Iatrogenic immunization with bovine thrombin: a mechanism for prolonged thrombin times after surgery.

Unexplained very-prolonged thrombin times (greater than 300 s) were found in plasma from four patients. Other coagulation variables were normal, and there was no history of coagulopathy. Mixing studies suggested the presence of thrombin inhibitors in patient plasma. Substitution of human thrombin for bovine thrombin in performing the thrombin time test resulted in normal clotting times, indicating that the inhibitory activity was directed primarily against bovine thrombin. Each patient had been treated with topical bovine thrombin during previous surgery. In the one patient with a preoperative thrombin time, the initial value was normal and prolongation began 16 days after surgery. An enzyme-linked immunoassay showed elevated levels of IgM or IgG antibodies to bovine thrombin in each patient tested. Affinity-purified antibodies to bovine thrombin from patient serum prolonged the thrombin time of normal plasma. These results suggest that iatrogenic immunization by intraoperative exposure to bovine thrombin is responsible for antibodies to bovine thrombin, which accounts for the prolonged thrombin times found in some patients after surgery.

Analysis of the market driven nursing shortage.

Donley and Flaherty look anew at the nursing shortage; they explain why the supply-side orientation cannot explain or resolve the problem. The mere increase in our numbers is not a solution in today's complex care environment. The authors explain the market-driven shortage and the changes in the work environment essential for ameliorating the shortage.

Seven caring functions of black grandmothers in adolescent mothering.

Exploration of the involvement of Black grandmothers (N = 12) in the primary infant care activities of young adolescent mothers is the focus of this descriptive study. Seven categories of "grandmother caring functions" are identified from an analysis of interview data: managing, caretaking, coaching, assessing, nurturing, assigning, and patrolling. The importance of the grandmother in the functioning of the low-income extended Black family and the maternal role process of the adolescent mother is discussed.

Grandmother functions in multigenerational families: an exploratory study of black adolescent mothers and their infants.

This qualitative study explores the functions of 19 Black grandmothers who are engaged in the care of their adolescent daughters' infants. Interviews with grandmothers revealed seven functions related to this role: managing, caretaking, coaching, assessing, nurturing, assigning, patrolling. Suggestions for future research with Black grandmothers are given.
