The frequency of detection of serum antibodies against pituitary cells was determined in 32 patients with the primary empty sella syndrome. Antibodies reacting with corticotropin-secreting mouse AtT20 and PRL-secreting rat GH3 cells were found in 24 (75%) and 15 (47%), respectively, of the 32 patients; 14 patients (44%) had antibodies reacting with both cell lines. In patients with pituitary adenomas, the prevalence of antipituitary antibodies was significantly lower than in those with the empty sella syndrome; 1 of 9 acromegalic patients had antibodies reacting with GH3 cells, and 2 of 9 prolactinoma patients and 1 of 7 patients with nonfunctioning adenomas had antibodies reacting with both AtT20 and GH3 cells. Among 6 patients with idiopathic diabetes insipidus, 1 patient had antibodies reacting with AtT20 and GH3 cells, and 2 patients had antibodies reacting with either AtT20 or GH3 cells. None of 5 patients with established autoimmune diseases (3 with systemic lupus erythematosus and 2 with autoimmune adrenal failure) had antipituitary antibodies in their serum. These results suggest that pituitary antibodies may be related to the development of pituitary atrophy and the primary empty sella syndrome, and that the test may be clinically useful as a screening test for the empty sella syndrome.
Cardiovascular disease is the most common cause of death in the elderly. In fact, the cardiovascular disease is the number one cause of death in women and the number two cause in men of 65 years or over in Japan. We studied the clinical characteristics and significances of acute myocardial infarction as they relate to the aged.
We investigated serum CA 125 levels in 120 patients with benign gynecologic disease, and examined variations in CA 125 levels during the menstrual cycle in six normally cycling women. CA 125 levels were elevated transiently during menstruation, suggesting that blood samples taken during menstruation can give a false-positive result. Whereas the CA 125 values during the nonmenstrual part of the cycle were significantly higher in the patients with adenomyosis and advanced endometriosis (stages III and IV) than in those with a normal pelvis, there was no difference between the patients with mild endometriosis (stages I and II) and the normal-pelvis group. CA 125 levels during menstruation were significantly elevated, not only in women with adenomyosis and advanced endometriosis, but also in those with mild endometriosis, compared with those with a normal pelvis. These data suggest that the measurement of serum CA 125 during menstruation may increase the rate of detection of endometriosis over that in studies done during other phases of the cycle.

With recent advances in the development of detection methods, the number of STD (Sex transmitted disease) cases detected is greater than ever. Among many STD, Chlamydia trachomatis (= CT), a microbe that has been given a great deal of attention in the urologic field as a cause of nonspecific urethritis and has a characteristic life cycle, has been gradually proved to exist commonly and to become a cause of various obstetrical and gynecological diseases. However, as it is clinically still unknown which symptoms are actually influenced by CT, we studied infection with CT in 706 cases, consists of a group of patients with some symptoms and an asymptomatic group mainly consisting of pregnant women. As a result, the positive rate was found to be 12.3% (87 cases); 10.8% (21 out of 194) in asymptomatic pregnant women, 11.0% (13 out of 118) in patients with cervicitis, and 14.9% (30 out of 202) in patients with adnexitis including PID. As to age, the positive rate was highest, 23.1%, in patients under 20 years old. In fact, 1) CT positive pregnant women are not rare, and 2) CT was detected in high frequency in the young generation under 20 years old; and in these young women, the possibility of transmission of the disease to the infant in the birth canal should be considered in connection with future pregnancy and delivery. Tubal sterility may also result. We concluded that it was necessary to establish a satisfactory examination system.
Three patients with advanced cervical carcinoma were treated with two courses of cis-platinum (CDDP) intra-arterial administration. Histology of all patients revealed squamous cell carcinoma, keratinizing type, and all patients had stage IIIb diseases. CDDP at a dose of 50 mg was infused through each catheter of which tips were fixed in the bilateral internal iliac arteries via the opposite femoral arteries. The course of therapy was repeated at an interval of three to four weeks, so the total dose of CDDP in each patient was 200 mg. The bleeding from lesions, which occurred intermittently before therapy in all patients, was reduced after the primary course of therapy and completely stopped after the second one. Marked reduction of lesion on computed tomograph (CT) was confirmed in all patients. The reduction in the product of perpendicular diameters of measurable lesions on CT ranged from 40 to 50%. Adverse effects such as nephrotoxicity, myelosuppression and gastrointestinal toxicity were mild as compared with patients receiving intravenous administration of CDDP. The excellent clinical effects and tolerance observed in this study indicated that an intra-arterial administration of CDDP has a highly therapeutic efficacy in the treatment of squamous cell carcinoma of the cervix.

