A case of acute myelogenous leukaemia associated with eosinophilia: cytogenetic study of eosinophilic colonies showing the origin of the normal clone.

We describe a case of acute myelogenous leukaemia presenting with remarkable eosinophilia in relapse. Since the patient had chromosomal abnormalities, haemopoietic as well as eosinophilic colonies grown in culture were cytogenetically analysed to determine the origin of the eosinophils. Eosinophilic colonies as well as erythroid bursts revealed a normal karyotype, while a short-term culture of peripheral blood cells in relapse revealed an abnormal karyotype which had been observed before treatment. These data clearly demonstrate that the eosinophilia in this case originated from a normal clone. The cytogenetic analysis of eosinophilic colonies is a useful technique for the definite diagnosis of eosinophilic leukaemia or reactive eosinophilia in patients having marker chromosomes.

Promyelocytic crisis of chronic myelogenous leukemia without t(15;17).

A case of promyelocytic crisis of chronic myelogenous leukemia is reported. The amount of promyelocytoid blasts in peripheral blood and bone marrow were 70% and 58%, respectively, at the onset of the blastic phase. Cytogenetic analyses during the blastic phase did not reveal t(15;17); however, t(3;21) in addition to the t(9;22) in the karyotype of bone marrow or peripheral blood cells was observed.

36Cl- and 86Rb+ uptake in rat parotid acinar cells.

36Cl- uptake was markedly (85 per cent) inhibited by the loop diuretics, furosemide and bumetanide. Partial replacement of Na+ in the incubation medium (from 137 to 5 mM) reduced 36Cl- uptake 30 per cent; total replacement reduced uptake to 40-45 per cent of control values. Partial replacement of K+ (from 5.8 to 1 mM) decreased 36Cl- uptake approx. 45 per cent; complete replacement resulted in minimal levels of 36Cl- uptake (less than 10 per cent of controls). Ouabain reduced 36Cl- uptake approx. 55 per cent in the absence of bumetanide, but was without effect in its presence. 86Rb+ uptake was reduced approx. 85 per cent with bumetanide; complete replacement of Cl- with I- or gluconate-, decreased 86Rb+ uptake by 55 or 40 per cent respectively. The results support the notion that the bulk of Cl- influx in rat parotid acinar cells is via a loop diuretic-sensitive Na+/K+/Cl- co-transport mechanism as may occur in other secretory epithelia.

Selectivity and contribution of lecithin: cholesterol acyltransferase to plasma cholesterol ester formation.

Selectivity factors (Vm/Km) for human and rat lecithin: cholesterol acyltransferases (LCAT) for the transfer of various acyl groups from the 2-position of phosphatidylcholine were determined. By multiplying these values by the proportions of acyl groups at the 2-position of phosphatidylcholine, one can predict the proportions of molecular species of cholesterol ester which will be synthesized by LCAT. In human subjects fasted overnight, the molecular composition of plasma cholesterol ester was found to reflect the LCAT selectivity relatively accurately. This result supports the concepts that hepatic acyl-CoA:cholesterol acyltransferase (ACAT) does not contribute significantly to the synthesis of plasma cholesterol ester and that removal of cholesterol ester from plasma is not selective with respect to molecular species under these conditions. In contrast to the results with humans, the molecular composition of plasma cholesterol ester formed in spontaneously hypertensive rats fed a high-cholesterol diet and then fasted overnight differs from that which is predicted from LCAT selectivity and the proportion of various fatty acids at the 2-position of phosphatidylcholine: these results suggest that cholesterol ester is formed mainly via the ACAT reaction.

Expression of monocytic and T-lymphocytic phenotype in a variant form of acute promyelocytic leukaemia (M3).

