Increased membrane fluidity of erythrocytes in a case with malignant hyperthermia.

In a case with malignant hyperthermia (MH), membrane fluidity of erythrocytes was studied by electron spin resonance (ESR)-spin labeling technique. The measurement was made with samples obtained during and after pyrexic crisis until full recovery. The results indicated that membrane fluidity increased during pyrexic crisis and that these changes were serially restored. As the physicochemical characteristics of the erythrocyte membrane resemble those of cellular and cell organella membrane, it was concluded that membrane composition and configuration might have been altered pathophysiologically during the crisis of MH in our case.

Japanese patients with leukaemia following the use of Thorotrast including a patient with marked chromosomal rearrangements.

It is estimated that 20,000-33,000 persons have been injected with Thorotrast in Japan. By August 1984, 12 patients with leukaemia following the use of Thorotrast have been reported. Their clinical and haematological data indicate that most of the patients were males over 50 years of age and that the most frequent type of leukaemia involved the stem cells common to the granulocytic, erythroid and/or megakaryocytic lines. This assumption was supported by the evidence of marked chromosomal rearrangements in 100% of the cells from the bone marrow in our case. The median latent period from the administration of Thorotrast to the onset of leukaemia in Japan was 35 years, ranging from 16 to 45 years, which indicates that patients who were injected with Thorotrast should be carefully followed up in the future.

[Production, absorption and excretion of phenols in intestinal obstruction].

In intestinal obstruction, phenols were produced in the distended loop proximal to obstruction by enteric bacteria. Clinically, in 17 cases of non-strangulated intestinal obstruction, phenols were detected in 15 cases and mean concentration of phenols was 4.2 +/- 9.7 micro g/ml(mean +/- 1 SD). In the fraction of phenols, p-cresol was detected in 15 cases and mean concentration was 3.8 +/- 7.7 and phenol was detected in 4 cases and mean concentration was 0.5 +/- 2.6. Phenols were decreased as clinical improvement of intestinal obstruction. Enteric bacteria in enteric juice ranged from 10(4) to 10(10)/ml and its change paralleled to phenols concentration. Mean urinary concentration of phenols in intestinal obstruction was increased to 297 +/- 415 mg/day compared to control (less than 50 mg/day). Its change also paralleled to phenols concentration in enteric juice. Closed ileal loop was made in dogs and phenols were infused in the loop. Phenols were increased in the portal vein 5 min after the infusion and in the femoral vein 60 min after the infusion. Phenols, which was thought to be toxic to the host, were proved to be produced in the distended intestine and excreted from the kidney.

[Management of Crohn's disease].

Our therapeutic management to Crohn's disease was analyzed retrospectively. Among 17 cases with Crohn's disease, 14 (82%) were operated. Operative indications were due to obstruction (7), perforation (2), fistula (2), stenosis (2) and intractability (1). Postoperative recurrence was developed in 8 out of 14 (57%) and 4 were reoperated. Most of recurrent lesions were around the previous anastomosis. Operation per se does not cure the Crohn's disease but operation should be indicated when complicated obstruction, fistula and perforation. Nutritional treatment has been also performed 8 times. Crohn's disease activity index (CDAI) was decreased in all but one. In individual item of CDAI, frequency of bowel movement, abdominal pain and general status were improved but items such as anal lesion, fistula and fever were not improved. CDAI values were turned to remission value in all of cases with complicated stenotic lesions, but it remained in active stage in all cases with complicated fistula, obstruction and abscess. Nutritional treatment seemed less effective in cases complicated fistula, obstruction and abscess.
