[Classification and clinical assessment of postoperative functions in anorectal anomalies].

There were some problems in International Classification of Anorectal Anomalies. Recto-Urethral Fistula: The level of recto-urethral Fistule and that rectal pouch are different from case to case. Hypertrophied genital folds: this has no value in the diagnosis of low type. Recently the Japan Study Group of Anorectal Anomalies has proposed clinical score, which is based on four parameters, rectal sensation, continence, constipation and soiling. But this score has also some problems. We have proposed a new clinical assessment.

[Studies on fetal heart and aorta by ultrasonic electroscanner and time-motion mode].

The ultrasonic studies were performed on 203 normal fetuses ranging from 16 weeks to 42 weeks gestational age. Structures of fetal heart and aorta were visualized by real time imaging. Fetal UCG were recorded by TM mode simultaneously under the guidance of an ultrasonic electro-scanner. Aortic root diameter, descending aortic diameter, left atrial dimensions, mitral valve diastolic excursion, left ventricular dimensions and right ventricular dimensions were measured in fetal UCG. The following correlations were obtained. Three-dimensional holographic display of images of otological specimens.

Three-dimensional displays of anatomical structures and clinical findings are very persuasive and instructive. Using multiplex holograms, we designed a display of three-dimensional images of otological specimens. Multiplex holograms, reported by Cross of the United States in 1975, enable reconstruction of three-dimensional moving images and are used for artistic display as well as for teaching in medicine and general education. Multiplex holograms were recorded in a two-step process. The first step is to make a series of original cine-pictures of an object from different horizontal directions, rotating it on a turntable. In the second step, one frame of the original film is recorded on a narrow strip hologram. All frames of the original film are recorded one after another and a complete multiplex hologram can be synthesized. In the reconstruction stage, the multiplex hologram is formed into a cylinder and illuminated from below by a small white light source. Reconstructions of the three-dimensional bright images of the object inside the cylindrical holographic screen are shown.

Optic hypoplasia with pituitary dwarfism (Kaplan-Grumbach-Hoyt syndrome, or DeMorsier syndrome).

A case of a 14-year-old boy with optic hypoplasia and pituitary dwarfism was presented. Eye Examination showed typical hypoplasia of the left optic nerve and mild hypoplasia of the right optic nerve. Except for dwarfism and nystagmus, the appearance of the patient was not unusual. Computed tomography revealed an enlargement of the suprachiasmatic cistern, and the presence of the septum pellucidum. The pituitary function tests revealed complete deficiency in GH, and poor or intermediate secretion in ACTH, TSH, PRL, LH and FSH. Urine volume and specific gravity were normal. Therapy with human growth hormone has been successful. It was indicated that hypopituitarism was probably of hypothalamic origin and early recognition of the syndrome is important in view of the possibility of treating growth retardation in some blind, or near blind children.

[The changes of alpha 2-plasmin inhibitor, and notably urokinase activities in blood measured by synthetic chromogenic substrates S-2444 after urokinase administration].

Thrombolytic therapy with Urokinase (UK) has often been successful but it is very difficult to determine the effective dosage of UK. It is reported that after UK administration, plasmin fibrinolytic activity was immediately inhibited by alpha 2-Plasmin inhibitor (alpha 2-PI). In this study, we used UK on patients with myoma to prevent the occurrence of thrombosis after operation and the initial decrease in alpha 2-PI activities following UK administration was investigated to determine the minimum effective dosage of UK required to suppress alpha 2-PI. An attempt was also made to measure UK activity in blood by means of chromogenic substrate S-2444, and in some cases by administering 60,000 I.U. UK, alpha 1-Antitrypsin (alpha 1-AT), alpha 2-Macroglobulin (alpha 2-M), Antithrombin III (AT III) and Plasminogen (Plg) were measured at the same time. The results were as follows: 1) By the drip infusion method. In all doses, alpha 2-PI and UK activity showed no remarkable change. 2) By the one shot method. a) A decrease in alpha 2-PI was observed following both 48,000 and 60,000 I.U. UK administration. It was noted that in the case of 48,000 I.U. UK, alpha 2-PI showed the lowest level, 60% of the pre-administration level. b) UK activity showed a gradual increase in the case of 60,000 I.U. UK only and large changes in the other cases. c) alpha 1-AT, alpha 2-M, AT III and Plg produced no remarkable changes. This indicated that the effective dosage of UK for suppressing alpha 2-PI was at least 48,000 I.U. UK with the one shot method, and alpha 2-PI is a reliable indicator of the effectiveness of UK therapy.

Studies on experimentally induced hydrosalpinges in rabbits--experimental approach for tubo-microsurgery.

Hydrosalpinx proved macroscopically and microscopically which was quite similar to that observed in the damage human fallopian tube could be induced by ligation of the rabbit fallopian tube. Where ligatures have been placed on two places of the fallopian tube, hydrosalpinx can be induced in 100 percent of cases if ligation is done securely. About 1-2 weeks after ligation, no change of ciliated cells was observed in the mucosal fold in SEM findings, nor was there much change in the external shape of non-ciliated cells even though hydrosalpinx was induced. With the lapse of time after development of hydrosalpinx, however, both of these cells on the surface of the mucosal fold gradually changed to loose their normal shapes. After 15 weeks, ciliated cells all but disappeared to be replaced by large, filled, irregular non-ciliated cells. Accordingly, hydrosalpinx in the early stage can be cured fully by tubal salpingostomy using microsurgery. In the case of hydrosalpinx many hours after ligation, however, end-to-end anastomosis of the normal regions should be performed since no recovery can be expected.
