Management and evaluation of non-supervised home exercise program in a convalescent phase of acute myocardial infarction.

Out of 636 patients with acute myocardial infarction (AMI) admitted to our institution, 183 patients enrolled in our non-supervised home exercise program immediately after their discharge from the hospital. The first 40 patients were randomized to control and training group, while the remaining 143 patients were included in the training group. Before and after the trial, all patients underwent cardiopulmonary exercise testing; submaximal graded treadmill exercise test with the application of expiratory gas analysis. In the training group, patients performed 2 km walk-jog exercise everyday for 1 month, keeping their heart rate (HR) at 90-100% of that in the anaerobic threshold. HR during exercise was monitored by patients themselves, using HR-meter. The anaerobic threshold significantly increased in the training group; while control group had no significant changes. VO2 and HR significantly increased at the same Borg's indices. Psychological improvement was also obtained in the training group compared to control group. It is concluded that non-supervised home exercise program is effective and easily applicable in the convalescent phase of AMI.

Insulin release from the pancreas and fuel metabolism during late gestation in chemically diabetic rats.

The effects of chemical diabetes and fasting on fuel metabolism and insulin secretory activity in late pregnancy were investigated. Female Wistar rats were made chemically diabetic (CD) by intravenous injection of streptozotocine (30 mg/kg) 2 weeks before conception. When CD pregnant rats were fed, plasma glucose and insulin levels were not significantly different from those of normal pregnant rats. Ketone body levels, however, were higher in CD pregnant rats than in normal pregnant rats, indicating insulin resistance in CD rats. Insulin secretion from the perfused pancreas caused by arginine or glucose was markedly decreased in CD pregnant rats. The pregnant rats were fasted for 2 days, from day 19 to 21 of gestation. Plasma glucose and insulin concentrations decreased similarly in the two groups, whereas ketone body concentrations in CD pregnant rats were significantly higher than those in normal pregnant rats. Glucose-induced insulin secretion by the perfused pancreas was markedly attenuated by fasting and was not significantly different in normal and CD pregnant rats. These observations suggest that diabetes mellitus accelerates starvation in late gestation, due to increased insulin resistance and poor insulin secretion, and that fasting in diabetic pregnancy amplifies ketogenesis.

Meningioma associated with subdural hematoma--case report.

The case of a 49-year-old female with a left parietal convexity meningioma associated with an acute subdural hematoma is described. She was admitted because of sudden onset of severe headache accompanied by nausea and vomiting. She was also confused, and 6 hours after admission she developed lethargy, right hemiplegia, and left mydriasis with no pupillary reaction to light. Computed tomography disclosed a round, extra-axial mass in the left parietal region; it was heterogeneously enhanced. Emergency craniotomy, performed after carotid angiography, revealed a tumor with a massive underlying subdural hematoma. The histological diagnosis was meningotheliomatous meningioma, and there were many meningothelial cells within the hematoma.

Mechanism for the bitter tasting potency of peptides using O-aminoacyl sugars as model compounds.

In order to study the role of hydrophobicity in bitter peptides, several O-aminoacyl sugars, in which amino acids or peptides were attached to the 2- and 3-position of methyl alpha-D-glucopyranoside, were synthesized and sensory analyses were carried out. It was found that the bitterness increased as the hydrophobicity of compounds increased, implying that the bitterness receptor recognizes the hydrophobicity of bitter peptides. A structure for the bitterness receptor is also discussed.

[A bone metastasis of hepatocellular carcinoma].

Reported is a case of a bone metastasis of a hepatocellular carcinoma (HCC). A 61-year-old man with an unresectable HCC, which had been detected on spontaneous rupture, complained of a severe neck pain and numbness of both arms. A roentgenogram of the cervix showed destruction in the spinous process of the third cervical vertebra, and an MRI revealed a vertebral tumor compressing the spinal cord. Thus, the tumor was surgically removed. A pathological examination of the tumor revealed it to have the characteristic features on an HCC. One month after its removal, however, an iliac bone metastasis that was causing pain was found, and, again, an excision of the new tumor was performed. Pain symptoms disappeared after these operations. Treatment of an HCC bone metastasis is palliative, however, it is beneficial in maintaining the quality of the patient's remaining life.

