Primary choriocarcinoma of the lung.

Choriocarcinoma was found in the lung of a 34-year-old woman. Examination of the patient's entire body, especially the genital tract, failed to disclose foci of choriocarcinoma other than that in the right lung. After surgery, the levels of human chorionic gonadotropin in the blood and the urine fell. It was concluded that the choriocarcinoma of the lung was, in fact, the primary tumor. The genesis of choriocarcinoma was also studied in ten patients who died after delivery or abortion. Autopsy disclosed trophoblasts in the pulmonary arteries in nine of these ten patients. These findings suggest that primary choriocarcinoma in women is due to pulmonary embolism caused by trophoblasts at the time of abortion or delivery.

What muscle is responsible for lateral pharyngeal wall movement?

In an attempt to discover what muscle is responsible for medial movement of the lateral pharyngeal wall, the levator muscle was anesthetized with lidocaine under visual control by fiberscope in 3 normal subjects. The results indicate that the medial movement of the lateral pharyngeal wall results from the contraction of the levator. The attendant roll-up of the uvula toward the nasal side induced by the anesthesia was regarded as the action of the uvular muscle without the counterbalance of levator action.

Identification and characterization of a B cell growth inhibitory factor (BIF) on BCGF-dependent B cell proliferation.

The culture supernatants of Con A-activated human peripheral blood mononuclear cells (PBM) contained at least two regulatory factors upon B cell proliferation. One was B cell growth factor (BCGF), which activated antigen-stimulated B cells to proliferation and clonal expansion, and the other was its inhibitory factor, arbitrarily named B cell growth inhibitory factor (BIF). This BIF inhibited the effect of BCGF on anti-mu-stimulated B cells or the monoclonal mature B cell line (CLL-T.H.) obtained from the peripheral blood lymphocytes of B cell-type chronic lymphocytic leukemia patients, which were activated only with BCGF and without adding other proliferating stimuli (e.g., anti-mu). BIF activity was detected in the 24 hr culture supernatants of Con A-activated human PBM in FCS containing medium and also in serum-free RPMI 1640 medium. This substance with BIF activity could not be derived from FCS. Con A-induced BIF (m.w. of 80,000 and an isoelectric point of pH 5.4) was analyzed by Sephadex G-200 gel filtration and chromatofocusing. BIF was stable at pH 2.0 and at 56 degrees C for 30 min. Partially purified BIF had no effect on cell viability and almost no interferon activity (less than 1 IU/ml). BIF with high titer had a slight but significant inhibition on TCGF-dependent T cell growth and on PHA or Con A responses, but the extent of these inhibitions was far less than that of BCGF-dependent B cell growth. Absorption of BIF with Con A blasts made its inhibition on T cell growth even less. On the other hand, BIF activity could not be absorbed with Con A blasts but was almost absorbed with large numbers of CLL-T.H. cells. BIF had almost no inhibitory effect on the proliferation of a mouse fibroblast cell line (NIH 3T3), a mouse myeloma cell line (NS-1), human lymphoid cell lines (MOLT-4, HSB-2, and Daudi), or a human myeloid cell line (K-562). BIF-producing cells were estimated to be T cells and were identified as T8+ T cells. On the other hand, Con A-induced BCGF was demonstrated to be produced predominantly by T4+ T cells. These results show that human B cell proliferation is regulated by interaction between T4+ and T8+ cells via soluble factors, namely BCGF and BIF, respectively.

[Comparison of immunochemotherapy and chemotherapy of stage IV gastric carcinoma].

Fifty-eight patients with stage IV gastric carcinoma were studied for the 24-month survival rate after receiving immunochemotherapy (Mitomycin-C + FT 207 + OK 432 + PSK, 29 cases) and chemotherapy Mitomycin-C + FT 207, 29 cases). There were no differences in the background factors influencing survival time between the two groups. The survival rate of the immunochemotherapy group was higher than, that of chemotherapy group throughout the observation period, in addition to statistical significance in some periods. The 50% survival rate was 10 months, in the chemotherapy group and 18 months in immunochemotherapy group. The difference in survival between the nonresected cases with immunochemotherapy and the resected cases with chemotherapy was smaller than that between nonresected cases and resected cases of the chemotherapy or immunochemotherapy group. These results suggested that possibility that immunochemotherapy prolonged the survival of patients with stage IV gastric carcinoma.

