The prognostic and therapeutic value of second-look laparotomy in advanced ovarian cancer.

Of 267 patients with ovarian cancer FIGO stages III and IV, 157 underwent second-look laparotomy after combination chemotherapy consisting of cis-platinum and cyclophosphamide with and without doxorubicin. At second-look operation 45% had macroscopic tumour, 15% microscopic tumour, and 40% complete pathological response. Survival 3 years after second look was: complete pathological response 74%; microscopic disease 24%; visible tumour less than 1 cm 28%; visible tumour greater than 1 cm 17%; negative cytology 59%; and positive cytology 18%. Of the patients with macroscopic tumour, 29% had all visible tumour removed at second look. Three-year survival for these patients was about 45%.

Diagnostic accuracy of outpatient endocervical curettage using conventional and Vabra curettage of the cervix.

With the object of examining the diagnostic accuracy of and pain reaction to endocervical curettage (ECC) either by using a new instrument, Vabra mc cervix, or with a conventional 3 mm metal curette, 298 patients with histologically verified cervical intraepithelial neoplasia (CIN) or cytological suspicion have been involved in a consecutive prospective randomized trial. One hundred and forty-eight patients underwent curettage with the Vabra instrument (vabrasio) followed by conventional curettage; 150 underwent the same procedure, but in reverse order. In 114 patients, CIN was ascertained in one or both trials. Vabra mc cervix found normal histology in 23 of these patients (20%), whereas conventional curettage showed normal histology in 58 patients (51%). Quantitatively, Vabra mc cervix extracted significantly more tissue material than did conventional curettage. The pain intensity when using of Vabra mc cervix was less severe. The present study showed diagnostic accuracy of Vabra mc cervix to be 80% (95% confidence limits: 63.38-80.73), compared with 49% (95% confidence limits: 35.17-54.35) when using conventional curettage.

Waterborne paints. A review of their chemistry and toxicology and the results of determinations made during their use.

This work presents information on the composition of waterborne construction paints used in Denmark, data from determinations of chemicals evaporating from paints applied with a brush or roller, and the toxicology of these chemicals. Seven product types were selected to illustrate the composition of the paints. Measurements at two workplaces were used as models for the work environment during painting. Evaporating chemicals were collected on Tenax TA and analyzed with capillary column gas chromatography after thermal desorption. In smudging work, waterborne paints may result in skin irritation and/or sensitization when safety precautions are not taken. Irritation of the mucous membranes may be expected if airing is not sufficient. This irritation may lead to headache mediated by trigeminal nerve stimulation. No other health hazards (eg, brain damage) are expected. The available information indicates that waterborne paints are a clear improvement over traditional paints which use white spirit as the main solvent.

Relief from pain localized to the lower back in early labour. A double blind trial of ketocaine, a new, highly effective, local, cutaneous analgesic (A 2358, Ane-Pad), and placebo.

Pain localized to the lower back in the first stage of labour in 47 primiparous women was treated by epicutaneous application of ketocaine or placebo compresses. Twenty-three patients were treated with ketocaine compresses (Ane-Ped), and 24 patients with compresses soaked with isotonic saline. The pain relieving effect was almost the same in both treatment groups. Pain localized to the lower back was reduced in about 50% of the patients after one hour's application of ketocaine or placebo compresses. Supplementary analgesics had to be administered in about 60% of the patients in each group. No maternal or foetal systemic side effects related to the treatment were registered. Local erythema on the application area was only reported in the ketocaine group. The frequency of feeling of warmth during the application of compresses of ketocaine were high compared to the placebo group, 78% and 29% respectively.
