Nuclear genes associated with a single Brassica CMS restorer locus influence transcripts of three different mitochondrial gene regions.

Previous studies have shown that the mitochondrial orf224/atp6 gene region is correlated with the Polima (pol) cytoplasmic male sterility (CMS) of Brassica napus. We now extend this correlation by showing that the effects of nuclear fertility restoration on orf224/atp6 transcripts cosegregate with the pol restorer gene Rfp1 in genetic crosses. We also show, however, that the recessive rfp1 allele, or a very tightly linked gene, acts as a dominant gene, designated Mmt (modifier of mitochondrial transcripts), in controlling the presence of additional smaller transcripts of the nad4 gene and a gene possibly involved in cytochrome c biogenesis. A common sequence, TTGTGG, maps immediately downstream of the 5' termini of both of the transcripts specific to plants with the Mmt gene and may serve as a recognition motif in generation of these transcripts. A similar sequence, TTGTTG, that may be recognized by the product of the alternate allele (or haplotype), Rfp1, is found within orf224 just downstream of the major 5' transcript terminus specific to fertility restored plants. Our results suggest that Rfp1/ Mmt is a novel nuclear genetic locus that affects the expression of multiple mitochondrial gene regions, with different alleles or haplotypes exerting specific effects on different mitochondrial genes.

Composition of human zona pellucida as revealed by SDS-PAGE after silver staining.

The macromolecular composition of zona pellucida (ZP) isolated from human oocytes and embryos was characterized by one- and two-dimensional sodium dodecyl sulfate polyacrylamide gel electrophoresis (1-D- and 2-D-SDS-PAGE) under reducing conditions after silver staining. ZP specimens obtained after in vitro fertilization were removed from pools of heavily fragmented embryos and inseminated oocytes that failed to fertilize. For unfertilized oocytes, two major bands with an apparent molecular weight of, respectively, 96 and 76-54 kDa were observed after 1-D-SDS-PAGE and silver staining. When ZP were isolated from fragmented embryos, the electrophoretic pattern showed a marked attenuation of the 96-kDa band. Silver-stained 2-D-SDS-PAGE analysis of ZP components from unfertilized oocytes revealed the presence of four protein trains: ZP1 (Mr = 92-80 kDa, pl = 4.9-5.9), ZP2 (Mr = 66-58 kDa, pl = 5.0-6.0), ZP3H (high) (Mr = 72-58 kDa, pl = 3.5-5.1), and ZP3L (low) (Mr = 62-54 kDa, pl = 3.5-5.1). The human ZP3 family (ZP3H and ZP3L) showed marked heterogeneity. Fertilization-associated changes were apparent in the electrophoretic pattern. ZP1 (Mr = 92-86 kDa, pl = 5.0-5.8) displayed a dramatic decrease in intensity, and a new component had migrated to a position similar to that of ZP2. This modification may have been responsible for one aspect of the zona reaction, and could have contributed to a zona block to polysperma.

Increased zona-binding ability after incubation of spermatozoa with proteins extracted from spermatozoa of fertile semen.

The involvement of proteins extracted from spermatozoa of fertile semen in sperm-zona binding was examined under hemizona assay conditions. One droplet of a suspension of spermatozoa was exposed to sperm proteins and then tested for zona binding, while a parallel semen suspension droplet incubated with culture medium served as a control. The reliability of the test was increased by relating the number of spermatozoa bound to each inseminated hemizona to the surface area of the hemizona and expressed as the binding index. For spermatozoa incubated with extracted proteins, the binding index was greater than (P = 0.001) that of controls (125.2 +/- 45.1 versus 63.6 +/- 29.2, respectively). As a first control, two other protein sources (fetal calf serum and human follicular fluid) were tested in the hemizona assay. No significant differences were found in zona binding for other protein-exposed spermatozoa compared with controls. As a second and reverse control, exposure of one hemizona to sperm proteins before insemination with untreated spermatozoa induced a marked decrease (P = 0.0003) in sperm binding, compared with that of the matched hemizona not exposed to sperm proteins (control) (3.4 +/- 1.4 versus 74.5 +/- 6.8, respectively). Taken together, these findings confirm the involvement of extracted sperm proteins in sperm-zona interactions. Therefore, in the cases in which fertilization in vitro fails because of a lack of sperm-zona binding, incubation of deficient spermatozoa with proteins extracted from spermatozoa of fertile ejaculates should restore their ability to interact with the oocyte and, thus, should enhance the prognosis for in vitro fertilization.

[Coronary artery disease in octogenarians: contribution of coronary angiography and evaluation of therapeutic possibilities].

