy and episcleritis. In this article, the authors report on the immunohistologic study of an episcleral biopsy obtained in a female patient with Berger's disease and frequent episodes of episcleritis. Numerous dimeric-IgA-secreting cells were demonstrated in the episcleral inflammatory infiltrate. It also should be mentioned that microhematuria often occurred within a few days after the beginning of episcleritis in this patient. These results suggest the participation of ocular mucosal immunity in some cases of IgA nephropathy.

Mapping in vivo initiation sites of RNA transcription and determining their relative use.

Runoff transcripts were generated on viral transcriptional complexes cleaved with restriction enzymes and incubated in vitro with [alpha-32P]UTP under pulse-chase conditions. As viral transcriptional complexes in vitro elongated the nascent RNA preinitiated in vivo, size analysis by gel electrophoresis of the runoff transcripts allowed identification of the in vivo initiation sites. Moreover, scanning the intensities of the runoff bands as they appeared in the autoradiogram of the gel allowed determination of the relative use of these sites. A model system in which the initiation sites of simian virus 40 late RNA were identified and their relative use determined is presented.