[Frequency and prognostic features of rheumatoid patients with remission-inducing agents--a comparison of different kinds of medication].

Recently, the disease activity has been believed to be effectively controlled by remission inducing agents such as gold salt and D-penicillamine in rheumatoid arthritis. However, there remain some doubts whether the prognosis of RA has been improved by these drugs or not. The frequency of complete remission in RA patients and their special features were studied to know the real state of the remission of RA under the present treatments. The subjects under investigation were 466 RA patients who were treated with remission-inducing agents for one to eight years. There were 90 male and 376 female patients diagnosed as definite or classical RA. Complete remission according to ARA criteria (Pinals R.S. et al. 1981) was noted in 33 (7.1%) of the 466 patients. However, relapse of the disease activity was observed in five patients during an observation period of two years on average. The followings were noted as the clinical and laboratory features of 26 cases with complete remission which could be observed for more than six months. A breakdown on sex revealed that the remission rate was 13.3% in male patients, compared to 5.6% in female patients. Duration of the disease for these complete remission cases was shorter for males than for females. Clinical examination of the cases with complete remission revealed that the most of them had no family history of RA, as well as no rheumatoid nodule or hip contracture.(ABSTRACT TRUNCATED AT 250 WORDS)

[Mechanical properties and curing depth of urethane tetramethacrylate-based composite resins].

Mechanical properties and curing depth of light-cured composite resins based on five types of urethane tetramethacrylate (UTeMA; IP-4 M, XY-4 M, MC-4 M, HM-4 M and TM-4 M) were investigated, and compared them with those of UDMA-based composite resins. Composites based on UTeMA monomers containing an aromatic or cyclohexane ring in their chemical structure (IP-4 M, XY-4 M and MC-4 M) showed a relatively rigid properties than those based on aliphatic UTeMA (HM-4 M and TM-4 M). The composite resins based on XY-4 M monomer showed a curing depth and transmission coefficient superior to the other composite resins. UTeMA-based composites showed a significantly higher rigid properties than UDMA-based composites, while their rigidity did not improve the flexural strength.

[Mechanical properties and cure depth of UDMA-based composite resins].

Mechanical properties and cure depth of visible light-cured composite resins based on six types of UDMA (IP-HEMA, IP-HPMA, XY-HPMA, MC-HPMA, UEDMA and UPDMA) monomers were investigated. Under wet conditions, the mechanical properties of the composite resins based on aliphatic UDMA (UEDMA and UPDMA) monomers were inferior to those based on the other UDMA monomers containing aromatic or cyclohexane rings in their chemical structures. The cure depth for these UDMA-based composite resins increased with increasing irradiation time. The composite resin based on the XY-HPMA monomer showed a cure depth and transmission coefficient superior to the other composite resins.

[Electron-probe microanalysis examination of acid-proof dentin layer].

To predict the presence and thickness of the acid-proof dentin layer by the method of combining the halogenated methacrylate and electron-probe microanalyzer (EPMA), 2-methacryloyloxyethyl hydrogen chloromaleate (CIMEM) used as a base monomer for a bonding agent and 2-bromoethyl methacrylate (2 BEM), 2-(4-bromophenyl)ethyl methacrylate (BPy1EM), 2-(4-bromo-1-naphthyl)ethyl methacrylate (BNEM) and 2,4,6-tribromophenoxy methacrylate (TriBr-PM) used as tracers were synthesized. The bond strength to dentin treated with 37% phosphoric acid solution for 30 sec. was not statistically different for the bonding agents with and without tracers. The SEM micrographs revealed that the layers, which may be acid-proof dentin layers, were 3-4 microns thick at the resin-dentin interface for all bonding agents. According to EPMA analysis, cps of C1Ka and BrLa increase at the same points and the acid-proof dentin layer thickness was about 4 microns.

[Epilepsy after operation for ruptured intracranial aneurysm].