A rat hepatocyte cell line was cultured in Higuchi's medium with fetal calf serum and insulin and labeled with 35SO2/4-. The cells were treated with a number of ligands to displace the heparan 35SO4 proteoglycan (HSPG) from the pericellular matrix. Maximum release was obtained with D-mannose-6-PO4 (50 mM), D-glucose-6-PO4 (50 mM), myo-inositol-2-PO4 (2-5 mM), myo-inositol hexaphosphate (2-5 mM), and DL-myo-inositol-1-PO4 (1-2 mM). D-myo-Inositol-1,3,4-(PO4)3 (1 mM) and L-myo-inositol-1-PO4 (2 mM) were intermediate in their ability to release the cell surface HSPG, whereas heparin (2 mg/ml), yeast phosphomannan (4 mg/ml), D-xylose-1-PO4 (50 mM), D-glucose-6-SO4 (50 mM), and myo-inositol hexasulfate (5 mM) were ineffective. When 35SO2/4- was added to cell cultures, the total cell surface HSPG increased linearly, but the percentage of the total cell surface [35SO4]HSPG that was released by myo-inositol-PO4 increased with time during the labeling period, reaching a maximum of 65% after 5 h. When cells were labeled for 12 h without insulin in the medium, the maximum amount of cell surface HSPG that was released by myo-inositol-PO4 was reduced to 30%. However, when cells labeled in the absence of insulin were treated with phosphatidylinositol-specific phospholipase C and then myo-inositol-PO4, the release of the cell surface [35SO4]HSPG was increased to 73%. When the [35SO4]HSPG that was released from the cell surface by treatment with myo-inositol-PO4 was added to cultures of unlabeled hepatocytes, it was taken up very rapidly and a portion of the internalized HSPG was converted to free heparan SO4 chains which appeared in the nucleus. Uptake was Ca2+- and Mg2+-independent. The amount of [35SO4]HSPG taken up was markedly reduced when the myo-inositol-PO4-releasable [35SO4]HSPG was pretreated with trypsin, thermolysin, alkaline borohydride, or alkaline phosphatase. When the cells were grown in inositol-deficient medium or in the presence of myo-inositol-PO4, the amount of heparan SO4 found in the nucleus was markedly reduced, and the cells no longer exhibited contact inhibition. These effects of myo-inositol deficiency on the growth and nuclear heparan SO4 were accentuated by addition of LiCl to the cultures to prevent phosphatidylinositol synthesis from the endogenous myo-inositol-PO4.(ABSTRACT TRUNCATED AT 400 WORDS)
The maximum static pressure in the anal canal was measured using an open-tip manometric method with a continuous infusion system. The values obtained from 49 postoperative patients with imperforate anus were expressed as a percentage of those values obtained from 108 aged-matched controls. The value was observed to be 50 per cent or more in those patients with translevator type, showing normal defecation, 30-50 per cent in those with supralevator type, showing normal defecation or constipation, and 30 per cent or less in those with supralevator type, showing incontinence, and 30 per cent or less in those who had undergone surgery on the anal area at another hospital and required re-operation at our department, showing normal continence or incontinence.
The effects of sodium arsenite (arsenite) on the cholinergic system in the brain of the mouse were investigated in vitro and compared with those of N-ethylmaleimide (NEM) and iodoacetate, both of which are alkylating sulfhydryl reagents. Arsenite, at concentrations greater than 10(-4) M, inhibited depolarized and nondepolarized release of acetylcholine (ACh) from cerebral slices, the synthesis of ACh in the slices, high-affinity uptake of choline into synaptosomes and activity of acetylcholinesterase (AChE). On the other hand, arsenite potentiated dose-dependently the activity of choline acetyltransferase (ChAT). N-Ethylmaleimide and iodoacetate showed inhibitory effects similar to those of arsenite. However, some exceptions were that N-ethylmaleimide did not have any effect on the nondepolarized release of ACh while iodoacetate had no effect on high affinity uptake of choline and activity of AChE. In contrast to arsenite, N-ethylmaleimide and iodoacetate inhibited the activity of ChAT. Neither of arsenite, N-ethylmaleimide nor iodoacetate showed any effect on the binding of [3H]quinuclidinyl benzilate to muscarinic ACh receptors. Although arsenite is thought to inhibit the cholinergic system in brain in vivo, its potentiating effect on ChAT and inhibition of AChE may reduce this harmful effect.

In 16 untreated patients with hyperthyroidism due to Graves' disease, serum antidouble stranded DNA antibody, measured by RIA, was positive (greater than 20 U/ml) in 14. In methimazole-treated patients with T3-suppressible thyroid uptake, anti-DNA antibody was found in 9% (3 of 35). The frequency of positive tests in methimazole-treated patients with T3-nonsuppressible thyroid uptake and in surgically treated patients was 24% (5 of 21) and 57% (4 of 7), respectively. Among anti-DNA antibody-negative (less than 9 U/ml) and weakly positive (10-19 U/ml) patients, those with T3-suppressible thyroid uptake had lower anti-DNA antibody titers than those with T3-nonsuppressible thyroid uptake. Among 32 patients with Hashimoto's thyroiditis, anti-DNA antibody was positive in 7. None of the patients with simple goiter had positive or weakly positive anti-DNA antibody results. Although the quantity of antibodies did not correlate well in individual patients, the rates of positive TSH binding-inhibiting immunoglobulin and anti-DNA antibody tests were roughly comparable in these patient groups. None of these patients with thyroid disease associated with anti-DNA antibody had clinical or other serological evidence suggestive of systemic lupus erythematosus or related collagen vascular disorders. The finding of anti-DNA antibody provides a new aspect of immunological abnormality associated with hyperthyroidism of Graves' disease.