An increasing number of octogenarians is being referred for coronary angiography with a view to myocardial revascularisation. Based on a retrospective study of 168 octogenarians undergoing coronary angiography between January 1987 and December 1992, the authors reviewed the indications, the risks and therapeutic decisions taken in those patients with significant coronary artery disease. The population studied had acute myocardial ischaemia in 88.7% of cases, a low incidence of previous myocardial infarction and severe extracardiac pathology. Significant coronary lesions were observed in 90% of patients (151) with multiple vessel diseases in 57.8% of cases. The mortality attributed to coronary angioplasty was 0.59% (1 case) and the morbidity was 4.7%. The number of disease vessels did not influence the decision as 75.5% of single vessel, 75% of double vessel and 77.3% of triple vessel disease patients were referred for coronary surgery or angioplasty. Of the patients undergoing a revascularisation procedure (n = 108), those referred for surgery (n = 22) had more severe coronary disease than those referred for angioplasty (p < 0.05). The hospital mortality was high about 8%, irrespective of the therapeutic decision. The 3 year actuarial survival was 70% in the group undergoing myocardial revascularisation (whether by angioplasty or coronary surgery) and 53.6% in subjects treated medically. These results show that coronary angiography in a selected elderly population is possible although the mortality and morbidity should not be underestimated. A high proportion of these patients (72%) is referred for myocardial revascularisation.

[Atrial insertion of accessory pathways in permanent reciprocating junctional tachycardia].

Permanent reciprocating junctional tachycardia (PRJT) is an uncommon type of Tachycardia, usually associated with a retrogradely conducting accessory pathway situated near to the ostium of the coronary sinus. This study reports the localisation of the site of atrial insertion of the accessory pathway, confirmed by the efficacy of intracardiac catheter ablation. Five patients (4 men), aged 14 to 45, experienced PRTJT at rates of 120 to 150/mn over a period of 2 to 15 years. 4 patients were normal, expected in 2 patients in whom they were 16 and 20%. One of these suffered a thromboembolic complication after pharmacological interruption of the tachycardia. The presence of an accessory pathway with decremential retrograde conduction was confirmed in all cases. Catheter ablation was successful in the medio-septal (2 cases), posteroseptal (1 case), lateral (1 case) and anteroseptal (1 case) regions. Six to 30 applications of 20 to 30 watts of radiofrequency energy were used in the 5 cases. In one case (right lateral accessory pathway), a fulguration procedure was necessary with a cathodic shock of 160 Joules. The AV conduction was preserved and retrograde conduction was normalised in all cases. All patients remained asymptomatic for a period of 10 to 43 months without antiarrhythmic therapy. The ejection fractions of the two patients with left ventricular dysfunction returned to normal. The authors conclude that the accessory pathways of PRJT may be situated in different regions of the right atrium. The efficacy of catheter ablation was 100 % in this form of tachycardia.

A fucose-containing epitope potentially involved in gamete interaction on the human zona pellucida.

The oligosaccharide moiety of human, porcine and bovine zonae pellucidae was studied with lectins and monoclonal antibodies specific for tri- or tetra-saccharidic epitopes containing at least one terminal alpha-L-fucose. Animal eggs were collected from follicular aspirates, human eggs were collected from in-vitro fertilization and embryo transfer programmes and pooled into six groups. By direct immunofluorescence, the lectins reactivity was detected for the animal or the human zonae pools in the same way. Reactivity of Aleuria aurantia lectin demonstrated the presence of alpha-L-fucose terminal residues in the zonae from the three species studied. By indirect immunofluorescence, the 2-25 antibody reactivity was detected in every pool of human zonae whereas there was no evidence of any antibody reactivity on animal zonae. Using an anti-Lewis-b blood group antibody (2-25), we observed expression of this antigen as an intrinsic component of the human zona pellucida, independently of patients' Lewis red blood cell phenotypes. Antibody 2-25 inhibited the spermatozoa-zona binding in a hemizona assay, suggesting that this fucose-containing antigen could be part of a sperm-zona receptor.

[Electrocardiographic evaluation of anti-ischemic effect of verapamil during angioplasty].

The anti-ischemic and cardioprotective effects of various angina drugs have been evaluated during angioplasty. This special situation of transitory regional ischemia was used to test verapami given by intra-coronary injection in 15 patients undergoing balloon dilatation of the left anterior descending artery, suffering from stable angina and with normal left ventricular kinetics. Each patient serving as his own control, parameters indicating oxygen consumption and clinical and electrocardiographic (ST segment changes) markers were measured during two successive inflations. Continuous monitoring of comparative variations between a first inflation without treatment and a second inflation after intra-coronary administration of 1 mg of verapamil revealed, following 60 seconds of ischemia, the absence of any change in heart rate, a moderate fall in mean blood pressure (101.7 as against 96.8 mmHg, p = 0.033) and a notable 60% reduction in the amplitude of maximal ST depression (5.07 +/- 3.08 mm as against 2.07 +/- 1.44 mm, p = 0.0002). These results confirm the anti-ischemic properties of verapamil, the action of which is based upon an indirect peripheral vasodilator model but also on a probable direct cardioprotective effect on anoxic myocardial regions.