Fifty-five patients who underwent surgery for ruptured aneurysm of the anterior circulation were evaluated with respect to postoperative epilepsy. Prophylactic treatment with anticonvulsants had been carried out in all patients. The overall incidence of the postoperative epilepsy was 23.6% (13/55). The mean latency between the operation and the first seizure was 11 months (range, 8 days to 5 years). The most important risk factors by statistical analyses were multiple aneurysms, preoperative severity, severe subarachnoid hemorrhage, intracerebral hematoma, normal pressure hydrocephalus, EEG abnormalities in the chronic stage, intracerebral low density area on CT-films and neurological deficit. Other risk factors were aneurysm of MCA and early operation for aneurysm. Our opinions concerning postoperative anticonvulsant therapy for aneurysmal patients are as follows. 1) Prophylactic treatment with anticonvulsant should be administered to all patients who have undergone aneurysm surgery. 2) The risk factors should be considered in determining the duration of medication with the anticonvulsant. 3) The treatment with the anticonvulsant should be performed while monitoring the clinical course, EEG findings, plasma concentration level of the anticonvulsant and side effects of the drugs.

[In situ immune complex glomerulonephritis induced by perfusion of cationized antibody].

In situ immune complex glomerulonephritis can be induced in the rat employing cationized antigen planted in the glomerular basement membrane (GBM) as a target for specific antibody. Another possible mechanism of in situ immune complex formation is antibody already present in the GBM to bind circulating antigen. Present study was performed in order to determine whether cationized antibodies planted in the GBM could react with anionic as well as cationic antigens to form immune deposits. Horse ferritin, rabbit antibody to horse native ferritin (f-Ab) and rabbit antibody to ovalbumin (o-Ab) were highly cationized as described by Danon et al. The ability of the cationized antibodies to precipitate antigens was estimated by Ouchterlony analysis. 500 micrograms/100 g body weight (b.w.) of cationized f-Ab or o-Ab was perfused into the left renal artery of male Wistar rats and 0.1-10.0 mg/100 g b.w. of either native or cationized ferritin or ovalbumin was injected respectively into the tail vein 1 hr later. Estimation of proteinuria was done and the kidneys were removed up to 5 days for immunohistological as well as electron microscopical examination. Cationized antibodies bound to anionic sites of the GBM and combined with subsequently injected cationized ferritin or native ovalbumin in situ, both leading to formation of subepithelial immune deposit with activation of C3 and caused mild proteinuria. Native ferritin, however, induced neither subepithelial immune deposit nor proteinuria, because it didn't permeate through the GBM. The presented model indicates that antibody molecules of high positive charge can bind to the GBM and react with specific antigens that are traversing the barrier to form immune deposits. This is independent of the charge of antigen provided that the antigen molecules are permeable into the GBM, as is the case with ovalbumin but not native ferritin.

Characterization and induction of a cell line established from a patient with erythroleukemia (FAB M6).

A human cell line, designated as NISI, was established from a patient with erythroleukemia (FAB M6). The presence of a chromosome marker in NISI line indicates that it is derived from the leukemic clone which bears the common marker. The cell line shows morphology of immature erythroblast and has myelocytic properties from surface immunophenotyping. The differentiation capacities of NISI cells by three inducers (hemin, phorbor 12-myristate 13-acetate and 1-25-(OH)2D3) were evaluated. Hemin treated cells showed a significant increase in erythroid antigen expression as well as an increase in number of benzidine positive cells. Phorbor 12-myristate 13-acetate treated cells demonstrated a significant increase in megakaryocytic antigen expression, and a slightly diminished CD36 antigen expression. The surface markers of 1-25-(OH)2D3 treated cells did not demonstrate significant changes. NISI cell line, a human erythroleukemia cell line, still retained the tendency for differentiation to megakaryocytic lineage.

Continued high albumin production by multicellular spheroids of adult rat hepatocytes formed in the presence of liver-derived proteoglycans.

Adult rat hepatocytes formed floating multicellular spheroids, when they were cultured with proteoglycan fraction isolated from rat liver reticulin fibers. Cells in the spheroid showed only low growth activity. Albumin production by the spheroids increased up to 1.5 micrograms/micrograms DNA/day (180 micrograms/mg Protein/day) during the first 6 days and remained constant thereafter. In contrast, the albumin production by the monolayer markedly decreased after 4 days. The spheroid culture appears to be more suitable than the monolayer in studying differentiated functions of adult hepatocytes.