A 46-yr-old woman was diagnosed as having M3 variant leukaemia in FAB subclassification with disseminated intravascular coagulation (DIC) and the 15;17 translocation. Immunologic examinations revealed that the blast cells exhibited monocytic (Ia+, Mo1+, Mo2+, My4+, My7+ and My9+) and T-lymphocytic (OKT11+, Leu4+ and Leu5b+) immunophenotype, with probably a biphenotypic population of cells. Expression of monocytic phenotype on the blast cells suggests that M3 variant includes a subtype with monocytic characteristics, and may be related to the morphologic monocyte-like feature of that of M3 variant.

[The function of the sympathetic nervous system determined by humoral factors in the maintenance hemodialysis patients].

Dialysis-induced hypovolemia in renal patients might accompany with decreased sympathetic tone and decreased baroreflex sensitivity. Present study was undertaken to elucidate the sympathetic nervous activity in terms of biogenic amines [plasma norepinephrine (NE), epinephrine (E) concentrations, dopamine-beta-hydroxylase activity (DBH) and plasma serotonin (5-HT) concentration] in 17 patients with chronic renal failure in whom renal hemodialysis were continuously performed. During pre-dialysis period, in these hemodialysis patients, heart rate, systolic blood pressure and plasma 5-HT were significantly elevated as compared with those on sex and age-matched controls; plasma NE and plasma DBH were not changed in these hemodialysis patients as compared with those in controls. After hemodialysis, increased plasma NE and increased DBH were observed in parallel with the significant weight loss. Mean blood pressure did not change after dialysis. Dialysis produced an increasing tendency of heart rate. Dialysis also produced a significant decrease in plasma 5-HT. Hemodialysis-induced weight loss was approximately 2 kg body weight after 5 hours dialysis; these patients were done 3 times dialysis during one week. However, hypotension and dizziness were not demonstrated under this condition. Present study revealed that long term maintenance hemodialysis produced increased sympathetic nervous system activity; the assumed explanation would be relevant to the simultaneous recovery of baroreflex sensitivity and sympathetic tone, because of the long-term continuous hemodialysis.

[Goblet cell carcinoid of the appendix].

A 50-year-old man was admitted with a diagnosis of acute appendicitis on January, 5, 1984 and an appendectomy was performed on the same day. The acutely inflamed appendix measured 7 cm in length, and a gross tumor, was seen at the apex. Histologic examination of the gross tumor, which was 1.7 cm in diameter, revealed a neoplasm representing a goblet cell carcinoid. Silver impregnation by both the Grimelius and Masson-Fontana methods disclosed a positive reaction in some neoplastic cells. Electron microscopically, endocrine granules with both limiting membranes and ahlos, 100-210 nm in diameter, were present in the neoplastic cells, Because of the relatively large size of the tumor and neoplastic invasion to the mesoappedix in this case and, in addition, the possible malignant nature of carcinoid tumor, ileoascendocolectomy was performed as an additional operation.

Cerebro-hepato-renal syndrome of Zellweger. Clinical and autopsy findings and a review of previous cases in Japan.

An autopsy case of cerebro-hepato-renal syndrome of Zellweger, which occurred in a 14-year-old Japanese girl, is reported. The autopsy revealed widely distributed cystic changes in addition to renal blastema of both kidneys, and the liver was cirrhotic. The case was complicated by anomalies such as high forehead, strabismus, and partial defect of chorioidea. So far there have been only 10 reported cases of cerebro-hepato-renal syndrome of Zellweger in Japan.

Photodynamic therapy with hematoporphyrin derivative in cancer of the upper gastrointestinal tract.

An historical overview is given of the development of photodynamic therapy (PDT) with hematoporphyrin derivative for the treatment of malignant tumors. Results in patients with superficial, and non-superficial esophageal cancer and with early gastric cancer is described, with case illustrations and follow-up. Due to the difficulty in early stage diagnoses and in determining all cases of lymph node involvement, this form of treatment should be employed primarily in inoperable early-stage cancer, to reduce the extent of resection, or to render previously inoperable cases operable. Combination with other treatment modalities and the effects of PDT on histologically different tumor types, need to be further examined.