Evaluation of velopharyngeal closure by CT scan and endoscopy.

Computerized tomography (CT) and endoscopy were used for the objective evaluation of velopharyngeal closure. In 19 patients with cleft palates and 9 normal subjects, CT scans of the velopharynx were made both at rest and during vowel phonation with a scanning time of 3.0 seconds and slicing width of 3 mm. At the same time, endoscopic observations of the velopharynx through the nose were carried out both at rest and during phonation. CT scan during phonation clearly demonstrated the mobility of the velopharynx, i.e., elevation of the soft palate, inward movement of the lateral pharyngeal walls, and protrusion of the posterior pharyngeal wall, in a single picture. Its disadvantage is exposure to x-rays and a rather complicated procedure. However, endoscopy is simple with no exposure to x-rays, but its disadvantage is occasional incomplete visualization because of the dead angle created by the elevated soft palate. Thus the combined use of CT and endoscopy can help to determine a rational choice of treatment for cleft palates.

Laryngeal fricative in cleft palate speech.

Some of the substitutions for or [integral of] in cleft palate speech, which was earlier regarded as a pharyngeal fricative, on the basis of listener's judgement, was found to be a laryngeal fricative by means of fluorovideoscopy and nasopharyngofiberscopy. The sound was most likely produced at the stricture between the posteriorly inclined epiglottis and the elevated arytenoids. During the production of laryngeal fricatives, the velopharyngeal port remained open just as for nasal sounds.

Prostacyclin producing activity of human umbilical, placental and uterine vessels.

Prostacyclin-like material producing activity of umbilical, placental and uterine vessels was studied. Umbilical arteries and veins were separated at sites 10-15cm and 1-2cm from insertion of the umbilical cord to the placenta. Placental arteries and veins were prepared from the first, second and third branches on the chorionic plate. Uterine vessels were obtained at abdominal hysterectomy. After incubation of each specimen in Tris buffer 1 ml (pH8.5, 0.5M) for 30 min at room temperature, the inhibitory effect of the medium on ADP induced platelet aggregation was measured and the prostacyclin-like material was quantified. These procedures were repeated consecutively four times in total for each specimen. Prostacyclin-like material production rate and its total production were calculated. In total prostacyclin-like material production, umbilical arteries and veins were much higher than placental arteries and veins respectively (p less than 0.001), but there was no significant difference between placental and uterine vessels. These results showed that prostacyclin-like material producing activity of blood vessels declined remarkably at the transitive region from umbilical to placental vessels. It seems that this distribution of vascular prostacyclin-like material producibility in the fetoplacental vascular system correlates with that of vascular reactivity to prostacyclin.

[Evaluation of chemotherapy of breast cancer (2). Clinical evaluation of the blood and tissue concentrations of FT-207 following intrarectal administration].

In order to examine the transfer of antitumor drug in breast cancer after administration of FT-207 suppository, FT and 5-FU concentrations in blood, tumor tissues, normal tissues and axillary lymph nodes were determined respectively. FT and 5-FU concentrations in tumor tissues were higher than those in normal tissues or lymph nodes in every cases, and there was a significant difference between FT and 5-FU concentration in blood and tumor tissues. Blood levels of FT and 5-FU remained quite constant at 16 and 20 hours after administration, respectively. Accordingly, it was suggested that the administration of FT-207 suppository to breast cancer was very effective as an adjuvant chemotherapy.

[Case of breast cancer associated with Recklinghausen's disease].

Malignant epithelial tumors associated with Recklinghausen disease are very rare. A 32-year-old female with Recklinghausen disease was admitted to our hospital complaining of a 5 X 4 cm left breast mass. A malignant tumor of the breast was diagnosed by mammography, echography and clinical features. Radical mastectomy was performed and pathological examination showed papillotubular adenocarcinoma. Three cases of breast cancer associated with Recklinghausen disease reported in the Japanese literature were reviewed.