A distinct form of tau is selectively incorporated into Alzheimer's paired helical filaments.

Tau, a microtubule-associated phosphoprotein, was identified as a definite component of paired helical filaments which progressively accumulate in Alzheimer's disease brain. To learn more about tau in the aged brain, we have isolated and sequenced a cDNA clone encoding tau from a cDNA library of an aged human brain. The cloned cDNA sequence included a new insert of 93 nucleotides, which added a fourth repeat to the three-repeat type of tau already reported. Perhaps, this four-repeat type of tau is predominant in normal aged brain. In contrast, the sequence analysis of paired helical filaments showed that the integrated tau is of three-repeat type. This indicates that a distinct form of tau is selectively incorporated into paired helical filaments.

A reticuloendothelial system-activating arabinoxylan from the bark of Cinnamomum cassia.

A neutral polysaccharide, named cinnaman AX, was isolated from the dried bark of Cinnamomum cassia Blume. It was homogeneous on electrophoresis and gel chromatography. It is composed of L-arabinose: D-xylose in the molar ratio of 4:3, and its molecular weight was estimated to be about 1.0 x 10(6). Methylation analysis, carbon-13 nuclear magnetic resonance and controlled Smith degradation studies enabled elucidation of its structural features. It showed remarkable reticuloendothelial system-potentiating activity in a carbon clearance test.

Some correlations between procaine-induced convulsions and monoamines in the spinal cord of rats.

The relationships between the convulsions induced by the local anesthetic procaine and monoamines in the spinal cord were investigated in rats. The levels of dopamine (DA) and serotonin (5-HT) in the spinal cord were time-dependently increased after procaine (170 mg/kg, i.p.), which induced clonic convulsions, but the level of norepinephrine (NE) was unchanged. The rats that died during convulsions had a marked increase in DA. Phenobarbital (25 mg/kg, s.c.) produced both depletion of DA and elevation of 5-HT in the spinal cord and completely protected rats against convulsions. A 5-HT precursor, 5-hydroxytryptophan (5-HTP; 20-80 mg/kg, i.p.), suppressed the convulsions in a dose-dependent manner as shown by a decrease in the incidence and a prolongation of the latency, but a DA precursor, 3,4-dihydroxyphenylalanine (L-DOPA; 20-80 mg/kg, i.p.), markedly shortened the latency. Furthermore, the effect of L-DOPA on the convulsions was inhibited by the combination of 5-HTP. alpha-Methyl-p-tyrosine (20-80 mg/kg, i.p.) or DL-p-chlorophenylalanine (20-80 mg/kg, i.p.), an inhibitor of NE and DA or 5-HT biosynthesis, had a slight effect on the convulsions. These results suggest that procaine causes significant elevations of rat spinal DA and 5-HT in the convulsive process and suggest that dopaminergic and serotonergic neurons may be associated with procaine-induced convulsions.

Effect of starvation on nutrition and insulin secretion in pregnant rats and their fetuses.

Pregnancy is thought to create a metabolic condition of accelerated starvation. To clarify this idea, the effect of fasting on pregnant rats (day 21 of gestation) and their fetuses was examined. Although pregnancy significantly increased plasma insulin, plasma ketone body concentrations in fed pregnant rats were higher than those of age-matched fed virgin rats. After 48 hr fasting (i.e., fasting during days 19-21 of gestation), plasma insulin was markedly decreased in virgin rats compared with term pregnant rats, while ketone bodies were significantly higher in pregnant rats than in virgin rats. Body weight was lower in fetuses from fasted mothers than those from fed mothers. Starvation also markedly diminished the insulin response to glucose in isolated, perfused pancreases in both virgin and pregnant rats. The amount of insulin released during glucose stimulation was greater in pregnancy, and the inhibitory effect of 48 hr fasting on insulin release was greater in virgin rats than in pregnant rats. It is possible, therefore, that in term pregnant rats a decrease in insulin release caused by fasting may cause more profound catabolism than in nongravid rats.