[Future of the treatment of male infertility].

The aim of this paper is to introduce inherent difficulties leading to relative delay in the field of male infecundity treatment. Four theoretical possibilities are exposed: infertility prevention improvement, qualitative or quantitative improvement of spermatogenesis, spermatozoa fecundity ability improvement and micro-fertilization procedures. These methods are exposed in their technical aspects and in their results. Critical analysis is conducted about some impressive reported series. Basic knowledge improvement seems to be necessary to explore new therapeutic ways.

Medical abortion after in vitro assisted fertilization.

Subzonal sperm insertion is an assisted insemination procedure to enhance IVF in patients with severe male factor. We report a lethal fetal abnormality that occurred in a SUZI-obtained pregnancy. It is essential that such cases be extensively collected to assess the feasibility of using this method in patients with severe semen defects.

[Cancer of the ovary and medically assisted reproduction].

Case reports and epidemiologic studies have recently indicated a possible link between ovarian stimulation, ovarian cancer and ovarian tumours. We sent a questionnaire to 116 French IVF centers to determine whether ovarian cancer had occurred in women treated during the last 10 years. The 19 cases of ovarian cancer reported have since been studied more thoroughly in a follow up questionnaire. Six can now be excluded, and the 13 remaining cases are compared. Five of these patients had fewer than 4 stimulation cycles here. Imputability was considered probable for 2 patients, possible for 5 patients, improbable for 3 and highly improbable for 3. Histologically, 6 (46%) cases involved borderline lesions. These results indicate that there is no need to change medical practice, but we are convinced that prospective epidemiologic research should be carried out immediately.

[The bovine zona pellucida: differences in macromolecular composition between oocytes, pre-treated with A-23187 or not, and embryos].

The SDS-PAGE method was used to determine the composition of isolated bovine zona pellucida (ZP). This egg extracellular coat appears to be characterized by 3 major glycoproteins (ZP1, ZP2, ZP3), with apparent molecular weight (MW) of 80-70 kD, 66-63 kD and 60 kD, respectively, as revealed by 1-dimensional SDS-PAGE. After 2-dimensional SDS-PAGE, the zona pellucida electrophoretic pattern indicates a fourth glycoprotein, thus called ZP4. SDS-PAGE analysis of ZP isolated from oocytes and embryos after transit through the genital tract of A23187 pretreated oocytes allowed the description of modifications in glycoproteinic composition.

Utility of zona pellucida drilling in cases of severe semen alterations in man.

OBJECTIVE: To evaluate the utility of zona drilling for human severe sperm alterations in in vitro fertilization (IVF). DESIGN: A prospective randomized study was conducted according to random assignment of collected oocytes to a control or zona drilling procedure. SETTING: Reproductive Biology Laboratory, University Hospital of Nantes, France. PATIENTS: Twenty infertile couples with severe semen defects and previous IVF failure. INTERVENTIONS: Ultrasound-guided transvaginal follicular aspiration. RESULTS: Fertilization rates were compared to assess the advantages of zona drilling: 93% (80/86) drilled oocytes survived and 18.75% (15) were fertilized, whereas only 3% (3/100) control oocytes were fertilized. The polyspermy rate for fertilized drilled oocytes was high (10/15; 66.6%). The normal fertilization rate after zona drilling remained very low (5/86; 5.8%) and was not statistically different from that of control oocytes (3/100; 3%). One implantation occurred after replacement of five drilled embryos in three patients but resulted in early miscarriage. Replacement of the single control embryo led to a progressive pregnancy and normal male birth. CONCLUSIONS: This study indicates that zona drilling does not improve IVF results in cases of severe semen alterations.

[Etiology of bacterial vaginosis (non-specific vaginitis)].

56 women who were diagnosed bioclinically as having a bacterial vaginal infection were studied, as were 35 women as a control group. The study was a semi-quantitative analysis of the vaginal bacterial flora, both aerobic and anaerobic. It shows that Gardnerella vaginalis and anaerobic bacteria such as Peptococcus, Peptostreptococcus, Bacteroïdes, Veillonella and Mobiluncus were associated in a statistically significant way with bacterial vaginitis. On the other hand Lactobacilli were less frequently found (p less than 0.001) than in the control group of women. The way in which the microbial flora is changed has been observed during attacks of vaginitis and is discussed, as is the importance of making the diagnosis and of treating this syndrome.
