The treatment of pyogenic bone and joint infections by closed irrigation-suction.

An improved method of closed irrigation-suction treatment of bone and joint infections is described. The method utilizes a crisscross flow-reversal system using a double tube which prevents obstruction of the tube. A series of 154 patients with acute and chronic bone and joint infections were treated with closed irrigation in 172 closed irrigation-suction procedures. One hundred forty-eight cases (86.0%) were treated successfully, 7 cases (4.1%) were improved, and 17 cases (9.9%) were failures. Blood concentration levels of antibiotics were examined during closed irrigation. Absorption of antibiotics from the wound was insignificant. Culture and sentivity tests of drainage during closed irrigation are used to monitor the cleansing of the wound. Serum hepatitis was seen in 4 cases within the hematogenous group and in 6 cases within the traumatic group. Postoperative bleeding during irrigation was an important complication.

[Study on cefazolin concentration in bone marrow (author's transl)].

Cefazolin (CEZ) concentration in the bone marrow of tibia was examined 1, 2 and 3 hours after intramuscular injection of 1 g. CEZ concentration in the bone marrow of tibia 1 hour after injection was 59.9+/-10.31 microgram/ml. Ratio to serum was 101.7%. CEZ concentration in the bone marrow of tibia 2 hours after injection was 42.5+/-7.77 microgram/ml. Ratio to serum was 102.7%. CEZ concentration in the bone marrow of tibia 3 hours after injection was 33.9+/-7.75 microgram/ml. Ratio to serum was 102.7%. Penetration capacity of CEZ into the bone marrow of tibia was excellent.

Estrogen receptor and alpha-fetoprotein in human breast cancer: brief communication.

Concentrations of estrogen receptor (ER) and alpha-fetoprotein were determined by dextran-coated charcoal assay and analyzed with Scatchard plots and radioimmunoassay, respectively, in cytosols of 72 human breast cancers. The values for ER ranged from 0 to 340 fmoles/mg cytosol protein. The concentrations of alpha-fetoprotein, which were low in all tumor cytosols examined, ranged from less than 0.1 to 1.1 ng/mg cytosol protein. No positive relationship was found between ER and alpha-fetoprotein concentrations. These results show that ER, but not alpha-fetoprotein, usually accounts for most of the high estrogen-binding capacity in cytosols of human breast cancers.

A cytoplasmic progesterone receptor in hen pituitary and hypothalamic tissues.

The cytosol fraction of the anterior lobe of the pituitary in laying White Leghorn hens was shown to possess a progesterone-binding component which perferentially bound progesterone. A similar component was also found in the cytosol of the hypothalamus and of the oviduct magnum. The dissociation constant (Kd) at equilibrium was determined as 1.68 +/- 0.10 (SE) X 10(-10) M in the pituitary, 3.29 +/- 0.10 x 10(-10) M in the hypothalamus, and 3.02 +/- 0.40 x 10(-10) M in the oviduct magnum, whereas the number of binding sites/mg cytosol protein was 5.99 +/- 0.084 (SE) X 10(-13) mol in the pituitary, 0.43 +/- 0.003 x 10(-13) mol in the hypothalamus, and 2.88 +/- 0.114 x 10(-13) mol in the oviduct magnum. The results suggest that a cytoplasmic progesterone receptor is present in the pituitary and hypothalamic tissue, as well as in the oviduct magnum, of the hen.

A new method of paired thyrotropin assay as a screening test for neonatal hypothyroidism.

A simple and reliable method of paired TSH assay was developed and used in screening for neonatal primary hypothyroidism. In this method, a paired assay is first done. Equal parts of the extracts of dried blood spots on filter paper (9 mm diameter) from two infants 4-7 days old are combined and assayed for TSH by double antibody RIA. If the value obtained is over the cut-off point, the extracts are assayed separately for TSH in a second assay to identify the abnormal sample. Two systems, A and B, with different cut-off points were tested. On the basis of reference blood samples (serum levels of TSH, 80 microU/ml in system A and 40 microU/ml in system B), the cut-off point was selected as follows: upper 5 (A) or 4 (B) percentile in the paired assay and values of reference blood samples in the second individual assay. Four cases (2 in A and 2 in B) of neonatal primary hypothyroidism were found among 25 infants (23 in A and 2 in B) who were recalled from a general population 41,400 infants (24,200 in A and 17,200 in B) by 22,700 assays. This paired TSH neonatal hypothyroidism.
